Objective To simulate modern social stress using a pre-pregnancy chronic mild stress(CMS)model and to explore the mechanisms of emotional, behavioral, and neurodevelopmental changes in male offspring of pre-pregnancy liver qi stagnation female mice through corticosterone(CORT)-brain-derived neurotrophic factor(BDNF)extracellular signal-regulated kinase(ERK)1/2 signaling cascade-mediated hippocampal injury. This study aimed to elucidate the impact of negative life events on offspring and the interventional mechanism of Shuyu Capsules. Methods CMS stress was used to induce pre-pregnancy depression in female rats(liver qi stagnation state),followed by intervention with Shuyu and fluoxetine capsules. After screening, male rats were mated and 12 male offspring from each group were selected for behavioral testing and detection of serum CORT levels by enzyme-linked immunosorbent assay.BDNF,ERK1/2,phospho(p)-ERK1/2,cAMP-response element binding protein(CREB),and p-CREB protein levels in the hippocampus were detected by Western Blot, and BDNF,ERK1,ERK2,and CREB mRNA levels in the hippocampus were detected by reverse transcription-polymerase chain reaction(RT-PCR),to verify the effects of pre-pregnancy CMS on the BDNF-ERK1/2-CREB signaling pathway and to investigate the key micro-mechanisms of Shuyu Capsules on emotional and learning memory-related behaviors of male offspring of females with pre-pregnancy liver qi stagnation syndrome. Results The distance, number of entries, and duration of stay in the central area in open-field experiments were significantly reduced in offspring in the model group(all P<0.05).The escape latency during the exploration period of the water-maze experiment was significantly prolonged(P<0.05)and the swimming distance, duration of the target quadrant, and number of platform crossings were significantly reduced(P<0.05,P<0.05,P<0.01),the suspension time and frequency in the forced-swimming experiment were increased(P<0.05,P<0.01),and the incubation period was shortened(P<0.05)in offspring in the model group.Prophylactic treatment with Shuyu Capsules and fluoxetine improved the depression-like behavior and cognitive impairment in the offspring in the model group.Biochemical tests showed that CORT levels were increased in the CMS model group(P<0.05),BDNF,p-ERK1/2,and p-CREB protein levels in the hippocampus were decreased(all P<0.05),and BDNF,ERK1,ERK2,and CREB mRNA levels were significantly reduced(P<0.01,P<0.05,P<0.01,P<0.05).Treatment with Shuyu Capsules and fluoxetine increased the CORT content and BDNF,ERK1/2,and CREB protein and mRNA levels in male offspring to varying degrees. Conclusions High levels of CORT in offspring act selectively on the hippocampus, exerting adverse effects on the emotional and learning memory functions of rats by downregulating the BDNF-ERK1/2 signaling cascade. The Chinese medicine Shuyu Capsules can reduce the impact of an adverse intrauterine environment on offspring development by correcting abnormal levels and pathways of glucocorticoids.

Chronic pain causes physical suffering and can have major psychological impacts in patients.Chronic pain can induce depressive disorder, and clinical studies have consistently shown that chronic pain and depression frequently co-occur,suggesting the possibility of shared pathogenic mechanisms underlying these conditions.Acupuncture,as an alternative therapy, has been widely used for analgesia and to treat depression, with demonstrated clinical efficacy. The therapeutic mechanism of acupuncture is related to neural and endocrine regulation. This review considers the mechanism of chronic pain accompanied by depression,in relation to the brain regions and neural circuits affected by acupuncture treatment. This review provides a new approach for the treatment of depression caused by chronic pain.

Anxiety is a major emotional disorder manifested in the individual's expectation of future threats. The incidence rate of anxiety is about 7.3%,with the highest lifetime prevalence rate among mental health conditions. The mechanism of anxiety overlaps with depression, and anxiety is a typical symptom of various mental diseases or emotional disorders in traditional Chinese medicine. The high rates of comorbidity and disability pose serious threats to people's health.Animal models are important tools for studying anxiety and are of great use for deciphering the pathogenesis of anxiety and for developing drugs. The traditional paradigm of stress-induced anxiety, however,is relatively limited.Based on traditional theory combined with clinical and animal experimental data,we propose a new hypothesis of"insufficiency of ZhiYi'causing anxiety, defined as"an anxiety state induced by the inability of an individual to meet their own needs, limited or lacking after multiple attempts, rendered hindered and powerless by an inability to meet their desires".This hypothesis is more in line with the typical manifestations of despair, lack of pleasure, and social withdrawal in clinical patients, and is supported by traditional theory and experimental data showing"hunger but unable to eat, food but unable to obtain, and gain but not full".Based on this, the established modeling paradigm is easy to apply, with good repeatability and low cost, and can be used to establish anxiety models in rats and mice,to provide a theoretical and model basis for the development and pharmacological evaluation of anti-anxiety drugs.

