1.Effects of brief mindfulness-based stress reduction on preoperative anxiety in patients undergoing painless gastrointestinal endoscopy

Objective:To investigate the effects of brief mindfulness-based stress reduction(MBSR)on preoperative anxiety in pa-tients undergoing painless gastrointestinal endoscopy.Methods:We enrolled 100 patients scheduled to undergo elective painless gas-trointestinal endoscopy at The First Affiliated Hospital of Zhengzhou University from September 2024 to April 2025.The inclusion cri-teria were:age,18-60 years;body mass index,18.0-28.0 kg/m2;American Society of Anesthesiologists physical status,class Ⅰ orⅡ;and no gender restriction.The patients were assigned to experimental group(n=50)or control group(n=50)using a random num-ber table.A dedicated nursing team implemented the brief MBSR protocol.At 30 minutes before endoscopy,both groups underwent anxiety assessment using the Amsterdam Preoperative Anxiety and Information Scale(APAIS).All the patients received routine preop-erative education.Guided by the nurses,the experimental group received the brief MBSR intervention consisting of mindful body scan-ning,mindful breathing,and mindful music listening,for 12 minutes each at 30 and 15 minutes before the procedure.We recorded the APAIS score,bispectral index(BIS),heart rate(HR),systolic blood pressure(SBP),diastolic blood pressure(DBP),and mean arterial pressure(MAP)at 30 minutes before the procedure(T0),after brief MBSR(T1),and immediately before anesthesia induction(T2);the length of stay in the post-anesthesia care unit(PACU)and postoperative adverse reactions;and the APAIS score and degree of sat-isfaction of patients at discharge from the PACU(T3).Results:Com-(all P<0.05).However,no significant differences were observed be-pared with the control group,the experimental group exhibited sig-nificantly lower APAIS scores,significantly reduced BIS values,and significantly lower HR values at T1 and T2 and a significantly lower APAIS score and a significantly higher degree of satisfaction at T3 tween the groups in SBP,DBP,MAP,postoperative adverse events,or PACU length of stay at any time point(all P>0.05).Conclusion:Brief MBSR is an effective non-pharmacological intervention to cope with perioperative negative emotions in patients undergoing pain-less gastrointestinal endoscopy,which can alleviate preoperative anxiety,reduce electroencephalographic arousal,and improve patient satisfaction.
2.The practice of rational drug use management in hospital under the reform of DRGs payment methods

Objective To establish an effective management mode, play the full role of DRGs in rational drug use, formulate the pharmaceutical clinical path, and intervene the prescription behavior of doctors, which could improve the level of rational drug use in the hospital through the management practice of rational drug use under the payment method of DRGs in a third-grade hospital. Methods A drug entering mechanism, a rational drug use supervision mechanism and an active and rational drug use data exposure mechanism based on DRGs were established, and the rational drug use indicators such as drug consumption index, average drug cost, and auxiliary drug use were accurately positioned based on the information platform, and the accurate and refined management of rational drug use was realized through the daily supervision of the office of the Pharmaceutical Affairs Committee. Results and Conclusion Under the multi-dimensional and multi-level rational drug use management based on DRGs, the effect of rational drug use was evaluated after the reform of DRGs payment method, and all the index of rational drug use in our hospital were continuously improved.
3.Echinatin inhibits malignant behaviors and immune escape of lung cancer A549 cells by activating the STING/TBK1/IRF3 signaling pathway

Objective:To investigate the effects of echinatin(Ech)on the malignant biological behavior and immune escape of lung cancer A549 cells and its underlying mechanisms.Methods:Normal lung epithelial cells(BEAS-2B)and A549 cells were routinely cultured and treated with different concentrations of Ech for 24h.Cell viability was assessed using the MTT assay,and concentrations of 20,30,and 40 μmol/L were selected for subsequent experiments.A549 cells were divided into the following groups:control group(0 μmol/L Ech),low-(20 μmol/L),medium-(30 μmol/L),and high-concentration(40 μmol/L)Ech groups(Ech-L,Ech-M,Ech-H),and high-dose Ech combined with the pathway inhibitor H-151(1.0 μmol/L)group(Ech-H+H-151).Cell proliferation,migration,and invasion were evaluated using the EdU assay,wound-healing assay,and Transwell assay,respectively.Western blotting(WB)assay was applied to detect the expression of proteins related to proliferation,migration,invasion,and the STING/TBK1/IRF3 signaling pathway.Subsequently,A549 cells were co-cultured with CD8+T cells,and trypan blue staining was used to detect CD8+T cell viability.The levels of type Ⅰ interferon(IFN-Ⅰ)in the co-culture supernatants were detected by WB,while the levels of programmed death ligand 1(PD-L1),interleukin-10(IL-10),interleukin-4(IL-4),and transforming growth factor-β(TGF-β)were determined using ELISA.Results:Ech inhibited the viability of A549 cells in a dose-dependent manner(all P<0.05)but had no significant effect on the viability of BEAS-2B cells.Ech dose-dependently inhibited the proliferation,migration and invasion of A549 cells(all P<0.05),as well as the protein expression of cyclinD1,Ki67,MMP2,MMP9,STING,p-TBK1 and p-IRF3(all P<0.05).These effects were partially reversed by H-151.Ech dose-dependently promoted the survival of CD8+T cells co-cultured with A549 cells(all P<0.05),enhanced IFN-Ⅰexpression(all P<0.05),and inhibited the secretion of PD-L1,IL-10,IL-4,and TGF-β(all P<0.05),with H-151 partially reversing these effects(all P<0.05).Conclusion:Ech inhibits malignant biological behavior and immune escape of lung cancer A549 cells by activating the STING/TBK1/IRF3 signaling pathway.
4. Exploring the outbreak point for clinical pharmacists to provide pharmaceutical care in nephrology department

