1.Performance of Computer-Aided Detection Software in Tuberculosis Case Finding in Township Health Centers in China

Background::Computer-aided detection (CAD) software has been introduced to automatically interpret digital chest X-rays. This study aimed to evaluate the performance of CAD software (JF CXR-1 v3.0, which was developed by a domestic Hi-tech enterprise) in tuberculosis (TB) case finding in China.Methods::In 2019, we conducted an internal evaluation of the performance of JF CXR-1 v3.0 by reading standard images annotated by a panel of experts. In 2020, using the reading results of chest X-rays by a panel of experts as the reference standard, we conducted an on-site prospective study to evaluate the performance of JF CXR-1 v3.0 and local radiologists in TB case finding in 13 township health centers in Zhongmu County, Henan Province.Results::Internal assessment results based on 277 standard images showed that JF CXR-1 v3.0 had a sensitivity of 85.94% (95% confidence interval [CI]: 77.42%, 94.45%) and a specificity of 74.65% (95% CI: 68.81%, 80.49%) to distinguish active TB from other imaging conditions. In the on-site evaluation phase, images from 3705 outpatients who underwent chest X-ray detection were read by JF CXR-1 v3.0 and local radiologists in parallel. The imaging diagnosis of local radiologists for active TB had a sensitivity of 32.89% (95% CI: 22.33%, 43.46%) and a specificity of 99.28% (95% CI: 99.01%, 99.56%), while JF CXR-1 v3.0 showed a significantly higher sensitivity of 92.11% (95% CI: 86.04%, 98.17%) ( p < 0.05) and maintained high specificity at 94.54% (95% CI: 93.81%, 95.28%). Conclusions::CAD software could play a positive role in improving the TB case finding capability of township health centers.
2.Dexmedetomidine pretreatment enhances protective effect efficacy of pericytes agaist acute lung injury in septic mice

Objective To investigate the effect and mechanism of dexmedetomidine(Dex)pretreated pericytes(Dex-PCs)on acute lung injury(ALI)in septic mice.Methods Mouse model of sepsis-ALI was established with intraperitoneal injection of lipopolysaccharide(LPS)at a dose of 10 mg/kg.A total of 96 C57BL/6J mice were randomly divided into sham operation(Sham)group,ALI group,PC treatment group and Dex-PCs treatment group.The mice of the PCs and Dex-PCs groups received a tail venous injection of 5×105 PC cells,respectively,whereas those of the Sham and ALI groups received equal volume of normal saline.Flow cytometry,immunofluorescence assay,whole-body volume tracing system and ELISA were used to observe the changes in the colonization of PCs,pulmonary vascular permeability,lung function,serum TNF-α and IL-6 levels,as well as the survival rate and survival time of mice at 72 h after LPS stimulation.After PCs and Dex-PCs were exposed to 10 μg/mL LPS,the level of intracellular reactive oxygen species(ROS)was measured.Results Compared with the Sham group,the ALI group had increased permeability of pulmonary vascular endothelial cells,extensive extravasation of Evans blue,severely destructed of lung tissue structure and massive inflammatory cell infiltration,higher lung wet/dry weight ratio,elevated serum TNF-α and IL-6 levels,and declined lung function,and no mice survived for 72 h after modeling(P<0.05).Both PC cell treatment effectively alleviated the pulmonary vascular endothelial leakage,reduced Evans blue content per unit tissue,improved lung pathological structure,lung function and inflammatory responses,and significantly improved the survival rates in the PC group and the Dex-PC group(P<0.05).What's more,the therapeutic effect of Dex-PC cells was significantly better than that of the PC cells(P<0.05).LPS stimulation induced ROS accumulation greatly in PCs,but no such effect was observed in the PCs after Dex pretreatment(P<0.05).Conclusion Dex pretreatment significantly enhances PCs'protective effect on pulmonary vascular endothelial barrier functions in septic mice,which may be due to its enhancing anti-oxidative capacity of PCs.

4.Effect of mesenchymal stem cells combined with immunosuppressants on immune rejection in a rat model of liver transplantation

