1.Prevalence of chronic diseases, chronic disease prevention and control literacy and its influencing factors among middle-aged and elderly residents in Wenzhou from 2020 to 2023

Objective:To analyze the prevalence of chronic diseases, the level of chronic disease prevention and control literacy and its influencing factors among middle-aged and elderly residents in Wenzhou from 2020 to 2023.Methods:Based on a cross-sectional research design, 19 528 usual residents aged 45-69 were retrospectively selected with a multi-stage stratified random sampling method from the health literacy monitoring survey in Wenzhou from 2020 to 2023. The χ2 tests and trend χ2 tests were employed to analyze chronic disease prevalence rates and health literacy possession rates across different population subgroups. Binary logistic regression models were used to identify determinants of chronic disease prevention health literacy among middle-aged and elderly residents in Wenzhou. The results were standardized using data from Wenzhou′s Seventh National Population Census. Results:The numbers of middle-aged and elderly respondents was 5 528, 4 822, 4 575 and 4 603 from 2020 to 2023, respectively, and the prevalence of self-rated chronic diseases was 39.56%, 37.33%, 41.29% and 39.00%, respectively (the standardized rate was 33.49%, 33.58%, 36.43% and 35.34%, respectively) in the each year, and the rates of chronic disease prevention and control literacy was 24.76%, 27.98%, 24.44% and 28.13%, respectively (the standardized rate was 27.95%, 30.11%, 27.57%, 29.78%, respectively). The rates of chronic disease prevention and control literacy was on an upward trend in middle-aged and elderly respondents in Wenzhou from 2020 to 2023 ( χ2trend=5.997, P=0.014). From 2020 to 2023, chronic disease prevention health literacy showed a positive educational gradient, with significantly higher levels observed among more educated populations (both P<0.05). Except for 2020, the residents with household income≥30 000 yuan had a higher chronic disease prevention and control literacy than those with household income less than 30 000 yuan (both P<0.05); in 2021 and 2022, the chronic disease prevention and control literacy in the people of 45-59 years was higher than that in the individuals of 60-69 years (both P<0.05); in 2020 and 2022, the rural residents had a higher chronic disease prevention and control literacy than the urban residents, and the rates of chronic disease prevention and control literacy of employees or retirees in government institutions/doctors/teachers were higher than that in the peasants (both P<0.05); in 2023, the chronic disease prevention and control literacy of the smokers was lower than that in the non-smokers ( P=0.014). Conclusions:The prevalence of chronic diseases among the middle-aged and elderly residents in Wenzhou is relatively low, people with chronic diseases, especially those with multiple chronic diseases, have a low level of chronic disease prevention and control literacy. To prevent and control chronic diseases among them, interventions should be differentiated by the chronic disease status, occupation and household income.

In recent years,protein and polypeptide drugs have developed rapidly and have been widely used to treat cancer,hepatitis,and other diseases.The activity of proteins and polypeptides is closely related to their structure.Protein structures can be divided into four levels:primary,secondary,tertiary,and quaternary structures.The primary structure determines the advanced structure and biological functions of proteins.To ensure drug safety,it is necessary to carry out structural characterization and quality control.The advanced structure is the basis for proteins to express their functions and activities,and changes in spatial structure can lead to alterations in functions.This article reviews the progress of research on the primary and advanced structure characterization of proteins and polypeptides,including techniques such as reversed-phase high-performance liquid chromatography,Edman degradation,mass spectrometry,and spectroscopy.It aims to provide a reference for the structural characterization and quality control of other protein and polypeptide drug products.

5.Prevalence and influencing factors of cigarette and e-cigarette use among high school students in Wenzhou