AIM: To introduce the entry point of clinical pharmacists on developing pharmaceutical care in the department of nephrology to further explore more pharmaceutical care entry point and to better promote rational drug use. METHODS: To summarize the work of the clinical pharmacists participate in the formulation and optimization of anti-infection therapy; participate in the formulation of parenteral nutrition program; conduct medication and adherence education; conduct pharmaceutical monitoring; provide evidence-based medical analysis for the off-label drug use in the department of nephrology by case sharing. RESULTS: Clinical pharmacists provide pharmaceutical care in the department of nephrology. This improves the medication compliance, avoids adverse reactions, optimize therapeutic regimen. CONCLUSION: Developing various pharmaceutical care in the department of nephrology will be safe, effective and economic for drug application.
5.Application of a ASK model-based four-step training method combined with workshops in training on the prevention and control of nosocomial infection for medical students

Objective:To explore the effects of applying a four-step training method based on an attitude-skill-knowledge (ASK) model combined with workshops in training on the prevention and control of nosocomial infection among medical students, and to provide a reference for reforming nosocomial infection prevention and control training for medical students and establishing a nosocomial infection prevention and control training system.Methods:Medical students from five medical schools entering clerkships during 2018 to 2020 were selected. They were divided into control group (218 medical students in 2018-2019 receiving conventional nosocomial infection control training) and experimental group (216 medical students in 2019-2020 receiving nosocomial infection control training based on the ASK model-based four-step method combined with workshops). The two groups were compared in terms of the compliance rate of hand hygiene, the pass rates of nosocomial infection control theory and skill examinations, the correct rate of medical waste disposal, the incidence of occupational exposure, and the degree of satisfaction with teaching.Results:All the indices of the experimental group were significantly better than those of the control group: the compliance rate of hand hygiene ( χ2=4.92, P=0.027), the correct rate of medical waste disposal ( χ2=5.13, P=0.023), the pass rate of nosocomial infection control theory examination ( χ2=4.03, P=0.038), the pass rate of nosocomial infection control skill examination ( χ2=6.71, P=0.010), and the degree of satisfaction with teaching [(98.03±2.16) vs. (92.53±2.01), P<0.001]. No one had occupational exposure in the experimental group, while there were three cases in the control group. Conclusion:The ASK model-based four-step method combined with workshops is effective in nosocomial infection prevention and control training for medical students. This innovative training method improves students' satisfaction with clinical teaching, providing a basis for the establishment of a training system for the prevention and control of nosocomial infection.
6.Application and practice of standardized patient teaching of online appointment system for interns in cardiology department

Objective:To explore the effect of online appointment system of standardized patients (SP) in cardiology practice teaching.Methods:The undergraduate students who entered the cardiology department of Shanghai Ninth People's Hospital, Shanghai Jiao Tong University School of Medicine for clinical practice in 2018 and 2019 were selected as teaching objects, and they were divided into traditional teaching group ( n=30) and appointed SP teaching group ( n=30). After the teaching, SPSS 19.0 software was used for data analysis to compare the teaching effect of the two groups. Results:The scores of SP teaching group were higher than those of traditional teaching control group, with statistical significance ( P<0.05). Conclusion:Through constructing SP database and typical case database, online appointment system of SP for medical history inquiry and doctor-patient communication skills training can effectively solve the problems such as lack of clinical practice cases and doctor-patient contradiction, and greatly improve the teaching efficiency.
7.The role of indocyanine green fluorescence imaging in laparoscopic partial splenectomy

