1.Value of apparent diffusion coefficient in predicting efficacy of neoadjuvant concurrent chemoradiotherapy for rectal cancer

Objective To discuss the value of the apparent diffusion coefficient(ADC)in the prediction of therapeutic effect of neoadjuvant concurrent chemoradiotherapy(nCRT)in rectal cancer.Methods The clinical data of 52 patients with pathologi-cally diagnosed rectal cancer who underwent neoadjuvant chemoradiotherapy(nCRT)and received pelvic MR1 examinations before and 8 weeks after nCRT at the 900th Hospital of Joint Logistics Support Force from January 2019 to February 2023 were analyzed retrospectively.The ADC values before and after nCRT were obtained,and ΔADC and ΔADC%were calculated.The 52 patients were divided into a sensitive group(n=40)and a non-sensitive group(n=12)based on the tumor regression grade(TRG).The ADC values before nCRT,ΔADC and ΔADC%of the above two groups were compared by Mann-Whitney U test.The ADC values after nCRT of the two groups were compared by independent samples t-test.Spearman's rank correlation coefficient was used to evaluate the correlation of ΔADC and ΔADC%with TRG.The baseline data of the two groups were compared by independent samples t-test,Mann-Whitney U test and chi-square test.Binary Logistic regression was used to analyze the factors predicting efficacy.ROC was drawn to assess the ability of ΔADC in predicting nCRT efficacy,and determine optimal cut-off value.Results There was no statistical difference between the sensitive group and the non-sensitive group with regard to ADCs before and after nCRT(P>0.05).There were statistical differences between the above two groups with regard to ΔADC and ΔADC%(P<0.05).ΔADC and ΔADC%showed negative correlation with TRG(r=-0.378,-0.368,P<0.05).Univariate analysis showed that there were statistically significant differences in tumor diameter between the two groups(P<0.05).Multivariate analysis showed that ΔADC was an independent predictor of the efficacy.ROC curve analysis showed the optimal cut-off value of ΔADC was 236.600×10-6 mm2/s.Conclusion The ADC values before and after nCRT have limited predictive significance for assessing the efficacy of nCRT,whereas ΔADC can predict nCRT efficacy effectively and provide references for subsequent clinical treatment.[Chinese Medical Equipment Journal,2024,45(12):61-66]

To compare the pancreatic proteomics and autophagy between Rehmanniae Radix-and Rehmanniae Radix Praeparata-treated mice with type 2 diabetes mellitus(T2DM). The T2DM mouse model was established by high-fat diet coupled with streptozotocin(STZ, intraperitoneal injection, 100 mg·kg~(-1), once a day for three consecutive days). The mice were then randomly assigned into a control group, low-(5 g·kg~(-1)) and high-dose(15 g·kg~(-1)) Rehmanniae Radix groups, low-(150 mg·kg~(-1)) and high-dose(300 mg·kg~(-1)) catalpol groups, low-(5 g·kg~(-1)) and high-dose(15 g·kg~(-1)) Rehmanniae Radix Praeparata groups, low-(150 mg·kg~(-1)) and high-dose(300 mg·kg~(-1)) 5-hydroxymethyl furfuraldehyde(5-HMF) groups, and a metformin(250 mg·kg~(-1)) group. In addition, a normal group was also set and each group included 8 mice. The pancreas was collected after four weeks of administration and proteomics tools were employed to study the effects of Rehmanniae Radix and Rehmanniae Radix Praeparata on protein expression in the pancreas of T2DM mice. The expression levels of proteins involved in autophagy, inflammation, and oxidative stress response in the pancreatic tissues of T2DM mice were determined by western blotting, immunohistochemical assay, and transmission electron microscopy. The results showed that the differential proteins between the model group and Rehmanniae Radix/Rehmanniae Radix Prae-parata group were enriched in 7 KEGG pathways, such as autophagy-animal, which indicated that the 7 pathways may be associated with T2DM. Compared with the control group, drug administration significantly up-regulated the expression levels of beclin1 and phosphorylated mammalian target of rapamycin(p-mTOR)/mTOR and down-regulated those of the inflammation indicators, Toll-like receptor-4(TLR4) and Nod-like receptor protein 3(NLRP3), in the pancreas of T2DM mice, and Rehmanniae Radix showed better performance. In addition, the expression levels of inducible nitric oxide synthase(iNOS), nuclear factor erythroid 2-related factor 2(Nrf2), and heine oxygenase-1(HO-1) in the pancreas of T2DM mice were down-regulated after drug administration, and Rehmanniae Radix Praeparata demonstrated better performance. The results indicate that both Rehmanniae Radix and Rehmanniae Radix Praeparata can alleviate the inflammatory symptoms, reduce oxidative stress response, and increase the autophagy level in the pancreas of T2DM mice, while they exert the effect on different autophagy pathways.

