To compare the performance differences between the multidisciplinary team (MDT) model and the conventional diagnostic and treatment model for lung cancer, and to explore a high-quality development pathway for optimizing lung cancer diagnostic and treatment resources. A retrospective analysis was conducted on electronic medical record data of lung cancer patients at Shanghai Chest Hospital from March 2025 to December 2025. Patients were divided into an MDT group and a conventional care group based on whether they were admitted to the integrated oncology ward. Statistical analyses were performed using the Mann-Whitney A total of 4, 758 patients with primary lung cancer were included, comprising 365 (7.7%) in the MDT group and 4, 393 (92.3%) in the conventional care group. After adjusting for confounding factors, the MDT model significantly reduced hospitalization frequency during the observation period by 48.8% ( The MDT model for lung cancer significantly reduces hospitalization frequency; however, its effect on cost per hospitalization is population-selective, with increased costs in early-stage (stage Ⅰ) patients and decreased costs in late-stage (stages Ⅱ and Ⅳ) patients. The implementation of the MDT model should adopt precise patient stratification management, prioritizing the optimal patient population to achieve the optimal allocation of medical resources.

Objective:To analyze the prognostic influencing factors in patients with isocitrate dehydrogenase (IDH) wild-type glioma, and further evaluate the value of surgical resection in prognosis. Methods:The clinical data and molecular pathological information of 647 patients with IDH wild-type glioma were retrieved from the Chinese Glioma Genome Atlas (CGGA) database (from 2006 to 2019). The clinical characteristics were recorded, including gender, age, initial symptoms, preoperative Karnofsky performance status (KPS) score, tumor location, tumor laterality, extent of resection, O6-methylguanine-DNA methyltransferase (MGMT) promoter methylation status and postoperative treatment. Kaplan-Meier survival curve was plotted to calculate overall survival (OS) and progression-free survival (PFS), and comparisons were performed using the log-rank test. Multivariate Cox regression analysis was used to identify the independent influencing factors of prognosis in patients with IDH wild-type glioma. Results:Among the 647 patients with IDH wild-type glioma, there were 120 cases of WHO grade Ⅱ, 115 cases of grade Ⅲ, and 412 cases of grade Ⅳ; the median OS was 20.3 months. There were statistical differences in age, initial symptoms, tumor location, preoperative KPS score, extent of resection, MGMT promoter methylation status and postoperative treatment among patients with different WHO grades (P<0.01), but there were no statistical difference in gender and tumor laterality (P>0.05). The median OS and PFS in patients with WHO grade Ⅱ were significantly longer than those in patients with WHO grade Ⅲ and WHO grade Ⅳ (169.9 months vs. 24.7 and 14.4 months, 138.8 months vs. 17.7 and 11.4 months), the indexes in patients with WHO grade Ⅲ were significantly longer than patients with WHO grade Ⅳ, and there were statistical differences (P<0.05). The median OS and PFS in patients with total resection were significantly longer than those in patients with subtotal resection and partial resection (37.2 months vs. 20.3 and 8.5 months, 29.5 months vs. 17.6 and 6.0 months), the indexes in patients with subtotal resection were significantly longer than patients with partial resection, and there were statistical differences (P<0.05). Among 153 patients with MGMT promoter methylation who received postoperative chemoradiotherapy, the median OS and PFS in patients with total resection (67 cases) were significantly longer than those in patients with subtotal resection (64 cases) and partial resection (22 cases): 28.8 months vs. 18.4 and 9.0 months, 25.6 months vs. 16.6 and 6.5 months), the indexes in patients with subtotal resection were significantly longer than patients with partial resection, and there were statistical differences (P<0.05). Among 202 patients with MGMT promoter non-methylation who received postoperative chemoradiotherapy, the median OS and PFS in patients with total resection (81 cases) were significantly longer than those in subtotal resection (81 cases) and partial resection (40 cases): 31.3 months vs. 21.9 and 14.0 months, 22.7 months vs. 16.8 and 9.7 months, and there were statistical differences (P<0.05), but there were no statistical difference in the indexes between patients with subtotal and patients with partial resection (P>0.05). Multivariate Cox regression analysis result showed that age, extent of resection, WHO grade (grade Ⅲ/grade Ⅳ) and postoperative treatment were independent OS and PFS influencing factors in patients with IDH wild-type glioma (OS: HR = 1.327, 3.295, 3.406/7.964 and 1.597; 95% CI 1.079 to 1.633, 2.627 to 4.132, 2.225 to 5.216/5.339 to 11.880 and 1.288 to 1.981; P<0.01. PFS: HR = 1.282, 2.655, 3.143/6.511 and 1.477; 95% CI 1.039 to 1.583, 2.123 to 3.322, 2.049 to 4.819/4.376 to 9.690 and 1.181 to 1.849; P<0.05 or <0.01).Conclusions:The age, extent of resection, WHO grade and postoperative treatment are independent prognosis influencing factors in patients with IDH wild-type glioma. Treatment strategies should fully consider the clinical and molecular pathological characteristics to achieve maximal safe tumor resection and optimal survival benefit.

