1.Research advances in traditional Chinese medicine for the treatment of hepatocellular carcinoma by regulating immune cells

Hepatocellular carcinoma (HCC) is a common malignant tumor with a high mortality rate, an insidious onset, and complex pathological mechanisms. In the tumor microenvironment, tumor-promoting immune cells protect tumor cells from immune attacks, while dysfunction of anti-tumor immune cells causes the inhibition of immune response, thereby leading to the continuous deterioration of cancer. In recent years, traditional Chinese medicine has shown good efficacy in the treatment of HCC, and it can inhibit the proliferation and metastasis of cancer cells by regulating immune cells. By analyzing related articles in China and globally, this article summarizes how immune cells affect the progression of HCC through the immunosuppressive pathway and how traditional Chinese medicine exerts an anti-HCC effect by regulating immune cells, in order to provide theoretical basis and reference for optimizing the treatment of HCC.
2.Application value of chromosomal microarray analysis for the detection of low-level mosaicisms in amniotic fluid samples and analysis of rare cases.

OBJECTIVE:
To assess the value of chromosomal microarray analysis (CMA) for the detection of low-level mosaicisms in amniotic fluid samples, and to retrospectively analyze the rare cases of mosaicisms.
METHODS:
Chromosomal karyotype of the fetus was determined by G-banding analysis of cultured amniotic fluid cells. CMA was used to detect copy number variation of fetal chromosomes, and fluorescence in situ hybridization (FISH) was used to determine the proportion of fetal chromosomal mosaicisms in uncultured amniotic fluid cells.
RESULTS:
Among 825 prenatal samples, 4 cases of true fetal mosaicisms were detected, which yielded an incidence of 0.48%. Two cases were sex chromosomal mosaicisms, and two were autosomal mosaicisms, which involved chromosomes 8 and 9, respectively. All cases were verified by G-banding analysis of cultured amniotic fluid cells, CMA, and/or FISH.
CONCLUSION
CMA has a great value for detecting low-level mosaicisms in amniotic fluid samples, though the positive results need to be verified by other techniques and should be interpreted with caution. The review of rare cases can provide a basis for prenatal genetic counseling.

3.Development and validation of a nomogram for predicting positive surgical margins after robot-assisted radical prostatectomy