ObjectiveTo explore the occurrence, in vitro culture effect and clinical outcomes of embryo contamination during in vitro fertilization and embryo transfer (IVF-ET) cycles. MethodsA retrospective analysis was made on the embryo contamination cases during IVF-ET treatment at Reproductive Medicine Centre, Department of Obstetrics and Gynecology, Sun Yat-sen Memorial Hospital, Sun Yat-sen University from January 2010 to December 2020. We compared the contamination of 4 different insemination methods, contamination on 3 different in vitro culture days (D1, D2, D3) and contamination of 3 different samples (follicular fluid, semen and culture medium). The source of contamination and types of microbes were identified. We also compared in vitro culture and clinical outcomes between two groups (42 cases of all embryos contaminated and 28 cases of partial embryos contaminated). ResultsAmong the 29 583 cycles examined, 70 cycles had microbial contamination (0.24%), and all contaminated embryos were from in vitro fertilization (IVF) source rather than intracytoplasmic sperm injection (ICSI) source. The contamination rate on D2 was the highest (54.3%), followed by those on D1 (32.9%) and D3 (12.9%). Compared with follicular fluid, semen is the most common source of contamination from D1-D3. The most common contaminants identified was Enterococcus faecalis in follicular fluid (18.0%), while it was Escherichia coli in semen (59.6%) and culture medium (66.7%). Moreover, the types of bacteria in culture medium were not consistent with those in follicular fluid and semen in 5 cases of microbial contamination. Compared with the Total contaminated group, the Partial contaminated group showed a significant decrease in No available embryo rates (6/28 vs. 34/42, 21.4% vs. 81.0%, P<0.001) and a rising trend in formation of blastocyst rates (5/12 vs. 2/7, 41.2% vs. 28.6%, P=0.656), meanwhile, the clinical pregnancy cases and live births cases after transplantation of fresh and frozen-thawed cycles in Partial contaminated group were higher than those in the Total contaminated group. ConclusionICSI can effectively reduce embryo contamination. Embryo contamination mainly occurred on D2 in vitro culture and the most common contamination source is Escherichia coli in semen. Partial embryo contamination may still result in good in vitro culture effect and clinical outcomes during IVF-ET cycles.

Objective:
To investigate the effectiveness and adverse effects of Cyberknife stereotactic body radiotherapy (SBRT) on liver metastases from PCa.
METHODS:
From June 2009 to September 2016, we treated 20 cases of PCa liver metastases by Cyberknife SBRT, at a total dose of 36 (30-50) Gy, on 1-3 liver metastatic lesions, for 3-5 times, with a prescription isodose line of 70-92%. We assessed the therapeutic effect according to the modified Response Evaluation Criteria in Solid Tumors (mRECIST), calculated the survival and disease-control rates using the Kaplan-Meier method, and analyzed the adverse events based on the National Cancer Institute Common Terminology Criteria for Adverse Events-Version 4.0 (CTCAE 4.0).
RESULTS:
Of all the cases treated, complete response (CR) was found in 8 (40.0%), partial response (PR) in 9 (45.0%), stable disease (SD) in 2 (10.0%), and progressive disease (PD) in 1 (5.0%), with a local control rate (CR+PR) of 85.0% and a disease-control rate (CR+PR+SD) of 95.0%. Among the 14 patients with elevated PSA, 10 (71.4%) showed a significant decrease after treatment. The median follow-up time was 17 months, the 1- and 2-year survival rates were 85.0% and 15.0%, respectively, and the median survival time of the 20 patients was 16.5 months (95% CI: 12.12-22.88). Cyberknife SBRT was well tolerated in all the patients, with only a few mild adverse events (mainly grades 1 and 2 but no 4 and 5) during the whole course of treatment.
CONCLUSIONS
Cyberknife SBRT is safe and effective in the treatment of PCa liver metastases, with a high local control rate, and capable of reducing the PSA level and raising the long-term survival rate of the patients.
8.Design of thermal alarm sensing controller of high-frequency electric knife against risk of burns