Objective:To investigate the effectiveness of methylene blue in pain management after costal cartilage removal. Methods:A prospective, randomized controlled trial was conducted from June 2023 to March 2024. Female patients undergoing rhinoplasty with autologous costal cartilage transplantation were randomly divided into a methylene blue group and a control group. Before costal cartilage harvesting, patients in the methylene blue group received a 5 ml injection of 0.1% methylene blue solution into the skin and subcutaneous tissue of the costal cartilage donor site at the lower edge of the seventh costal cartilage or below the breast contour. Patients in the control group received an equal volume of normal saline injected into the same area. Postoperative management included routine observation and pain control (using oral analgesics and topical analgesia). Pain scores were assessed 24 hours after surgery using a visual analog scale (VAS, 0-10 points, higher scores indicate more severe pain) and a numerical rating scale (NRS, 0-10 points, higher scores indicate more severe pain), as well as the frequency of oral analgesics. Patients were followed up for postoperative complications within one month after surgery, and patient satisfaction with pain control was assessed using a self-made 5-point questionnaire (higher scores indicate greater patient satisfaction). Quantitative data were compared between groups using the independent sample t-test; qualitative data were compared between groups using the chi-square test. P<0.05 was considered statistically significant. Results:A total of 112 female patients were enrolled. Fifty-six patients were in the methylene blue group, aged (35.4 ± 5.6) years (range, 18-55 years), and 56 patients were in the control group, aged (36.1 ± 6.0) years (range, 19-54 years). The methylene blue group had significantly lower oral analgesic use 24 hours after surgery than the control group [(1.5±0.5) times vs. (4.7±1.2) times], with statistically significant differences ( P<0.05). The methylene blue group also had significantly lower VAS scores (3.2±1.2 vs. 5.8±1.3) and NRS scores (3.5±1.0 vs. 6.2±1.1) 24 hours after surgery than the control group ( P<0.05). At one-month follow-up, the incidence of postoperative complications in the methylene blue group was significantly lower than that in the control group [10.7% (6/56) vs. 21.4% (12/56)]. Patient satisfaction in the methylene blue group was significantly higher than that in the control group [4.5 ± 0.6 vs. 3.2 ± 0.8]. The differences were statistically significant ( P< 0.05). No serious adverse reactions were observed in either group. Conclusion:Methylene blue has a good analgesic effect after costal cartilage transplantation, reducing the need for analgesics, and no significant adverse reactions were observed.

