2.Association between skeletal muscle function, insulin resistance and type 2 diabetes mellitus

Objective:To investigate the association of skeletal muscle function(including muscle strength and physical performance) and insulin resistance as well as type 2 diabetes mellitus(T2DM).Methods:The retrospective study included 942 patients who visited the Department of Geriatrics at Renji Hospital, Shanghai Jiao Tong University School of Medicine, between October 2020 and July 2024. Low skeletal muscle function was defined as either reduced muscle strength or impaired physical performance. Muscle strength was assessed by grip strength, while physical performance was evaluated using the 5-time chair stand test. The associations between muscle function, insulin resistance, and T2DM were analyzed.Results:A significant association was observed between decreased skeletal muscle function and a higher prevalence of T2DM( P=0.001). Further analysis revealed that decreased physical performance was significantly associated with increased T2DM prevalence( P<0.001), whereas reduced muscle strength showed no significant association with T2DM prevalence( P=0.331). Linear regression analysis indicated that both the homeostasis model assessment of insulin resistance(HOMA2-IR) and fasting blood glucose levels increased significantly with longer chair stand times( P<0.05). Restrictive cubic spline(RCS) analysis demonstrated a nonlinear relationship between chair-rising time and HOMA2-IR. Notably, when the cumulative chair-rising time exceed 8.1 s, HOMA2-IR increased significantly with prolonged chair stand time. Logistic regression analysis showed that compared with patients with normal physical performance, those with decreased physical performance had significantly higher odds of T2DM( OR=2.64, P<0.001) and insulin resistance( OR=2.34, P=0.002). Conclusion:Decline in physical performance is significantly positively associated with insulin resistance and the risk of T2DM. Morever, when the cumulative chair stand time exceed 8.1 s, HOMA2-IR increases progressively with further prolongation of chair stand time.
3.Role of Mfn2-mediated mitochondrial homeostasis in hypoxia-reoxygenation injury in human colorectal adenocarcinoma cells

Objective:To evaluate the role of mitochondrial fusion protein 2 (Mfn2)-mediated mitochondrial homeostasis in hypoxia-reoxygenation injury in human colorectal adenocarcinoma cells.Methods:Human colorectal adenocarcinoma cell line Caco-2 was cultured in vitro and divided into 4 groups ( n=30 each) using a random number table method: control group (C group), hypoxia-reoxygenation group (HR group), transfection with negative control virus plus hypoxia-reoxygenation group (Vector+ HR group) and transfection with Mfn2 plus hypoxia-reoxygenation group (Mfn2+ HR group). Mfn2 was packaged by lentivirus and transfected into Caco-2 cells, and the hypoxia-reoxygenation model was established through exposing cells to hypoxia for 12 h followed by 2 h reoxygenation. The cell viability and levels of lactate dehydrogenase (LDH) were measured. The expression of Mfn2, Occludin, Claudin-1, E-cadherin and microtubule-associated protein 1 light chain 3 Ⅰ (LC3Ⅰ), LC3Ⅱ, P62, translocase of outer mitochondrial membrane (TOM20) and cytochrome c oxidase Ⅳ (COX Ⅳ) was detected by Western blot. The levels of ATP content and ROS were determined. Results:Compared with C group, the cell viability and ATP content were significantly decreased, the LDH activity and ROS level were increased, the expression of Mfn2, Occludin, Claudin-1 and E-cadherin was down-regulated, the expression of LC3Ⅱ was up-regulated, and the expression of P62, TOM20 and COX Ⅳ was down-regulated in HR group ( P<0.05). Compared with HR group, the cell viability and ATP content were significantly increased, the LDH activity and ROS level were decreased, the expression of Mfn2, Occludin, Claudin-1 and E-cadherin was up-regulated, the expression of LC3Ⅱ was down-regulated, and the expression of P62, TOM20 and COX Ⅳ was up-regulated in Mfn2+ HR group ( P<0.05), and no significant change was found in the aforementioned parameters in Vector+ HR group ( P>0.05). Conclusions:Mfn2 may alleviate hypoxia-reoxygenation injury in human colorectal adenocarcinoma cells by maintaining mitochondrial homeostasis.

6.Etiological analysis of influenza-like cases in Taizhou City, Zhejiang Province from 2013 to 2022

ObjectiveTo analyze the etiological results of influenza-like case surveillance in Taizhou, Zhejiang Province from 2013 to 2022, to timely understand the epidemic trend of influenza viruses and the change rule of dominant virus strains, and to provide reference for the prevention and control of influenza in this region. MethodsInfluenza virus nucleic acid was detected by real-time PCR in 24 183 influenza-like cases. ResultsThe positive rate of influenza virus in 24 183 samples was 18.43%, the highest positive type was seasonal H3 (37.34%). There was no a significant difference in positive rate between different genders (χ2=0.148, P=0.701). There was significant difference in the positive rate among different age groups (χ2=496.626, P<0.001), and the highest positive rate was found in the 25‒59 age group (22.56%). The positive rate in different years was statistically significant (χ2=1 670.922, P<0.001). The positive rate from 2013 to 2019 showed an upward trend (χ2=30.559, P<0.001). The lowest positive rate was in 2020 (0.04%), and the positive rate from 2021 to 2022 showed an upward trend (χ2=304.465, P<0.001). The dominant strains were different in different monitoring years. There was a significant difference in the positive rate of influenza in different months (χ2=1 652.455, P<0.001), and the peak of influenza was mainly concentrated in December‒March and July‒August. ConclusionFrom 2013 to 2022, the positive rate of influenza virus in Taizhou showed a wavy dynamic change, and the dominant strains were different in different years, presenting alternate epidemic characteristics. It is necessary to strengthen the etiological surveillance of influenza virus and improve the prevention and control measures with influenza vaccine.
7.Association of skeletal muscle fat index with insulin resistance and type 2 diabetes mellitus

