1.Perspectives on Colorectal Cancer Screening in A Multiethnic Population in Kuala Lumpur using the Health Belief Model: A Qualitative Study

Colorectal cancer (CRC) carries a significant burden in most world regions. However, its screening uptake remains low. This study aimed to explore awareness and perspectives on CRC screening program in a multiethnic population and their preference for CRC screening decision aid. In-depth interviews were conducted until data saturation was reached. All interviews were audiotaped, transcribed verbatim, translated to English and analysed thematically using hybrid inductive and deductive approaches. 17 informants from three main ethnic groups (Malay, Chinese and Indian) with various levels of risk for developing CRC were recruited. Awareness on CRC screening program was found to be low. Majority of informants never heard of CRC screening program. Among 11 eligible informants, only five experienced CRC screening uptake. Thematic analysis of the transcripts yielded six major themes; knowledge on CRC, screening process, authority’s role, curability, willingness to screening and preference for decision aid, and theywere mapped onto the Health Belief Model. Specific multiethnic perspectives found included preference for traditional medicine coming from all ethnics, and reliance in God coming from Malay informants. Majority preferred short videos as CRC screening decision aid, in the form of animation and live-action screenplay. Exploration of perspective of CRC screening helps in producing impactful decision aids. Future efforts should focus on developing short videos that incorporate population’s perspectives and can be disseminated through electronic media.
2.Impact of COVID-19 on General Surgical Emergencies in a Tertiary Care Public Hospital in Malaysia

Introduction:
COVID-19 pandemic placed unprecedented strain on hospital resources and manpower. This study assesses the impact of Malaysia’s nationwide lockdown on the incidence, types, severity, and outcomes of emergency general surgery (EGS) cases at a tertiary care hospital.
Methods:
This retrospective cohort study analysed the data of patients who underwent EGS procedures for the period 2017 and 2021.
Results:
During the pandemic period, there was a 48.3% reduction in EGS cases (n=889) compared to the pre-pandemic period (n=1720). Significant reductions were observed in surgeries for appendicitis, soft tissue infection, colorectal, urology, peptic ulcer, and vascular disorders in the COVID period. Conversely, procedures for biliary disorders and hernia complications increased by 7.4% and 5.5%, respectively. Length of hospital stay increased by 57.8% for patients with small bowel disorders, while ICU/HDU admissions rose by 43.5% for colorectal cases (P < 0.05). There was no significant difference in mortality rates between the two periods.
Conclusions
The overall reduction in EGS procedures during the COVID-19 period was consistent with global trends. Despite the challenges posed by the pandemic, the outcomes related to duration of stay, ICU/HDU utilisation, and mortality remained comparable to the pre-pandemic period. These findings highlight the adaptability of healthcare professionals in maintaining continuity and quality of emergency surgical care during a crisis.

3.Unraveling the Sugar Rush: A Cross-sectional Study of Knowledge, Attitudes, and Practices Related to Sugar- Sweetened Beverages Consumption among Malaysian Young Adults

Introduction: The consumption of sugar-sweetened beverages (SSBs) among young adults in Malaysia has witnessed
a notable increase in recent years, giving rise to concerns regarding the potential adverse health effects, including
obesity and diabetes. Despite the urgent need for targeted interventions, there is a dearth of research examining the
knowledge, attitudes, and practices (KAP) related to SSBs among Malaysian young adults. To address this research
gap, the present study aims to assess the level of KAP concerning SSB intake and its associated factors in this specific
population. Methods: A cross-sectional study was conducted involving 408 Malaysian young adults aged between
15 and 29 years. Respondents completed an online questionnaire survey, which encompassed the assessment of
socio-demographic characteristics and KAP regarding SSB consumption. The collected data were analyzed using
IBM’s Statistical Package for the Social Sciences (SPSS) version 22.0. Spearman correlation tests were performed
at a significance level of 0.05. Results: The majority of respondents demonstrated moderate knowledge (60%) and
practices (60%), while exhibiting a high positive attitude (100%) towards reducing SSB consumption. Significant
correlations were observed between knowledge and attitude (p = 0.003, rs = 0.148) and between knowledge and
practice (p = 0.005, rs = 0.138). Moreover, the attitude scores were found to be significantly associated with gender
(χ2
(1, N = 408) = 10.421, p = 0.001) and locality (χ2
(1, N = 408) = 4.106, p = 0.043), while the practice scores
exhibited a significant association with ethnicity (p = 0.006). Conclusion: This study provides valuable insights into
the relationship between KAP related to SSB consumption and socio-demographic factors among Malaysian young
adults. The findings underscore the importance of utilizing these insights to develop strategic interventions that aim
to curtail SSB consumption and promote a healthy lifestyle, ultimately contributing to the reduction of non-communicable diseases. Health authorities are encouraged to capitalize on these findings to design targeted interventions
tailored to the unique needs and circumstances of this population.

