1.Clinical Efficacy of Shenqi Yangxin Decoction in Treatment of Patients with Ischemic Cardiomyopathy and Its Effect on Serum H2S and Ca2+

ObjectiveTo evaluate the clinical efficacy of Shenqi Yangxin decoction in the treatment of ischemic cardiomyopathy (ICM) with Qi and Yin deficiency and blood stasis syndrome and its effect on serum hydrogen sulfide (H2S) and calcium ion (Ca2+). MethodsA total of 64 ICM patients with Qi and Yin deficiency and blood stasis syndrome who met the inclusion criteria were randomly divided into a control group (n=32) and a treatment group (n=32). All patients received conventional Western medicine treatment. The treatment group was additionally given Shenqi Yangxin decoction. The TCM syndrome score, Minnesota Living with Heart Failure Questionnaire (MLHFQ) score, left ventricular ejection fraction (LVEF), N-terminal pro-B-type natriuretic peptide (NT-proBNP), 6-minute walk test (6MWT), New York Heart Association (NYHA) cardiac function classification, and serum H2S and Ca2+ levels were compared between the two groups pre- and post-treatment. ResultsTwo cases dropped out from each group during the study. Finally, 30 patients in each group were included in the analysis. There were no significant differences in age, gender, course of coronary heart disease, underlying diseases, and laboratory tests between the two groups. Compared with baseline, the TCM syndrome score, MLHFQ score, and NT-proBNP in both treatment group and control group decreased significantly (P<0.01), LVEF, 6MWT, and H2S increased significantly (P<0.01), and serum Ca2+ increased (P<0.05). Compared with the control group after treatment, the MLHFQ score and NT-proBNP in the treatment group decreased (P<0.05), the TCM syndrome score decreased significantly (P<0.01), LVEF, 6MWT, and serum Ca2+ increased (P<0.05), and H2S increased significantly (P<0.01). The improvement degree of the NYHA cardiac function classification in the treatment group was higher than that in the control group, but there was no significant difference. ConclusionShenqi Yangxin decoction is effective in treating ICM patients with Qi and Yin deficiency and blood stasis, which could significantly improve cardiac function and quality of life, and its therapeutic effect may be related to the regulation of serum H2S and Ca2+ levels.
2.Impacts of ionizing radiation dose on NADPH oxidase expression in mouse intestine

Objective:To investigate the effects of different doses of ionizing radiation on the changes in nicotinamide adenine dinucleotide phosphate (NADPH) oxidase and reactive oxygen species (ROS) levels in the intestines of mice.Methods:C57BL/6 mice aged 6-8 weeks were randomly assigned to four groups (0 Gy, 0.1 Gy, 0.2 Gy, and 0.5 Gy; n=10/group) and subjected to single whole-body irradiation using a 60Co γ-ray source at a dose rate of 13 mGy/min. At 20 weeks post-irradiation, jejunal, ileal, and colonic tissues were collected. Immunohistochemistry, Western blotting, and reverse transcription-polymerase chain reaction (RT-PCR) were employed to assess mRNA and protein expression of NADPH oxidase components. Hydrogen peroxide (H 2O 2) levels and the expression of Nuclear Factor-kappa B (NF-κB), a transcriptional regulator of Dual Oxidase 2 (DUOX2), were also measured. Results:Compared with the 0 Gy group, mice in the 0.5 Gy group exhibited shortened villus length in the jejunum, villus fusion in the ileum, and increased crypt spacing in the colon, with statistically significant differences ( t=2.48, P < 0.05). No significant differences were observed in other dose groups compared to the 0 Gy group ( P > 0.05).The expression of H 2O 2 in the jejunum, ileum, and colon of the 0.1 Gy group was significantly elevated compared to the 0 Gy group ( t=4.12, 3.12, 3.12; P < 0.05). In the 0.5 Gy group, H 2O 2 expression in the jejunum and colon increased nearly twofold relative to the 0 Gy group ( t=8.67, 8.69; P < 0.05).At 20 weeks post-irradiation, DUOX2 protein expression levels in the jejunum, ileum, and colon were markedly higher in irradiated mice than in the 0 Gy group ( t=3.03, 10.29, 2.74; P < 0.05). DUOX2 mRNA levels in the jejunum, ileum, and colon of the 0.1 Gy group were significantly upregulated compared to the 0 Gy group ( t=12.75, 4.12, 11.14; P < 0.05). Additionally, NOX4 mRNA expression increased in the jejunum of the 0.2 Gy group ( t=4.54, P < 0.05) and in the ileum of the 0.1 Gy group ( t=4.13, P < 0.05).The nuclear factor kappa-B (NF-κB), a transcriptional regulator of DUOX2, showed an upward trend in expression in the jejunum, ileum, and colon of the 0.1 Gy group compared to the 0 Gy group, with statistically significant differences ( t=8.73, 8.18, 7.02; P < 0.05). Conclusion:Low-dose radiation induces long-term effects on the intestinal tract. Specifically, 0.5 Gy irradiation causes mild morphological alterations in the jejunum, ileum, and colon, while 0.1 Gy irradiation promotes the upregulation of DUOX2, a NADPH oxidase, in intestinal tissues.

