Myelodysplastic syndrome (MDS) is a malignant hematologic tumor, which is currently difficult to cure. The theory of Xuanfu was proposed by Liu Wansu, which is unique in the clinical evidence of Chinese medicine and is less frequently applied to hematological diseases. The application of Xuanfu theory in myelodysplastic syndrome provides new ideas for the treatment of the disease. The abnormal flow of Qi, blood and fluids caused by the occlusion of the Xuanfu is the cause of toxic stasis obstruction, which is the pathogenesis of toxic stasis obstruction. Thus, the method of dispersion of Bone from Xuanfu, the external treatment of Xuanfu, and regulation of liver qi and Xuanfu help to return to normal of opening and closing function of Xuanfu, and release toxic stasis. In this paper, we analyzed the evidence of toxin-stasis obstruction in myelodysplastic syndrome from the theory of Xuanfu, aiming to provide a feasible theoretical basis for clinical treatment of the disease.
3.Clinical Experience of LI Yan in Treating Coronary Heart Disease from the Spleen and Stomach
Journal of Guangzhou University of Traditional Chinese Medicine 2024;41(1):213-218
Coronary heart disease(CHD)is a common cardiovascular disease in clinic, which can be classified as"chest qi-obstruction"and"heart pain"in traditional Chinese medicine.Professor LI Yan believes that CHD can be differentiated as the syndrome of deficiency in origin and excess in superficiality, and the deficiency in origin is characterized by the deficiency of spleen and the failure of spleen and stomach in transportation, while the excess in superficiality manifests as the obstruction of visible pathogens such as phlegm turbidity, blood stasis,qi stagnation, and cold accumulation. The clinical treatment should focus on regulating the spleen and stomach, using Astragali Radix,Atractylodis Macrocephalae Rhizoma,Codonopsis Radix,Schisandrae Chinensis Fructus to strengthen the spleen and nourish the heart in order to treat the root cause. Moreover, therapies of activating yang to remove turbidity, activating blood to remove stasis, soothing liver to rectify qi,and warming yang to dissipate the cold can be adopted. Therefore, the symptom and root cause will be treated simultaneously, the unblocking and supplementing effect will be achieved, and then the spleen-qi is vigorous, the heart qi is in abundance, the blood stasis and the phlegm are removed, and the chest yang can be stretched. As a result, the heart vessels are unblocked, and the symptoms of chest qi-obstruction will be relieved.Professor LI Yan's experience from the perspective of spleen and stomach can provide reference for the clinical diagnosis and treatment of CHD with traditional Chinese medicine.

5.Research progress on bipolar disorder and pregnancy complications and neonatal risk
Chinese Journal of Nervous and Mental Diseases 2024;50(1):60-64
Pregnant women with bipolar disorder(BD)are a high-risk pregnancy state.Necessary psychotropic drug treatment, special stress reactions, bad living habits, and fluctuations in pregnancy hormones all increase the risk of pregnancy to a certain extent.Risks of complications such as hypertension, gestational diabetes, premature birth and spontaneous abortion.Drugs can penetrate the placental blood-brain barrier and enter the maternal-fetal microcirculation.Combined with the effects of genetic genes and the environment, they can induce neurodevelopmental abnormalities in the fetus, leading to congenital malformations, attention deficit hyperactivity disorder, autism spectrum disorder and other diseases after birth. The mode of delivery may also be affected.Women with BD often give birth by caesarean section. It is particularly important to weigh the choice of drug types and dosages, which will help improve the accuracy of clinical risk management and disease control of pregnancy-related mental disorders.

Objective To identify and validate co-expressed genes associated with myocardial ischemia/reperfusion injury(MI/RI)and necrotic apoptosis by bioinformatics analysis. Methods Gene expression profile data for MI/RI were obtained by GSE67308 and GSE19875 datasets from the Gene Expression Omnibus(GEO)database.Differential expression analysis was conducted on the GSE67308 dataset to identify differentially expressed genes(DEGs),followed by gene set enrichment analysis and biological pathway analysis.More-over,immune cell infiltration analysis was performed on the GSE67308 dataset.Necrotic apoptosis-related genes were retrieved from the Molecular Signatures Database and the Kyoto Encyclopedia of Genes and Genomes(KEGG).A protein-protein interaction(PPI)network was constructed by overlapping DEGs with these necrotic apoptosis-related genes to identify key genes.Furthermore, the expression pat-terns of these key genes across various cardiac cell types were analyzed using a single-cell sequencing analysis platform, and validation of key gene expression was performed using the GSE19875 dataset. Results A total of 1054 DEGs were identified, comprising 363 upregu-lated and 691 downregulated genes.Gene enrichment analysis revealed that DEGs were primarily associated with processes related to apoptosis, immune responses, and intracellular signaling regulation. Moreover, biological pathway analysis demonstrated that DEGs were predominantly involved in the regulation of signaling pathways such as tumor necrosis factor(TNF)and NF-κB.Immune infiltration anal-ysis indicated a high degree of immune infiltration, particularly with natural killer cells and monocytes,in MI/RI myocardial tissue.PPI network analysis identified Il1b,TNF,Birc3,and Ripk1as crucial genes in the context of necrotic apoptosis.Single-cell sequencing anal-ysis showed the elevated expression of key genes within white blood cells. In comparison to the control group, the MI/RI model group in the GSE19875 dataset exhibited significantly increased expression of Il1b,TNF,Birc3,and Ripk1(P<0.01).Conclusion MI/RI is strongly correlated with the TNF signaling pathway and the NF-κB signaling pathway, both of which play pivotal roles in regulating necrotic apop-tosis.Il1b,TNF,Birc3,and Ripk1emerge as key genes that concurrently regulate both MI/RI and necrotic apoptosis. It is plausible that IL-1b,TNF,Birc3,and Ripk1 may serve as critical regulatory factors in the context of necrotic apoptosis during MI/RI.

Chinese Journal of Practical Nursing 2024;40(6):434-440
Objective:To explore the current situation of binary coping in patients with perimenopausal syndrome and analyze its influencing factors, in order to provide a basis for improving the level of binary coping. Methods:Using convenience sampling method, a total of 210 patients with perimenopausal syndrome and their spouses from the First Affiliated Hospital of Heilongjiang University of Traditional Chinese Medicine were cross-sectional surveyed by a general data questionnaire, the Binary Coping Scale, and the Modified Kupperman Score Scale. The influencing factors of binary coping level in patients with perimenopausal syndrome were explored by univariate analysis and variance decomposition model analysis. Results:A total of 200 valid questionnaires were retrieved. The patients aged (50.52 ± 2.89) years old. The binary coping score was (79.64 ± 22.74) points. The variance decomposition model analysis showed that marriage age, type of medical insurance, number of children, education level, family monthly income, spouse′s education level, presence of major comorbidities in spouse, modified Kupperman score, presence of generalized anxiety in spouse were the main influencing factors of binary coping in patients with perimenopausal syndrome (all P<0.05). Conclusions:The binary coping scores of patients with perimenopausal syndrome are lower than normal, and considering the influence and involvement of patients' spouses, nursing staff should pay special attention to patients who are married relatively early, have more children, have lower education levels, and have lower family monthly incomes. Additionally, attention should be given to spouses who experience widespread anxiety, have a lower level of education, and suffer from major chronic diseases. By developing and implementing comprehensive intervention measures aimed at improving the Kupperman score and the level of binary coping, both parties can be encouraged to support each other more effectively, thereby improving the marital relationships of patients during the perimenopausal period.
