1.Differences in scapular kinematics between healthy individuals and rotator cuff tear patients based on biplane X-ray

BACKGROUND:Due to the complex movement of the scapula,which is a six-degree-of-freedom activity in three-dimensional space,it is difficult to measure it accurately using traditional methods.The image and model matching technology based on dual-plane X-ray is a three-dimensional measurement method that has gradually developed and matured in recent years.Two high-speed cameras are used to project and shoot from orthogonal directions.Compared with a single perspective,this method has advantages in observation range and reduction of out-of-plane errors,and is suitable for the study of scapula kinematics.OBJECTIVE:X-ray biplane and image-model registration technology were used to explore the differences in scapular kinematics between normal individuals and patients with rotator cuff tears,providing a basis for the treatment and rehabilitation of rotator cuff tear patients.METHODS:From April 2023 to January 2024,10 patients with normal shoulders and 10 patients with rotator cuff tears who met the inclusion criteria were enrolled from Guanghua Hospital Affiliated to Shanghai University of Traditional Chinese Medicine.The subjects underwent a shoulder CT scan to create a 3D model and a local scapular coordinate system.X-ray biplane images were taken during shoulder abduction with two C-arm machines.The 2D and 3D images were registered to compare scapular kinematic differences between the two groups at 0°,15°,30°,45°,60°,75°,and 90° of abduction,including scapular rotation angle and displacement distance.RESULTS AND CONCLUSION:(1)During shoulder abduction,the scapula of both groups showed upward rotation,but the upward rotation of the rotator cuff tear group was greater than that of the control group,and the difference was significant when the abduction was 30°-90°(P<0.01).At the same time,the scapula internal rotation of both groups gradually increased,but the rotator cuff tear group was significantly greater than the control group when the abduction was 45°-90°(P<0.01).In addition,the anteroposterior tilt of the scapula of the two groups was significantly different when the abduction was 15°-90°(P<0.01).The scapula posterior tilt of the control group gradually increased during abduction,while the scapula of the rotator cuff tear group tilted forward except for a slight posterior tilt at 15°-30° abduction.(2)In terms of displacement,the upward displacement of the rotator cuff tear group was less than that of the control group during abduction,and the difference was significant at 15°-90°(P<0.05),but there was no significant difference in lateral and anterior-posterior displacement between the two groups(P>0.05).(3)Rotator cuff tear can cause scapular dyskinesis,characterized by increased upward rotation,internal rotation,and abnormal forward tilt during shoulder abduction.Identifying and addressing scapular dyskinesis is crucial for treating rotator cuff tear.

3.Flexible subtraction CE-Boost technique combined with low dosage contrast agents for CT pulmonary angiography

Objective To investigate the value of flexible subtraction CE-Boost technique combined with low dosage contrast agents for CT pulmonary angiography(CTPA).Methods A total of 68 patients who would undergo CTPA examination due to suspected pulmonary embolism(PE)were prospectively enrolled and randomly divided into study group(n=34)and control group(n=34)using block randomization method.After injecting 25 ml contrast agents at a flow rate of 2.5 ml/s in study group or 50 ml contrast agents at a flow rate of 3.5 ml/s in control group,CTPA scanning were performed with identical parameters.For images in study group,hybrid iterative reconstruction was performed,followed by flexible subtraction CE-Boost post-processing to obtain CE-Boost CTPA.For images in control group,conventional CTPA was obtained with hybrid iterative reconstruction.Subjective and objective evaluations of image quality were compared between groups.Taken the final clinical diagnosis as standard,the accuracy rate of diagnosing PE were compared between groups.Results There were 7 cases of pulmonary artery main trunk PE and 15 cases of pulmonary lobe-level PE in study group,while in control group there were 8 cases and 17 cases.No statistical difference of subjective scores of CTPA was found between groups(P>0.05).CT values of the main pulmonary artery,bilateral pulmonary artery trunks and lower lobes of both lungs,signal-to-noise ratio or contrast-to-noise ratio in CTPA were not significantly different between groups(all P>0.05),while no significant difference of the accuracy rate of CTPA for diagnosing PE of pulmonary artery main trunk(100%[7/7]vs.100%[8/8])nor pulmonary lobe-level PE(86.67%[13/15]vs.88.24%[15/17])was detected between groups(all P>0.05).Conclusion Flexible subtraction CE-Boost technique combined with low dosage contrast agents for CTPA could reduce contrast agent dosage without affecting image quality.
4.Influence of Da Vinci robot-assisted laparoscopic partial nephrectomy via different approaches on renal function and intestinal function recovery in the treatment of central renal tumors

