1.Comprehensive management ability of Helicobacter pylori screening in primary hospitals of Jiangsu Province

Objective:Based on the activities of Helicobacter pylori ( HP) screening in Jiangsu Province in 2024, to evaluate the overall management ability in HP screening, testing, treatment and follow-up in primary medical facilities. Methods:From May 15 to October 18, 2024, the data of HP screening and treatment were retrospectively collected from 79 township health centers, community hospitals, and community health service centers in Jiangsu Province. The rates of screening completion, urea breath test (UBT) completion, treatment rate, UBT follow-up completion, and HP eradication were analyzed. Chi-square test was used for statistical analysis. Results:The completion rate of HP screening was 94.45% (15 489/16 400). There were 6 604 cases (42.64%) with serum HP antibody positive among the 15 489 individuals who completed screening. The positive rate of serum HP antibody in males was higher than that in females (44.77%, 2 643/5 904 vs. 41.32%, 3 961/9 585), and the difference was statistically significant ( χ2=17.69, P<0.001). The positive rates of serum HP antibody in screened individuals aged 18 to 19, 20 to 39, 40 to 59, and 60 to 75 years old were 22.38% (32/143), 36.12% (1 168/3 234), 45.01% (3 240/7 199), and 44.05% (2 164/4 913), respectively, and the difference was statistically significant( χ2=100.73, P<0.001). Among the 6 604 HP antibody-positive individuals, 4 381 cases completed UBT, with a UBT completion rate of 66.34% (4 381/6 604). There were 3 197 individuals with both HP serum antibody and UBT positive, the consistency rate of the 2 tests was 72.97% (3 197/4 381). Totally 2 737 cases received treatment, with a treatment completion rate of 85.61% (2 737/3 197); 2 327 individuals underwent UBT follow-up, with a follow-up completion rate of 85.02% (2 327/2 737). During follow-up, the result of UBT was negative in 1 982 individuals, and the HP eradication rate was 85.17% (1 982/2 327). Conclusions:There are deficiencies in the completion rate of HP screening, testing, treatment, and follow-up in primary hospitals, especially in the completion rate of UBT, which may be related to cognitive insufficiency for HP in residents. It is necessary to strengthen the training of physicians′ abilities in primary hospitals, optimize the allocation of drug resources, enhance health education, and increase residents′ participation and compliance.
2.Clinical correlation of P2X7R and NLRP3 inflammasomes in prostatic hyperplasia tissue

Objective The aim of this study is to detect the expression of purinergic 2X7 receptor(P2X7R)and the NLRP3 inflammasome in benign prostatic hyperplasia(BPH)tissues and to analyze the clinical correlations.Methods Twelve patients undergoing surgery for BPH were enrolled.Based on the presence or absence of inflammatory cell infiltration in HE-stained tissue sections,the patients were divided into inflammation group and non-inflammation group.Preoperative routine ex-aminations excluded surgical contraindications,and International Prostate Symptom Score(IPSS)questionnaires,urinary flow rate measurements,expressed prostatic secretions(EPS)analysis,and prostate-specific antigen(PSA)tests were conducted.Prostate tissues obtained during surgery were subjected to HE staining,immunofluorescence/Western blot to detect the expression of NLRP3,P2X7R,Caspase-1,Cleaved-Caspase-1,IL-1 β,and TNF-α,and immunofluorescence to assess lymphocyte infiltra-tion.SPSS 26.0 software was used to analyze correlations between the expression levels of NLRP3 and P2X7R in prostate tissues and indicators including Caspase-1,Cleaved-Caspase-1,IL-1 β,TNF-α,lymphocyte count,IPSS score,urinary flow rate,EPS,and PSA.Results In BPH tissues,the expression levels of NLRP3 and P2X7R were positively correlated(P<0.05).The ex-pression levels of NLRP3 and P2X7R were positively correlated with Caspase-1,Cleaved-Caspase-1,IL-1 β,TNF-α,and lym-phocyte count(P<0.05).NLRP3 and P2X7R expression levels were positively correlated with white blood cell count in EPS,but showed no correlation with IPSS score,lecithin body count in EPS,maximum urinary flow rate and PSA(P<0.05).Conclusion P2X7R and NLRP3 in prostate tissues exacerbate local inflammatory responses,which may be an important mecha-nism in BPH development.However,they are not correlated with IPSS score,lecithin body count in EPS,maximum urinary flow rate and PSA.

