1.Amelioration of depressive behaviours in CUMS rats via PI3K/AKT pathway and HPA axis by Anshen Jieyu decoction

Objective:To explore the effects of anshen jieyu decoction on hippocampal morphology and regulation of PI3K/AKT signalling pathway with hypothalamic-pituitary-adrenal axis expression in CUMS depressed rats.Methods:Rats were divided into a control group(Control),CUMS model group(CUMS),and ASJYD treatment group(CUMS+ASJYD).Rats in the CUMS and CUMS+ASJYD groups were exposed to 10 different chronic stressors to induce depressive-like behaviors.After modeling,the CUMS+ASJYD group received ASJYD via gavage.Depressive-like behaviors were assessed using the sucrose preference test and open field test.Serum levels of adrenocorticotropic hor-mone(ACTH)and corticosterone(CORT)were measured by ELISA.Hippocampal neuronal morphological changes were observed via Nissl staining,mitochondrial ultrastructure was examined using transmission electron microscopy,and hippocampal PI3K and AKT protein phosphorylation levels were detected by Western blot.Results:Compared with the Control group,the CUMS group showed significantly reduced sucrose consumption,preference rate,total travel distance in the open field,and central zone activity time(P<0.01).Serum ACTH and CORT levels were elevated(P<0.01),with disorganized hippocampal cell arrangement,reduced cell count,mitochondrial swelling,cristae blurring/rupture observed under electron microscopy,and decreased phosphorylation levels of PI3 K and AKT proteins(P<0.01).After ASJYD treatment,the CUMS+ASJYD group exhibited significant improvements in sucrose consumption,preference rate,locomotor activity,and central zone exploration(P<0.01),accompanied by reduced ACTH/CORT expression,alleviated hippocampal pathology,restored mitochondrial integrity with clear cristae,and increased PI3K/AKT phosphorylation(P<0.01).Conclusion:Anshen Jieyu decoction significantly improved the depression-like be-haviour of CUMS rats,which may be achieved by down-regulating the hyperactive hypothalamic-pituitary-adrenal axis,regulating the expression of PI3K/AKT signaling pathway,and ameliorating the damage to hippocampal neurons and mi-tochondria.
2.Analysis of the Construction and Path of China's Hospital Digital Transformation Model Based on the Multiple Case Study

Objective To investigate the drivers,stakeholders,core tasks,and differentiated development models of digital transformation in Chinese hospitals,develop a hospital digital transformation model,and propose advancement pathways.Methods Leveraging ROCCIPI theory,socio-technical systems theory,and social network theory,a multiple case study approach was employed to analyze four representative Chinese hospitals,examining the driving factors,social network relationship,and core tasks of digital transformation.Results Hospital digital transformation is a complex process driven by regulations,opportunities,and capabilities,requiring efficient collaboration among stakeholders focused on patient services,clinical operations,hospital management,and security.It identified three development models-ecology-oriented,regional integration,and grassroots enhancement—based on the distinct characteristics of the hospitals.A theoretical model for digital transformation in four Chinese hospitals was developed,along with proposed pathways and strategies.Conclusion It presents a digital transformation model and advancement pathways for hospitals through multiple case analyses,addressing the limited perspectives of existing research and providing a reference for practice.
3.Luspatercept combined with roxadustat in the treatment of refractory myelodysplastic neoplasms with ring sideroblasts: a prospective, randomized, single-center study

