1.Comprehensive Assessment and Analysis of the Informational Development Level in Public TCM Hospitals Nationwide Based on Comprehensive Index Method

Objective To learn the current status of informational development in public TCM hospitals in China,and to provide references for promoting the informational development.Methods It makes a comprehensive assessment of the informational level in 2 539 public TCM hospitals nationwide with comprehensive index method,and then analyse the index.Results(1)The construction rate of telemedicine systems in tertiary public TCM hospitals reached 76.96%,and the rate in secondary hospitals is 57.63%.(2)In terms of functional level of electronic medical record application,34.42%of tertiary public TCM hospitals failed to meet the requirement of Level 4,and 40.92%of secondary hospitals failed to meet the requirement of Level 3.In terms of Interconnection standardization maturity level,86.72%of tertiary public TCM hospitals failed to meet the requirement of Level 4.(3)The informational level is not balanced among different typesand different regions in tertiary public TCM hospitals.In secondary public TCM hospitals,there are differences among types of hospitals.Conclusion There are advantages on the development of Internet consultation and telemedicine systems in public TCM hospitals.The standardization of information may be a key factor that affect the lack of data connectivity and smart hospital management.There are differences in the level of informatization among different types of hospitals.So TCM hospitals is recommended to explore the way of development that suit for them.Some hospitalswell-developed should lead the way of development.
2.Research on the coupled and coordinated measurement of high-quality development of public traditional Chinese medicine hospitals in China based on the new development philosophy

Objective:To construct a measurement index system for the high-quality development of public traditional Chinese medicine (TCM) hospitals and conduct an analysis of the coupling coordination degree, providing a reference for the high-quality development of public TCM hospitals.Methods:The characteristics and principles for the high-quality development of public TCM hospitals were conceptualized and operationalized based on the new development philosophy. The Delphi method was adopted to establish a coupled and coordinated measurement index system for the high-quality development of public TCM hospitals. The empirical study was conducted using the data from 240 teritiary grade A public TCM hospitals at the manicipal level and above in 2022, selected from the comprehensive statistical system of the Monitoring and Statistical Research Center. The comprehensive evaluation index method was adopted to comprehensively evaluate the level of high-quality development, and the coupling coordination degree model was used to calculate the level of coupling coordination development.Results:The constructed measurement index system for the high-quality development of public TCM hospitals included 5 first-level indicators (inheritance and innovation, service quality improvement, efficient operation, element openness, and resource sharing), 10 second-level indicators and 32 third-level indicators. The 240 public TCM hospitals basically presented a development level state of resource sharing>element openness>efficient operation>service quality improvement>inheritance and innovation in the subsystem dimensions. The average coupling coordination degree among subsystems was 0.498, which belonged to the primary coordination stage. The coupling coordination degree of element opening and resource sharing with other subsystems was relatively high, while the coupling coordination degree of inheritance and innovation with other subsystems was relatively low.Conclusions:The role of inheritance and innovation in driving the development of public TCM hospitals still needs to be improved. There are significant differences in the coupling and coordination degree among subsystems. It is suggested that the capacity building of advantageous specialties in TCM be further strengthened, the quality and level of services be improved, the regional coordinated development strategy of public TCM hospitals be further implemented, the regional development gap be narrowed, and the coordinated development of various subsystems of high-quality public TCM hospitals be promoted.
3.Reform Challenges and Strategic Recommendations for the Operation and Management of Public Traditional Chinese Medicine Hospitals under the DRG Version 2.0 Payment System

The reform of payment based on disease diagnosis related grouping (DRG) is an important measure to deepen the medical security system. Its implementation will have a profound impact on the operation and management of public TCM hospitals from four aspects: medical service efficiency, cost, quality, and medical resource allocation. At the same time, there are also difficulties in reform, such as the contradiction between personalized diagnosis and treatment in TCM and standardized payment in DRG 2.0, compatibility issues between TCM medical record data and DRG 2.0 grouping system, limitations of special case negotiation mechanism, and difficulty in cost control and accounting. In response, this study formulated targeted strategies and recommendations intended to facilitate the effective implementation of the DRG 2.0 payment system, and bolster the operational and administrative efficacy of public TCM hospitals, culminating in a favorable scenario of tripartite synergy among "healthcare providers, insurers, and patients".
4.Entropy Weight TOPSIS Method Based on the National Public Traditional Chinese Medicine Hospitals Analysis of the Current Situation of Chinese Medicine Specialty Services

