Background:
s/Aims: Sarcomatoid hepatocellular carcinoma (HCC) is a rare histological subtype of HCC characterized by extremely poor prognosis; however, its molecular characterization has not been elucidated.
Methods:
In this study, we conducted an integrated multiomics study of whole-exome sequencing, RNA-seq, spatial transcriptome, and immunohistochemical analyses of 28 paired sarcomatoid tumor components and conventional HCC components from 10 patients with sarcomatoid HCC, in order to identify frequently altered genes, infer the tumor subclonal architectures, track the genomic evolution, and delineate the transcriptional characteristics of sarcomatoid HCCs.
Results:
Our results showed that the sarcomatoid HCCs had poor prognosis. The sarcomatoid tumor components and the conventional HCC components were derived from common ancestors, mostly accessing similar mutational processes. Clonal phylogenies demonstrated branched tumor evolution during sarcomatoid HCC development and progression. TP53 mutation commonly occurred at tumor initiation, whereas ARID2 mutation often occurred later. Transcriptome analyses revealed the epithelial–mesenchymal transition (EMT) and hypoxic phenotype in sarcomatoid tumor components, which were confirmed by immunohistochemical staining. Moreover, we identified ARID2 mutations in 70% (7/10) of patients with sarcomatoid HCC but only 1–5% of patients with non-sarcomatoid HCC. Biofunctional investigations revealed that inactivating mutation of ARID2 contributes to HCC growth and metastasis and induces EMT in a hypoxic microenvironment.
Conclusions
We offer a comprehensive description of the molecular basis for sarcomatoid HCC, and identify genomic alteration (ARID2 mutation) together with the tumor microenvironment (hypoxic microenvironment), that may contribute to the formation of the sarcomatoid tumor component through EMT, leading to sarcomatoid HCC development and progression.

ObjectiveTo assess the therapeutic effectiveness and safety of the traditional Chinese medicine compound Shenlong decoction in addressing the symptoms of pulmonary deficiency and stasis in patients with idiopathic pulmonary fibrosis (IPF). MethodsSixty eligible patients with lung deficiency and collateral stasis syndrome of IPF were randomly assigned to the observation (30 patients) and control groups (30 patients). All patients underwent standard Western medical therapy. Additionally, the observation group received Shenlong decoction granules, while the control group received a placebo. Both treatments were packaged in four doses of 10.5 g each, taken twice daily for three months. The indexes of the patients during the treatment cycle were observed, and the main indexes include traditional Chinese medicine (TCM) syndrome scores and 6 min walk test (6MWT). The secondary indexes include pulmonary function test [actual value/expected value of total lung volume (TLC%),actual value/expected value of vital capacity(FVC%),actual/predicted diffusing capacity of the lung for carbon monoxide(DLCO%),actual/predicted forced expiratory volume in one second (FEV1%),and FEV1/ forced vital capacity (FVC)],blood gas analysis [arterial blood diathesis partial pressure of oxygen (PaO2),partial pressure of carbon dioxide (PaCO2),and arterial oxygen saturation (SaO2)],serum inflammatory factors [transforming growth factor-β1 (TGF-β1),interleukin-4 (IL-4),interleukin-13 (IL-13),interleukin-12 (IL-12),and gamma-interferon (IFN-γ)],and quality of survival evaluation [St George's Respiratory Questionnaire (SGRQ) score]. The patients' clinical manifestations were determined at the end of the treatment, and the occurrence of adverse events was recorded. ResultsA total of 53 patients completed the study, comprising 27 in the control group and 26 in the observation group. Upon completion of the treatment period, the control group achieved a total effective rate of 33.33% (9/27),whereas the observation group demonstrated a total effective rate of 53.85% (14/26),which was statistically superior to the control group (χ2=4.034,P<0.05). After the treatment, the TCM syndrome scores,6MWT,DLCO%,FEV1%,PaO2,PaCO2,TGF-β1,IL-4,IL-13,IL-12,and IFN-γ in the two groups were all significantly improved (P<0.01). Compared with those in the control group after treatment at the same period, the TCM syndrome scores,6MWT,PaO2,and PaCO2 were significantly improved in the observation group after 60 days and 90 days of medication (P<0.01). Three months after the end of medication, the SGRQ score in the observation group showed significant improvement when compared to that in the control group (P<0.05),and no severe adverse events were reported during the follow-up period. ConclusionCompound Shenlong decoction can alleviate clinical symptoms such as shortness of breath and wheezing in patients with lung deficiency and collateral stasis syndrome of IPF,enhance exercise tolerance, improve the quality of life, and have certain potential advantages in improving pulmonary function.

