1.Mechanism Prediction of Banxia Baizhu Tianmatang and Danggui Shaoyaosan Intervention in Ménière's Disease Based on LC-MS Technology Combined with Network Bioinformatics

ObjectiveThis study aims to analyze the pharmacodynamic material basis and multi-target mechanism of action of Banxia Baizhu Tianmatang combined with Danggui Shaoyaosan in the treatment of Meniere's disease(MD). MethodsUltra-performance liquid chromatography-high resolution mass spectrometry (UPLC-HRMS) (mobile phase: gradient elution with 0.1% formic acid aqueous solution-acetonitrile. Mass spectrometry scanning range: m/z 90-1 300) was used to identify the chemical components of the compound recipe and components absorbed into blood. The core mechanism was predicted by combining network pharmacology (target screening via SwissTargetPrediction and GeneCards databases, and construction of protein-protein interaction (PPI) network by STRING) and molecular docking (evaluated by Autodock, with binding energy ≤ -5.0 kcal·mol-1). For animal experiment validation, 36 Sprague Dawley (SD) rats were divided into a blank group, a model group (postauricular injection of lipopolysaccharide (LPS) at 1 mg·kg-1), low/medium/high-dose Chinese medicine groups (5.94, 11.88, and 23.76 g·kg-1·d-1, respectively), and Western medicine group (betahistine at 0.1 mg·kg-1·d-1). After eight weeks of intervention, the gene and protein expressions in cochlear tissue were detected. Results①A total of 2 831 chemical components and 173 components absorbed into blood were identified, with terpenoids showing the highest absorption rate into blood(10.28%). ②60 common drug-disease targets were screened, with core targets including tumor necrosis factor-α(TNF-α),interleukin-6(IL-6), Toll-like receptor 4(TLR4), angiotensin-converting enzyme(ACE), and endothelial nitric oxide synthase 3(NOS3).These targets were enriched in the nuclear factor-κB(NF-κB) signaling pathway and renin-angiotensin system(P<0.05). Molecular docking showed that the active component YC-1 had a strong binding ability to TNF(binding energy-9.66 kcal·mol-1). ③In animal experiments, the high-dose Chinese medicine group significantly down-regulated the expression of pro-inflammatory factors TNF mRNA(P<0.01)and up-regulated vascular regulatory factors NOS3 protein(P<0.01), and alleviated cochlear pathological damage[hematoxylin eosin (HE) score: from 4 to 2]. ConclusionThis compound recipe synergistically regulates the TNF/NF-κB inflammatory pathway and ACE/NOS3 vascular homeostasis pathway through flavonoids, triterpenoids, and other components, thereby inhibiting endolymphatic hydrops and cochlear damage. It provides a scientific basis for the theory of "simultaneous treatment of phlegm and blood stasis" in traditional Chinese medicine.
2.Association of urinary cadmium levels with peripheral leukocyte classification counts among middle-aged and older adults aged 40-89 in selected areas of China

Objective:To investigate the association of urinary cadmium levels with peripheral leukocyte classification counts among middle-aged and older adults aged 40 to 89 years in selected areas of China.Methods:The research was based on the survey of the impact of soil quality of agricultural land on human health in typical areas conducted in 2019-2020. A total of 5 600 middle-aged and older adults aged 40 to 89 years were included by using a multi-stage stratified random sampling method. Baseline characteristics of the subjects were collected and physical examinations were performed. Random midstream urine was collected to measure urinary cadmium and urinary creatinine and fasting venous blood was collected to measure the leukocyte count, neutrophil count, lymphocyte count, monocyte count and eosinophil count. The linear mixed effect model was used to analyse the association of urinary cadmium levels with leukocyte classification counts, and the dose-response relationship between them was analyzed by using the restricted cubic spline (RCS) function.Results:The age of the subjects was (63.17±12.02) years; 2 851 (50.91%) were males; and the M ( Q 1, Q 3) of urinary creatinine-corrected urinary cadmium levels was 2.69 (1.52, 4.69) μg/g·creatinine. After adjusting for confounding factors, the results of linear mixed effects model analysis showed that for each 1-unit increase in urinary creatinine-corrected urinary cadmium level, the percentage change [% (95% CI)] of leukocyte count and lymphocyte count was -1.70% (-2.61%, -0.79%) and -1.57% (-2.86%, -0.26%), respectively. RCS function showed a negative linear relationship between urinary creatinine-corrected urinary cadmium levels and leukocyte counts and lymphocyte counts, respectively (all Pnon-linear>0.05). Conclusion:Urinary cadmium levels are negatively associated with leukocyte count and lymphocyte count among middle-aged and older adults aged 40 to 89 years in selected areas of China.

