1.Incidence and Mortality of Liver Cancer in Fusui County of Guangxi Zhuang Autonomous Region in 2019 and Trend from 2011 to 2019

[Purpose]To analyze the incidence and mortality of liver cancer in Fusui County of Guangxi Zhuang Autonomous Region in 2019 and the trend of change from 201 1 to 2019.[Meth-ods]Based on the incidence and mortality data of liver cancer in Fusui County from 2011 to 2019,the crude incidence rate,crude mortality rate,age-standardized rate by Chinese standard population(ASIRC/ASMRC),age-standardized rate by world standard population(ASMRW/ASM-RW),disability-adjusted life years(DALY),years of life lost(YLL),years of lived with disability(YLD)were calculated.The Joinpoint linear regression model was used to calculate the average annual percentage change(AAPC)of the ASIRC/ASMRC and DALY rate of liver cancer in Fusui County from 2011 to 2019.[Results]In 2019,there were 345 new cases and 298 deaths of liver cancer in Fusui County,accounting for 28.49%and 35.86%of all new cases and death cases of malignant tumors in Fusui County,respectively.The crude incidence rate,ASIRC and ASIRW were 74.71/105,67.55/105 and 65.63/105,respectively;the crude mortality rate,ASMRC and ASMRW were 64.53/105,58.78/105 and 56.88/105,respectively.In 2019,ASIRC/ASMRC of men were higher than those of women(105.32/105 and 95.67/105 for men;27.26/105 and 19.11/105 for women).The peak incidence and mortality rates were in the 65~69 and 80~84 age groups,re-spectively.The incidence rate of liver cancer in Fusui County declined from 82.50/105 in 2011 to 67.55/105 in 2019,with an AAPC of-3.01%(P=0.024),but the mortality rate did not have sig-nificant changes(AAPC=-1.44%,P=0.224).The rate of DALY decreased from 14.51/105 to 10.36/105,with an AAPC of-2.82%(P=0.050).[Conclusion]The incidence rate and DALY rate of liv-er cancer in Fusui County showed an overall decreasing trend from 2011 to 2019,but still re-mained at a high level.It is necessary to further strengthen the prevention,screening and early treatment of liver cancer in this region,especially for men and the elderly.
2.Analysis of thirst sensation in patients with cirrhosis and its influencing factors

Objective To understand the current situation of thirst in patients with cirrhosis and analyze its influencing factors,in order to improve medical staff's attention to thirst symptoms in patients with cirrhosis and provide theoretical basis for clinical intervention.Methods A total of 220 patients with cirrhosis who were hospitalized in the infection department of a tertiary A general hospital in Wuhan from March to June 2024 were selected by convenience sampling method.General data questionnaire,Numerical Score Scale and Thirst Distress Scale were used to investigate the factors affecting thirst sensation in patients with cirrhosis.Results A total of 202 valid questionnaires were collected,and the effective questionnaire recovery rate was 91.82%.The results showed that the incidence of thirst in patients with cirrhosis was 59.41%;the score of thirst was 3.00(3.00,6.00)points,and the mean score of thirst was 3.09.Among them,43.07%of the patients with cirrhosis were in the moderate to severe level of thirst.The score of Thirst Distress Scale is 6.00(6.00,17.00)points,and the average score of thirst distress was 11.62 points,which was in the medium level.47.03%of patients with cirrhosis indicated thirst distress,and 23.76%of patients with cirrhosis which was in the moderate and severe level of distress.The results of multiple linear regression analysis showed that the degree of ascites,the use of diuretics,the stage of disease,and the degree of thirst distress were the factors influencing the degree of thirst in patients with cirrhosis(P<0.05).Gender,marital status,degree of ascites,use of diuretic drugs,disease stage and degree of thirst were the factors influencing degree of thirst distress in patients with cirrhosis(P<0.05).Conclusion The incidence and severity of thirst in patients with cirrhosis are relatively high,and are affected by many factors.Medical staff should pay more attention to the management of thirst symptoms in patients with cirrhosis,and formulate targeted nursing measures or nursing programs according to the related influencing factors of thirst,so as to improve the comfort level of patients and improve the disease experience of patients.
3.The rise,development and application of patient-derived immunocompetent organoid in the treatment of bladder cancer

