Diabetes mellitus is a chronic metabolic disorder, currently affecting more than 530 million people worldwide. Based on differences in pathogenesis, it is classified into type 1 diabetes mellitus (T1DM) and type 2 diabetes mellitus (T2DM). T1DM is primarily caused by autoimmune destruction of pancreatic β cells, whereas T2DM results from insulin resistance due to β cell failure and dysfunction. Current treatments for diabetes include insulin injections, oral medications, and islet transplantation. As a curative therapy, islet transplantation still faces challenges such as limited donor cell availability, systemic immunosuppression-related side effects, and low long-term survival of transplanted islets. Transplantation of stem cell-derived islet organoids is expected to replace primary human islet cells and overcome these issues, making it one of the most promising therapies for T1DM and advanced-stage T2DM. This review provides an in-depth overview of the development of islet transplantation, with a focus on recent progress in stem cell differentiation into islet organoids and the engineering of islet organoids. Meanwhile, we also discuss the current opportunities and future challenges in islet organoid transplantation. These explorations and efforts lay the groundwork for future clinical applications and offer hope for a potential cure for diabetes.

3.Electroacupuncture improves morphological structure of the detrusor muscle and bladder function in rats with spinal cord injury

BACKGROUND:Numerous clinical and basic studies have shown that electroacupuncture can improve the function of neurogenic bladder after suprasacral spinal cord injury. OBJECTIVE:To observe the effects of electroacupuncture on bladder function and connective tissue growth factor expression in rats with suprasacral spinal cord injury. METHODS:Forty-eight female Sprague-Dawley rats were randomly divided into four groups(n=12 per group):the blank group did not receive any treatment;the sham-operated group only exposed the T8 subvertebral spinal cord;in the model group established,a T8 subvertebral spinal cord transection injury model was established;in the electroacupuncture group,the T8 subvertebral spinal cord transection injury model was established,and electroacupuncture intervention at Ciliao(BL32),Zhongji(RN03)and Sanyinjiao(SP06)was given at 19 days after modeling,20 minutes once a day,for 10 continuous days.After the intervention,the relevant indicators were detected. RESULTS AND CONCLUSION:Urodynamics:Compared with the blank group,the leakage point pressure,maximum bladder capacity and maximum bladder pressure of rats in the model group increased(P<0.05).Compared with the model group,the leakage point pressure,maximum bladder capacity and maximum bladder pressure of rats in the electroacupuncture group decreased(P<0.05).Hematoxylin-eosin staining:Compared with the blank group,the bladder epithelial cells in the model group were arranged in a disordered manner,the lamina propria was destroyed,the detrusor muscle bundles were hypertrophied,the muscle fibers were arranged in a disordered manner,and the tissue edema was obvious.Compared with the model group,the bladder epithelial cells in the electroacupuncture group were arranged in a regular and orderly manner,and the degree of bladder fibrosis and tissue edema was relatively reduced.Masson staining:The degree of bladder detrusor muscle fibrosis was severe in the model group and it was lighter in the electroacupuncture group than in the model group.Transmission electron microscopy:Mitochondria in the bladder tissue in the model group were swollen and vacuolated,the morphology of the detrusor muscle was twisted and distorted,and the muscle gap was widened.Compared with the model group,mitochondria in the electroacupuncture group had a slightly clearer contour and were less vacuolated,and the muscle gap was narrowed.Western blot detection:The protein expression of connective tissue growth factor in the detrusor muscle of the bladder was elevated in the model group compared with the blank group(P<0.05).Compared with the model group,the protein expression of connective tissue growth factor in the bladder detrusor muscle was decreased in the electroacupuncture group(P<0.05).To conclude,electroacupuncture at Ciliao(BL32),Zhongji(RN03)and Sanyinjiao(SP06)acupoints can improve the morphology,structure and function of the bladder in rats with suprasacral spinal cord injury,and the mechanism of action may be related to the down-regulation of connective tissue growth factor protein expression in the detrusor muscle.