Objective To explore the influence of Da Vinci robot-assisted different approaches of laparoscopic partial nephrectomy(LPN)on the renal function and intestinal function recovery in the treatment of central renal tumors.Methods Totally 100 patients with central renal tumors who received Da Vinci robot-assisted LPN in the Central Hospital of Enshi Tujia and Miao Autonomous Prefecture were selected from November 2022 to March 2024.According to the consent of the patients,they were divided into transperitoneal group and retroperitoneal group by envelope drawing method,with 50 cases in each group.The transperitoneal group adopted transperitoneal approach,while the retroperitoneal group received retroperitoneal approach.The surgical status and postoperative recovery status,renal function[serum creatinine(Scr),glomerular filtration rate(GFR)and blood urea nitrogen(BUN)],intestinal function recovery status[restore exhaust time,plasma diamine oxidase(DAO),D-lactic acid and urinary lactulose/mannose(L/M)]and complications were compared between groups.Results The surgical time,intraoperative blood loss,renal warm ischemia time,restore exhaust time,urinary catheter indwelling time and hospitalization time with(126.76±23.89)h,(115.86±6.93)ml,(25.41±6.57)min,(2.29±0.56)d,(4.89±0.93)d and(11.08±1.92)d in transperitoneal group were longer or more than the retroperitoneal group[(111.58±21.86)h,(86.27±6.71)ml,(17.08±5.16)min,(1.73±0.43)d,(3.16±0.62)d and(9.29±2.07)d,all P<0.05].1 month after surgery,the SCr level in transperitoneal group with(83.04±10.13)μmol/L was lower than(88.55±9.16)μmol/L in retroperitoneal group while the GFR value with(58.27±6.19)ml/min was higher than(54.36±6.05)ml/min in retroperitoneal group(P<0.05).The levels of DAO,D-lactic acid and the value of urine L/M with(3.39±0.57)mg/L,(2.09±0.31)mg/L and(0.08±0.02)in transperitoneal group at 24 hours after surgery,which were higher than the retroperitoneal group[(3.07±0.46)mg/L,(1.72±0.25)mg/L and(0.05±0.02),P<0.05].The total incidence rates of postoperative complications in transperitoneal group and retroperitoneal group were 12.00%and 8.00%(P>0.05).Conclusion The two approaches of LPN assisted by Da Vinci robot have their own advantages in the treatment of central renal tumors.Transperitoneal approach is more conducive to improving renal function,but retroperitoneal approach is more conducive to shortening the postoperative recovery time and promoting the intestinal function recovery and has lower incidence rates of complications.

7.Pancreaticoduodenectomy for duodenal papillary carcinoma:tumor size and lymph node metastasis affect long-term survival

Objective:To investigate the clinicopathological factors affecting the long-term survival of patients with duodenal papillary carcinoma (DPC) after pancreaticoduodenectomy (PD).Methods:The clinicopathological and follow-up data of patients with DPC who underwent PD at the First Affiliated Hospital of Anhui Medical University and the Second Hospital of Anhui Medical University from Jan 2015 to Dec 2021 were retrospectively analyzed.Results:All 73 cases have been followed-up. The median follow-up time was 60 months. Multivariate analysis of COX proportional risk model showed that positive lymph node metastasis and tumor size over 2.5 cm were common independent risk factors for OS and DFS. Lymph node metastasis was confirmed pathologically in 20 patients. Multivariate analysis results of Logistic regression model showed that smoking, tumor breaking through the serous layer and tumor low differentiation were independent risk factors for lymph node metastasis.Conclusions:Poor prognosis was associated with tumors that were larger than 2.5 cm, and with lymph node metastases. Preoperative smoking history, tumor breaking through the serous layer and low tumor differentiation were the predictors of positive lymph node metastasis.
8.A case of primary small intestinal mucosa associated lymphoid tissue lymphoma presenting with recurrent ileus

Primary gastrointestinal extranodal marginal zone lymphoma of mucosa associated lymphoid tissue (MALT) is more likely to occur in stomach, and may also in esophagus, small intestine, colon and rectum. But primary small intestinal MALT lymphoma is relatively rare, lack of specific clinical manifestations and easily to be misdiagnosed. This paper reports a case of primary small intestinal MALT lymphoma with intestinal obstruction as initial symptom and segmental ulcerative lesion, providing reference for clinical diagnosis and treatment.
9.Effects of TAMs and CXCL1 on the biological behavior of hepatocellular carcinoma cell line Huh7

Objective:
To investigate the effects CXCL1 derived from TAMs on the progression of HCC by CXCL1 in tumor microenvironment (TME) .
Methods :
PMA induced human monocyte (THP-1) to obtain undifferentiated macrophages (M0) and then co-cultured with Huh 7 HCC cells.The biomarkers of macrophage phenotype and mRNA level of HCC were analysed by qRT-PCR. Colony formation assay,cell viability assay and transwell assay were performed to evaluate the biological alteration of HCC cells in TME.Epithelial-mesenchymal transition (EMT) molecular genetics were detected using western blot.The levels of CXCL1 in TME were measured by ELISA assay.
Results :
High CXCL1 expression in HCC patients predicted poor prognosis.Abundant macrophages were found in CXCL1 high expression HCC tissues.In macrophages and HCC co-cultured model,significantly increasing CXCL1 expression was detected in both two cells and the biomarkers of M2 macrophages,CD163 and CD206 were elevated.The growth and migration of HCC cells were promoted.
Conclusion
Co-culture of macrophages with HCC cells induces macrophages pro-tumor phenotype and CXCL1 secretion which promotes the progression of HCC.
