Functional dyspepsia (FD) is a prioritized disease category where traditional Chinese medicine (TCM) demonstrates distinct therapeutic advantages. The current western medicine treatment for FD is mainly based on proton pump inhibitors and prokinetic agents, with digestive enzymes, probiotics and antidepressants serving as adjuvant medication, yet such therapies still have certain limitations. TCM treatment for FD includes oral administration of Chinese herbal formulas and Chinese patent medicines, as well as external TCM therapies such as acupuncture and moxibustion, acupoint application, hot medicinal compress therapy, rubbing with ointment, medicinal iontophoresis, auricular acupoint therapy and tui na (Chinese medical massage). The combined treatment of FD with integrated TCM and western medicine can significantly improve clinical effectiveness and reduce adverse reactions. The common mechanisms underlying the therapeutic effects of both TCM and western medicine revolve around the core pathological processes of FD, mainly focusing on restoring gastrointestinal motility, regulating the levels of brain-gut peptides, modulating intestinal microecology, and ameliorating inflammatory status. The differential mechanisms lie in the precise targeting feature of western medicine versus the holistic-regulating and multi-target characteristics of TCM, and the two approaches exert a synergistic effect to enhance efficacy. This paper proposes to leverage the advantages of TCM in holistic regulation and the strengths of western medicine in targeted treatment, so as to provide personalized and comprehensive treatment regimens for FD patients.
2.Application value of total cerebral small vessel disease score combined with three-dimensional arterial spin labeling in predicting the prognosis of patients with acute ischemic stroke after intravenous thrombolysis
Journal of Apoplexy and Nervous Diseases 2026;43(1):15-19
Objective To investigate the value of total cerebral small vessel disease (CSVD) score combined with three-dimensional arterial spin labeling (3D-ASL) in predicting the prognosis of patients with acute ischemic stroke (AIS) after 90 days of intravenous thrombolysis. Methods A total of 189 AIS patients who were consecutively admitted to Department of Neurology, Changshu Hospital Affiliated to Nantong University, from January 2021 to June 2023 and underwent intravenous thrombolysis with alteplase were enrolled, and multimodal brain MRI was performed to assess total CSVD load and cerebral blood flow in the area of responsibility. A multivariate logistic regression analysis was used to identify the independent influencing factors for poor 90-day prognosis in AIS patients undergoing intravenous thrombolysis. The receiver operating characteristic (ROC) curve was used to investigate the value of total CSVD score and 3D-ASL used alone or in combination in predicting the prognosis of AIS. Results The multivariate regression analysis showed that diabetes (OR=3.601,95%CI 1.448‒8.954,P=0.006), total CSVD score ≥3 points (OR=4.077, 95%CI 1.785‒9.313,P=0.001), and baseline NIHSS score (for every 1-point increase:OR=1.167, 95%CI 1.061‒1.283, P=0.001) were independent risk factors for poor prognosis in AIS patients on day 90 after intravenous thrombolysis, while ASL hyperperfusion was an independent protective factor against poor prognosis (OR=0.16, 95%CI 0.050‒0.356,P=0.014).The ROC curve analysis showed that the predictive model based on CSVD score and ASL perfusion had a C-value of 0.844 (95%CI 0.786‒0.902, P<0.05) and thus had the best predictive efficacy. Conclusion Total CSVD score combined with 3D-ASL can more comprehensively reflect the whole brain circulation network and has a better effect than each indicator alone in predicting the 90-day prognosis of AIS patients undergoing intravenous thrombolysis.

Background In recent years, the National Health Commission of China organized surveys on the awareness of nutrition and health knowledge among Chinese adults aged 18 to 64. Shanghai, as a core sample with benchmark significance in terms of economic level, geographical location, and population structure, still lacks sufficient real-time and effective research data. Objective To estimate the awareness rate of nutrition and health knowledge of Shanghai residents aged 18-64 in Shanghai from 2022 to 2023, identify shortcomings in nutritional health knowledge and potential influencing factors to clarify the core issues that need to be addressed urgently. Methods According to the questionnaire and scoring standard designed by the National Institute of Nutrition and Health of Chinese Center for Disease Control and Prevention, a stratified random sampling strategy was employed by urban/rural areas, regions, age, and gender and considering invalid questionnaires and refusal rates. Therefore, 1680 residents were selected in 4 survey sites in Shanghai. The survey subjects were grouped according to gender, marital status, age, education level, and occupation. t-test or analysis of variance was used to compare the average scores of nutritional health knowledge among different groups of respondents; chi-square test was used to analyze the awareness rate of nutritional health knowledge among respondents in different groups; and binary logistic regression was used to analyze the influencing factors. Results The nutrition and health knowledge score among Shanghai residents aged 18–64 was 73.45 ± 11.51, with an awareness rate of 53.14%. The food characteristics category had the lowest correct response rate at 66.39%. The mean scores of different genders were significantly different (t=−2.12, P=0.034), and the female group had higher scores than the male group. Significant differences in average scores and awareness rates were observed among age (F=10.95, P < 0.001; χ2=24.62, P < 0.001), educational level (F=46.01, P < 0.001; χ2=119.34, P < 0.001) , and industry groups (F=10.97, P < 0.001; χ2=56.30, P < 0.001). The logistic model showed that better knowledge awareness of nutrition and health in residents with high school, technical school, and above education (OR=1.959−3.622) than those with primary education or below; and worse in argo-food processing industry; manufacturing industry; agriculture, forestry, animal husbandry, and fishery; and other occupational residents than those in medical and health institutions (OR=0.458-0.504). Conclusion At present, residents’ understanding of food characteristics is particularly weak. Education level and occupation are the key factors influencing residents’ awareness rate of nutrition and health knowledge. Targeted and effective measures should be taken to improve the nutritional quality of residents.

