1.Ancient and Modern Application and Key Information Analysis of Classic Formula Erchentang

Erchentang is a classic formula widely used by medical practitioners throughout history. In this paper,ancient and modern literature of Erchentang were collected, and bibliometrics was employed to analyze its historic evolution,prescription meaning,herbs origin, processing method,preparation methods, and clinical application. A total of 84 pieces of data were collected, and 58 pieces of data involving 53 ancient medical Chinese books were screened, sorted, and processed. Combined with research of modern scholars,the research has found that the Erchentang originated from the Taiping Huimin Huiye Shijie Fang compiled by the Imperial Medical Bureau of the Song Dynasty. The basic information about the origin of the drugs is quite clear. Pinelliae rhizoma in the formula is the dried tuber of Pinellia ternata. Citri exocarpium rubrum is the dried mature peel of Citrus reticulata and its cultivated varieties, with the inner white membrane removed. Poria is the whitest dry sclerotia of Poria cocos; Glycyrrhizae radix et rhizoma is the dried root and rhizome of the Glycyrrhiza uralensis. The dosage is 5.70 g Pinelliae rhizome and Citri exocarpium rubrum, 3.43 g Poria, and 1.69 g Glycyrrhizae radix et rhizoma praeparata cum melle. During the decoction process, the above-mentioned herbs should be chopped, with 300 mL water, 7 g ginger in thick slices, and 2 g Mume fructus added, and it was then simmered together to 180 mL. After removing the medicinal residue, it can be taken warmly. Erchentang has the effect of drying dampness and resolving phlegm, regulating Qi and harmonizing the middle. It can be used in treating the syndrome of phlegm and dampness,as well as symptoms such as frequent cough,white phlegm,fullness in chest and diaphragm,nausea and vomiting,limb drowsiness,anorexia,dizziness,palpitations,white and greasy tongue coating, and slippery pulse. The above results provide reference for future research and development of Erchentang.
2.Randomized Controlled Clinical Observation on the Treatment of Lumbar Disc Herniation of Cold-Dampness Obstruction Type with Hot Ironing of Haitongpi Formula (海桐皮方) Combined with Three Movements Technique of Qinggong Spinal Manipulation

ObjectiveTo observe the clinical efficacy and safety of hot ironing with Haitongpi Formula (海桐皮方, HF) in the treatment of lumbar disc herniation (LDH) of cold-dampness obstruction type. MethodsA total of 70 patients with cold-dampness obstruction type LDH were randomly divided into a treatment group and a control group, with 35 cases in each group. Both groups received three movements technique of Qinggong Spinal Manipulation (QSM) as the basis for treatment. In addition, the treatment group received hot ironing with HF, while the control group applied Diclofenac Sodium Gel externally. The treatment duration for both groups was 14 days. The clinical efficacy was compared between groups. Japanese Orthopedic Association (JOA) score, visual analog scale (VAS) for pain, pain pressure threshold (PPT) for lumbar positive response points, and traditional Chinese medicine (TCM) symptom scores were compared, on day 7, and day 14 of treatment, as well as on day 7 and day 14 of follow-up. The lumbar curvature index (LCI) was also compared before treatment and on day 14 of treatment. Adverse reactions during the study were recorded for both groups. ResultsA total of 35 patients in the treatment group and 34 patients in the control group were included for final analysis. The clinical total effective rate of the treatment group (91.43%, 32/35) was significantly higher than that of the control group (82.35%, 28/34, P<0.05). Both the JOA score and PPT of the two groups increased on day 7 and day 14 of treatment, and on day 7 and day 14 of follow-up. VAS scores and TCM symptom scores both decreased. The LCI of both groups increased on day 14 of treatment (P<0.01). Compared with the control group at the same time points, on day 14 of treatment and day 7 and day 14 of follow-up, the treatment group had higher JOA scores and PPT, and lower VAS scores and TCM symptom scores. The LCI of the treatment group increased on day 14 of treatment (P<0.05 or P<0.01). One case in the control group showed mild skin allergy, with no other adverse reactions observed in either group. ConclusionBased on three movements technique of QSM, hot ironing with HF shows better clinical efficacy than external Diclofenac Sodium Gel in the treatment of cold-dampness obstruction type LDH. It can significantly reduce lumbar pain, increase pain pressure threshold, improve clinical symptoms, lumbar function, and lumbar curvature, with good safety.
3.Association of CRP and interleukin-6 levels with cardiac function in patients with myocardial infarc-tion