Objective:To investigate the risk factors for positive surgical margins(PSM)after robot-assisted radical prostatectomy(RARP),and to develop and validate a predictive nomogram.Methods:We retrospectively analyzed the clinicopathological data of 874 prostate cancer patients who underwent RARP performed by a single surgeon at the First Medical Center of Chinese PLA General Hospital between January 2012 and December 2018. Patients were divided into positive surgical margin(n=327)and negative surgical margin(n=547)groups based on postoperative margin status.The PSM group had significantly higher preoperative median tPSA[31.200(19.050,54.400)ng/ml vs. 15.050(9.840,27.590)ng/ml, P<0.01],higher proportion of patients with PSAD>1 ng/ml 2[49.5%(162/327)vs. 21.2%(116/547), P<0.01],biopsy Gleason score ≥8[33.3%(109/327)vs. 21.2%(116/547), P<0.01],ISUP grade 4-5[33.3%(109/327)vs. 21.2%(116/547), P<0.01],clinical T stage ≥cT 3[11.3%(37/327)vs. 4.2%(23/547), P<0.01],and high-risk classification[82.3%(269/327)vs. 55.9%(306/547), P<0.01]compared to the negative surgical margin group. Conversely,the PSM group had a lower prevalence of hypertension[29.7%(97/327)vs. 40.2%(220/547), P=0.002].Patients were randomly split into a training cohort(n=656,75%)and an internal validation cohort(n=218,25%). An external validation cohort included 71 patients who underwent RARP by different surgeons between January 2014 and December 2016. No significant differences in baseline characteristics were observed between cohorts( P>0.05).Univariate and multivariate logistic regression analyses identified independent predictors of PSM,which were incorporated into a nomogram. Predictive performance was assessed using receiver operating characteristic(ROC)curves,decision curve analysis(DCA),and calibration curve. Internal and external validations were performed. Results:The PSM group had longer postoperative hospitalization[6(5,8)vs. 6(5,7)days, P=0.028],higher rates of pathologic Gleason score ≥8[41.5%(115/277)vs. 24.9%(111/446), P<0.01],ISUP grade 4-5[41.5%(115/277)vs. 24.9%(111/446), P<0.01],pT 3 stage[52.3%(171/327)vs. 17.4%(95/547), P<0.01],pN 1 stage[12.8%(42/327)vs. 3.8%(21/547), P<0.01],extracapsular extension[52.3%(171/327)vs. 17.4%(95/547), P<0.01],and seminal vesicle invasion[34.6%(113/327)vs. 9.1%(50/547), P<0.01].Multivariate analysis identified elevated tPSA( OR=1.014,95% CI 1.004—1.024,P=0.006)and PSAD ≥0.15 ng/(ml/g)( OR=11.638,95% CI 1.450—93.396,P=0.021)as independent risk factors for PSM. The area under the ROC curve(AUC)of the nomogram constructed based on the above variables was 0.770(95% CI 0.735—0.805). The AUC values for the internal and external validation sets were 0.698(95% CI 0.630—0.767)and 0.643(95% CI 0.513—0.774),respectively. The calibration curve demonstrated good agreement between the predicted and observed outcomes,and the DCA indicated that the predictive model has potential clinical utility in decision-making. Conclusion:tPSA and PSAD were identified as independent risk factors for PSM. The nomogram constructed based on these two independent predictive variables effectively predicted PSM after RARP.
4.Side-to-side anastomosis of superficial temporal artery and middle cerebral artery bypass using in-situ intravascular suture technique: a report of 10 cases

Objective:To investigate the feasibility and clinical efficacy of side-to-side anastomosis of superficial temporal artery (STA) and middle cerebral artery (MCA) bypass using in-situ intravascular suture technique.Methods:A retrospective analysis was conducted on the clinical data of 10 adult patients who were treated with side-to-side microvascular anastomosis of STA-MCA bypass to improve intracranial blood supply, between February 2024 and September 2024 in the Department of Neurosurgery of the Fourth Affiliated Hospital of Soochow University. Among the patients, 2 were of symptomatic MCA occlusion and 8 of Moyamoya disease. Diameter of STA and MCA, length of anastomosis and blocking time of MCA were recorded. Indocyanine green video angiography (ICG-VA) was performed to evaluate the immediate patency of the STA-MCA side-to-side anastomosis. Digital subtracted angiography (DSA) was performed at 1 week after the surgery to evaluate the patency of the STA-MCA bypass anastomosis, then follow-up DSA was performed at 1, 3 and 6 months after surgery to further evaluate the postoperative anastomotic patency. Neurological function was evaluated regularly with the modified Rankin Scale (mRS).Results:All of the 10 side-to-side STA-MCA bypass anastomoses were successfully performed using in-situ intravascular suture technique. The scalps of all patients healed well. The diameters of STA and MCA were 1.4-2.0 (1.76±0.27) mm and 0.8-1.4 (0.98±0.20) mm, respectively. The average length of the anastomoses was 3.5-5.0 (4.45±0.60) mm. The blocking time of MCA was 12.0-29.0 (21.50±6.62) min. A 100% vessel patency rate was achieved immediately after vessel anastomosis and at 1 month after surgery. DSA examinations were performed at 3 months after surgery on 6 patients and at 6 months after surgery on 1 patient, and all the anastomoses were found in full patency. All patients were included in the postoperative follow-up that lasted for 3 to 7 months. All patients recovered well without new neurological dysfunction. The mRS of 8 patients remained at 0 point before and after surgery. Two patients had improved postoperative mRS of 1 point from that of 2 points before surgery.Conclusion:STA-MCA side-to-side microvascular bypass anastomosis can be performed safely and efficiently using in-situ intravascular suture technique. It could efficiently reduce the incidence of perioperative complications. Meanwhile, it can self-regulate the blood flow and maximise the potential capability of STA.
5.Diagnostic value of electromyographic tremor indicators for Parkinson's disease based on Logistic regression model