AIM:To establish an optimal protocol for isolating and extracting mouse hepatocellular carcinoma H22 cell-detrived microparticles(H22-MPs),evaluate their stimulating effect on dendritic cells(DC),and investigate the cytotoxic capacity of CD8+T cells induced by H22-MPs on hepatocellular carcinoma cells.METHODS:The H22-MPs were extracted by high-speed differential centrifugation, and the morphology of H22-MPs was observed by transmis-sion electron microscopy. The particle size distribution of H22-MPs was detected by dynamic light scattering, and the ex-pression of vesicle surface markers was detected by Western blot. The protein components and pathways involved by MPs were analyzed by labeling free quantitative proteomics combining with bioinformatics GO and KEGG enrichment methods. The effect of MPs antigen on the expression of CD11c,a mature marker of DC,was detected by laser confocal microscopy.Next,CD8+T cells were sorted by magnetic beads, and the secretion of tumor necrosis factor-α(TNF-α),interferon-γ(IFN-γ)and interleukin-2(IL-2)in the supernatant of CD8+T cells stimulated by DC loaded with H22-MPs was detected by ELISA.Lactate dehydrogenase(LDH)test was conducted to evaluate the specific killing effect of CD8+T cells induced by DC loaded with H22-MPs on hepatocellular carcinoma cells.RESULTS:The results showed that the morphology, size and vesicle markers of H22-MPs met the requirements. Most of the H22-MPs protein antigens were derived from its parent cell H22 and coordinated various signal transduction pathways in the cells.Subsequently,it was detected that H22-MPs increased the expression of CD11c on the surface of DC,and H22-MPs were mainly localized in the lysosomes of DC.In addition,DC loaded with H22-MPs stimulated CD8+T to release high levels of TNF-α,IFN-γ and IL-2,and it also pro-moted CD8+T to overexpress CD69,thus inducing CD8+T to killhepatocellular carcinoma cells specifically.CONCLU-SION:H22-MPs promote the maturation of DC,and the mature DC may induce CD8+T cells to play an anti-hepatocellu-lar carcinoma immune response by promoting the activation of CD8+T cells.

The aim of this updated guideline is to provide comprehensive recommendations for the management of gout in patients with common comorbidities, such as chronic kidney disease(CKD), cardiovascular disease(CVD), diabetes, osteoarthritis(OA), and gastrointestinal disorders. This guideline was developed by a multidisciplinary expert panel consisting of specialists in endocrinology, rheumatology, nephrology, cardiology, gastroenterology, and methodology. The development process adhered to standard methodologies, including PICO(population, intervention, comparator, and outcomes) question deconstruction, systematic literature review, the Grading of Recommendations Assessment, Development and Evaluation(GRADE) for evidence and recommendation evaluation, Delphi voting, and expert consensus. The guideline presents 26 evidence-based recommendations addressing 7 clinical questions for patients with hyperuricemia and gout in the context of comorbidities. Key recommendations include the maintenance of strict serum urate targets, particularly for patients with CKD stage≥3, chronic gouty arthritis, and OA, in order to prevent disease progression. In patients with CVD or diabetes, intra-articular triamcinolone is preferred over systemic glucocorticoids. Prioritized anti-inflammatory treatments for patients with CKD, gastrointestinal diseases and OA are recommended. The guideline also introduces emerging therapies, such as interleukin-1 inhibitors and selective urate transport inhibitors, as potential treatment options for refractory cases. The update offers a comprehensive, patient-centered approach to managing gout, particularly in individuals with associated comorbidities. Multidisciplinary collaboration and emerging new treatments and evidence ensure the optimization of the recommendations.
