Clinical characteristics and MRI diagnosis of fibrothecoma and ovarian fibroma

Abstract:
Objective To analyze the clinical features and MRI manifestations of fibrothecoma and ovarian fibroma,and to explore the value of MRI in diagnosis and differential diagnosis of these two tumors.Methods The MRI manifestations of 26 fibrothecomas and 9 ovarian fibromas confirmed by surgery and pathology were analyzed retrospectively.Results The mean age of patients with fibrothecomas was(44.8±13.0)years old,and the mean age of patients with ovarian fibromas was(48.3±17.7)years old.There were no statisti-cal differences in the number and mean age of menopausal group and premenopausal group.There were 25 cases(96.2％)of unilat-eral lesions and 1 case(3.8％)of bilateral lesions in 26 cases of fibrothecomas.All ovarian fibromas were unilateral lesions(100％).The average length of all fibrothecomas was(7.69±4.12)cm,and the lesions were mainly solid(88.9％).The average length of all ovarian fibromas was(5.24±2.30)cm,and the lesions were all solid(100％).On T1WI,the solid components of fibrothecomas were mainly isointensity(100％)and were mainly iso-and hypointensity on T2WI(63.0％).The solid components of ovarian fibromas were mainly isointensity on T1WI(77.8％)and were mainly iso-and hypointensity on T2WI(77.8％).On T2WI,the"pomegranate seed-like","rosette-like",and"twisted wire-like"were found in 40.7％of fibrothecomas and 55.6％of ovarian fibromas.The appar-ent diffusion coefficient(ADC)values of fibrothecomas and ovarian fibromas were about(1.55±0.32)× 10-3mm2/s vs(1.15±0.18)× 10 3 mm2/s,which showed statistical difference.Conclusion Fibrothecomas and ovarian fibromas are mainly solid tumors.The"pomegranate seed-like","rosette-like",and"twisted wire-like"on T2WI are characteristic manifestations and ADC value is helpful to distinguish the two tumors.
