1.Comorbidity of abnormal spinal curvature and screening myopia among primary and middle school students

Objective:
To investigate the comorbidity of abnormal spinal curvature and screening myopia among primary and middle school students in Taizhou City, Zhejiang Province, so as to provide the reference for the prevention and control of abnormal spinal curvature and myopia among children and adolescents.
Methods:
Students from nine counties (cities, districts) in Taizhou City were selected using a stratified cluster random sampling method. Basic information, sleep, physical activity, diet, electronic device usage, frequency of seat changes, after class study, reading and writing posture, and eye-use habits were collected through questionnaire surveys. Abnormal spinal curvature was screened according to the Technical Guidelines for the Prevention and Control of Abnormal Spinal Curvature in Children and Adolescents. Myopia was screened based on Specification for Screening of Refractive Error in Primary and Middle School Students (WS/T 663—2020). The influencing factors for the comorbidity of abnormal spinal curvature and screening myopia were analyzed using a multivariable logistic regression model.
Results:
A total of 13 596 students were surveyed, including 7 423 males (54.60%) and 6 173 females (45.40%). The average age was (13.45±2.59) years. The prevanlece of the comorbidity of abnormal spinal curvature and screening myopia, was 2.90% (394 cases). Multivariable logistic regression analysis showed that students who used computers for <1 hour in the past 7 days (not use as the reference, OR=1.312, 95%CI: 1.023-1.684), after class study for ≥3 hours in the past 7 days (OR=1.513, 95%CI: 1.068-2.143), and frequently/always read books or watched screens under direct sunlight (OR=1.604, 95%CI: 1.018-2.526) had a higher risk of the comorbidity of abnormal spinal curvature and screening myopia. In contrast, the students who slept for >9 h/d (OR=0.638, 95%CI: 0.411-0.991), engaged in outdoor activities for ≥3 h/d in the past 7 days (OR=0.645, 95%CI: 0.427-0.975), and changed seats once per month or more (OR=0.740, 95%CI: 0.573-0.955) had a lower risk of the comorbidity of abnormal spinal curvature and screening myopia.
Conclusion
The comorbidity of abnormal spinal curvature and screening myopia among primary and middle school students is mainly influenced by sleep, physical activity, electronic device use, frequency of seat changes, after class study and eye-use habits.
2.Establishment of a prognostic model for HER2 low expression breast cancer with lung metastasis

Purpose This study aimed to evaluate the consistency of human epidermal growth factor receptor 2(HER2)status between primary breast cancer lesions and lung metastatic lesions and to establish a prognostic model for predicting the survival rate of HER2 low expression(HER2-low)breast cancer patients with lung metastasis.Methods Clinicopathological data from a cohort of 252 patients with breast cancer and lung metastasis were retrospec-tively analyzed.Results 50.00%of the patients had HER2-low expression in metastatic lesions,and HER2-low ex-pression was the most prevalent subgroup in both primary and metastatic lesions.A discordance in HER2 status be-tween primary and metastatic sites was observed in 28.07%of cases.The most frequent shift was from HER2-zero in the primary tumor to HER2-low expression in the metastasis(12.28%of all cases).Estrogen receptor(ER)status,menopausal status,and histological type were identified as independent prognostic factors for overall survival(OS)by univariate and multivariate Cox regression analyses.A prognostic model incorporating these factors was constructed to predict 3-year and 5-year survival.The model demonstrated area under the curve(AUC)values of 0.765 and 0.780 for 3-year and 5-year OS in the training cohort,and 0.667 and 0.706 in the validation cohort,respectively.Conclu-sion HER2-low expression is the most common subtype among breast cancer patients with lung metastasis.The ob-served shift from HER2-zero in primary lesions to HER2-low in metastases underscores the clinical necessity of re-biop-sy at metastatic sites.The developed prognostic model effectively predicts OS in this patient population.
3.Perceived stress and ego depletion of college students: the mediating role of emotional eating and the moderating role of peer relationship