Objective:To investigate the levels of C reactive protein(CRP)and interleukin(IL)-6 in patients with myocardial infarction(MI)and their association with cardiac function.Methods:We enrolled 140 MI patients ad-mitted to Sanya Harbin Medical University Hongsen Hospital Co.,Ltd between January 2020 and December 2022,as MI group.According to coronary stenotic severity,they were divided into mild group(n=62),moderate group(n=41)and severe group(n=37),and 75 healthy individuals who underwent physical examination simultaneously were selected as control group.Serum CRP,IL-6,cardiac troponin Ⅰ(cTnⅠ)and creatine kinase isoenzyme MB(CK-MB)levels were measured by automatic biochemical analyzer,and left ventricular ejection fraction(LVEF)was assessed by echocardiography.Above-mentioned indexes were compared between MI group and control group,and among groups of different coronary disease severity.Pearson/Spearman correlation was used to analyze the asso-ciation of serum CRP and IL-6 levels with Gensini score,cTnⅠ,CK-MB levels,and LVEF in MI patients.Diag-nostic value of serum CRP and IL-6 levels for MI was analyzed by receiver operating characteristic(ROC)curve.Results:Compared with participants in control group,those in MI group had significant higher serum CRP,IL-6,cTnⅠ and CK-MB,and significant lower LVEF(P<0.001 all).Serum CRP,IL-6,cTnⅠ and CK-MB levels sig-nificantly increased,and LVEF significant decreased in the order of mild group,moderate group and severe group(P<0.01 all).Pearson/Spearman correlation analysis showed that serum CRP and IL-6 levels were positively cor-related with Gensini score,cTnⅠ,and CK-MB levels(r=0.496~0.646,P<0.001all),and negatively correlated with LVEF(r=-0.530,-0.572,P<0.001 all)in MI patients.ROC curve showed that the area under the curve(AUC)of CRP combined IL-6 diagnosing MI was 0.856(95%CI 0.802~0.900),which was significantly higher than those of CRP(AUC=0.770,95%CI 0.708~0.824)and IL-6(AUC=0.793,95%CI 0.733~0.845)alone(Z=3.295,2.877,P<0.01 all).Conclusion:Serum CRP and IL-6 levels rise in MI patients,and they are closely associated with increased severity of coronary artery disease and reduced cardiac function.

Hypothermia is a clinical syndrome characterized by core body temperature<35℃,caused by significant heat loss from body surface in cold environment.As a systemic cold injury,it can be lethal if treatment is delayed.Emergent diagnosis and treatment of hypothermia are expected to improve the prognosis of patients.In 2005,the U.S.Army Research Institute of Environmental Medicine(USARIEM)issued guidelines for the prevention and management of cold injuries,but there has been no corresponding standard in China.Therefore,Emergency Branch of Chinese Medical Rescue Association,Emergency Medical Equipment Society of China Association of Medical Equipment,Integrated Rehabilitation Medical Branch of Chinese Medical Rescue Association,and Pre-Hospital Emergency Care Working Committee of Chinese Aging Well Association jointly developed the Chinese Expert Consensus on Emergent Treatment of Hypothermia(2025 edition).The consensus covers the pathophysiology,etiology and epidemiology,diagnosis and severity grading,prehospital treatment,and in-hospital treatment of hypothermia,including 15 recommendations in total,aiming to provide guidance for the relevant clinical rescue work.
5.Related factors of sense of coherence in patients with cancer receiving proton and heavy ion radiotherapy

