1.Dosimetric study of volumetric modulated arc therapy for pediatric Wilms'tumor under flattening filter free and flattening filter modes

Objective To compare the dosimetric differences in volumetric modulated arc therapy(VMAT)for pediatric Wilms'tumor under flattening filter(FF)and flattening filter free(FFF)modes.Methods Ten children with Wilms'tumor who received radiotherapy at some hospital from August 2022 to October 2023 were selected retrospectively,for whom the VMAT plans were designed under FF and FFF modes.The two kinds of plans were compared in terms of planning target volume(PTV)dosimetric parameters,exposure dose to organ at risk(OAR),monitor units,gamma pass rate and treatment time.Results The FFF plan behaved significantly better than the FF plan in D2,D98 and HI(P<0.05);the two kinds of plans had no statistically obvious differences in D50 and CI(P>0.05).The FFF plan gained advantages over the FF plan in Dmean,V5 and V10 of the contralateral kidney,Dmean,V5 and V10 of the liver and Dmax of the spinal cord,and the differences were statistically significant(P<0.05).The FF plan outperformed the FFF plan in gamma pass rate and monitor units,while the FFF plan had the treatment time shorter than that of the FF plan,and the differences were statistically significant(P<0.05).The two kinds of VMAT plans both had the gamma pass rates higher than 95%.Conclusion The VMAT plans under both FF and FFF modes meet clinical treatment requirements,and the FFF mode has advantages over the FF mode in terms of PTV dose distribution,exposure dose to OAR and treatment time,which contribute to enhancing the treatment outcome and cooperation of the children.[Chinese Medical Equipment Journal,2024,45(4):70-73]
2.Feasibility of ultra-low-dose CT combined with advanced modeling iterative reconstruction algorithm for tuberculosis screening in adults

Objective To explore the feasibility of the ultra-low-dose CT energy spectrum purification technique combined with the advanced modeling iterative reconstruction(ADMIRE)algorithm for adult tuberculosis screening.Methods Totally 120 patients with secondary tuberculosis(typeⅢ)admitted to some hospital from May to December 2022 were selected and divided into groups A,B,C,and D with a random number table,with 30 patients in each group.Group A underwent conventional dose scanning with a tube voltage of 120 kV,a tube current of 110 mAs and was treated with the filtered back projection reconstruction algorithm;group B went through low-dose scanning with a tube voltage of 120 kV and a tube current of 70 mAs,and was treated with ADMIRE algorithm(level 3);group C was exposed to low-dose scanning with a low tube voltage of 100 kV and a tube current of 110 mAs,and was treated with the ADMIRE algorithm(level 3);group D was subjected to ultra-low-dose scanning with a tube voltage of Snl00 kV and a tube current of 110 mAs,and treated with the ADMIRE algorithm(level 3).The four groups were compared in terms of effective dose(ED),image subjective score and objective evaluation index and etc.SPSS 17.0 software was used for data processing.Results The differences between the four groups were statistically significant in ED value and subjective score,signal-to-noise ratio(SNR)and contrast noise ratio(CNR)of mediastinal window images(P<0.05),while not in subjective score,SNR and CNR of lung window images(P>0.05).Conclusion The ultra-low-dose CT energy spectrum purification technique combined with the ADMIRE algorithm reduces the radiation dose in the examination of adult tuberculosis patients while the image quality meets the clinical diagnostic requirements.[Chinese Medical Equipment Journal,2024,45(4):74-77]
3.Design of new type of acupressure wrist-ankle strap and its value for treating mild anxiety insomnia

Objective To design a new type of acupressure wrist-ankle strap and evaluate its clinical effect on the treatment of patients with mild anxiety insomnia.Methods The acupressure wrist-ankle strap was composed of a fixation band and an acupressure part.Totally 94 insomnia patients with mild anxiety at some hospital from October 2020 to January 2023 were selected as the subjects and divided into an observation group(n=48)and a control group(n=46)with the random number table method.The observation group was treated with the acupressure wrist-ankle strap,and the control group was given with placebos.The two groups were compared before and after treatment in terms of Pittsburgh sleep quality index(PSQI)and Hamilton anxiety scale(HAMA)with two-way repeated measures ANOVA.SPSS 21.0 software was used for data analysis.Results The PSQI and HAMA scores of the two groups were enhanced significantly after treatment(P<0.05).The two groups had statistically significant difference in the effect of the time factor on PSQI and HAMA scores(P<0.05),and also in the effect of the time x group interaction on PSQI and HAMA scores(P<0.05).Condusion The acupressure wrist-ankle strap contributes to improving the sleep and anxiety of mild anxiety insomnia patients in a self-administered,non-invasive,painless,and non-adverse manner.[Chinese Medical Equipment Journal,2024,45(4):78-82]
4.Disposition of medical support equipment support unit for medical service support of synthetic battalion

Objective To analyze the disposition of medical support equipment support units for medical service support of the synthetic battalion to carry out efficiently demand matching and replenishment for war loss,so as to enhance the efficiency of deployment and decision making of medical support equipment.Methods War loss of the medical support equipment was analyzed with queuing theory with the support service of the synthetic battalion as the basic research framework and desired equipment intact rate in the battlefield as the goal.A M/M/1/∞/∞ model and a M/M/c/∞/∞ model were established to investigate the disposition process of medical support equipment support units by simulating actual conditions of the battlefield.Multi synthetic battalions were assumed to be deployed to carry out 2 missions with different tasks and combat intensities,of which 40 synthetic battalions were involved in for mission 1 and 35 synthetic battalions for mission 2.The disposition of medical support equipment support units was calculated with the two models constructed.Results With the M/M/c/∞/∞ model it's suggested 5 medical support equipment support units be deployed to serve for 40 synthetic battalions;with the M/M/1/∞/∞ model only one unit was employed to support 35 battalions.The two models both met the requirements of 90%intact rate.Conclusion The research contributes to the decision making of medical support equipment support forces during synthetic battalion medical support and enhances the precision support of medical support equipment.[Chinese Medical Equipment Journal,2024,45(4):83-87]

9.Design of patient lifting aid

Objective To design a lifting aid to enhance the loading and unloading efficiency of casualties in emergency rescue.Methods The lifting aid was composed of a lifting mechanism,a motorized control unit and a manual control unit,which used a pulley set to enhance working efficiency,achieved lateral patient translation via a separable and movable sleeping pad and realized electric and manual control by a motor and lever respectively.An equivalent scaled-down model was made to test the forces at the lever under 0,5,10,20 and 30 kg bearing weights to verify the performance of the aid.Results The trend of the tensile force exerted on the lever under five sets of different loads was in accordance with the results of the theoretical analysis.Conclusion The lifting aid with easy operation ensures patient safety and enhances the loading and unloading efficiency while saving the workload.[Chinese Medical Equipment Journal,2024,45(4):111-115]