Objective:To explore the effects of perceived stress on ego depletion of college students, as well as the mediating role of emotional eating and the moderating role of peer relationship.Methods:A cross-sectional survey of 1 088 college students was conducted using the perceived stress scale, the Dutch eating behavior questionnaire, the self-control resource depletion scale, and the peer relationship measurement from December 2023 to April 2024.PROCESS Macro program in SPSS 25.0 software was used to test the mediating effect of emotional eating and the moderating effect of peer relationship.Results:(1)The score of perceived stress, emotional eating, peer relationship and ego depletion were 39.26±8.35, 39.19±12.15, 2.00(1.00), and 18.19±7.15, respectively.(2)Perceived stress was positively correlated with emotional eating, ego depletion, and peer relationship( r=0.36, 0.61, 0.25, all P<0.01). Emotional eating was positively correlated with ego depletion and peer relationship( r=0.40, 0.19, both P<0.01). And ego depletion was positively correlated with peer relationship( r=0.23, P<0.01).(3)Emotional eating played a partial mediating role in the effect of perceived stress on ego depletion( β=0.077, 95% CI=0.053-0.104), and the mediating effect accounted for 12.38%(0.077/0.622) of the total effect.(4)Peer relationship played a moderating role between perceived stress and emotional eating. Under low peer relationship, perceived stress had a significant positive predictive effect on emotional eating( βsimple=0.46, P<0.01), and under high peer relationship, the predictive effect of perceived stress on emotional eating was significantly weaker( βsimple=0.26, P<0.01). Conclusions:Perceived stress not only directly affects ego depletion, but also indirectly affects ego depletion through emotional eating in college students.High levels of peer relationship can weaken the impact of perceived stress and high emotional eating on ego depletion.
4.MITF expression in acral melanoma tissues and its association with clinical,pathological characteristics and prognosis

Background and purpose:The microphthalmia-associated transcription factor(MITF)plays a complex role in melanoma pathogenesis and progression.It is known to regulate multiple processes both in melanocytes and melanoma cells.While numerous studies have explored MITF in cutaneous melanoma(CM),research in acral melanoma(AM)is still limited.This study retrospectively analyzed the correlation between MITF expression and clinical,pathological characteristics and prognosis in AM patients,providing a basis for prognosis evaluation and personalized treatment plan formulation for patients.Methods:Patients who underwent primary resection of AM at Fudan University Shanghai Cancer Center from March 2008 to February 2022 were included.All surgical samples were diagnosed by clinical histopathology and used to construct the tissue microarray(TMA).This study was approved by the medical ethics committee of Fudan University Shanghai Cancer Center(approval number:2203-ZZK-69-3).Cutting complete tissue microarray and evaluating MITF expression levels by immunohistochemistry(IHC)staining were carried out.The results were independently assessed and scored by three pathologists.Clinical and pathological data were collected from the hospital's electronic medical record system,and each patient's data was matched to their corresponding tissue sample on the chip.Patients were stratified into two groups based on MITF expression levels.Statistical analyses were performed to assess differences in clinical,pathological characteristics and survival outcomes between these two groups.Results:A total of 137 AM patients were included.MITF expression was significantly associated with T stage,N stage,American Joint Committee on Cancer(AJCC)stage,clark level,sentinel lymph node status,and presence of ulceration.Among these,N stage and ulceration were independent risk factors for high expression of MITF after adjusting for confounding factors.Survival analysis showed that AM patients with high MITF expression or higher T stage were associated with shorter disease-free survival(DFS).Patients with high MITF expression showed no significant difference in overall survival(OS)between observation or cytokine therapy and adjuvant immune checkpoint inhibitor(ICI)therapy,whereas those with low MITF expression derived significant survival benefits from ICI treatment.Conclusion:A higher N stage or the presence of ulceration indicates high MITF expression in tumor cells,with high MITF levels serving as a warning signal for early recurrence,metastasis,and even death.Patients with low MITF expression could receive improved OS with early adjuvant ICI therapy.MITF could not only serve as an auxiliary diagnostic marker for melanoma but also provide a basis for clinical prognosis assessment and the formulation of personalized treatment plans.
5.Three-dimensional deep neural network integrating transfer learning for preoperative coronary CTA classification in atrial fibrillation patients

Objective To develop a three-dimensional(3D)deep neural network based preoperative classification model for coronary computed tomography angiography(CTA)in atrial fibrillation patients,and to explore the effects of transfer learning on the performance of medical image classification models,thereby providing preoperative decision support for catheter ablation to advance atrial fibrillation treatment toward precision and personalization.Methods Utilizing 3D ConvNet and 3D ResNet as backbone network,the three-dimensional classification features were extracted from coronary CTA sequences.The publicly available pre-trained weights were used for transfer learning.The model performance was evaluated through metrics such as confusion matrix,classification accuracy,and area under the curve(AUC).A comparative analysis was also conducted to evaluate the performance differences between the transfer learning model and the initialized training model.Results Transfer learning yielded significant performance improvements over the initialized training models,attaining AUC improvement of 9.1%-16.7%and accuracy enhancement of 6.2%-23.5%.Among all models,3D-ResNet18 model with MedicalNet pre-training weights performed the best,achieving an AUC of 0.77 and an accuracy of 0.71.Conclusion The proposed three-dimensional deep network enhanced by transfer learning can effectively identify atrial fibrillation patients requiring additional ablation besides pulmonary vein isolation through preoperative coronary CTA,which will assist clinicians in optimizing surgical strategies and improving treatment outcomes,thereby reducing long-term postoperative recurrence rates.