Objective To investigate the diagnostic value of electromyographic(EMG)tremor indicators for Parkinson's disease(PD)using the Logistic regression model.Methods A total of 65 patients with PD(PD group)and 39 patients with essential tremor(ET)(ET group)were enrolled and underwent EMG tremor analysis.General information,disease-related data,and EMG tremor characteristics were compared between the two groups.Multivariate Logistic regression analysis was performed to screen for independent influencing factors of PD,and receiver operating characteristic(ROC)curves were plotted.The area under the curve(AUC)was used to evaluate the diagnostic value of EMG tremor indicators for PD.Results Compared with the ET group,the PD group had a higher proportion of patients with unilateral onset and those with tremor spectrum frequency≥2 times,and a lower proportion of patients with a family history of tremor(P<0.05).The tremor peak frequencies in the resting,postural,and weight-bearing(1 000 g)states were lower in the PD group than in the ET group(P<0.05).There were statistically significant differences in the tremor rhythm patterns between the two groups in the resting and weight-bearing states(P<0.05),with the PD group dominated by alternating contraction patterns and the ET group by synchronous contraction pat-terns.Multivariate Logistic regression analysis revealed that the tremor peak frequency in the weight-bearing state,the tremor rhythm pattern in the resting state,and the frequency of tremor spectrum were independent influencing factors of PD(P<0.05).The ROC curves showed that the AUCs of the tremor peak frequency in the weight-bearing state,the tremor rhythm pattern in the resting state,and the frequency of tremor spectrum for diagnosing PD were 0.886,0.750,and 0.779,respec-tively.The combination of these three indicators yielded the highest AUC(0.936)for diagnosing PD,with a sensitivity of 81.54%and a specificity of 94.87%.Conclusion The tremor peak fre-quency in the weight-bearing state,the tremor rhythm pattern in the resting state,and the frequency of tremor spectrum provided by EMG tremor analysis can serve as clinical indicators for early diagno-sis of PD,and their combined use offers higher diagnostic value,which can be used to differentiate PD from ET.

10.Study on the Application of Cyclosporine A in Patients with Unexplained Re-peated Implantation Failure

Objective:To investigate the clinical efficacy of Cyclosporine A(CsA)in patients with unexplained repeated implantation failure(URIF),and to analyze the changes of peripheral blood lymphocyte subsets after CsA treatment.Methods:105 patients with URIF who underwent frozen-thawed embryo transfer(FET)in the De-partment of Reproductive Medicine in The Second Affiliated Hospital of Kunming Medical University from Septem-ber 30,2021 to March 1,2022 were selected.After informed consent,the patients were divided into CsA group(n=52)and control group(n=53)according to whether they received CsA treatment or not.Pregnancy outcomes and changes in lymphocyte subset were compared between the two groups.Results:The embryo implantation rate and clinical pregnancy rate in CsA group were higher than those in the control group,the difference was statisti-cally significant(48.91%vs.32.56%,P=0.027;53.85%vs.32.08%,P=0.024).The CsA group had a lower ear-ly abortion rate than the control group(10.71%vs.23.53%),but the difference was not statistically significant(P=0.25).The percentage of CD3-CD16+CD56+in CsA group was significantly decreased after treatment[(16.15±5.37)%vs.(18.23±7.10)%,P=0.012],it was also lower than that in the control group[(16.15±5.37)%vs.(18.67±5.16)%,P=0.018].Conclusions:CsA treatment can significantly improve the clinical preg-nancy rate and embryo implantation rate of frozen-thawed embryo transfer in patients with URIF,which may be a-chieved by promoting the distribution of peripheral blood lymphocytes to the direction of embryo implantation,es-pecially by down-regulating the percentage of CD3-CD16+CD56+.CsA has a certain application prospect in the field of assisted reproduction.