4.Analysis of CMV and EBV infection in healthy populations in China before and after the COVID-19 pandemic

Objective:This study aimed to assess the infection status of cytomegalovirus (CMV) and Epstein-Barr virus (EBV) in healthy populations in China over the past decade and analyze the differences in CMV and EBV infection and related risk factors in healthy populations before and after the lifting of coronavirus disease 2019 (COVID-19) pandemic control measures.Methods:This study retrospectively analyzes the CMV and EBV infection status of 8 827 healthy donors who underwent prehematopoietic stem cell transplantation screening at Peking University People's Hospital from January 2014 to December 2023. Logistic regression analysis was conducted to determine the risk factors for CMV and EBV infection.Results:The CMV and EBV IgG positivity rates were 94.52% and 95.40% among the healthy donors, respectively, with no significant differences before and after the lifting of pandemic control measures (all P value>0.05). However, IgG antibody titers increased [CMV: (100.44±36.50) U/ml vs (109.98±36.31) U/ml, P<0.001; EBV: (281.57±226.79) U/ml vs (361.08±268.58) U/ml, P<0.001] after lifting the COVID-19 restrictions. However, the CMV IgM positivity rate remained unchanged. The EBV IgM positivity rate significantly increased after lifting measures (2.77% vs 6.29%, P<0.001), reaching 8.10% within 3 months. Further analysis of the factors affecting EBV IgM positivity revealed that gender ( OR=1.479, 95% CI 1.169-1.872, P=0.001), age[compared with the group younger than 18 years, the 18-50-year age group ( OR=0.584, 95% CI 0.421-0.820, P=0.002), the >50-year age group ( OR=0.389, 95% CI 0.248-0.610, P<0.001) ], and the lifting of COVID-19 restrictions ( OR=2.360, 95% CI 1.287-3.047, P<0.001) were independent factors influencing EBV IgM positivity in the general population. The EBV IgM positivity rate in individuals under 18 years old was not affected by gender or the lifting of COVID-19 restrictions when stratified by age group. Both genders ( OR=1.499, 95% CI 1.138 - 1.975, P=0.004) and the lifting of COVID-19 restrictions ( OR=2.608, 95% CI 1.940-3.507, P<0.001) were independent factors affecting EBV IgM positivity in the 18-50-year age group. The lifting of COVID-19 restrictions ( OR=2.222, 95% CI 1.101-4.484, P=0.026) was the sole independent factor affecting EBV IgM positivity in individuals over 50 years old. Conclusions:Previous infection rates of CMV and EBV are high in healthy populations in China, which increase with age. COVID-19 infection may increase EBV reactivation rates in healthy individuals, with a more pronounced effect on those aged >18 years.

6.Bibliometric Study on Health Utility Value Obtained by Mapping Method in Pharmacoeconomic Evaluation

OBJECTIVE:To know about the research status of health utility value obtained by mapping method in pharmacoeconomic evaluation ,and to provide reference for bibliometric study in pharmacoeconomic evaluation . METHODS : Using“Mapping method ”“Health utility value ”“Cost-utility”“Utility point system ”as Chinese and English keywords ,retrieved from CNKI ,Wanfang database ,PubMed,Medline,Ebsco,Ovid and Wiley database ,empirical journal documents published from the inception to Dec. 31st,2018 about using mapping method to obtain health utility value were collected. The bibliometrics was used to statistically analyze basic information of included literature ,the construction and test of the model ,the type of the best model and so on. RESULTS :The 124 included documents were all published in English journal. In the construction and testing of the mapping model ,the most frequently used econometric methods ,performance evaluation indicators and model testing methods were ordinary least squares (OLS),mean absolute error (MAE)and residual normality test ,application frequency of which were 97 times(31.60%),89 times(24.93%)and 62 times(21.09%). There are 117 articles that define the best mapping model ,of which 101 articles(86.32%)have the best direct mapping effect. Most of the non-utility measurement scales adopted specific scales (92 articles,77.97%),and a few literatures adopted the universal scale (26 articles,22.03%). The most utility measurement scales were 3-level European 5-dimensional health scale (79 articles,66.95%). CONCLUSIONS :The domestic empirical researches that use the mapping method to obtain health utility values need to be developed yet. A series of mapping models successfully developed by foreign scholars not only provide the feasibility of using non-utility measurement scales for cost-utility analysis ,but also provide more ideas for China to choose the corresponding econometric methods ,evaluation indicators and mapping methods in the empirical research of the mapping method in the future.
7.Construction of a lentiviral low expression of miR-139-5 p vector and validation of its transduction efficiency in H9 c2 cells