7.Impact of early triglyceride reduction on clinical outcomes in patients with extremely severe hypertriglyceridemia-induced acute pancreatitis

Objective:To investigate the association between early reduction (within 3 days of admission) of serum triglyceride (TG) to 5.65 mmol/L and clinical outcomes in patients with acute pancreatitis extremely severe hypertriglyceridemia-induced (HTG-AP).Methods:The clinical data were derived from the PERFORM database, which prospectively collected clinical information on 613 HTG-AP patients admitted to 38 medical centers across China between November 2020 and June 2023 with serum TG level ≥11.3 mmol/L at admission. This study further screened extremely severe HTG patients with TG≥22.6 mmol/L. Patients were divided into the target-reaching group (TG≤5.65 mmol/L on day 3 after admission) and non-target-reaching group (TG>5.65 mmol/L on day3 after admission). The effect of early reduction of serum triglyceride to standard level on organ failure was observed. The primary outcome was organ failure-free days (OFFD) to 14 days of admission. Secondary outcomes included the presence of organ failure on day 7 and day 14, persistent organ failure (POF) to day 14, new-onset organ failure to day 14, maximum sequential organ failure assessment (SOFA) score to day 14, incidence of infected pancreatic necrosis (IPN) by day 60 of admission, length of ICU and hospital stay, mortality by day 60 of admission. The linear regression model was used for multivariate analysis. The subgroup forest plot was drawn to assess the effect of early reduction of TG on OFFD in different subgroups. The Kaplan-Meier method was used to plot the cumulative recurrence rate curve for the time to organ failure resolution and Log-Rank test was used for comparison between two groups.Results:Overall, 212 patients with HTG-AP were enrolled, with 118 in the target-reaching group and 94 in the non-target-reaching group. There was no significantly statistical difference on baseline TG level between two groups, but patients in non-target-reaching group had higher total cholesterol, LDL and CRP than those in target-reaching group. The median OFFD with 14 days of admission in target-reaching and non-target-reaching group was 14.0(11.0, 14.0) and 14.0(13.0, 14.0) days, respectively, without significant difference ( P=0.279). Moreover, there was no significant difference on the presence of organ failure on day 7 and day 14, POF to day 14, new-onset organ failure to day 14, maximum SOFA score to day 14, incidence of IPN by day 60 of admission, length of ICU and hospital stay, mortality by day 60 of admission between two groups. Results of multivariate analysis revealed that there was no significant difference on OFFD between two groups. Subgroup analyses were performed according to age, body mass index, baseline acute physiology and chronic health evaluation-Ⅱ (APACHE-Ⅱ) score, and whether there was organ failure at baseline. However, no association of early reduction of TG with OFFD to day 14 was shown in any subgroup. Among the 67 patients with organ failure at baseline, there was no significant difference in the time to organ failure resolution between target-reaching group and non-target-reaching group ( HR=1.256, 95% CI 0.746-2.116; P=0.343). Conclusions:For patients with acute pancreatitis complicated with extremely severe HTG (TG≥22.6 mmol/L), rapid TG decline (TG≤5.65 mmol/L on day 3 after admission) is not associated with reduced incidence and shorter duration of organ failure.

9.Study on the application of autologous umbilical cord blood priming of CPB circuit in cardiac surgery for congenital heart disease

Objective:To explore the clinical value of autologous umbilical cord whole blood(UCB) priming of the cardiopulmonary bypass(CPB) circuit in neonatal cardiac surgery for congenital heart disease(CHD).Methods:This prospective non-randomized controlled trial included neonates undergoing CHD surgery at Guangdong Provincial People’s Hospital from August 2024 to January 2025. The experimental group used autologous UCB for CPB circuit priming, while the control group used adult allogeneic blood(AAB) priming when UCB was unavailable. Preoperative characteristics, intraoperative CPB and aortic cross-clamping(ACC) times, postoperative ICU stay duration, mechanical ventilation time, and hospitalization length were compared.Results:There were no significant differences in preoperative baseline characteristics between the two groups( P>0.05). At the end of surgery, red blood cell count(RBC), hemoglobin level(Hb), and creatine kinase(CK) showed no significant differences between the groups( P> 0.05). Additionally, perioperative left ventricular ejection fraction(LVEF) demonstrated no statistically significant variations( P>0.05). At surgery completion, the UCB group exhibited lower hematocrit(HCT) and higher blood lactic acid(Lac) levels but these differences resolved by 6 hours postoperatively( P>0.05). The UCB group had higher maximum vasoactive-inotropic scores(VISmax) within 48 hours and longer ICU stays, though total hospitalization and mechanical ventilation durations showed no significant differences( P>0.05). Conclusion:Autologous UCB priming reduces AAB requirements and has minimal impact on postoperative cardiac and pulmonary function recovery, or homeostasis., which is safe and feasible. This study provides evidence supporting the clinical application of UCB priming in CPB circuits.