Objective To understand the situation of Traditional Chinese Medicine (TCM) characteristic services in public TCM hospitals,and to provide reference and basis for hospitals to give full play to the advantages of TCM characteristics.Methods Taking the survey data of the 2022 National Comprehensive Statistical System of TCM as the main assessment content,the entropy weight TOPSIS method was used to analyse the situation of TCM characteristic services in TCM hospitals nationwide through five TCM characteristic indicators of outpatient services and four TCM characteristic indicators of inpatient services.Results (1) In outpatient services,"the proportion of outpatients using non-pharmacological TCM"has the greatest weight (20.40%);in inpatient services,"the proportion of patients discharged with TCM as the main treatment"has the greatest weight (20.13%).(2) The highest score of TCM specialty service index of TCM hospitals is 0.854,and the lowest score is 0.035;the capability of TCM specialty service in Southwest,Central South and East China is relatively stronger;the Ci values of provincial-level,municipal-level,and county-level TCM hospitals and the following hospitals are 0.497,0.487,and 0.423 respectively,and that of the tertiary-level and secondary-level TCM hospitals are 0.495 and 0.416.(3) The TCM specialty service indexes of tertiary specialized TCM hospitals,tertiary ethnomedicine hospitals,and tertiary general TCM hospitals are higher,at 0.549,0.508,and 0.496 respectively.Conclusion The overall performance of TCM hospitals nationwide in TCM speciality services is fair,but there are differences between regions,hospital levels and hospital types.

8.Study on the medication law of treatment for acute mastitis in Pu Ji Fang based on the ancient and modern medical record cloud platform

Objective:To analyze the medication characteristics and the rules of prescription for the treatment of acute mastitis in Puji Fang; To provide reference for the clinical treatment of acute mastitis in modern TCM. Methods:Prescriptions of the chapters of " Ruyong" and " Duru" contained in Pu Ji Fang? Fu Ren Zhu Ji Men were screened, and the prescription data were analyzed by using the ancient and modern medical record cloud platform V2.3.5 for drug frequency, properties, tastes, meridian tropism, and efficacy. Clustering analysis of high-frequency drugs and analysis of association rules were performed. Results:A total of 141 prescriptions were included, with 201 kinds of Chinese materia medica. The top ten drugs with the high frequency of use were Glycyrrhizae Radix et Rhizoma, Paeoniae Radix alba, Rhei Radix et Rhizoma, Angelicae Sinensis Radix, Astragali Radix, Olibanum, Trichosanthis Fructus, Ginseng Radix et Rhizoma, Scutellariae Radix, Gleditsiae Spina. The main property was cold, the main taste was sweet, the main meridian was spleen meridian, and the main efficacy was heat clearing and detoxification. Three types of high-frequency drugs could be obtained by clustering analysis, namely, Ginseng Radix et Rhizoma-Astragali Radix-Angelicae Sinensis Radix-Rhei Radix et Rhizoma, Scutellariae Radix-Paeoniae Radix alba-Glycyrrhizae Radix et Rhizoma, Trichosanthis Fructus-Olibanum-Gleditsiae Spina. Through correlation analysis of high-frequency drugs, it was found that there was a great correlation between Astragali Radix and Paeoniae Radix alba, Angelicae Sinensis Radix, Glycyrrhizae Radix et Rhizoma, Ginseng Radix et Rhizoma.Conclusions:The pathogenesis of acute mastitis in Pu Ji Fang is mainly damp-heat toxin and blood stasis, and deficiency of qi and blood. The treatment for strengthening healthy qi focuses on tonifying qi and blood of the liver and spleen, while the treatment for eliminating pathogenic factors focuses on removing dampness, clearing heat and toxin, resolving phlegm, dispersing stasis and removing blood stasis. In addition, according to the condition, the treatment will be increased or decreased with the disease.
9.Fibrinogen-to-albumin ratio predicts the outcome of patients with acute ischemic stroke receiving intravenous thrombolysis