In recent years,patient-derived tumor organoid(PDTO)models have rapidly emerged as important tools in cancer research,thanks to their unique ability to preserve the characteristics of primary tumors.These models provide a reliable preclinical research platform for screening individualized chemotherapy and targeted therapies for patients.However,traditional PDTOs lack immune cells and cannot replicate the tumor immune microenvironment,which restricts their utility in evaluating immunotherapies.To address this challenge,researchers have developed composite models that incorporate both tumor cells and immune cells,known as patient-derived immunocompetent tumor organoids(PDITOs).PDITOs have shown excellent performance in the preclinical evaluation of immunotherapies,particularly with PD-1/PD-L1 immune checkpoint inhibitors,with some studies reporting up to 100%accuracy in predicting patient responses to immunotherapy.As a common malignancy of the urinary system,bladder cancer has benefited from the application of PDITOs in drug screening and personalized immunotherapy evaluation,demonstrating significant potential.This paper aims to review the rise and development of PDITOs,and compare the advantages and limitations of using different methods to construct PDITOs,so as to explore their application in the treatment of bladder cancer.
4.Research progress on en bloc resection of bladder tumor:surgical techniques and pathological assessment

En bloc resection of bladder tumor(ERBT),as an innovative procedure for conventional transurethral resection of bladder tumor(cTURBT),has become an important treatment option for non-muscle invasive bladder cancer(NMIBC).Although ERBT has been recommended by international guidelines,its technical standardization,verification of long-term survival benefits and stratification of indications are still the focus of future research.This article first provides an overview of the surgical techniques of ERBT,and then elaborates on the advantages and disadvantages of the three energy sources of ERBT,namely,electrocautery,laser,and Hybridknife.In terms of pathological assessment,current research suggests that the detection of detrusor muscle in surgical specimens can reflect the quality of the surgery.The integrity of pathological specimens and diagnostic consistency have promoted the progress of sub-staging of T1 bladder cancer and have also driven more research on microscopic indicators related to prognosis.Finally,this article reviews whether repeated transurethral resection can be avoided to help optimize individualized treatment for bladder tumor.
5.The research progress of bladder preservation strategies for high-risk non-muscle invasive bladder cancer

High-risk non-muscle invasive bladder cancer (HR-NMIBC) is associated with a higher risk of recurrence and progression. The standard treatment involves intravesical Bacillus Calmette-Guérin (BCG) instillation following transurethral resection of the bladder cancer. However, in cases of BCG unresponsiveness, radical cystectomy is typically recommended. Due to the high complication rates and significant impact on quality of life associated with this procedure, bladder-preserving treatment (BPT) strategies have emerged as an alternative. This review summarizes recent advances in BPT, including combined chemoradiotherapy, immune checkpoint inhibitors, targeted therapies, localized precision treatments, and novel immunomodulators. With ongoing technological and therapeutic innovations, BPT has become an effective alternative for patients who are unable or unwilling to undergo surgery, reshaping the treatment landscape of HR-NMIBC.

8.Construction of device-related pressure injury prevention program in orthopedics patients based on evidence-based and SSKINS clinical management model

Objective:To construct a device-related pressure injury prevention program for orthopedic patients based on evidence-based and SSKINS (S: skin, S: surface, K: keep moving, I: incontinence, N: nutrition, S: self-care) clinical management model, and to provide reference for clinical nursing practice.Methods:In June 2023, a research team was set up to retrieve the literature related to device-related pressure injury in orthopedic patients, and the evidence was extracted and screened with reference to the SSKINS clinical management model, and the first draft of the protocol was formulated based on clinical practice. The final plan was determined through two rounds of expert consultations from October to December 2023.Results:The response rate of the two rounds of expert correspondence was 100%(20/20). The expert authority coefficients were 0.897 and 0.907, the overall Kendall coordination coefficients of the two rounds were 0.116 and 0.136 respectively, and the Kendall coordination coefficients of all levels were 0.104-0.213 (all P<0.001). The final protocol contains 6 first-level items (assessment and examination, the use and adjustment of device, prophylactic dressings, skin protection, nutritional support, health education) and 31 second-level items. Conclusions:The proposed orthopedic device-related pressure injury prevention program is scientific and practical, and can provide a reference for the prevention and management of clinical medical staff.
