BACKGROUND

There is an increasing population of elderly patients with diabetes. Malnutrition has been associated to higher morbidity and mortality among these patients. Currently, there are limited data on malnutrition and its risk factors among elderly patients with diabetes in the Philippines.

This study determined the prevalence, clinical profile and risk factors associated with malnutrition and identify the association of malnutrition with glycemic status and insulin resistance among elderly patients with diabetes.

This is a cross-sectional study involving 117 elderly patients with diabetes seen at a tertiary hospital in Manila, Philippines. Demographic, anthropometric, and clinical data were collected. Mini-Nutritional Assessment-Short form (MNA-SF), Simple FRAIL questionnaire and Mini-cog assessment were administered. Patients were categorized into normal, at risk for malnutrition, and malnourished using the MNA-SF. Comparative and logistic regression analyses were performed to identify the clinical profile and possible risk factors.

The prevalence of malnutrition was 1.71% with 29.06% at risk for malnutrition. There was no significant difference in demographic, anthropometric and biochemical parameters between the different nutrition statuses. High BMI, central obesity, and increased insulin resistance were observed across all nutrition status. Frail patients had almost five times increased likelihood (OR=4.94, p=0.043) of developing malnutrition. Good glycemic control had two-fold decreased likelihood (OR=0.44, p=0.050) of malnutrition. Insulin resistance was not associated with malnutrition.

Malnutrition is prevalent among elderly patients with diabetes. Frailty and poor glycemic control increased the risk of malnutrition. Therefore, malnutrition screening should be routinely performed among these patients. Diabetes management among elderly patients should include maintaining good glycemic control and preventing frailty and its complications.

BACKGROUND

Telemedicine as a response to the need for consultation amidst limited face to face interaction during the pandemic became part of the services of most tertiary hospitals in Davao City. In order to ensure the client’s needs are addressed by existing telemedicine services, it is important to look at the level of client satisfaction and identify factors that influence it.

The main objective of the study was to determine the level of satisfaction and the factors influencing it among the patients of the Family Medicine Department of a tertiary hospital in Davao City on the telemedicine services they received.

The study employed a descriptive cross-sectional design utilizing a self-administered telemedicine satisfaction questionnaire developed from the Clinician and Group Adult Visit Survey 4.0 (beta) and Telehealth Usability Questionnaire (TUQ). Participants were 18 years old and above and sought teleconsultation in the institution during the pandemic. Excluded were those who did not give consent. The data gathered were analyzed using an Epi-Info software to determine what factors were associated with level of satisfaction, a chi-square test was done for nominal and ordinal data while a t-test or ANOVA as applicable was used for continuous data.

A total of 278 patients were surveyed. More than ninety-five percent of the respondents perceived high satisfaction rating on the telemedicine service while less than five percent were dissatisfied. Eight factors were identified that seem to affect level of satisfaction. These include employment status, network connection and frequency of telemedicine.

The study identified that the telemedicine in SPMC DFCM provides high level of satisfaction among patients availing the service. It was also identified that factors such as employment status, network connectivity and frequency of using the service affects the level of satisfaction.

BACKGROUND

Globally, cancer remains to be one of the leading causes of mortality and poses significant burden to patients and families. A growing body of evidence suggests that early palliative care results in more favorable patient and caregiver outcomes compared to standard oncology care alone. 

This study aims to determine the effects of early palliative care on caregivers of adult cancer patients in terms of quality of life, psychological distress, and satisfaction with care. 

Systematic database searches in PubMed, Google Scholar, Cochrane library and Clinicaltrials.gov registry was conducted to select eligible studies for this review. Keywords, limit fields and Boolean operators were used for the search. Qualitative and quantitative analyses through the Review manager 5.4 software were done to compare quality of life, psychological distress, and satisfaction with care among caregivers given early palliative care vs. caregivers given standard oncology care. 

This review revealed inconclusive evidence on the benefits of early palliative care on caregiver outcomes when compared to standard care. We reviewed outcomes of early palliative care including quality of life, psychological distress, and satisfaction with care, but evidence remains unclear due to paucity of studies. Larger, high-caliber trials with more robust evidences, refinement of measurement tools catered to caregivers with higher sensitivity, and standardization of EPC protocol are needed to detect effect of EPC. 

