1.Point-of-Care Ultrasound-Based Assessment of Sarcopenia to Predict Revisits or Mortality in Older Adults at the Emergency Department: A Prospective Observational Study

Background:
Older patients visiting the emergency department (ED) are at increased risk of adverse outcomes, including ED revisits and mortality. Sarcopenia quantification by point-of-care ultrasound (POCUS) may be a useful bedside screening tool, especially when traditional frailty screening instruments, reliant on history taking, cannot be used. This study evaluated whether POCUS measurement of rectus femoris cross-sectional area (RFcsa) can predict adverse outcomes in older patients visiting the ED.
Methods:
In this single-centre prospective study, patients aged ≥70 years presenting to the ED of a Dutch university hospital and enrolled in the Acutelines data and biobank were included. RFcsa was measured using POCUS. ROC-analysis assessed the overall accuracy of RFcsa for prediction of the primary outcome, which was defined as the composite of ED revisit or death within 3 months. Logistic regression determined the added value of RFcsa to Karnofsky Performance Score (KPS).
Results:
During the study period, a total of 68 patients were included. Twenty-six patients (38%) met the primary endpoint. RFcsa showed excellent intra-rater reliability (interclass correlation coefficient=0.98). However, the accuracy to predict the composite endpoint was low, with an area under the curves of 0.53 (0.39–0.66) for unadjusted RFcsa and 0.51 (0.36–0.66) for sex-adjusted RFcsa. The addition of RFcsa in a multivariate logistic regression model with KPS did not increase the overall explained variance in the primary endpoint.
Conclusion
In older patients presenting in the ED, POCUS-measured RFcsa does not predict ED revisits or death within 3 months. (Trial Registration No. NCT05369962 at ClinicalTrials.gov)

Background:
and Purpose Whether perfusion-diffusion mismatch modifies treatment effect of mechanical thrombectomy (MT) for large strokes is poorly known. To address this question, we conducted a post hoc secondary analysis of the Large Stroke Therapy Evaluation (LASTE) randomized controlled trial (RCT).
Methods:
The LASTE RCT compared MT plus best medical treatment (BMT) to BMT alone in patients with large infarct (Alberta Stroke Program Early CT Score 0–5) in the 0–7-hour timewindow. This secondary analysis was restricted to patients with available baseline MR perfusionweighted imaging. We investigated the potential heterogeneity of MT treatment according to the presence of perfusion-diffusion mismatch, defined as a mismatch ratio ≥1.2, calculated as the time-to-maximum >6 seconds cerebral volume divided by ischemic core volume. The primary outcome was better functional outcome (favorable shift in the distribution of modified Rankin Scale [mRS] at 90 days, analyzed using generalized odds ratio [GenOR]).
Results:
A total of 102/324 patients were included, among whom 55 (54%) had a perfusiondiffusion mismatch. No significant treatment effect heterogeneity by diffusion-perfusion mismatch was observed for the primary outcome (GenOR for better functional outcome: 1.70 [95% confidence interval, CI, 0.95 to 3.05] and 1.04 [95% CI, 0.57 to 1.87] in patients with and without mismatch, respectively; ratio of GenORs: 1.63 [95% CI, 0.71 to 3.74]; P for heterogeneity=0.24) or for the secondary efficacy and safety outcomes.
Conclusions
Our study found no evidence of heterogeneity of treatment effect with respect to benefit or safety in patients with unrestricted infarct size at baseline treated with MT by presence of perfusion-diffusion mismatch. An individual participant-data meta-analysis of RCT is needed for definitive conclusions.

Background:
and Purpose In patients with ischemic stroke and isolated cervical internal carotid artery occlusion (c-ICA-O), endovascular therapy (EVT) can improve cerebral perfusion. To maintain vessel patency, EVT is frequently combined with carotid artery stenting (CAS). We assessed the efficacy and safety of emergent CAS during EVT for isolated c-ICA-O.
Methods:
This retrospective multinational cohort study (42 centers) included consecutive patients who underwent EVT for isolated c-ICA-O within 24 hours from the time last seen well. Patients who underwent emergent CAS were compared with those who did not. Co-primary outcomes were c-ICA vessel patency and symptomatic intracerebral hemorrhage (sICH) 24 hours post-EVT. Secondary outcomes included any intracerebral hemorrhage (ICH) at 24 hours and disability at 3 months (modified Rankin Scale [mRS] shift). Outcomes were adjusted using inverse probability of treatment weighting.
Results:
Of 317 patients (mean age, 68.6 years [standard deviation, 12.9]; median National Institutes of Health Stroke Scale 11 [interquartile range, 6–17]; 26.8% female), 219 (69.1%) underwent CAS, whereas 98 (30.9%) did not. At 24 hours, vessel patency was more common after CAS (83.5% vs. 40.7%; adjusted odds ratio [aOR], 9.45; 95% confidence interval [CI], 4.91–18.17); sICH rates did not differ (2.3% vs. 3.1%; aOR, 0.92; 95% CI, 0.18–4.73). Any ICH was more common after CAS (19.3% vs. 9.3%; aOR, 2.50; 95% CI, 1.12–5.60). CAS was not associated with mRS at 3 months (adjusted common odds ratio, 0.98; 95% CI, 0.62–1.56).
Conclusions
In patients undergoing EVT for isolated c-ICA-O, emergent CAS was technically effective and reasonably safe. More frequent vessel patency in patients who underwent CAS did not translate into improved functional outcome at 3 months.

