1.The in vitro and in vivo inhibitory effects of metformin on esophageal squamous cell carcinoma cells

OBJECTIVE To explore the in vitro and in vivo inhibitory effects and mechanism of metformin on the malignant biological behavior of esophageal squamous cell carcinoma (ESCC) cells by the hypoxia inducible factor-1α (HIF-1α)/interleukin-8 (IL-8) signaling pathway. METHODS Human ESCC TE1 cells were assigned into blank group, metformin low-, medium-, and high-dose groups (0.5, 1, 2 mmol/L), IDF-11774 (HIF-1α inhibitor) group (20 μmol/L), and high-dose metformin+HIF-1α activator dimethyloxalylglycine (DMOG) group. After 24 h treatment, cell proliferation [measured by the positive rate of 5-ethynyl- 2′-deoxyuridine (EdU) and optical density at 450 nm (OD450 value)], apoptosis, invasion and migration as well as mRNA expressions of proliferating cell nuclear antigen (PCNA), Bcl-2 interacting mediator of cell death (Bim), migration and invasion enhancer 1 (MIEN1), and matrix metalloproteinase-9 (MMP-9), and protein expressions of HIF-1α and IL-8 in the cells were detected. The xenograft tumor model of nude mice was established. Thirty nude mice were randomly divided into blank group, metformin low-, medium-, and high-dose groups (i.g. administration of metformin 62.5, 125, 250 mg/kg+i.p. administration of equal volume of normal saline), IDF-11774 group (i.g. administration of 50 mg/kg IDF-11774+i.p. administration of equal volume of normal saline) and high-dose metformin+DMOG group (i.g. administration of metformin 250 mg/kg+i.p. administration of DMOG 250 mg/kg), with 5 mice in each group. They were given relevant medicine, once a day, for 4 consecutive weeks; the mass and volume of the tumor and protein expressions of HIF-1α and IL-8 in the tumor tissue were determined. RESULTS The EdU positive rate, OD450 value, cell invasion number, scratch healing rate, mRNA expressions of PCNA, MIEN1 and MMP-9, protein expressions of HIF-1α and IL-8, as well as the mass and volume of transplanted tumors and protein expressions of HIF-1α and IL-8 in tumor tissues were decreased by metformin in concentration/dose-dependent manner (P<0.05). Additionally,metformin increased the apoptosis rate and mRNA expression of Bim in cells (P<0.05). The trend of changes in corresponding indicators in the IDF-11774 group was consistent with that in the metformin groups, whereas DMOG could significantly attenuate the aforementioned effects of high-concentration/high-dose metformin (P<0.05). CONCLUSIONS Metformin can inhibit the proliferation, invasion, migration of TE1 cells, and tumor growth of nude mice, and induce cell apoptosis, the mechanism of which may be related to the inhibition of HIF-1α/IL-8 signaling pathway.
2.The splicing factor HNRNPH1 regulates Circ-MYOCD back-splicing to modulate the course of cardiac hypertrophy.

OBJECTIVES:
To explore the mechanism of Circ-MYOCD back-splicing and its regulatory role in myocardial hypertrophy.
METHODS:
Sanger sequencing and RNase R assays were performed to verify the circularity and stability of Circ-MYOCD, whose subcellular distribution was determined by nuclear-cytoplasmic fractionation. Bioinformatics analysis and mass spectrometry from pull-down assays were conducted to predict the RNA-binding proteins (RBPs) interacting with Circ-MYOCD. In rat cardiomyocytes H9C2 cells, the effects of HNRNPH1 and HNRNPL knockdown and overexpression on Circ-MYOCD back-splicing were evaluated. In a H9C2 cell model of angiotensin II (Ang II)-induced myocardial hypertrophy, the expression of HNRNPH1 was detected, the effects of HNRNPH1 knockdown and overexpression on progression of myocardial hypertrophy were assessed, and the regulatory effect of HNRNPH1 on Circ-MYOCD back-splicing was analyzed.
RESULTS:
Sanger sequencing confirmed that the junction primers could amplify the correct Circ-MYOCD sequence. RNase R and nuclear-cytoplasmic fractionation assays showed that Circ-MYOCD was stable and predominantly localized in the cytoplasm. Bioinformatics analysis and mass spectrometry from the Circ-MYOCD pull-down assay identified HNRNPH1 and HNRNPL as the RBPs interacting with Circ-MYOCD. In H9C2 cells, HNRNPH1 knockdown significantly enhanced while its overexpression inhibited Circ-MYOCD back-splicing; HNRNPH1 overexpression obviously increased the expressions of myocardial hypertrophy markers ANP and BNP, while its knockdown produced the opposite effect. In Ang II-induced H9C2 cells, which exhibited a significant increase of HNRNPH1 expression and increased expressions of ANP and BNP, HNRNPH1 knockdown obviously increased Circ-MYOCD expression, decreased MYOCD expression and lowered both ANP and BNP expressions.
CONCLUSIONS
HNRNPH1 regulates Circ-MYOCD back-splicing to influence the progression of myocardial hypertrophy.