Soal selidik sebagai alat untuk mengukur faktor sosiobudaya berkaitan COVID-19 masih belum digunakan secara meluas oleh kebanyakan penyelidik. Di Malaysia, tiada soal selidik yang dibina dan disahkan untuk tujuan tersebut. Oleh itu, tujuan kajian ini adalah untuk membina dan mengesahkan soal selidik sebagai alat ujian yang baru bagi mengukur faktor sosiobudaya berkaitan COVID-19 dalam kalangan Orang Asli di Malaysia.
Metodologi:
Kajian rintis ini melibatkan 30 orang penduduk Orang Asli di Pos Lenjang, Kuala Lipis, Pahang. Indeks Kesahan Kandungan (CVI) digunakan untuk menguji kesahan kandungan soal selidik, manakala kesahan konstruk dan kebolehpercayaan konsistensi dalaman soal selidik ditentukan menggunakan Exploratory factor analysis (EFA) dan Cronbach’s Alpha.
Hasil Kajian:
Satu set soal selidik terdiri daripada empat konstruk telah dibina iaitu kepercayaan, amalan perubatan tradisional, adat dan budaya tradisional, dan sokongan sosial. Sebanyak 16 item telah dikekalkan di dalam EFA dengan nilai Cronbach’s Alpha setiap konstruk adalah 0.617, 0.778, 0.773 dan 0.779.
Rumusan
Soal selidik yang telah dibina ini berjaya divalidasi dan boleh digunakan untuk mengukur faktor sosiobudaya berkaitan COVID-19 dalam kalangan Orang Asli. Walau bagaimanapun, penambahbaikan masih perlu dilakukan dari masa ke semasa untuk memastikan soal selidik ini relevan terutamanya jika ia digunakan ke atas populasi atau dari negara yang berbeza dari Malaysia, dengan mengambil kira juga penyakit yang berbeza.

Chediak-Higashi syndrome (CHS) is a rare haematological and immunodeficiency disorder that occurs in childhood leading to recurrent infections, bleeding tendencies and progressive neurological dysfunction. Partial oculocutaneous albinism occurs in almost all cases. The exact prevalence is unknown, and the disease is caused by over 70 identified mutations in the lysosomal trafficking regulator gene. The presence of a bright polychromatic appearance from hair shaft and abnormally large intracytoplasmic granules, especially within neutrophils and platelets in the bone marrow is highly suggestive. Treatment is largely supportive, and the only curative treatment is through an allogeneic hematopoietic stem cell transplant.Without transplant, most patients will enter an accelerated phase of hemophagocytic lymphohistiocytosis (HLH) which carries a high mortality rate. We present a young male with CHS who we had followed through and eventually developed a fulminant accelerated phase.We believe this is only the second reported case of CHS in Malaysia.
6.Evaluation of the antinociceptive activities of natural propolis extract derived from stingless bee Trigona thoracica in mice

Background:
Stingless bee propolis is a popular traditional folk medicine and has been employed since ancient times. This study aimed to evaluate the antinociceptive activities of the chemical constituents of aqueous propolis extract (APE) collected by Trigona thoracica in a nociceptive model in mice.
Methods:
The identification of chemical constituents of APE was performed using high-performance liquid chromatography (HPLC). Ninety-six male Swiss mice were administered APE (400 mg/kg, 1,000 mg/kg, and 2,000 mg/kg) before developing nociceptive pain models. Then, the antinociceptive properties of each APE dose were evaluated in acetic acid-induced abdominal constriction, hot plate test, and formalin-induced paw licking test. Administration of normal saline, acetylsalicylic acid (ASA, 100 mg/kg, orally), and morphine (5 mg/kg, intraperitoneally) were used for the experiments.
Results:
HPLC revealed that the APE from Trigona thoracica contained p-coumaric acid (R2 = 0.999) and caffeic acid (R2 = 0.998). Although all APE dosages showed inhibition of acetic acid-induced abdominal constriction, only 2,000 mg/kg was comparable to the result of ASA (68.7% vs. 73.3%, respectively). In the hot plate test, only 2,000 mg/ kg of APE increased the latency time significantly compared to the control. In the formalin test, the durations of paw licking were significantly reduced at early and late phases in all APE groups with a decrease from 45.1% to 53.3%.
Conclusions
APE from Trigona thoracica, containing p-coumaric acid and caffeic acid, exhibited antinociceptive effects, which supports its potential use in targeting the prevention or reversal of central and peripheral sensitization that may produce clinical pain conditions.
7.Role of TGF-β1/SMADs signalling pathway in resveratrol-induced reduction of extracellular matrix deposition by dexamethasone-treated human trabecular meshwork cells

