OBJECTIVES:
To evaluate the feasibility, efficacy and safety of Sentinel cerebral embolic protection device (CEPD) during transcatheter aortic valve replacement (TAVR). This study is a subgroup analysis of the China Moderate to Severe Valvular Heart Disease Registry, which has been registered at the Chinese Clinical Trial Registry (ChiCTR2300075006).
METHODS:
Patients undergoing TAVR with the Sentinel CEPD from October 2023 to September 2024 were retrospectively enrolled. A total of 80 patients were included, with a median age of 72 (68, 76) years, including 52 males (65.0%) and 28 females (35.0%); 62 patients (77.5%) with tricuspid valves, and 18 patients (22.5%) with bicuspid valves; 34 patients (42.5%) with type Ⅰ aortic arch, 24 patients (30.0%) with type Ⅱ aortic arch, 12 patients (15.0%) with type Ⅲ aortic arch, and 10 patients (12.5%) with bovine-type aortic arch. Clinical data of the patients were summarized and analyzed. The primary endpoints were success rate of Sentinel CEPD implantation, as well as all-cause death, symptomatic stroke, transient ischemic attack, and Sentinel CEPD access vessel complications during hospitalization and within 30 days postoperatively.
RESULTS:
In the 80 patients, self-expanding valves were used in 68 cases (85.0%) and balloon-expandable valves in 12 cases (15.0%). Seventy-nine patients (98.8%) successfully underwent TAVR with Sentinel CEPD deployment. Macroscopically visible debris was captured in 92.5% (74/80) by filters of Sentinel CEPD. Although the procedure time for Sentinel CEPD placement was slightly longer in patients with bovine-type aortic arch, there was no statistically significant difference in deployment time among different aortic arch types (P>0.05). During hospitalization and within 30 days postoperatively, only one case of transient ischemic attack occurred, and there was no all-cause mortality, symptomatic stroke, or access-site vascular complications related to the Sentinel CEPD observed.
CONCLUSIONS
The Sentinel CEPD demonstrates high feasibility across aortic arch types, potential efficacy in embolic capture, and excellent safety in TAVR.

Androgenetic alopecia (AGA) is currently the most common hair loss disease, which is the result of a combination of genetic and environmental factors, and manifests itself as miniaturization of hair follicles around the top of the head and frontotemporal region, which eventually leads to hair reduction or disappearance. The pathogenesis of AGA is closely related to a variety of signaling pathways, which include the Wnt/β-catenin signaling pathway, bone-forming protein (BMP) signaling pathway, insulin-like growth factor (IGF) signaling pathway, fibroblast growth factor (FGF) signaling pathway, hedgehog (SHH) signaling pathway, and so on. Several cytokines are involved in these signaling pathways, including β-catenin, BMP2, IGF-1, and FGF5. An in-depth study of AGA-related signaling pathways and related cytokines can help to provide a comprehensive understanding of the pathogenesis of AGA, provide a certain theoretical basis for the discovery of new signaling pathways, and provide a new research direction for the treatment of AGA.

Chinese Medical Ethics 2024;37(9):1037-1045
The thinking process of medical diagnosis requires a comprehensive and analytical judgment. However,in both judgment processes, human thinking has inevitable limitations. With the development of artificial intelligence(AI)technology, symbolism and connectionism can effectively compensate for the deficiencies of human thinking in some respects, making more accurate diagnostic judgments, but they also have their limitations. It is conceivable that the integration and development of symbolism and connectionism will assist in enhancing the capabilities of medical-assisted diagnosis. However, there is still a tension between accuracy, transparency, and interpretability regarding the application of connectionism in AI,and many concerns about AI have arisen due to its lack of transparency and interpretability.By sorting out the thought process of medical diagnosis, organizing the differences among various AI paradigms and the changes in semantic philosophy and truth concepts behind AI,ethical concerns about the application of AI can be alleviated to a certain extent, and the practical application of AI in the field of medical diagnosis can be promoted.

Chinese Journal of Epidemiology 2024;45(10):1448-1454
Patients with chronic kidney disease (CKD) have a relatively high risk of cardiovascular disease (CVD). Risk stratification guided by CVD risk prediction models is essential for managing CKD populations. We reviewed the outcome events, predictive variables, modeling methods, and predictive performance of CVD risk prediction models in CKD populations. We found a large variability in predictive outcomes, number of predictors, and sample sizes across studies. The models tended to overestimate the CVD risk of CKD populations. There are few independently validated or constructed CVD risk prediction models for CKD populations in developing countries, and in particular, there is a lack of independent external validation studies of model calibration. Future studies should comply with the reporting standards of risk prediction models to better support the application of CVD risk prediction models for CKD populations.