INTRODUCTION: The COVID-19 pandemic has significantly increased reliance on digital devices for education, leading to heightened concerns about digital eye strain (DES) among students. This study aimed to investigate the association between screen time and the degree of DES among first to third-year medical students at a private medical school from August to September 2023. METHODS: An analytical cross-sectional design was employed, involving 194 participants who completed a self administered questionnaire, including the Computer Vision Syndrome Questionnaire (CVS-Q). Data were analyzed using descriptive statistics and relative risk calculations. RESULTS: The mean daily screen time was 6.94 hours, with 79.38% of participants reporting symptoms of digital eye strain. A significant association was found between screen time and DES, with a positive risk ratio of 1.304 for those spending 4-8 hours on screens compared to those with less than 4 hours. CONCLUSION This study highlights the growing prevalence of DES among medical students during the pandemic, emphasizing the need for educational institutions to implement strategies that mitigate screenrelated health risks. Recommendations include awareness programs, ergonomic guidelines and regular eye check-ups to promote ocular health.

BACKGROUND

Moyamoya angiopathy (MMA) is a rare cause of stroke, yet its incidence and outcomes are understudied in Southeast Asia, despite being the third most populous geographical region in the world. Our aim was to enhance understanding of MMA in this population and present a case from our experience

A systematic literature search on MMA in the Southeast Asian population was performed via PubMed, Scopus, and Wiley Online Library, including local journals and databases up until May 2024. Descriptive statistics were used to synthesize data. We also present a case of a 36 year-old Filipino female diagnosed with MMAfollowed by superficial temporal artery-middle cerebral artery (STA-MCA) bypass and resolution of symptoms.

Out of the 275 articles retrieved, 23 articles detailing 37 patients of Southeast Asian nationality were included in this review. Stroke predominated (76.3%, n=29) as initial presentation, followed by seizures (36.8%, n=14), movement disorders (n=4, 10.5%) and headaches (n=4, 10.5%). Treatment included antiplatelet therapy for 57.9% (n=22), and anti seizure medications for 26.3% (n=10) of cases. Only 21% (n=8) underwent surgical intervention, primarily via direct bypass. Outcomes based on the Modified Rankin Scale (MRS) showed 94.7% (n=36) had retained a functional score of MRS ≤2.

Our review underscores the scarcity of data on MMAin Southeast Asians, and highlights the varied clinical presentation. Effective management requires a tailored approach, with surgical management showing favorable outcomes. Future research should focus on longitudinal studies to assess long-term outcomes, explore the genetic and environmental factors, and develop standardized treatment protocols to improve patient care in this ethnically diverse population.

Background: The global pandemic caused by COVID-19, the subsequent improvement in health situation caused by anti- COVID-19 vaccination and the developments in digital technology prompted changes in undergraduate medical education as to content delivery and assessment. This study determined the difference in the performance of first year medical students in traditional face-to face, online and Hyflex teaching strategy. Methods: A non-concurrent cohort study was done to determine the performance of students in three annual subjects in Year 1 undergraduate medical education for school years 2018 to 2023. One-way ANOVA at p=0.001 determined significance of differences of variables. Results: There was no difference in the profile of students as to their sex, pre-medicine course and scores in the National Medical Admission Test (NMAT). The performance in the written examinations in Anatomy and Physiology showed significant difference (p=0.001) when the conduct of examinations was shifted online on an emergency basis. There was no difference (p=0.001) in performance in the laboratory conference sessions in Physiology, as compared to the focused group discussion sessions in Biochemistry and practical examinations in Anatomy. Conclusion Significant difference in the performance of first year medical students was observed only during the emergency shift to online examinations, otherwise the performance was similar in traditional face-to face, online and Hyflex teaching strategy.