Objective To investigate the effect of mesenchymal stem cells(MSCs)combined with immunosuppressants(IS)on immune rejection in a rat model of liver transplantation.Methods F344 rats were divided into Normal group(without any intervention),PS group(injected with an equal volume of normal saline),MSC group(injected with MSC),IS group(injected with IS),and MSC+IS group(injected with MSC and IS),with 8 rats in each group.For all rats except those in the Normal group,the Kamada's double-cuff method was used to establish a model of orthotopic liver transplantation,without reconstruction of the hepatic artery.HE staining and Masson staining were performed for rat liver tissue,and the degree of liver fibrosis was analyzed;immunohistochemical experiments were used to measure the infiltration of T cells and NK cells,and immunofluorescence assay was used to analyze macrophage M2 polarization.A one-way analysis of variance was used for comparison of continuous data between multiple groups,and the least significant difference t-test was used for further comparison between two groups.The Kaplan-Meier method was used to plot survival curves,and the log-rank test was used for survival analysis.Results Compared with the PS group,the MSC+IS group had a significantly prolonged survival time(P<0.01),and the MSC group,the IS group,and the MSC+IS group had a significant improvement in the histological structure of the liver and a significant reduction in the degree of liver fibrosis(all P<0.000 1),as well as a significant reduction in the infiltration of NK and T cells(all P<0.000 1)and a significant increase in the degree of macrophage M2 polarization(all P<0.000 1).The MSC+IS group had a significantly better effect than the MSC group and the IS group.Conclusion MSCs combined with IS can improve liver histopathology,reduce inflammatory cell infiltration,promote macrophage M2 polarization,and exert an immunosuppressive effect in rats after liver transplantation.
5.Application value of indocyanine green injection via percutaneous transhepatic portal vein puncture under ultrasound guidance during laparoscopic anatomical liver resection

Objective:To investigate the application value of indocyanine green (ICG) injection via percutaneous transhepatic portal vein under ultrasound guidance during laparoscopic anatomical liver resection.Methods:The retrospective and descriptive study was conducted. The clinicopatho-logical data of 17 male patients with hepatocellular carcinoma who underwent laparoscopic anatomical liver resection in Mengchao Hepatobiliary Hospital of Fujian Medical University from October 2022 to May 2024 were collected. The patients were aged 59(range, 27-74)years. Patients underwent ICG fluorescence-guided laparoscopic anatomical liver resection after ICG injection via percutaneous transhepatic portal vein under ultrasound guidance. Obsevation indicators: (1)surgical situations; (2) postoperative situations; (3) follow-up. Measurement data with normal distribution were represented as Mean± SD. Measurement data with skewed distribution were represented as M(range). Count data were represented as absolute numbers. Results:(1) Surgical situations. All 17 patients were successfully punctured and injected with ICG via percutaneous transhepatic portal vein under ultrasound guidance, with a puncture time of 6(range,3-12)minutes. The fluorescence staining successfully displayed at the target liver segment without infiltration, and the distance from the fluorescence staining border to the tumor edge was greater than 1.0 cm, which was consistent with preoperative three-dimensional reconstruction imaging. Among the 17 patients, 11 cases had punctures in one branch of the portal vein, 4 cases had punctures in two branches, and 2 had punctures in three branches. The operation time was (229±51)minutes, volume of intraoperative blood loss was 200(range, 100-300)mL. No blood transfusion or conversion to open surgery was required during the operation. (2) Postoperative situations. Two of 17 patients had a small pleural effusion of grade Ⅰ-Ⅱ of Clavien-Dindo classification, and were recovered without treatment. The duration of postoperative hospital stay was 5(range, 5-6)days. Postoperative pathological examina-tion showed hepatocellular carcinoma with negative surgical margin. The tumor diameter was 4.5(range, 1.8-12.5)cm. (3) Follow-up. All the 17 patients were followed up for 14(range, 2-30)months, without tumor recurrence.Conclusion:Injection of ICG via percutaneous transhepatic portal vein under ultrasound guidance during laparoscopic anatomical liver resection is safe and feasible.

7.Study on protective effects and mechanism of ziyuglycoside Ⅰ on acute lung injury in sepsis rats

OBJECTIVE To investigate the protective effects and mechanism of ziyuglycoside Ⅰ on acute lung injury in sepsis rats based on network pharmacology, and conduct experimental verification. METHODS The network pharmacology was used to predict the potential target of ziyuglycoside Ⅰ in the treatment of acute lung injury following sepsis. The rat model of sepsis was reproduced by cecum ligation and puncture for experimental verification. Totally 192 SD rats were randomly divided into the sham operation group (Sham group), sepsis group (Sep group), conventional therapy group (CT group) and ziyuglycoside Ⅰ group (Zg Ⅰ group), respectively. Sham group and Sep group were given sterile normal saline, and CT group and ZgⅠ group were given relevant volume of Ringer’s solution and ziyuglycoside Ⅰ. The arterial blood gas, serum inflammatory factors, lung wet/dry mass ratio, pathological changes of lung tissue, pulmonary vascular permeability, the expressions of pulmonary vein tight junction protein 1 (ZO-1) and vascular endothelial cadherin (VE-cadherin) protein and 72-hour survival were observed in each group. RESULTS Results of network pharmacology showed that there were 47 potential targets of ziyuglycoside Ⅰ in the treatment of sepsis. The results of gene ontology function enrichment analysis and Kyoto encyclopedia of genes and genomes pathway enrichment analysis showed that the mechanism could 598486924@qq.com be correlated with biological processes such as positive regulation of reactive oxygen species metabolism, wound healing, regulation of endothelial cell proliferation, cell activation, blood vessel development, response to oxidative stress, etc., and with signaling pathway such as apoptosis, tight junction, HIF-1 signaling pathway, etc. The results of experimental verification showed that compared with Sham group, pH value and the level of partial arterial oxygen pressure were decreased significantly in Sep group (P<0.05), while the level of partial pressure of carbon dioxide, serum levels of tumor necrosis factor α, interleukin 6 were increased significantly (P<0.05); the ratio of lung wet/dry mass was increased significantly (P<0.05); the protein expressions of ZO-1 and VE-cadherin were decreased significantly (P<0.05); 72 h survival rate decreased,the survival time was significantly shortened (P<0.05); the results of pathological observation of lung tissue showed that the rats’ alveoli were extensively ruptured, the alveolar wall was thickened and accompanied with edema, and there was obvious inflammatory cell infiltration; the results of pulmonary vascular permeability observation showed that the lung surface of rats was dark, with a large amount of Evans blue exudation, and the left lower lung was obviously dark blue. Compared with Sep group, the levels of above indexes almost were reversed significantly in CT group and ZgⅠ group (P<0.05); the lung histopathology and pulmonary vascular permeability were significantly improved, and the recovery degree of ZgⅠ group was greater than that of CT group, which was close to the results of Sham group. CONCLUSIONS Ziyuglycoside Ⅰ can significantly reduce inflammatory reaction and acute lung injury in septic rats, which is related to vascular function and tight junction signal pathway.
8.Pathophysiological features of rat hemorrhagic shock under high temperature conditions