5.Thoughts and diagnostic strategies of traditional Chinese medicine in diagnosing and treating male infertility

The pathogenesis of male infertility is intricate and complex,and now,Western medicine is primarily based on empirical drug treatment.Male infertility is one of the advantageous diseases of traditional Chinese medicine,and traditional Chinese medicine has obvious effect in improving male reproductive function,which is one of the important means of clinical treatment of this disease.Based on the classical ancient books and modern research,and based on the traditional tonifying kidney method,we have innovatively put forward the concept of treating male infertility with the principle of"deficiency of kidney essence as the root,tonifying kidney and benefiting essence as the method,and regulating yin and yang mildly as the rule",which has achieved good clinical efficacy.At the same time,combined with the long-term clinical practice of our team,we have proposed five major diagnosis and treatment strategies for male infertility,i.e.,combining disease identification with syndrome identification,combining macroscopic with microscopic approaches,combining holistic with localized treatments,combining traditional Chinese medicine with surgery,and combining treatment of the disease and treatment of the person.In addition,we have summarized some of the current problems in the treatment of male infertility with traditional Chinese medicine and put forward corresponding suggestions,with a view to promoting the development and application of traditional Chinese medicine in the treatment of male infertility.
6.Systemic sclerosis caused by paclitaxel liposomes

A 55-year-old female patient with ovarian cancer received bevacizumab (intravenous infusion of 400 mg on day 1) and paclitaxel liposomes (intravenous infusion of 180 mg on day 1) 9 years after surgery due to disease progression, with 21 days as a cycle of chemotherapy. Due to the improvement of the patient′s general condition, the chemotherapy regimens for the 3rd and 4th cycles were adjusted to addtionally give intravenous infusion of carboplatin 500 mg on day 1, and in the 5th cycle, the dose of bevacizumab was increased to 700 mg. Starting from the 2th cycle of chemotherapy, after each intravenous infusion of paclitaxel, the patient immediately experienced itching symptoms on the distal skin of both upper limbs, which were mild and could be relieved on their own. After the 5th cycle of chemotherapy, the patient′s itching symptoms worsened progressively. Four days later, keratinization of the skin on both hands and forearms of upper limbs appeared, with tight skin and waxy luster, gradually developing towards the proximal end. The modified Rodnan skin score was 1-2 (mild to moderate skin thickening). Laboratory tests showed positive results for anti-nuclear antibodies and anti-centromere antibodies.Pathological examination of skin biopsy tissue of the right forearm showed dermal fibrosis, involving subcutaneous adipose tissue and epidermal atrophy. It was considered to be localized cutaneous systemic sclerosis caused by paclitaxel liposomes. Paclitaxel liposomes was stopped and anti-inflammatory and topical application of glucocorticoids locally treatments were given for 3 days, the patient′s itching symptoms were improved. At a 2-month follow-up, the patient′s itching symptoms were improved and did not recur.

8.Acute hyperextension spinal cord injury in children: A retrospective study

Objective:To summarize the clinical characteristics and prognosis of acute hyperextension spinal cord injury (SCI) in children, and to provide some recommendations for the treatment and prevention of this disease.Methods:Reviewed the data of children of SCI after sustained or repeated hyperextension of the spine at Wuhan Union Hospital and Wuhan Children's Hospital from September 2010 to September 2020. According to the American Spinal Injury Association impairment scale (AIS grade), the patients were divided into complete SCI group and incomplete SCI group. The age, symptoms and evolution after injury, neurological level of injury, imaging data, laboratory examination data, prognosis and complications of the two groups were analyzed. Retrospectively summarize the characteristics of this type of injury.Results:Forty-four cases of acute hyperextension SCI in children were included. Their age ranged from 3 to 10 years old, 95% of them were under 8 years old and 95% of them were female. There was no significant difference in age at injury and time of dance training between children with complete SCI and incomplete SCI. Back and leg pain, lower limb weakness or paresthesia, and rapidly progress to complete or incomplete SCI in a short period were typical symptoms. All blood test results anddiagnostic analysis of cerebrospinal fluid were unremarkable or negative. There was no fracture or dislocation in the whole spine. Magnetic resonance imaging showed a longitudinally extended intramedullary high-intensity signal in the thoracolumbar spinal cord. Complete SCI accounted for 60% of all cases, and the prognosis was poor with spinal cord atrophy and various complications.Conclusion:Children younger than 10 years old after sustained or repeated hyperextension of the spine may suffer acute hyperextension SCI. Children with complete SCI have poor prognosis and serious complications. Therefore, prevention of this type of injury is the best strategy.
9.A comparison of ultrasound guided with assisted technologies versus computed tomography guided percutaneous thermal ablation for liver cancer at hepatic dome on safety and short-term efficacy