Objectives:To explore whether short-course radiotherapy (SCRT)-based total neoadjuvant therapy (TNT) combined with PD-1 inhibitors could further promote tumor regression and improve the prognosis. Methods:This is a prospective, multicenter, two-arm randomized controlled, seamless phase Ⅱ/Ⅲ trial for proficient mismatch repair or microsatellite stable (pMMR/MSS) locally advanced rectal cancer (LARC). Eligible patients were randomly assigned to the iTNT (TNT+PD-1) group or the TNT group. Patients in the TNT group received SCRT (5 Gy×5) followed by 4 cycles of CAPOX or 6 cycles of mFOLFOX chemotherapy, with the iTNT group receiving SCRT followed by the same regime in combination with 4 cycles of Sintilimab. Total mesorectal excision (TME) surgery or watch and wait (W&W) was performed after neoadjuvant therapy and then 2 cycles of same regimen as before were recommended. The primary endpoints are the complete response (CR) rate for phase Ⅱ trial and 3-year disease-free survival (DFS) for phase Ⅲ trial. A total of 588 patients will be enrolled for the phase Ⅱ/Ⅲ trial. Short-term efficacy and safety data from the initial 100 treated patients were analyzed as planned. Results:From 2022-8-31 to 2023-5-24 the initial 100 patients were enrolled from 10 hospitals in China, 76.0%(76/100) patients were male, and the median age was 61 years (21-74 years). More patients had tumors located in the lower rectum (78.0%, 78/100), staged T3-4 (97.0%, 97/100) and N1-2 (93.0%, 93/100), and about half of the tumors invaded the mesorectal fascia (52.0%, 52/100) and with extramural vascular invasion (51.0%, 51/100). Analyses were performed according to the per-protocal (PP) set. All patients in the iTNT group ( n=52) and the TNT group ( n=48) completed SCRT; The 4-cycle chemotherapy±Sintilimab completion rates were 86.5% and 100.0% in the iTNT and TNT groups, respectively. In the iTNT group, 82.7% (43/52), 11.5% (6/52), and 5.8% (3/52) of the patients received 4, 3, and 2 cycles of PD-1 inhibitor. After TNT, 68 patients underwent radical surgery and 15 patients achieved cCR and adopted W&W. The pathological complete response (pCR) rates were 48.5% (16/33) and 17.1% (6/35) in the iTNT and TNT groups, with CR rates of 50.0% (25/50) and 26.1% (12/46), respectively. The incidence of treatment-related grade 3-4 adverse events was 26.9% (14/52, iTNT group) and 18.8% (9/48, TNT group), with thrombocytopenia and leukopenia being the most common. Among patients receiving immunotherapy, grade 3 immunotherapy-related adverse events occurred in 2 (3.8%, 2/52) patients: one case was pancreatitis, another case was hepatitis combined with myositis and myocarditis. Conclusion:The preliminary results show that SCRT-based TNT combined with PD-1 inhibitors could further improve the CR rate for LARC without unexpected serious adverse events.

Journal of Nanjing University of Traditional Chinese Medicine 2025;41(1):26-29
At present, the main reason for the lagging development of emotional disease theory and the backward development of targeted drugs is the lack of suitable and effective animal models, and model evaluation is the key and difficult point in the preparation of emotional disease animal models. The theory of syndrome element differentiation in traditional Chinese medicine is closely matched with the theory of behavioral elements in animal behavior, which is expected to bring theoretical changes and methodological progress in related fields.Based on this background, this paper systematically sorted out and clarified the theoretical conception of animal model e-valuation of emotional diseases:guided by the thinking mode of syndrome differentiation theory of TCM,an attempt was made to estab-lish a complete model evaluation system. It also pointed out the realization path of animal model evaluation of emotional disease based on syndrome element differentiation theory:collection of behavioral elements in natural state and analysis of time sequence, phenotypic identification of abnormal behaviors under stress state, and equivalent transformation of model animal syndrome discrimination.