Deposition of extracellular matrix (ECM) in the trabecular meshwork (TM) increases aqueous humour outflow resistance leading to elevation of intraocular pressure (IOP) in primary open-angle glaucoma, which remains the only modifiable risk factor. Resveratrol has been shown to counteract the steroid-induced increase in IOP and increase the TM expression of ECM proteolytic enzymes; however, its effects on the deposition of ECM components by TM and its associated pathways, such as TGF-β-SMAD signalling remain uncertain. This study, therefore, explored the effects of trans-resveratrol on the expression of ECM components, SMAD signalling molecules, plasminogen activator inhibitor-1 and tissue plasminogen activator in dexamethasone-treated human TM cells (HTMCs). We also studied the nature of molecular interaction of trans-resveratrol with SMAD4 domains using ensemble docking. Treatment of HTMCs with 12.5 µM trans-resveratrol downregulated the dexamethasone-induced increase in collagen, fibronectin and α-smooth muscle actin at gene and protein levels through downregulation of TGF-β1, SMAD4, and upregulation of SMAD7. Downregulation of TGF-β1 signalling by trans-resveratrol could be attributed to its effect on the transcriptional activity due to high affinity for the MH2 domain of SMAD4. These effects may contribute to resveratrol's IOP-lowering properties by reducing ECM deposition and enhancing aqueous humour outflow in the TM.
8.Prevalence of Food Addiction among People with Diabetes in an Urban Health Center (Ketagihan Makanan di Kalangan Pesakit Diabetes di Klinik Kesihatan Kawasan Bandar)

Food Addiction (FA) is a concept of dependence on substance related to the consumption of palatable foods high in fat,
sugar, and salt. This dependence may influence dietary modification advice as dietary modification is an essential
component in managing people with diabetes. The objective of this study is to determine the prevalence of FA among
people with diabetes. A cross-sectional study was conducted between June and August 2021 at Klinik Primer, Pusat
Perubatan Universiti Kebangsaan Malaysia (Cheras, Kuala Lumpur) involving 142 people with diabetes. FA was
assessed using the validated Bahasa Melayu version of Yale Food Addiction Scale 2.0. The median age of participants
was 58 years (IQR=10), and the majority (88%, n=125) of them were above the age of 45 years. A large majority) had
central obesity (90.1%, n=128) and were overweight and obese (91%, n=129). The median duration of diabetes was 10
years (IQR=11) with more than two-thirds (63.4%, n=90) of them having diabetes for more than 5 years. Majority
(73.9%, n=105) also had poor glucose control with median HbA1c level of 7.5% (IQR =2). Our study found that the
prevalence of FA among people with T2DM was low (2.8%, n=4). This could be because the participants were mostly
from the older age group with long standing disease. These two features may have influenced their eating behaviour,
becoming more compliant and adapted to diabetic diet advice received over the long duration of the condition. A similar
study among young diabetics and those with new onset disease may better reflect FA among this group of people.
9.Poverty related diseases amongst Malaysia’s low-income community: a review

Poverty, as proven by several studies, is a driving force behind poor health and hygiene practices.
This review attempts to outline common communicable and non-communicable diseases that
disproportionately affect Malaysia’s 2.91 million low-income households. The current study also looks
into the government’s housing and healthcare programmes for this demographic to improve their
health and well-being. The initial examination yielded incredibly little research on this marginalised
community, with event reporting typically generalised to the Malaysian community as a whole rather
than analysing disease incidences based on household income, which would better reflect povertydriven diseases. As a result, there is an acute need for more accurate information on the epidemiology
of diseases among the poor in order to address this public health issue and provide conclusions that
can drive policy designs.
10.Aggregatibacter actinomycetemcomitans Endocarditis in a Patient with Chronic Periodontitis: A Case Report

The HACEK organisms consist of the non-influenzae Haemophilus sp., Aggregatibacter sp., Cardiobacterium sp.,
Eikenella corrodens and Kingella sp. are responsible for a sizable percentage of infective endocarditis cases worldwide with the mortality rate of 18%. Amongst them, Aggregatibacter actinomycetemcomitans is the most common
pathogen strongly associated with infective endocarditis. A. actinomycetemcomitans forms part of the oral microbiota and is also the etiological agent of periodontitis. Here, we present a case of a 37-year-old man with underlying
obstructive uropathy, that sought treatment for postural hypotension and symptomatic anaemia with fever. Later,
he had developed decompensated congestive cardiac failure with aortic regurgitation. A cardiac echocardiogram
revealed the presence of vegetation on the aortic valve. Blood culture grew A. actinomycetemcomitans, and he
was treated with furosemide and ceftriaxone. A further dental examination showed the patient is having chronic
periodontitis, which could be the possible source of A. actinomycetemcomitans causing infective endocarditis. The
patient was then transferred to the National Heart Centre for the first time for further management after completion of
4 weeks of intravenous antibiotics. As the pathogen is fastidious, rapid and newer technology like MALDI-TOF mass
spectrometry provides rapid and accurate identification for appropriate patient clinical management.