6.Meta-analysis of Efficacy and Safety of Single or Dual Antiplatelet Therapy for Short-term Complications after Transcatheter Aortic Valve Implantation

OBJECTIVE: To systematically evaluate effectiveness and safety of single antiplatelet therapy (SAPT) versus dual antiplatelet therapy (DAPT) on short-term complications after transcatheter aortic valve implantation (TAVI), and to provide evidence-based reference for clinical treatment. METHODS: Retrieved from PubMed, Cochrane clinical controlled trials registry, Web of Science, CNKI, Wanfang database, CBM and Chinese Clinical Trial Registry, RCTs and observational studies about effectiveness (all-cause mortality, incidence of stroke and incidence of myocardial infarction 30 days after operation) and safety (the incidence of bleeding events at 30 days after operation) of SAPT versus DAPT on short-term complications of TAVI were collected during the date of database establishment to Jan. 2019. After data extraction of included studies and quality evaluation with Cochrane system evaluator manual 5.1.0 (for RCT) and the Newcastle-Ottawa Scale (NOS) (for observational studies), Meta-analysis was conducted by using Rev Man 5.3 statistical software. RESULTS: Totally 3 RCTs and 7 cohort studies were included, involving 3 188 patients. Results of Meta-analysis showed that the incidence of all-cause mortality 30 days after operation [OR=0.48, 95% CI (0.32, 0.73), P<0.001] and the incidence of bleeding events 30 days after operation [OR=0.43, 95%CI (0.30, 0.59), P<0.001] in SAPT group were significantly lower than DAPT group, with statistical significance. There was no statistical significance in the incidence of stroke 30 days after operation [OR=0.63, 95%CI (0.38, 1.06) , P=0.08] or the incidence of myocardial infarction 30 days after operation [OR=1.09, 95%CI (0.46, 2.59), P=0.85] between 2 groups. CONCLUSIONS: Compared with DAPT, SAPT can decrease the incidence of all-cause mortality 30 days after TAVI and the incidence of bleeding events 30 days after TAVI.

Blood were sampled to analyze the lead level. Neurobehavioral tests were performed to determine their personal cognitive and memory ability. Questionnaires and physical examinations were administered to obtain the information of confounding factors. All students were divided into Q1-Q4 groups according to the quartile of their blood lead level. The relationship between the blood lead level and behavior was analyzed by multivariate logistic regression model and restricted spline regression model. Results The mean age of 697 students was (10.07±1.36) years old, and the median (interquartile range) of their blood lead level was 44.31 (35.42) μg/L. Multivariate logistic regression model showed that after adjusting for age, gender, body mass index and maternal culture level, compared with Q1 group, the OR (95%CI ) values of high digit symbol substitution test (DSST) scores and high overall memory quotient (MQ) scores in Q 3 group were 1.65 (1.01-2.70) and 2.10 (1.21-3.62), and the OR (95%CI ) value of high long term memory (LTM) scores in Q4 group was 0.53 (0.31-0.92). The results of the restricted spline regression model showed that the dose?response curves between the blood lead level and MQ/LTM test scores were both parabolic (P<0.05). Conclusion The blood lead level of school?age children from rural areas of Chongqing is the same as that from other areas of China, but slightly higher than that from other areas of Chongqing. Children with higher blood lead level have poor long?term memory ability.

Objective:
To investigate the effect of hypertensive disorder complicating pregnancy (HDCP) on the mortality and early complications of premature infants.
Methods:
The general clinical data of preterm infants with gestational age 24-36+ 6 weeks were collected from the cooperative units in the task group from January 1, 2013 to December 31, 2014.According to the severity of HDCP, the infants were divided into 4 groups: HDCP group, preeclampsia group, eclampsia group and non HDCP group, the mortality and major complications of preterm infants were compared, and the influencing factors were analyzed.