Objective:To investigate the current status of sense of coherence in patients with cancer under-going proton and heavy ion radiotherapy and analyze its related factors.Methods:A total of 241 patients with cancer undergoing proton and heavy ion radiotherapy in a certain hospital were selected.The Sense of Coherence Scale(SOC-13),Medical Coping Modes Questionnaire(MCMQ),General Self-Efficacy Scale(GSES),Re-silience Scale Specific to Cancer(RS-SC),and Social Support Rating Scale(SSRS)were used for the survey.Results:The total score of SOC-13 was(67.7±12.3).The results of multiple linear regression showed that age,total scores of GSES and SSRS were positively associated with the SOC-13 total scores(β=0.19,0.27,0.15),while place of residence and yielding scores were negatively associated with the SOC-13 total scores(β=-0.22,-0.30).Conclusion:It suggests that patients who experience adverse reactions to radiotherapy and unemployed/resigned have lower sense of coherence.Sense of coherence in patients with cancer undergoing proton and heavy ion radiotherapy are correlated with their age,place of residence,and self-efficacy,social sup-port and yielding levels.
6.Advantage of advanced diffusion weighted imaging over single index diffusion weighted imaging in distinguishing low-grade transitional zone prostate cancer from benign prostatic hyperplasia with inflammation

Objective To explore the advantage of advanced diffusion weighted imaging(DWI)over single index DWI in distin-guishing low-grade transitional zone prostate cancer(TZ-PCa)from benign prostatic hyperplasia(BPH)with inflammation.Methods Thirty-two patients with low-grade TZ-PCa(low-grade TZ-PCa group)and 38 patients with BPH with inflammation(BPH with inflammation group)confirmed by pathology were selected.All patients underwent routine MR sequence,single index DWI,and advanced DWI[including introvoxel incoherent motion diffusion weighted imaging(IVIM-DWI),stretched-exponential model(SEM),diffu-sion kurtosis imaging(DKI)]sequences scans.The related parameters of the lesion area[including apparent diffusion coefficient(ADC),diffusion coefficient(D),distributed diffusion coefficient(DDC),and mean kurtosis(MK)values]were measured.Independ-ent sample t-tests were used to compare differences in parameters values between the two groups.The receiver operating characteris-tic(ROC)curve was used to calculate the area under the curve(AUC)of varous parameters values in the differential diagnosis of low-grade TZ-PCa and BPH with inflammation.The difference in AUC between advanced DWI and single index DWI was compared using Z-test.Results The ADC,D,and DDC values of the low-grade TZ-PCa group were lower than those of the BPH with inflammation group(P<0.05).The MK value of the low-grade TZ-PCa group was higher than that of the BPH with inflammation group(P<0.05).The D,DDC,and MK values,and a combination of these parameters(D+DDC+MK)exhibited high diagnostic effi-cacy(all AUC>0.9).The diagnostic efficacy of these parameters was significantly higher than that of the ADC value(AUC 0.902,0.904,0.966,0.994 vs 0.725,all P<0.05).The combined param-eter demonstrated the highest AUC.Although the diagnostic efficacy of the combined parameter was not statistically different from the individual D,DDC,and MK values(P>0.05),the diagnostic specificity was significantly improved compared to the individual parame-ters.Conclusion Advanced DWI(IVIM-DWI,SEM,DKI)has a higher diagnostic efficacy in distinguishing low-grade TZ-PCa from BPH with inflammation than single index DWI.Combining the parameters(D+DDC+MK)further improves diagnostic specificity.
7.Characteristics and Risk Analysis of COVID-19 Infection in Patients with Multiple Myeloma after Autologous Hematopoietic Stem Cell Transplantation.

OBJECTIVE:
To retrospectively analyze the characteristics and influencing factors of COVID-19 infection in patients with multiple myeloma (MM) who underwent autologous hematopoietic stem cell transplantation (AHSCT).
METHODS:
The clinical data of MM patients who underwent AHSCT in Ruijin Hospital Affiliated to Shanghai Jiao Tong University School of Medicine from May 26, 2021 to December 26, 2022 were collected. The onset of COVID-19 infection, corresponding symptoms and laboratory tests were followed up in outpatient or by the means of telephone contact and online questionnaires. Related analysis was then performed.
RESULTS:
This study included 96 patients, and 72 cases among them were infected with COVID-19 while 24 cases were uninfected. Logistic regression analysis showed that vaccination did not significantly reduce the risk of COVID-19 infection, but patients who received two doses of the vaccine had a lower risk of developing moderate and severe disease than those who did not receive or received one dose (OR =0.06, P =0.029). Patients who received daratumumab before had a higher risk of COVID-19 infection (OR =5.78, P =0.039), while those with a history of immunomodulatory drugs (IMiDs) had the opposite effect (OR =0.31, P =0.028). The use of both drugs did not affect the severity of COVID-19 infection.
CONCLUSION
For MM patients undergoing AHSCT as first-line chemotherapy, COVID-19 vaccination does not significantly reduce the infection rate, but it plays a role in preventing moderate and severe cases. The application of antineoplastic drugs with different mechanisms has a certain impact on the susceptibility to the COVID-19, which should be considered comprehensively when creating treatment plans.