Objective:To investigate the diagnostic accuracy of serological indicators and evaluate the diagnostic value of a new established combined serological model on identifying the minimal hepatic encephalopathy (MHE) in patients with compensated cirrhosis.Methods:This prospective multicenter study enrolled 263 compensated cirrhotic patients from 23 hospitals in 15 provinces, autonomous regions and municipalities of China between October 2021 and August 2022. Clinical data and laboratory test results were collected, and the model for end-stage liver disease (MELD) score was calculated. Ammonia level was corrected to the upper limit of normal (AMM-ULN) by the baseline blood ammonia measurements/upper limit of the normal reference value. MHE was diagnosed by combined abnormal number connection test-A and abnormal digit symbol test as suggested by Guidelines on the management of hepatic encephalopathy in cirrhosis. The patients were randomly divided (7∶3) into training set ( n=185) and validation set ( n=78) based on caret package of R language. Logistic regression was used to establish a combined model of MHE diagnosis. The diagnostic performance was evaluated by the area under the curve (AUC) of receiver operating characteristic curve, Hosmer-Lemeshow test and calibration curve. The internal verification was carried out by the Bootstrap method ( n=200). AUC comparisons were achieved using the Delong test. Results:In the training set, prevalence of MHE was 37.8% (70/185). There were statistically significant differences in AMM-ULN, albumin, platelet, alkaline phosphatase, international normalized ratio, MELD score and education between non-MHE group and MHE group (all P<0.05). Multivariate Logistic regression analysis showed that AMM-ULN [odds ratio ( OR)=1.78, 95% confidence interval ( CI) 1.05-3.14, P=0.038] and MELD score ( OR=1.11, 95% CI 1.04-1.20, P=0.002) were independent risk factors for MHE, and the AUC for predicting MHE were 0.663, 0.625, respectively. Compared with the use of blood AMM-ULN and MELD score alone, the AUC of the combined model of AMM-ULN, MELD score and education exhibited better predictive performance in determining the presence of MHE was 0.755, the specificity and sensitivity was 85.2% and 55.7%, respectively. Hosmer-Lemeshow test and calibration curve showed that the model had good calibration ( P=0.733). The AUC for internal validation of the combined model for diagnosing MHE was 0.752. In the validation set, the AUC of the combined model for diagnosing MHE was 0.794, and Hosmer-Lemeshow test showed good calibration ( P=0.841). Conclusion:Use of the combined model including AMM-ULN, MELD score and education could improve the predictive efficiency of MHE among patients with compensated cirrhosis.
5.Comorbidity of myopia and obesity and the moderating role of lifestyle among primary and secondary school students in Inner Mongolia Autonomous Region in 2021

Objective:
To describe the current status of the prevalence of co-morbid myopia and obesity among 7-18 years students in the Inner Mongolia Autonomous Region in 2021 and to analyze the moderating effect of lifestyle in this association ,so as to provide scientific basis for the establishment of the mechanism of Co-morbidity,Shared Etiology,and Shared Prevention of common diseases in children and adolescents.
Methods:
A total of 139 630 primary and secondary school students aged 7-18 years from Inner Mongolia Autonomous Region were selected by stratified random cluster sampling method in September,2021. Myopia was determined using distance visual acuity examination and refractive error examination, and obesity was determined according to the BMI classification criteria for overweight, obesity screening of Chinese school age children and adolescents. Used a questionnaire, healthy lifestyles were determined according to the American Heart Association s Healthy Lifestyle Score by totaling the six scores for smoking, alcohol consumption, diet, exercise, screen time, and sleep duration.The χ 2 test was used to compare the association between group differences in the co-morbidity rate of myopia and obesity. The multivariable Logistic regression model was used to explore the influencing factors of the co-morbidity of myopia and obesity, and the stratified analysis was used to analyze the moderating effect of lifestyles on the prevalence of the co-morbidity.
Results:
The prevalence of myopia and obesity co-morbidity among students aged 7-18 years old in the Inner Mongolia Autonmous Region in 2021 was 13.7%, higher among boys than girls ( 15.5 % vs. 11.8%), higher among those aged 10-12 years old than 7-9,13-15,and 16-18 years old (14.7%,13.7%, 13.3%, 12.0%), higher among other ethnic minorities than Han Chinese and Mongolians (15.3%, 14.0%, 12.5%), higher in urban areas than that in suburban areas(15.3%, 13.0%), and middle economic level tracts were higher than poor and good tracts (14.8%, 12.9 %, 12.6%) ( χ 2=392.37,115.73,62.80,119.02,121.60, P <0.05). Multivariable Logistic regression modeling showed that unhealthy lifestyles ( OR=1.24, 95%CI=1.19-1.29 ) and middle level of lifestyle score ( OR=1.15, 95%CI=1.10-1.19 ) students had higher prevalence of co-morbidity, and the results were statistically significant among both boys and girls, the age groups of 10- 12, 13-15, and 16-18 years old, as well as the Han and Mongolian ethnic groups (all P <0.05).
Conclusion
In 2021, the current situation of myopia and obesity co-morbidity and unhealthy lifestyles among primary and secondary school students in the Inner Mongolia Autonomous Region are not optimistic.
6.Analysis of hospital management projects funded by National Natural Science Foundation of China during 2010—2020