Objective To study the prevalence of cigarettes and e-cigarettes, and to analyze its influencing factors among high school students in Wenzhou. Methods:A self-administered questionnaire survey was conducted among a total of 3 785students from senior high schools and vocational high schools in Wenzhou from September to November in 2020, who were selected by stratified cluster sampling. Their demographic information and the use of tobacco [including the use of cigarettes, e-cigarettes (currently smoking and smoking attempt), and the dual use of them] were asked in the questionnaire of “Global Youth Tobacco Survey”(GYTS). Proportion and rate were used to describe their demographic characteristics and smoking prevalence. Chi-square test was used to compare the difference in smoking prevalence of the high school students with selected characteristics. Multinomial logistic regression was used to analyze the influencing factors of tobacco use among the participants.Results:Among the 3 785 questionnaires, 3 740 were completed, and effective rate was 98.80%. The smoking attempt rate, current smoking rate, e-cigarette attempt rate, e-cigarette using rate, and dual using rate of the high school students in Wenzhou were 11.79% (441/3 740), 2.57% (96/3 740), 10.00% (374/3 740), 2.86% (107/3 740), 0.99% (37/3 740), respectively. Male students, vocational high school students, the students whose parents smoking showed higher smoking attempt rate, current smoking rate, e-cigarette attempt rate than female student, senior high school students and those parents who don′t smoke, respectively (all P<0.05). Best friend smoking [6.076 (2.872-12.855); 4.630 (2.656-8.071)], vocational high school [3.226 (1.877-5.546); 2.238 (1.432-3.496)] were risk factors for e-cigarette and the cigarettes use, belief that whether smoking or not does not make any difference in social activities [0.160 (0.077-0.331); 0.193 (0.096-0.387)], or smoking in social activities makes people more uncomfortable [0.037 (0.018-0.077); 0.155 (0.083-0.288)] were protective factors of e-cigarette and the cigarettes use. Additionally, passive smoking in past week [2.194 (1.129-4.262)], free tobacco products received [2.836 (1.018-7.905)], seeing smoking in campus [2.072 (1.111-3.863)] were all risk factors, while belief that smoking makes young people less attractive [0.280 (0.116-0.676)], not having been to tobacco retail outlets and unexposed to tobacco advertising or promotion [0.241 (0.123-0.473)] can protect senior middle school student away from cigarette. As to e-cigarette, boys were the risk factors [1.831 (1.100-3.050)]; as opposed to being a risk factor for cigarette use [1.942 (1.084-3.478)], rural area was a protective factor for e-cigarette use [0.639 (0.410-0.994)] among high school students in Wenzhou. Conclusions:High school students have relatively low rates of smoking cigarettes and e-cigarette in Wenzhou. Vocational students, best friend smoking are risk factors of e-cigarette and the cigarettes, beliefs that whether smoking or not does not make any difference or smoking makes people more uncomfortable in social activities are protective factors.
6. Major findings from the 4th Evaluation of the National Healthcare Improvement Initiative

Objective:
To carry out the 4th round of third-party evaluation on the implementation and effect of the 1st year of the 2nd Phase National Healthcare Improvement Initiative(abbreviated as Initiative)since 2015.
Methods:
The 4th round of the evaluation survey adopted the same methods, organization and execution, and technical roadmap as the former three rounds of evaluations.
Results:
The 4th round of evaluation was carried out from 18 March to 9 April, 2019 at 185 public hospitals in 31 provinces(autonomous regions, municipalities directly under the Central Government)and Xinjiang Production and Construction Corps.Facility survey, health professional survey and patient survey were conducted at each of the sample health facilities. A total of 120 782 valid questionnaires were collected from 144 non-psychiatric health facilities, 16 246 valid questionnaires were obtained from 41 psychiatric health facilities, and 252 cases of outstanding departments/hospitals in healthcare improvement were also collected. The average overall scoring of the 12 dimensions to assess Initiative implementation at 144 non-psychiatric health facilities was 84.4%. The overall outpatient satisfaction scoring was 91.1%, 96.7%for the inpatients. The overall inpatient satisfaction(family members inclusive) at 41 psychiatric health facilities was 93%. Areas remaining to be improved include day-surgery, telemedicine and medical social work. Compared with technical services, non-technical care should be further strengthened. The compensation, workload and work environment of the healthcare providers are still to be improved.
Conclusions
The implementation of the Initiative by health facilities has been greatly improved. The percentage of health facilities and patients who had positive perceptions of improved doctor-patient relationship has been increasing. Patient care experiences at public hospitals have been generally improved, and the implementation of promoting traditional Chinese Medicine practices also made progress. However, work satisfaction of healthcare providers was found to be rather low, compared to the high level of patient satisfaction.
