BACKGROUND:
Death burden of stroke is severe with over one-third rural residents in China, but there is still a lack of specific national and high-quality reports on the urban-rural differences in stroke burden, especially for subtypes. We aimed to update the understanding of urban-rural differences in stroke deaths.
METHODS:
This is a descriptive observational study. Data from the national mortality surveillance system, which covers 323.8 million with 605 disease surveillance points (DSPs) across all 31 provinces, municipalities, and autonomous regions in China. All deaths from stroke as the underlying cause from 2015 to 2020 according to DSPs. Crude mortality rate and age-standardized mortality rate (ASMR) were estimated through DSPs. Average annual percentage change was used to explain the change in mortality rate.
RESULTS:
From 2015 to 2020, the majority of deaths from all stroke subtypes occurred in rural areas. There were significant differences between the changes of urban and rural ASMRs. On the whole, the changes in urban areas were evidently better, and the ASMR differences were basically expanding. Stroke ASMR in urban China decreased by 15.5%. The rural ASMR of ischemic stroke increased by 12.9%. The rural and urban ASMRs of intracerebral hemorrhage decreased by 24.9% and 27.4%, and those of subarachnoid hemorrhage decreased by 29.5% and 40.4%, respectively. The highest ASMRs of all stroke subtypes and the increasing trend of ischemic stroke ASMR make rural males the focus of stroke management.
CONCLUSIONS
The death burden of stroke varies greatly between urban and rural China. Rural residents face unique challenges.

Objective To observe the impact factors of obesity(Ob)complicated with type 2 diabetes mellitus(T2DM).Methods Twenty Ob patients complicated with T2DM(group A)and 47 simple Ob patients(group B)were retrospectively enrolled. The blood glucose related indicators, including fasting blood glucose(FBG),fasting insulin(FINS)and glycosylated hemoglobin(HbA1c),homeostasis model assessment of insulin resistance(HOMA-IR),as well as fat quantitative parameters measured with dual-energy X-ray absorptiometry,CT or MRI were compared between groups, their correlations were analyzed, and the impact factors of Ob complicated with T2DM were explored. Results FBG,HbA1c and HOMA-IR in group A were all higher, while the total body fat mass percentage(TFM%)and leg fat mass percentage(LFM%)in group A were both lower than those in group B(all P<0.05).Proton density fat fraction(PDFF)of liver and lumbar bone marrow in group A were both higher than those in group B(both P<0.05).TFM%and LFM%in all 67 Ob patients were negatively correlated with FBG or HbA1c,while TFM%and Android region fat mass percentage(Android FM%)were both negatively correlated with HOMA-IR(r=-0.447—-0.263,all P<0.05).For all 67 Ob patients, the area of visceral adipose tissue(VAT)was positively correlated with HOMA-IR(r=0.339,P=0.006),liver PDFF was positively correlated with FBG,HbA1c and HOMA-IR,and PDFF of lumbar bone marrow was positively correlated with FBG(r=0.323-0.599,all P<0.05).Elevated LFM%was a protective factor, while elevated PDFF of liver and pancreas were both risk factors of Ob complicated with T2DM(all P<0.05).Conclusion LFM%and liver PDFF were both impact factors of Ob complicated with T2DM.

Objective:Osteosarcoma is a highly aggressive primary malignant bone tumor commonly seen in children and adolescents, with a poor prognosis.Anchorage-dependent cell death(anoikis)has been proven to be indispensable in tumor metastasis, regulating the migration and adhesion of tumor cells at the primary site. However,as a type of programmed cell death, anoikis is rarely studied in osteosarcoma, especially in the tumor immune microenvironment. This study aims to clarify prognostic value of anoikis and tumor immune microenvironment-related gene in the treatment of osteosarcoma. Methods:Anoikis-related genes(ANRGs)were obtained from GeneCards.Clinical information and ANRGs expression profiles of osteosarcoma patients were sourced from the therapeutically applicable research to generate effective therapies and Gene Expression Omnibus(GEO)databases.ANRGs highly associated with tumor immune microenvironment were identified by the estimate package and the weighted gene coexpression network analysis(WGCNA)algorithm.Machine learning algorithms were performed to construct long-term survival predictive strategy, each sample was divided into high-risk and low-risk subgroups, which was further verified in the GEO cohort. Finally, based on single-cell RNA-seq from the GEO database, analysis was done on the function of signature genes in the osteosarcoma tumor microenvironment. Results:A total of 51 hub ANRGs closely associated with the tumor microenvironment were identified, from which 3 genes(MERTK,BNIP3,S100A8)were selected to construct the prognostic model.Significant differences in immune cell activation and immune-related signaling pathways were observed between the high-risk and low-risk groups based on tumor microenvironment analysis(all P<0.05).Additionally,characteristic genes within the osteosarcoma microenvironment were identified in regulation of intercellular crosstalk through the GAS6-MERTK signaling pathway. Conclusion:The prognostic model based on ANRGs and tumor microenvironment demonstrate good predictive power and provide more personalized treatment options for patients with osteosarcoma.

Chinese Journal of Preventive Medicine 2024;58(6):910-916
Antimicrobial resistance poses a serious threat to public health and is one of the major challenges worldwide. As global social, economic, and environmental changes lead to increased exposure of populations to antimicrobials, the antimicrobial resistance of pathogens has accelerated and resulted in weakened clinical infection treatment effects. This article reviews the main mechanisms and driving factors of the production and spread of antimicrobial resistance from the perspective of "One Health"and discusses methods and strategies for controlling antimicrobial resistance from multiple dimensions. It also looks forward to the prospects of research and prevention of drug resistance to explore antimicrobial resistance prevention and control strategies based on "One Health".