This study presents an overview of our technique and the perioperative outcomes for the first 100 patients who underwent minimally invasive endoscopic-assisted off-pump multivessel bypass grafting (endoscopic coronary artery bypass [endo-CAB]) at the Catharina Hospital in Eindhoven.
Methods:
The first 100 patients undergoing multivessel endo-CAB from May 2022 to March 2024 were included in this retrospective, single-center, observational study (N=100).The study encompassed both elective and urgent surgical revascularization. In all cases, endoscopic-assisted harvesting of the internal mammary artery, radial artery, or saphenous vein was performed, followed by beating-heart anastomoses through a mini-thoracotomy.
Results:
A total of 226 distal anastomoses were performed, utilizing 102 left internal mammary arteries, 80 radial arteries, 30 right internal mammary arteries, and 14 saphenous veins. On average, each patient had 2.3 anastomoses. A Y graft configuration was employed in 78 patients, in-situ bilateral internal mammary artery inflow in 19 patients, and a proximal aortic graft in 3 patients. Four patients underwent concurrent arrhythmia surgery.Eleven patients received hybrid revascularization. There was 1 conversion to sternotomy (1%) and 3 instances where cardiopulmonary bypass was required (3%). The median operation time was 3.3 hours (interquartile range, 3.0–3.7 hours), and the median hospital stay was 4.0 days (interquartile range, 3–4 days). The in-hospital mortality rate was 1%.
Conclusion
Multivessel off-pump endo-CAB surgery can be safely performed with endoscopic-assisted conduit harvesting. Combining the benefits of a minimally invasive and anaortic approach may improve perioperative outcomes for patients requiring surgical revascularization. Further studies are necessary to establish the role of this technique in routine coronary surgery.

Bats are flying mammals with unique immune systems that allow them to hold many pathogens.
Hence, they are recognised as the reservoir of many zoonotic pathogens. In this study, we performed
molecular detection to detect coronaviruses, paramyxoviruses, pteropine orthoreoviruses and dengue
viruses from samples collected from insectivorous bats in Krau Reserve Forest. One faecal sample from
Rhinolophus spp. was detected positive for coronavirus. Based on BLASTN, phylogenetic analysis and
pairwise alignment-based sequence identity calculation, the detected bat coronavirus is most likely to
be a bat betacoronavirus lineage slightly different from coronavirus from China, Philippines, Thailand
and Luxembourg. In summary, continuous surveillance of bat virome should be encouraged, as Krau
Reserve Forest reported a wide spectrum of biodiversity of insectivorous and fruit bats. Moreover,
the usage of primers for the broad detection of viruses should be reconsidered because geographical
variations might possibly affect the sensitivity of primers in a molecular approach.
10.Feline mammary carcinoma‑derived extracellular vesicle promotes liver metastasis via sphingosine kinase‑1‑mediated premetastatic niche formation

Background:
Feline mammary carcinoma (FMC) is one of the most prevalent malignancies of female cats. FMC is highly metastatic and thus leads to poor disease outcomes. Among all metastases, liver metastasis occurs in about 25% of FMC patients. However, the mechanism underlying hepatic metastasis of FMC remains largely uncharacterized.
Results:
Herein, we demonstrate that FMC-derived extracellular vesicles (FMC-EVs) promotes the liver metastasis of FMC by activating hepatic stellate cells (HSCs) to prime a hepatic premetastatic niche (PMN). Moreover, we provide evidence that sphingosine kinase 1 (SK1) delivered by FMC-EV was pivotal for the activation of HSC and the formation of hepatic PMN. Depletion of SK1 impaired cargo sorting in FMC-EV and the EV-potentiated HSC activation, and abol‑ ished hepatic colonization of FMC cells.
Conclusions
Taken together, our findings uncover a previously uncharacterized mechanism underlying liver-metas‑ tasis of FMC and provide new insights into prognosis and treatment of this feline malignancy.