Objective:To investigate the correlation and predictive value of fibrinogen-to-albumin ratio (FAR) and clinical outcomes after intravenous thrombolysis in patients with acute ischemic stroke.Methods:Patients with acute ischemic stroke received intravenous thrombolysis treatment at Fuyang People's Hospital from November 2019 to August 2022 were retrospectively included. Their clinical, imaging, and laboratory data were collected. After 3 months of onset, a modified Rankin Scale was used for clinical outcome evaluation and a score >2 were defined as poor outcome. Multivariate logistic regression analysis was used to determine the correlation between FAR and poor outcome. Receiver operating characteristic (ROC) curve was used to evaluate the predictive value of FAR on adverse outcomes after intravenous thrombolysis in patients with acute ischemic stroke. Results:A total of 162 patients were included. There were 114 patients (70.4%) in the good outcome group and 48 (29.6%) in the poor outcome group. Univariate analysis showed that the baseline National Institutes of Health Stroke Scale (NIHSS) score, fasting blood glucose, fibrinogen, and FAR in the poor outcome group were significantly higher than those in the good outcome group, while the serum albumin was significantly lower than that of the good outcome group (all P<0.05). Multivariate logistic regression analysis showed that the baseline NIHSS score (odds ratio [ OR] 1.176, 95% confidence interval [ CI] 1.082-1.227; P<0.001), fasting blood glucose ( OR 1.206, 95% CI 1.018-1.430; P=0.030), and FAR ( OR 1.448, 95% CI 1.449-1.824; P=0.002) were the independent risk factors for poor outcome. The ROC curve analysis showed that the area under the curve of FAR for predicting poor outcomes was 0.706 (95% CI 0.616-0.796). When the FAR cutoff value was 8.06, the sensitivity and specificity were 66.7% and 78.2%, respectively. Conclusion:There is a significant correlation between FAR and outcomes after intravenous thrombolysis therapy in patients with acute ischemic stroke. Higher FAR has certain predictive value for poor outcomes of patients.
10.Analysis on the medication rules of Pu Ji Fang for treating acne externally based on data mining

Objective:To analyze the medication rules in the ancient book Pu Ji Fang for the external treatment of acne based on data mining method. Methods:By screening out the methods of treating acne externally in Pu Ji Fang and establishing a standardized medical record database, this paper adopted the web version of Ancient and Modern Medical Record Cloud Platform to calculate the frequency, properties, flavors, and meridians of those medicines, and conduct cluster analysis by using IBM SPSS Modeler 18.0 software to analyze the association rules. Results:A total of 87 prescriptions were selected, including 164 kinds of Chinese materia medica, among which. Radix Angelicae, Ligusticum Wallichii, Rhizoma Typhoni and lead powder are frequently appeared. The properties of those medicines are mainly warm, cold and mild; the flavors of those medicines are mainly spicy, acrid, sweet and bitter, and the meridians mainly belongs to lung, spleen, stomach and liver meridians. The medical pair and group with the strongest associationion are Ligusticum Wallichii- Radix Angelicae and Rhizoma Typhonii- Radix Angelicae- Ligusticum Wallichii. Those freuently appeared medicines could be grouped into three categories. The paste dosage that was frequently appeared has strong correlation with tallow, mercury and lead powder, while the powder dosage that was frequenctly appeared has strong correlation with Angelica Dahurica, Radix Saponicae, Gleditsia sinensis, Radices Ligustici Sinensis and Ligusticum Wallichii. Conclusions:The application of data mining method could preliminarily reveal the medication rules of Pu Ji Fang for the external treatment of acne. The main treatment method is XinSanFaYue. The three categories of Chinese materia medica are used to treat the syndrome of asthenic habitus attacked by exogenous pathogenic factors, exterior attacked by wind heat and hot blood stasis respectively, showing the rules of treating acne externally before Ming Dynasty and providing references for the clinical treatment of acne.