Objective:To compare the safety and short-term efficacy of percutaneous thermal ablation for liver cancer at hepatic dome guided by ultrasound with assisted technologies or computed tomography.Methods:Patients who underwent thermal ablation of liver cancer at hepatic dome from January 2016 to October 2019 in the Xixi Hospital affiliated to Zhejiang Chinese Medical University were studied. Using the different guidance methods, the patients were divided into the ultrasound guidance combined with assisted technologies group (the ultrasound guided group) and the CT guided group. Assisted technologies included contrast-enhanced ultrasound, fusion imaging and artificial ascites. The complications during and within one week of ablation were recorded to compare the safety between the two guidance methods. One month after ablation, contrast-enhanced CT or contrast-enhanced MRI was performed to evaluate the short-term efficacy.Results:Of 52 patients who were included in the study, 39 had no previous treatment while 13 had recurrent liver cancer (7 underwent previous surgical resection and 6 thermal ablation). There were 41 males and 11 females, age ranged from 38.0 to 76.0 (57.3±10.0) years. The tumor size was 12.0 to 46.0 (30.7±8.8) mm. Of the 34 patients in the ultrasound guided group, the age ranged from 38.0 to 73.0 (56.6±10.2) years, and the tumor size ranged from 17.0 to 46.0 (30.6±8.7) mm. Of the 18 patients in the CT guided group, the age ranged from 39.0 to 76.0 (58.1±9.8) years, and the tumor size ranged from 12.0 to 45.0 (30.9±9.2) mm. There were no significant differences in age and tumor size between the two groups ( P>0.05). During and within one week of ablation, 3 patients developed hydrothorax in the ultrasound guided group and 8 patients developed hydrothorax, and 7 patients pneumothorax in the CT guided group. The difference between the two groups was significant ( P<0.05). All complications were successfully treated with conservative treatment. One month after ablation, the complete ablation rate was 91.2% (31/34) in the ultrasound guided group and 88.9% (16/18) in the CT guided group. There was no significant difference between the two groups ( P>0.05). Conclusion:The two methods of guidance resulted in similar short-term efficacy after thermal ablation for liver cancer at hepatic dome. Compared with computed tomography guidance, ultrasound guidance with assisted technologies significantly reduced the incidence of thoracic complication rates.
10.Survey of nuclear medicine practice in Guangdong in 2016

Objective To survey the basic situation of diagnosis and treatment in clinical nuclear medicine across Guangdong province,evaluate the risks of exposure to the radiation workers and public and explore the countermeasures to control radiation exposre.Methods A survey team was set up to survey,by filling questionnaires,the basic information on nuclear medicine practices for workers,equipment,radionuclide,frequency and dose to workers and radiation protection measures.Results A total of 71 nuclear medicine institutions was involved in the survey with 733 radiation workers in 2016 in Guangdong.The average annual effective dose to nuclear medicine workers was (0.55±0.66) mSv per year.The total pieces of nuclear medical equipment was up to 189 in 2016 including 59 SPECT/CT scanners (5 SPECT),28 PET/CT scanners and 54 thyroid scanners.Total activity of 1.15× 10s MBq in radiopharmaceuticals was used in 325 903 examinations and treatments with the number of frequency of 2.97 examinations per 1 000 population.Concluions There have been a rapid progress in practice of nuclear medicine over the past 20 years in Guangdong province with departments of nuclear medicine set up in 18 of 21 cities.Compared with 1998,the activity in radiopharmaceuticals used has increased by 414% in 2016 and the number of frequency of examination and treatment has increased by 111%.The rapid expansion of nuclear medicine practice has also caused potential risks of radiation to the public and environment.