Objective:To analyze the characteristics and hot spots of hospital management projects funded by National Natural Science Foundation of China(NSFC), for the reference for furthering improving the modernization level of hospital management.Methods:The NSFC funded research projects in hospital management during 2010-2020 was searched. Project name, keywords, approval year, affiliation and other informations were obtained. CiteSpace was used for bibliometric analysis of these research projects, while all the data acquired were subject to descriptive analysis and frequency analysis.Results:The number of hospital management projects funded by NSFC increased from 4 in 2010(0.93 million yuan) to 32(9.94 million yuan) in 2020, totaling 192 projects and 72.76 million yuan. These numbers consisted of 90 general projects(46.9%) and 79(41.1%) youth fund projects respectively. All the funded projects were distributed in sub-disciplines under the NSFC first-level discipline of macro management and policy, namely health management and policy(154) and medical/drug management and policy(38). The hotspots of the funded projects covered performance evaluation, payment method, public health system, health economics, hierarchical diagnosis and treatment, utility theory, regional medical alliance, essential medicine system, capitation and incentive strategy, and so on.Conclusions:Recent years are witnessing a rising trend in the number and grants of NSFC projects on hospital management. More and more funds are granted to universities, especially young researchers, and a hospital management research system has been initially taken shape. Hospital management in China should take into full account of the management characteristics and the status quo of the medical and health field, focus on performance appraisal incentive mechanism, hierarchical medical system, construction of medical alliances. and patient medical experience as well. In addition, the cooperation between hospitals and universities should be strengthened in order to promote the translation of research results.
7.Study on the psychologic status and personality traits of patients with obstructive sleep apnea hypopnea syndrome

Objective To explore the personality and character of obstructive sleep apnea hypopnea syndrome (OSAHS) patients.Methods Subjects,recruited from May 2013 to February 2014,were assigned to the severe OSAHS group (56 cases),mild-moderate OSAHS group (59 cases),and control group(42 cases) on the basis of apnea hyponea index (AHI).Subjects were assigned to the severe hypoxemia group(24 cases),mild-moderate hypoxemia group (91 cases) on the basis of PaO2.The psychological aspects of subjects were assessed by using the Minnesota multiphasic personality inventory (MMPI).Results Compared between OSAHS group and the control group,differences of 6 clinical scales depression (D),hysteria (Hy),masculinity(Mf),paranoia (Pa),anxiety (A),ego strength (Es) were signifieant(t value was 2.609,2.133,-2.294,2.520,2.041,2.675 respectively,all P < 0.05).The scores of OSAHS group were higher than the control group on five clinical scales,depression (D),hysteria (Hy),paranoia (Pa),anxiety (A),ego strength (ES).The scores of OSAHS group were lower than the control group on clinical scale masculinity (Mf).Compared between severe OSAHS group and mild-moderate OSAHS group,differences of 6 clinical scales depression (D),paranoia (Pa),psychasthenia (Pt) anxiety (A),manifest anxiety scale (MAS),dependency (Dy) were significant (t value was 2.460,2.086,2.181,2.121,2.954,1.982,respectively).The scores of severe OSAHS group were all higher than the mild-moderate OSAHS group on these six clinical scales.Compared between severe hypoxemia group and the contrast group,differences of 4 clinical scales depression (D),masculinity (Mf),paranoia (Pa),ego strength (Es) were significant (t value was respectively 2.992,-2.221,2.164,2.165,all P < 0.05).The scores of severe hypoxemia group were higher than the control group on 3 clinical scales,depression (D),paranoia (Pa),ego strength (ES),and lower than the control group on clinical scale masculinity (Mf).Compared between severe hypoxemia group and mild-moderate hypoxemia group,psychasthenia (Pt) were significant(t value was 1.984).The scores of severe hypoxemia group were higher.Conclusions Compared with health people,OSAHS patients have special personality and character.The degree of OSAHS can infect the personality and character of OSAHS patients.