Objective:To evaluate the efficacy and safety of luspatercept combined with roxadustat in patients with refractory low-risk myelodysplastic neoplasms with ring sideroblasts (MDS-RS) patients.Methods:In this single-center, prospective, randomized controlled trial, patients with refractory MDS-RS were randomly assigned in a 1:2 ratio to receive either combination therapy (luspatercept + roxadustat) or luspatercept monotherapy. The primary endpoint was erythroid response at 12 weeks, while secondary endpoints included erythroid response at 24 weeks, achievement of transfusion independence ≥8 weeks within the first 12 weeks, and other hematologic indicators.Results:The combination therapy and monotherapy groups included 16 and 32 patients, respectively. Baseline demographic characteristics, laboratory tests, IPSS-R risk classification, transfusion burden, EPO levels, and previous treatment history were comparable between the two groups ( P>0.05). With similar doses of luspatercept and follow-up durations, no significant differences were observed between the groups at either 12 or 24 weeks in terms of erythroid response, transfusion independence, or other clinical indicators (all P-values>0.05). The incidence of adverse events was similar in both groups (all P-values>0.05) . Conclusion:Luspatercept combined with roxadustat shows comparable efficacy and safety to luspatercept monotherapy in the treatment of refractory low-risk MDS-RS. 4.Progress in animal models of perimenopausal depression

Perimenopausal depression seriously affects women's physical and mental health.It is caused primarily by gonadal function decline,which is typically characterized by low mood,anxiety,slow thinking,and loss of interest,and is accompanied by autonomic dysfunction and endocrine dysfunction.The pathogenesis of perimenopausal depression remains unclear and controversial.Many scholars have conducted scientific research that is based on animal models of perimenopausal depression.Indeed,a good animal model of perimenopausal depression is the basic premise for studying its pathophysiological mechanisms and for facilitating reliability of the scientific result.Therefore,this paper combines the modern research mechanisms of perimenopausal depression and the current status of animal models in China,and provides an overview,evaluation,and generalization of the modeling method,principles and result,to provide a scientific basis and reference for the selection of suitable animal models for scientific research experiments.

9.Use of autologous fat grafting in prevention and treatment of expansion complications

Objective:To explore the effectiveness of autologous fat grafting in the treatment of undesirable skin expansion.Methods:Patients' data were reviewed from 2011 to 2016, including the expanded regions with early signs of skin complications in face and neck. The effects of fat grafting group and control group were evaluated by follow-up records of expansion volume, skin thickness, skin texture and local capillary reaction.Results:Fat grafting could increase the thickness of expanded skin. It also improved the texture of expanded skin, with 0.83± 0.71 points before treatment and 1.30±0.66 points after treatment ( P=0.04). The local capillary reaction was also improved from 1.06±0.54 points before treatment and 1.45±0.51 points after treatment ( P=0.03). The expansion in the fat grafting group was 2.21±0.57 times before treatment and 2.94±0.83 times after treatment. In the control group, the expansion was 2.19 times when it showed early signs, and no obvious changes were observed during the follow-up period. Conclusions:Autologous fat grafting can effectively treat complications of skin expansion, prolong expansion process and promote tissue regeneration.
10.Effects of type D personality and perceived control on experiential avoidance in patients with coronary heart disease

Objective:To explore the effect of type D personality and perceived control on experiential avoidance in patients with coronary heart disease (CHD) , so as to provide a basis for reducing their experiential avoidance.Methods:From December 2018 to August 2019, a total of 365 patients with CHD who were admitted and treated in a ClassⅢ Grade A hospital in Anhui Province were selected as research objects by convenient sampling method. Type D Personality Scale-14 (DS-14) , Control Attitudes Scale-Revised (CAS-R) , Acceptance and Action Questionnaire 2nd Edition (AAQ-Ⅱ) were used to investigate those patients.Results:Among the 365 patients with coronary heart disease, 128 were type D personality, accounting for 35.1%. The score of CAS-R was (25.18±5.73) and that of AAQ-Ⅱ was (21.61±5.99) . Multiple linear regression showed that the negative emotions and social inhibition of type D personality, perceived control and residences had predictive effects on experiential avoidance in patients with CHD, and the joint explanation variation was 47.0%.Conclusions:The experiential avoidance of patients with CHD needs to be improved. Clinical nurses should actively assess whether patients with CHD have type D personality and help patients improve their perceived control so as to reduce their experiential avoidance.