Aim To construct a lentiviral low expres-sion of miR-139-5 p vector and validate its expression efficiency in H9c2 cells. Method Target sequence was designed according to the sequence of rat miR-139-5p. Oligonucleotide duplex was synthesized and cloned into the lentiviral vector pGC-LV. The recombi-nant lentiviral vector, pHelper 1. 0, and pHelper 2. 0 were co-transfected into 293T cells, packaging virus. Then H9 c2 cells were infected with the supernatant containing lentiviral particles, and its infection effi-ciency and miR-139-5 p expression were determined by fluorescent microscope and real-time quantitative PCR, respectively. Results A lentiviral low expression of miR-139-5p vector was successfully constructed. The infection efficiency in H9c2 cells reached over 95%, and the relative expression of miR-139-5p was significantly down-regulated. Conclusion The lentiviral low expression of miR-139-5p vector is successfully constructed, and the expression of miR-139-5p in infected H9c2 cells is inhibited effectively.
8.Research on Mathematical Logic in Syndrome Theory of TCM and False Proposition Features of“Element of Syndrome”

Through the study on mathematical logic relation in the syndrome theory of traditional Chinese medicine (TCM), it revealed that the mathematical logic relation among three basic elements in the judgment of syndromes, which were the disease cause (a), disease location (b), disease nature (c) and syndrome (z), was f(z)=a+b+c. The mathematical logic relation between syndrome (z) and symptom (zi) was f(z)= z1+z2+z3+…+zi. The obvious feature reflected by the complexity of syndromes was the construction number of symptoms. During the syndrome differentiation process, the mathematical logic relation between symptom and disease cause, location, nature and syndrome was z1+z2+z3+…+zi =a+b+c= f(z). However, syndromes generally exhibited a nonlinear relationship of point-set topology. After calculation, 79 major single TCM syndromes had a total of more than 6.5×105 different forms. The number within a certain range of syndrome group was approximately 6.6×10100. The super huge data “Googol” may be the root of complex TCM syndromes. The results showed that the researches on “element of syndrome” using the bayesian networks, the neural network algorithm and the algorithm of double levels of frequency power were irrelevant to the mathematical logic relation of the intrinsic relations of syndrome theory. Therefore, “element of syndrome” was a false proposition feature in the study of TCM basic theory. The established syndrome differentiation method with“element of syndrome” as its core was not conformed to the inherent law of TCM theory and clinical practice.
9.Approaches for developing a system model for health evaluation based on body constitutions of traditional Chinese medicine and order parameters.

From the point of view of systems science, human body can be considered as a complex system, and the human health system is a subsystem of it. Systems science conducts investigation in a holistic manner. As a theoretical method, it deals with the operation and evolution of systems from the macroscopic perspective, so this theory is similar to phenomenological theory of traditional Chinese medicine (TCM) in methodology. Naturally, numerous theories of systems science can be used in research of the human health systems of TCM. In this paper, the authors introduced synergetics, a theory of modern systems science, and its slaving principle, and in particular, analyzed the concept of order parameters related to the slaving principle and the relationship between body constitutions of TCM and order parameters. The body constitution of TCM can be treated as a slow variable in the human health systems. By using synergetics, the authors established a model of the human health system based on body constitutions of TCM. As an application of the model, the authors illustrated the argumentations in the theory of constitution being separable, the theory of a relationship between constitution and disease, and the theory of a recuperable constitution. To some extent, this work has made links between the TCM theory of body constitution and modern systems science, and it will offer a new thought for modeling the human health system.
10.Cardioprotection of ramipril and BQ-123 against myocardial ischemia/reperfusion oxidative injury in vivo in rats

Aim To investigate the protection of ramipril,BQ-123 and their combination against myocardial ischemia/reperfusion(I/R)injury in vivo in anesthetized rats,and to explore the mechanism of action of drugs on myocardial oxidation-antioxidation system.Methods Healthy male Wistar rats were divided into 5 groups randomly,sham operated(Sham)group,I/R group,ramipril(RAM)group,BQ-123(BQ)group and ramipril and BQ-123(R&B)group.All groups but not sham were subjected to I/R procedure.Twenty four hours before ligation,ramipril(1 mg·kg~(-1))was intragastrically administered to rats in RAM and R&B groups.The equal volume of normal saline was given to rats in other groups.BQ-123(10 μg·kg~(-1)· min-1)was infused intravenously from 10 min before ligation to the end of 30 min ischemia to rats in BQ and R&B groups.The equal volume of normal saline was given to other groups.HR,MAP and the change of ST-segment were observed;ventricular arrhythmias were monitored during ischemia;the infarct size was examined by TTC staining;the activity of myocardial T-SOD,Mn-SOD,CAT and the content of MDA were detected by spectrophotometer.Results Compared with I/R group,the elevation of ST-segment was decreased,onset of VPC and VT was delayed,duration of VPC and VT was shortened,incidence of VPC,VT and VF was decreased,IS and IS/AAR were improved,activity of T-SOD,Mn-SOD and CAT was increased,the content of MDA was decreased in RAM,BQ and R&B groups.Compared with RAM and BQ alone group,onset of VPC and VT,duration of VPC and VT,size,activity of T-SOD and Mn-SOD and content of MDA were changed dramatically in R&B group.Conclusions Ramipril,BQ-123 and the combined use of these two agents protected myocardium from I/R injury in vivo.The protective effects of the combination on delaying onset of VA,shortening duration of VA,decreasing infarct size and content of MDA,and increasing activity of SOD are better than those of using ramipril or BQ-123 alone.
