Abstract:
As a new type of transitional care mode involving doctors, nurses and patients, discharge plan is of great significance to the optimization of medical resources and the improvement of medical quality. It is widely used in adult chronic disease management. At present, domestic research on discharge plan of premature infants is still in the preliminary exploration stage. This paper expounds the connotation of discharge plan and the research status of discharge plan for premature infants at home and abroad in recent years, and analyzes the factors affecting the implementation of discharge plan for premature infants in China, aiming at promoting the promotion and development of discharge plan for premature infants.