Objective To design a thermal alarm sensing controller to avoid burns by the high-frequency electric knife in the operating room. Methods The system development of the controller was executed under Visual DSP, and a thermal alarm sensing control method based on multi-thread temperature feedback regulation was proposed for eliminating the risk of the burns by the high-frequency electric knife in the operating room. The functional and technical indexes of the controller were analyzed to construct its overall framework model, the control law was designed for the temperature feedback control of the controller, and proportion-integral-derivative (PID) control algorithm was applied to the improvement of the controller. Modular design was involved in the controller hardware which was composed of the temperature sensing AD module, baseline drift suppression module, high voltage adjust module, interrupt module and alarm communication circuit. Software process was designed, and then simulation experiment was carried out to test the performances of the controller. Results The controller gained advantages in high loading performance and high-frequency baseline at zero level to realize compensation, and was gifted with high anti-interference ability and precision when fulfilling the safety control of the high-frequency electric knife by multi-thread temperature regulation and overheating interrupt. Conclusion The controller behaves well in the alarm against the burns by the high-frequency electric knife in the operating room.

Objective To investigate the relationship between the levels of plasma adrenaline and norepinephrine and gene polymorphism of β1 adrenergic receptor G1165C in children with enterovirus 71 (EV71) infection in hand foot and mouth disease (HFMD). Methods The polymerase chain reaction (PCR) was used to detect the expression of gene polymorphism of β1 adrenergic receptor G1165C in vitro. The levels of plasma adrenaline and norepinephrine were measured by enzyme-linked immunosorbent assay (ELISA). Results The plasma norepinephrine level of severe group was significantly higher than the mild group in children with EV71 infection in HFMD (P < 0.05); however, the levels of plasma adrenaline in two groups had no statistical differences (P > 0.05); There was no significant difference in the distribution of β1 adrenergic receptor G1165C genotype and allele between EV71 infection group and healthy control group (P > 0.05). Further analysis of EV71 infection group by dividing it into mild and severe groups showed that there was no significant difference in the distribution of genotype and allele between these two groups as well (P > 0.05). There was no significant difference in the levels of epinephrine and norepinephrine in different genotypes of EV71 infection group (P > 0.05), and in the levels of plasma epinephrine and norepinephrine in the mild and severe groups (P > 0.05). Conclusions As the disease gets worse, the plasma norepinephrine level has a rising trend in children with EV71 infection in HFMD, which is an important indicator to evaluate the progress of the disease. However, the gene polymorphism of β1 adrenergic receptor G1165C have no significant correlation, not only with the susceptibility and severity of EV71 infection in hand, foot and mouth disease, but also with the levels of catecholamine.
10.Study on the Excretion of Liguzinediol in Rats by UPLC-PDA Method

ABSTRACT:OBJECTIVE To study the excretion of Liguzinediol and its metabolites in rats by UPLC-PDA method.METH-ODS Six SD rats were administered intravenously at a dose of 10 mg∕kg.Its urine,bile and feces were collected according to certain time points.The samples were treated by methanol and the supernatant was dried by N2 ,then dissolved by mobile phase.Liguzinediol and its metabolites in urine,bile and feces were determinated by UPLC-PDA.Conversion factor was con-firmed through the determination of UV absorption coefficient and molecular weight of prototype drug together with its metab-olites.Then the cumulative excretion of Liguzinediol in rats(Dose%)was calculated.RESULTS There were slight differences in the excretion process between female and male rats.The Dose% of Liguzinediol and its main metabolites in female rats u-rine,bile and feces respectively were 47.94%,1 6.67% and 0.648%,and the total Dose% was 65.26%.The Dose% of Ligu-zinediol and its main metabolites in male rats urine,bile and feces respectively were 35.00%,20.37% and 1.1 56%,and the total Dose% was 56.53%.CONCLUSION The UPLC-PDA method with the addition of conversion factor can be used to ex-plore the material balance of Liguzinediol in rats,and provide experimental evidence for clinical research.