Objective To investigate the pathophysiological features of rat hemorrhagic shock under high temperature conditions.Methods A total of 128 SD rats were assigned to high temperature group and normal temperature group according to the random number table,with 64 rats per group.Rats in high temperature group were pretreated at 342 for 12 h,and in normal temperature group were kept at 25℃.Hemorrhagic shock models in rats were produced by withdrawing 40% of the total blood volume via the femoral artery.Parameters of the two groups were measured including blood electrolytes (Na +,K +),plasma osmotic pressure,liver function [aspartate aminotransferase (AST),alanine aminotransferase (ALT)],kidney function [creatinine (CREA)],arterial blood gases (pH,PO2 and PCO2) and cardiac function [heart rate (HR),cardiac output (CO),cardiac index (CI) and DO2].Animal survival time and survival rate were detected.Results Before shock,high temperature group versus normal temperature group showed higher detections of Na+ concentration [(142.3 ± 2.2) mmol/L:(139.1 ±1.5) mmol/L],plasma osmotic pressure [(304.8 ± 4.7) mmol/L:(300.0 ± 1.9) mmol/L],HR [(462 ±30) times/min:(402 ± 44) times/min],CO [(0.892 ± 0.190) L/min:(0.713 ± 0.090) L/min] and CI [(0.0030±0.0006)L·min-1 · cm-2:(0.0023 ±0.0002)L· min-1 · cm-2] (P<0.05).After shock,normal temperature group showed further increased concentrations of Na + and K + and significantly enhanced AST,ALT and CREA compared to normal temperature group (P < 0.05).After shock,CO,CI and DO2 in high temperature group were decreased by 63%,63% and 69% respectively compared to those before shock,but less decrease was observed in normal temperature group.Rat survival rate and survival time were significantly reduced in high temperature group compared to normal temperature group (P < 0.05).Conclusion Pathophysiological changes of hemorrhagic shock in rats under high temperature are mainly manifested as impaired cardiac function,significantly increased concentrations of Na + and K + and significantly shortened survival time.
9.Inhibition of matrix seedling raising in winter on premature bolting of Angelica sinensis

Objective Inhibition of matrix seedling raising in winter greenhouse on premature bolting of Angelica sinensis.Methods Three factors of sowing periods,soil media,and seeds were tested in ortho-gonal design by repeated three times.Results In total 15 treatments,bolting percentage of A.sinensis in seven treatments were lower than 1%,among which the lowest was 0.14%;In the other seven treatments,the bolting percentages were 1%—5%,and in another treatment,it was 19.93%.No bolting happened in 40% of total 45 tested plots,and the bolting percentage was lower than 5% in other 46.7% tested plots.Stalk of winter raised seedlings started to produce at the beginning of August,which delayed 70 d compared to that of the traditional seedlings,bolting peak period of winter raised seedlings was in the middle of September,which delayed 100 d compared to that of the traditional seedlings.In total 15 treatments,100% of bolting plants only stalked,but no flowers produced in six treatments,over 50% of bolting plants only stalked,but no flowers produced in the other eight treatments,38.9% of bolting plants only stalked but no flowers produced in another one treatment.The sample test showed that ethanol extracts content of bolting plant root without flower was 45.93%.Influence in each one of these three factors to premature bolting percentage approached to the utmost notable difference,the influence sequence was sowing periods,seeds,and soil matrixes.Conclusion Premature bolting percentage of A.sinensis is not only obviously decreased by matrix seedling raising in winter greenhouse,but also the bolting can possibly be avoided,and bolting date be also delayed greatly.