Objective:To observe the efficacy and safety of combined lienal polypeptide injection therapy in the treatment of Mycoplasma pneumoniae pneumonia(MPP)in children aged 3 to 14 years old in multiple clinical centers. Methods:A randomized,controlled,multi-center clinical study design was adopted.A total of 240 hospitalized children aged 3 to 14 years old with MPP from 7 hospitals from September 1,2023 to January 31,2024 were included.According to the severity of pneumonia, they were divided into the mild MPP group with 80 cases and the severe MPP/refractory MPP(SMPP/RMPP)group with 160 cases, and then randomly divided into the control group and the experimental group at a ratio of 1 ∶1,using the random number table method. After screening, subjects entered a treatment period of 5 to 7 days. The control group was treated with azithromycin, while the experimental group was treated with azithromycin plus lienal polypeptide injection .The recovery of lung CT,length of hospital stay, duration of fever, cough score, whether mild cases developed into severe or refractory cases, duration of hormone use,use of intravenous immunoglobulin(IVIG),bronchoscopy treatment, and immune function were observed between the two groups to evaluate the efficacy of lienal polypeptide injection.Adverse events after medication, vital signs, blood routine, urine routine, liver function, myocardial enzymes, renal function, and electrocardiogram were observed to evaluate the safety. Results:A total of 231 subjects have completed the trial in the 7 hospitals, including 118 cases in the experimental group and 113 cases in the control group.Main observation index:the rate of lung CT aggravation in the experimental group was lower than that in the control group(2.6% vs 15.3%, P<0.01),and the difference was statistically significant.Secondary indexes:there were no statistically significant differences in the length of hospital stay, duration of fever, cough score, duration of hormone use, whether IVIG treatment was used, the number of bronchoscopy treatment cases, and immunoglobulin between the two groups(all P>0.05).However,the rate of cases of plastic bronchitis(PB)found under bronchoscopy in the experimental group was lower than that in the control group(0 vs 18.8%, P=0.03),and the difference was statistically significant.Among the mild MPP(72 cases),there were no statistically significant differences in the length of hospital stay, duration of fever, cough score, duration of hormone use, whether IVIG treatment was used, the number of bronchoscopy treatment cases, and the improvement rate of lung CT between the two groups(all P>0.05).However,compared with the control group, the rate of cases developing into SMPP/RMPP in the experimental group was less(24.3% vs 48.6%, P=0.03),and the difference in IgG before and after treatment was small[0.53(-0.04,1.18)g/L vs 1.33(0.48,2.25)g/L, P=0.01].Among the SMPP/RMPP cases(159 cases),the rate of cases of PB found under bronchoscopy in the experimental group was less than that in the control group(0 vs 20%, P=0.04),and the rate of cases with aggravated lung CT in the experimental group was less than that in the control group(1.3% vs 19.5%, P<0.01),and the improvement rate of lung CT in the experimental group was higher than that in the control group(88.8% vs 75.3%, P=0.03),with statistically significant differences. There were no statistically significant differences in the length of hospital stay, duration of fever, cough score, duration of hormone use, whether IVIG treatment was used, the number of bronchoscopy treatment cases, and immunoglobulin between the two groups(all P>0.05).Two cases in the experimental group developed rashes, which improved after the drug was discontinued. There were no serious adverse reactions such as abnormal vital signs like dyspnea and cyanosis due to the use of lienal polypeptide injection. There were no obvious changes in blood routine, liver function, myocardial enzymes, renal function, electrocardiogram, and urine routine values before and after medication compared with the baseline. Conclusion:The combined use of lienal polypeptide injection in the treatment of MPP in children can reduce the probability of the transformation from mild cases to SMPP/RMPP,reduce the rate of aggravation of the image findings, promote the absorption of lung inflammation, reduce the rate of PB found under bronchoscopy, and has good safety.

Journal of Clinical Medicine in Practice 2025;29(8):28-34
Objective To construct machine learning(ML)model for predicting early neurologi-cal deterioration(END)after intravenous thrombolysis(IVT)in patients with acute ischemic stroke(AIS),and to analyze risk factors of END using Shapley additive explanations(SHAP).Methods A total of 97 AIS patients who received IVT were enrolled.Patients were divided into END group(18 cases)and non-END group(79 cases)based on whether they experienced END within 24 hours post-IVT.All patients were randomly divided into training set(n=68)and validation set(n=29)at ra-tio of 7 to 3.Univariate and least absolute shrinkage and selection operator(LASSO)analyses were performed to screen important feature variables associated with END from clinical data.Six ML algo-rithms,including random forest, light gradient boosting machine, decision tree, support vector ma-chine,k-nearest neighbors and extreme gradient boosting, were employed to construct predictive mod-els.Receiver operating characteristic(ROC)curves, calibration curves and clinical decision curve analysis(DC A)were used to evaluate the performance of each ML model. The SHAP method was introduced to interpret the optimal ML model. Results Among the six ML algorithm models, the random forest model was identified as best predictive model. In the training set,it achieved area un-der the curve(AUC)of 0.909,with specificity, precision, recall and F1 score being 0.873,0.856,0.910 and 0.825,respectively.In the validation set,its AUC was 0.915,with correspond-ing values of 0.824,0.800,0.945 and 0.834.Calibration curves and DC A demonstrated that the random forest model had higher prediction accuracy and clinical net benefit.SHAP variable impor-tance plots revealed that the top six contributing imaging factors to END were large-area cerebral in-farction,pre-thrombolysis National Institutes of Health Stroke Scale(NIHSS)score,door-to-needle time(DNT),history of atrial fibrillation, white blood cell(WBC)levels and history of diabetes. Conclusion ML models can effectively predict the risk of END in IVT patients, with the random forest model demonstrating the best predictive performance.Combining SHAP for model visualization interpretation aids clinicians in understanding the contribution of each feature variable to the predic-tion results, thereby facilitating targeted preventive treatment strategies.
