Analysis of assessment value of CSVD total burden score and patients'cognitive impairment under multimodal MRI

Abstract:
Objective:To analyze assessment value of multimodal magnetic resonance imaging(MRI)for clinical correlation between imaging manifestations of cerebral small vessel disease(CSVD)and patients'cognitive impairment.Methods:A total of 106 CSVD patients who underwent diagnosis and treatment in Zhangjiagang Hospital of Traditional Chinese Medicine from April 2020 to April 2022 were collected as research subjects.All patients underwent cranial MRI examination and Mini-Mental State Examination(MMSE)score.The patients'CSVD total burden score(a comprehensive score based on brain MRI,which includes different imaging markers of CSVD and has great value in reflecting the overall degree of brain damage,and the higher score represents higher overall burden of CSVD in patients)and volume of brain gray matter were analyzed.Pearson correlation analysis was used to assess the correlation between CSVD total burden score and MMSE score,general clinical characteristics,and volume of brain gray matter.Results:The results of Pearson correlation analysis showed that the CSVD total burden score was positively correlated with age and systolic blood pressure of patients(r=0.514,0.432,P<0.01),respectively,but there were no significant correlations between that and the gender,diastolic blood pressure,blood sugar,blood lipid-related indicators of patients(P>0.05).The CSVD total burden score was significantly positively correlated with the MMES score of patients(r=0.672,P<0.001).The CSVD total burden score was significantly negatively correlated with the volume of gray matter both bilateral insula and cerebellar vermis(r=-0.462,-0.571,P<0.01),respectively.The results of Pearson analysis about the correlations between brain gray matter volume and general clinical characteristics,and between brain gray matter volume and cognitive impairment of patients showed that the course of CSVD patients was significantly negatively correlated with the volumes of gray matter both bilateral insula and cerebellar vermis(r=-0.381,-0.334,P<0.05),respectively.The volumes of gray matter of bilateral insula,the left side of cerebellar thalamic cortex,and the cerebellar vermis were significantly positively correlated with the MMES score(r=0.447,0.392,0.416,P<0.05),respectively.Conclusion:The CSVD total burden score under multimodal MRI is closely with the cognitive impairment,brain gray matter volume,age,and systolic blood pressure of patients,which can be used as an effective means to assess the condition and cognitive impairment of CSVD patients,further to providing a theoretical basis for the clinical diagnosis and treatment of CSVD patients.