Objective To investigate the correlation between early clinical outcomes following endoscopic surgery for unilateral lumbar disc herniation and variations in the cross-sectional area of the sciatic nerve. Methods Fifty-two patients(n=52)with unilateral lumbar disc herniation were recruited from June 2023 to June 2024 at the Pain Department of the First Affiliated Hospital of Nanchang University. The cross-sectional area(CSA)values of bilateral sciatic nerves were measured at the gluteal sciatic tuberosity-great femoral rotor level using ultrasound before and on the 4th day after surgery. The change in nerve cross-sectional area ΔCSA and sciatic nerve swelling rate(SR-SN)were calculated.Patient BMI(Body Mass Index),visual analog scale(VAS),Japanese Orthopaedic Association score(JOA),Oswestry Disability Index(ODI),and Treatment Improvement Rate(TIR)were collected preoperatively and on postoperative day 4.Changes in sciatic nerve cross-sectional area in unilateral lumbar disc herniation and its relationship to clinical outcomes were analyzed using the scoring scales as controls. Results Compared to the preoperative period, there was a significant decrease in VAS and ODI scores,as well as an increase in JOA score on the 4th postoperative day(P<0.01).Furthermore,surgical intervention led to substantial improve-ment in clinical outcomes for the patients.Prior to surgery, the CSA value of the affected side of the sciatic nerve was greater than that of the healthy side(P<0.01).Although CSA value decreased on the affected side after surgery,it remained higher than that of the healthy side(P<0.01).Additionally,there was a significant difference between SR-SN values on both sides with higher values observed on the affected side(P<0.01).Notably,△CSA on the affected side exhibited a negative correlation with postoperative VAS score,JOA score, and treatment improvement rate(P<0.05);The SR-SN on the affected side exhibited a positive correlation with BMI and negative correlations with postoperative VAS score,JOA score, and treatment improvement rate(P<0.05).However,no significant correlations were observed between △CSA,SR-SN,and ODI score on the affected side.(P>0.05).Conclusions In patients undergoing endoscopic surgery for lumbar disc herniation,a greater change in the cross-sectional area of the affected sciatic nerve before and after the operation is associated with more pronounced improvement in early symptoms. However, further investigation is required to explore the relationship between this change and recovery of both lumbar spine and lower limb function.

BACKGROUND:Research has shown that vanillic acid has anti-inflammatory and anti-oxidative stress effects, but it is unclear whether it has a protective effect on endplate chondrocytes.OBJECTIVE:To explore the effect and mechanism of vanillic acid on endplate chondrocytes under inflammatory microenvironment.METHODS:(1)Primary endplate chondrocytes were isolated from the intervertebral disc of SD rats and identified by toluidine blue staining and collagenⅡ immunofluorescence.(2)The CCK-8 assay was employed to detect the effects of interleukin-1β and vanillic acid on the proliferation activity of endplate chondrocytes,in order to determine the concentration of vanillic acid for subsequent cell treatment.(3)An inflammatory microenvironment was simulated by adding 10 ng/mL interleukin-1β to the culture medium, and the endplate chondrocytes were treated with low, medium, and high mass concentrations of vanillic acid. The expression levels of inflammatory markers and extracellular matrix proteins were detected by western blot assay and immunofluorescence.(4)The expression of nuclear factor κB signaling pathway-related proteins was detected by western blot assay.RESULTS AND CONCLUSION:(1)The morphology of endplate chondrocytes in adherent culture was pike or triangular in shape, positive for toluidine blue staining and immunofluorescence for collagen Ⅱ,indicating that the experimentally extracted cells were endplate chondrocytes.(2)The CCK-8 assay results showed that treatment with 2.5,5,10,and 20 μg/mL vanillic acid for 24 hours did not significantly inhibit the proliferation of endplate chondrocytes.Compared with the interleukin-1β group, the viability of endplate chondrocytes treated with 5,10,and 20 μg/mL vanillic acid for 24 hours was significantly increased(P<0.05).Therefore,5,10,and 20 μg/mL vanillic acid was selected as the low, medium, and high dose groups for subsequent treatment of endplate chondrocytes.(3)Compared with the model group(complete medium containing 10 ng/mL interleukin-1β),the expression of NOD-like receptor thermal domain associated protein 3(NLRP3),matrix metalloproteinase 13,matrix metalloproteinase 3,and tumor necrosis factor alpha protein in the endplate chondrocytes of the low, medium, and high doses of vanillic acid groups were significantly reduced(P<0.05).(4)Compared with the model group, the protein expression of aggrecan and collagen Ⅱ in the endplate chondrocytes of the low, medium, and high dose groups of vanillic acid significantly increased(P<0.05).(5)Compared with the model group, the protein expression of phospho-nuclear factor κB and phospho-inhibitor of nuclear factor κB in the endplate chondrocytes of the low, medium, and high dose groups of vanillic acid was significantly reduced(P<0.05).(6)The above results indicate that vanillic acid may alleviate the inflammatory response and extracellular matrix degradation induced by interleukin-1β in rat endplate chondrocytes by inhibiting the activation of the nuclear factor κB signaling pathway.