This study aimed to comprehensively examine the association of gallstones, cholecystectomy, and cancer risk. Multivariable logistic regressions were performed to estimate the observational associations of gallstones and cholecystectomy with cancer risk, using data from a nationwide cohort involving 239 799 participants. General and gender-specific two-sample Mendelian randomization (MR) analysis was further conducted to assess the causalities of the observed associations. Observationally, a history of gallstones without cholecystectomy was associated with a high risk of stomach cancer (adjusted odds ratio (aOR)=2.54, 95% confidence interval (CI) 1.50-4.28), liver and bile duct cancer (aOR=2.46, 95% CI 1.17-5.16), kidney cancer (aOR=2.04, 95% CI 1.05-3.94), and bladder cancer (aOR=2.23, 95% CI 1.01-5.13) in the general population, as well as cervical cancer (aOR=1.69, 95% CI 1.12-2.56) in women. Moreover, cholecystectomy was associated with high odds of stomach cancer (aOR=2.41, 95% CI 1.29-4.49), colorectal cancer (aOR=1.83, 95% CI 1.18-2.85), and cancer of liver and bile duct (aOR=2.58, 95% CI 1.11-6.02). MR analysis only supported the causal effect of gallstones on stomach, liver and bile duct, kidney, and bladder cancer. This study added evidence to the causal effect of gallstones on stomach, liver and bile duct, kidney, and bladder cancer, highlighting the importance of cancer screening in individuals with gallstones.

Acute respiratory distress syndrome (ARDS) is a common respiratory emergency, but current clinical treatment remains at the level of symptomatic support and there is a lack of effective targeted treatment measures. Our previous study confirmed that inhalation of hydrogen gas can reduce the acute lung injury of ARDS, but the application of hydrogen has flammable and explosive safety concerns. Drinking hydrogen-rich liquid or inhaling hydrogen gas has been shown to play an important role in scavenging reactive oxygen species and maintaining mitochondrial quality control balance, thus improving ARDS in patients and animal models. Coral calcium hydrogenation (CCH) is a new solid molecular hydrogen carrier prepared from coral calcium (CC). Whether and how CCH affects acute lung injury in ARDS remains unstudied. In this study, we observed the therapeutic effect of CCH on lipopolysaccharide (LPS) induced acute lung injury in ARDS mice. The survival rate of mice treated with CCH and hydrogen inhalation was found to be comparable, demonstrating a significant improvement compared to the untreated ARDS model group. CCH treatment significantly reduced pulmonary hemorrhage and edema, and improved pulmonary function and local microcirculation in ARDS mice. CCH promoted mitochondrial peripheral division in the early course of ARDS by activating mitochondrial thioredoxin 2 (Trx2), improved lung mitochondrial dysfunction induced by LPS, and reduced oxidative stress damage. The results indicate that CCH is a highly efficient hydrogen-rich agent that can attenuate acute lung injury of ARDS by improving the mitochondrial function through Trx2 activation.

Reporting guidelines are structured checklists for researchers to follow when reporting spe-cific types of studies.As researches conducted in real-world settings to address practical issues,implementa-tion research has stringent requirements for the replicability of result and the transparency of reporting,making its reporting guidelines particularly important.This paper systematically introduces the reporting guidelines in the field of implementation science,outlines their classification systems and scopes of applica-tion,and focuses on explaining the core characteristics and functions of five key reporting guidelines,inclu-ding the Standards for Reporting Implementation Studies(StaRI),Reporting guidelines for implementation and operational research,the Template for Intervention Description and Replication(TIDieR),the Frame-work for Reporting Adaptations and Modifications-Enhanced(FRAME),and recommendations for specifying and reporting implementation strategies.Furthermore,combined with the PEDALs research paradigm in im-plementation science,this paper further clarifies the specific application pathways for reporting guidelines and discusses directions for refinement,aiming to provide references for researchers to select appropriate reporting guidelines.
