Abstract:
The number of patients with chronic diseases is increasing year by year,bring a series of chronic problems and challenges to patients and society. Western countries have formed a mature model and system for chronic disease. This paper mainly introduce chronic care models in western country,so as to help nurses and patients in our country to know the management of patients with chronic disease,and then to improve the outcome of patients with chronic disease in our country.