Background: Sarcopenia is a musculoskeletal disease involving the reduction of muscle mass, strength, and performance. Handgrip strength (HGS) measurements included in frailty assessments are great biomarkers of aging and are related to functional deficits. We assessed the association between potential influencing factors and HGS asymmetry in older Peruvian adults. Methods: We used a database of the Peruvian Naval Medical Center "Cirujano Santiago Távara" located in Callao, Peru. All the patients included were ≥60 years old and had an HGS measurement in the dominant and non-dominant hand. Results: From a total of 1,468 patients, 74.66% had HGS asymmetry. After adjustment, calf circumference weakness (adjusted prevalence ratio [aPR]=1.08; 95% confidence interval [CI], 1.01–1.15), falls risk (aPR = 1.08; 95% CI, 1.02–1.16), and an altered Lawton index (aPR=0.92; 95% CI, 0.84–0.99) were associated with HGS asymmetry. Conclusion Our findings suggest that HGS asymmetry should be measured along with other geriatric assessments used to evaluate health outcomes in the elderly to enhance health promotion and prevention aimed at preserving muscle strength to curb functional limitations in the elderly.

Background: Challenges of polypharmacy and the impact of coronavirus disease 2019 (COVID-19) pandemic in older patients require further investigation. This retrospective study analyzed the progression of polypharmacy and anticholinergic burden in older patients in a primary care setting before, during, and after the COVID-19 pandemic. Methods: This 3-year cross-sectional study (2019, 2020, and 2021) comprised a dynamic cohort of individuals aged ≥75 years, who attended the Arrabal Primary Care Center in Zaragoza, Spain. Older patients with polypharmacy (≥5 medications) were identified according to their electronic health records. We collected demographic and clinical data, including medication prescriptions, diagnoses, and anticholinergic risks, and performed descriptive and statistical analyses. Results: This study included a total of 1,928 patients with a mean age of 83.52±0.30 years. Over the 3-year study period, the mean number of medications prescribed increased, from 9.4 in 2019 to 10.4 in 2021. The prevalence of excessive polypharmacy (≥10 medications) increased from 39% in 2019 to 45% in 2021. The most commonly prescribed drugs were anilides, proton pump inhibitors, benzodiazepine derivatives, and platelet aggregation inhibitors. Women had a higher prevalence of illnesses and anticholinergic drug prescriptions than men. Conclusion The results of this study highlighted an upward trend in polypharmacy and excessive polypharmacy among older patients in primary care settings. Future research should focus on optimizing medication management and deprescribing strategies and minimizing the adverse effects of polypharmacy in this population.

Human reflexes are simple motor responses that are automatically elicited by various sensory inputs. These reflexes can provide valuable insights into the functioning of the nervous system, particularly the brainstem and spinal cord. Reflexes involving the brainstem, such as the blink reflex, laryngeal adductor reflex, trigeminal hypoglossal reflex, and masseter H reflex, offer immediate information about the cranial-nerve functionality and the overall state of the brainstem. Similarly, spinal reflexes such as the H reflex of the soleus muscle, posterior root muscle reflexes, and sacral reflexes provide crucial information about the functionality of the spinal cord and peripheral nerves. One of the critical benefits of reflex monitoring is that it can provide continuous feedback without disrupting the surgical process due to no movement being induced in the surgical field. These reflexes can be monitored in real time during surgical procedures to assess the integrity of the nervous system and detect potential neurological damage.It is particularly noteworthy that the reflexes provide motor and sensory information on the functional integrity of nerve fibers and nuclei. This article describes the current techniques used for monitoring various human reflexes and their clinical significance in surgery. We also address important methodological considerations and their impact on surgical safety and patient outcomes. Utilizing these methodologies has the potential to advance or even revolutionize the field of intraoperative continuous monitoring, ultimately leading to improved surgical outcomes and enhanced patient care.

Conducting and writing quality improvement studies is a skill that every family and community physician should learn and apply in their practice. As family and community physicians it is one of our responsibilities to provide quality health care to our patients.1 Simply put, “quality” is doing the right thing right the first time and doing it better the next. Similar to the doctor-patient interaction in a typical consult, where the physician’s objective is to learn as much as possible about the patient’s signs and symptoms and medical history in order to make the right diagnosis and thus render the right treatment. The physician strives to do all this on the first visit and hopes to keep up-to-date on the condition so that when another patient presents with the same condition, he or she will receive better and more current care.