Objective:To study the role of indocyanine green(ICG)fluorescence imaging in laparoscopic partial splenectomy (LPS).Methods:The data of 4 patients who underwent ICG fluorescence imaging technology for LPS at Beijing Luhe Hospital Affiliated to Capital Medical University from May 2017 to May 2020 were retrospectively analyzed. There were 3 females and 1 male, aged 46, 41, 27 and 12 years respectively. The extents of spleen preservation were compared between ICG fluorescence imaging with ordinary white light during operation. The residual splenic remnants were tested with fluorescence imaging after splenectomy, which showed fluorescence fading indicating good vascular perfusion.Results:ICG fluorescence imaging was performed on 4 patients. The operation time ranged from 180.0 to 250.0 min, and the intraoperative blood loss ranged from 40.0 to 200.0 ml. The postoperative hospital stay ranged from 4 to 14 days. There were no serious complications. Postoperative histopathology showed: splenic cyst ( n=1), splenic hemangioma ( n=2), and splenic laceration ( n=1). Conclusions:ICG fluorescence imaging technology had a significant role to play in partial splenectomy. This study showed this technique to improve safety of laparoscopic partial splenectomy.

9.Pathway and Effect Evaluation of Information Management for Hospital Pharmacy

OBJECTIVE:To promote the realization of information management for hospital pharmacy. METHODS:The practice of pharmacy information management pathway in our hospital was introduced in respects of drug supply,rational drug use monitoring,pharmacy information dissemination,etc. The effects of information management were evaluated in respects of work efficiency,the rate of drug dispensing error and rational drug use index,etc. RESULTS:The information management pathway of our hospital included drug purchase,warehousing and keeping system,drug distribution management system,reasonable drug use monitoring information module,etc. It realized automatic generation and issue of purchase plan,acceptance bar code of drug warehousing,real-time drug stocking and location changing,drug guarantee delivery in advance,window drug delivery and check barcode scanning,drug distribution quality information management,whole process temperature and humidity monitoring, unreasonable prescription after the event,in the event,in advance,early warning of drug use,real-time monitoring of drug use and administration of drugs for chronic diseases,etc. After the implementation of information management,the time of drug warehousing invoice input and drug stocking were shortened.Among drug dispensing error,the percentage of type error decreased from 0.005 2% during Jun.-Dec. 2015 to 0.001 6% during Jan.-Jun. 2016;that of number error decreased from 0.006 9% to 0.001 6%;that of other error dropped to zero. During 2014-2016,the ratio of drug cost to treatment cost decreased gradually;those of inpatient were 26.62%,24.91%,24.36%,respectively;those of outpatient were 44.06%,42.10%,41.32%, respectively. Utilization rates of antibiotics in the inpatients decreased gradually,and were 44.82%,44.14%,43.91%, respectively. CONCLUSIONS:Information construction plays an important role on hospital pharmacy,can effectively improve work efficiency,reduces medication error and promotes rational drug use.
10.Effect of the Balint group on improving the empathy and doctor-patient communication skills among medical students inclinical practice

Objectives Toexplorethe effect of the Balint group on improving the empathy and doctorpatient communication skills among medical students in clinical practice.Methods We investigated 500 clinicalmedicalstudentsin fourth grade by JSE-HP and SEGUE framework scalein Shanghai Jiao Tong university school of medicine between July 2016 and December 2016.60 students with insufficientempathy and doctor-patient communication skills were selected randomly.These students were thanrandomly divided into the Balint group (experimental group) and the routine clinical practice group(control group).The experimental group participated inthe Balint group activitiesin addition to routine clinical practice,whilethe control group only participated in routine clinical practice.After 20 weeks of the Balintgroup intervention,the two groups were invited to assess theempathy and doctor-patient communication skills again to analyze the effect of the Balintgroup on improving the empathy and doctor-patient communication ability among intern students.The evaluation result was analyzed by SPSS 22.0 software.Data comparison was conducted using pair t test,independent sample t test and x2 testing.Results There were no statistically significant differences in age,sex,whether only child or class leader and previous year theoretical exam scoresbetween the two groups (all P>0.05).For empathy,there was no statistically significant difference in terms of opinion,emotional care,empathy,and total scorebefore the routine clinical practicebetween the two groups (P>0.05) and they all improvedsignificantly after the routine clinical practice (all P<0.05).But the experimental group had higher scoresin all above scalesthan the control group (all P<0.05).For communication skills with patients,there were no statistically significant differencesbetween the two groupsin communication initiation,information collection,understanding of patients,information giving,end questioning,and SEGUE scoresbefore the routine clinical practice (all P>0.05),and they allimprovedsignificantly in all above 5 scales after the routine clinical practice (all P<0.05).While except for understanding of patients,the experimental group had higher gains in other scales than the control group (all P<0.05).Conclusion Use of the Barring group activities on the basis of routine clinical practice canimprove the empathy andthedoctor-patientcommunication skills in medical students.