Objective:To investigate the association of skeletal muscle fat index(SMFI) with insulin resistance and type 2 diabetes mellitus.Methods:This retrospective study included 1 100 patients from Department of Geriatric, Renji Hospital, Shanghai Jiao Tong University School of Medicine from October 2020 to October 2023. The SMFI measured by chest computed tomography(CT) is used as an indicator to evaluate the quality of skeletal muscle. Spearman and linear regression analysis were used to evaluate the association between SMFI and homoeostasis model assessment for insulin resistance(HOMA-IR). Multivariate logistic regression analysis and restricted cubic spline(RCS) were used to analyze the association of HOMA-IR with the prevalence of type 2 diabetes mellitus. Finally, interaction and stratified analyses were conducted according to age, sex, body mass index, estimated glomerular filtration rate, uric acid, skeletal muscle index(SMI), smoking, alcohol drinking, hypertension, and dyslipidemia.Results:SMFI showed a significant positive correlation with HOMA-IR( r=0.385, P<0.001). Each 1-unit increase in SMFI led to 0.009, 0.011, 0.004, 0.004, and 0.004 rise in HOMA-IR across adjusted models(all P<0.05). Then subjects were stratified into tertiles( T1, T2, and T3) according to SMFI. Logistic regression analysis revealed that the prevalence of type 2 diabetes mellitus significantly increased in T2( OR=2.37, 95% CI 1.30-4.31, P=0.005) and T3( OR=2.85, 95% CI 1.39-5.86, P=0.004) compared to T1. RCS analysis showed that the prevalence of type 2 diabetes mellitus increased with the increase of SMFI when SMFI<157.195. The results of subgroup analysis showed that female, under 60 years old, body mass index<24 kg/m 2, uric acid<420 μmol/L, low SMI, non-smoking, non-alcoholic drinking, and non-hypertensive individuals with higher SMFI were more likely to develop type 2 diabetes mellitus. Conclusions:Elevated SMFI is positively associated with insulin resistance and type 2 diabetes mellitus risk. Reducing SMFI below 157.195 may significantly lower the risk of type 2 diabetes mellitus.

9.Tracheal diverticula discovered during surgery: a report of 2 cases and literature review

To analyze the reasons why tracheal diverticula is easy to be missed or misdiagnosed in the diagnosis and treatment of thyroid tumors, and to strengthen the understanding of the clinical characteristics of the disease. The reasons of the misdiagnosis and missed diagnosis in similar cases were analyzed, and the anatomy, differential diagnosis, and examination methods by reviewing the relevant literature in the past 20 years were further analyzed. At the same time, a retrospective analysis was carried out on two recent clinical cases of tracheal diverticula discovered during surgery. Tracheal diverticula is easily confused with thyroid tumor and may be misdiagnosed for the following reasons: tracheal diverticula is asymptomatic in most patients; symptomatic tracheal diverticula has similar clinical symptoms to thyroid nodules; lack of character in imaging findings. General surgeons should improve their awareness and vigilance of tracheal diverticula. Neck CT should be listed as a routine examination before thyroid-related surgery.
10.Etiological analysis of acute respiratory tract infection in Taizhou of Zhejiang Province from 2020 to 2021

ObjectiveWe examined the principal respiratory pathogens in patients with acute respiratory tract infection in Taizhou, Zhejiang Province during 2020‒2021 to provide evidence for prevention, diagnosis and treatment of acute respiratory tract infection. MethodsFrom September 2020 to August 2021, a total of 2 831 cases with acute respiratory tract infection were collected from two influenza sentinel surveillance hospitals in Taizhou, which had then received the examination of 22 respiratory pathogens by multiple fluorescence quantitative PCR. ResultsThe total positive rate of respiratory pathogens in 2 831 samples was 14.13%, among which enterovirus (7.77%) and respiratory syncytial virus (1.59%) were the principal pathogens. Except enterovirus, there was no significant difference in the positive rate of pathogens detected by gender(P>0.05). Moreover, there was significant difference in pathogens by age (P<0.05), with the highest positive rate in 0‒4 years(35.21%). There was also significant difference in pathogens by seasons (P<0.05), with the highest positive rate in summer(20.54%). ConclusionThe positive rate of acute respiratory tract infection decreases significantly, compared with that before the COVID-19 epidemic. The differences in the positive rate differ significantly by age and seasons. Comprehensive consideration of diverse factors before diagnosis and the utilization of multiple fluorescent quantitative PCR can quickly and effectively determine the pathogens in the early stage of infection. Our findings may provide certain support for the diagnosis and treatment of acute respiratory infections in the context of COVID-19 in Taizhou.