3.Cognitive Disorders Awareness and Associated Risk Factors in Xizang Autonomous Region

To investigate the awareness of cognitive impairment disorders among residents of the Xizang Autonomous Region and its influencing factors, thereby providing a basis for targeted prevention and treatment efforts. From April to December 2024, a questionnaire survey was conducted among permanent residents aged ≥18 years (residing in the Xizang Autonomous Region for 180 days or more). The survey was primarily conducted online, supplemented by QR code distribution during community medical outreach by healthcare workers. Demographic information and data on awareness of cognitive disorders were collected, and an ordered Logistic regression model was used to analyze influencing factors in the overall population and stratified by occupation. A total of 327 questionnaires were collected, with 14 excluded (13 for not meeting residency requirements and 1 for self-reported diagnosis of cognitive impairment), leaving 313 valid questionnaires. The average age of respondents was 42.0±11.9 years; 108 (34.5%) were male, and 205 (65.5%) were female. Most respondents were from Lhasa (78.6%, 246/313); 179 (57.2%) were healthcare workers, and 134 (42.8%) were non-healthcare workers. Regarding awareness of cognitive impairment disorders, 7.3% (23/313) were "unaware", 75.7% (237/313) were "partially aware", and 16.9% (53/313) were "well aware".Ordered Logistic regression analysis revealed that education level of high school or below ( Awareness of cognitive impairment disorders among residents of the Xizang Autonomous Region needs improvement. Educational level, occupation, and prior contact with cognitive impairment patients significantly influence disease awareness. Enhancing overall education levels and using vivid clinical case presentations in health education and public outreach are key strategies to improve public awareness of cognitive impairment disorders.
4.Clinical efficacy of endoscopically-assisted, minimally-invasive transposition reconstruction with a double-bundle flexor hallucis longus for chronic Achilles tendon ruptures

Objective:To investigate the clinical efficacy of endoscopically-assisted, minimally-invasive transposition reconstruction with a double-bundle flexor hallucis longus (FHL) for chronic Achilles tendon ruptures.Methods:A retrospective study was conducted to analyze the clinical data of 36 patients with chronic Achilles tendon rupture who had been treated by endoscopically-assisted, minimally-invasive transposition reconstruction with a double-bundle FHL at Foot and Ankle Surgery Center, Foshan Hospital of Traditional Chinese Medicine from May 2015 to April 2020. The cohort consisted of 30 males and 6 females with an age of (42.1±16.5) years. Their body mass index was (28.5±4.5) kg/m 2, their length of Achilles tendon defect (51.0±15.0) mm, and their duration from injury to surgery (13.1±10.4) weeks. The Achilles tendon rupture score (ATRS), American Orthopedic Foot and Ankle Society (AOFAS) ankle-hindfoot score, and Victorian Institute of Sports Assessment-Achilles (VISA-A) score were recorded and compared between pre-surgery and the last follow-up. All postoperative complications were record. Results:All patients were followed up for (41.3±3.5) months. No incision complications were noted. At the last follow-up, ATRS was (90.3±5.6) points, AOFAS ankle-hindfoot score (91.0±2.0) points, and VISA-A score (91.7±2.9) points, all significantly higher than the preoperative values [(24.3±8.2) points, (49.7±9.3) points, and (25.7±7.8) points] ( P < 0.05). Conclusion:The clinical efficacy of the endoscopically-assisted, minimally-invasive transposition reconstruction with a double-bundle FHL for chronic Achilles tendon ruptures is definite, leading to a low rate of complications.
5.Application of deep learning models based on super-resolution endorectal ultrasound in predicting perineural invasion in rectal cancer

Objective:To develop a deep learning model based on super-resolution endorectal ultrasound(ERUS)images for the preoperative prediction of perineural invasion(PNI)in patients with rectal cancer,thereby providing a reference for risk stratification and individualized treatment planning.Methods:A retrospective analysis was conducted on 382 patients with rectal cancer who underwent total mesorectal excision at Fujian Medical University Union Hospital between June 2019 and February 2024. Patients were randomly divided into a training set( n=305)and a test set( n=77)at a ratio of 8∶2,and further grouped into PNI-negative group and PNI-positive group subgroups based on pathological results. Super-resolution ultrasound images were generated from original ERUS images using a generative adversarial network(GAN). Deep convolutional neural networks were developed based on features from intratumoral and peritumoral regions to identify the optimal region of interest(ROI). The dSR5_ResNet18 and dSR5_ResNet50 models were constructed using the super-resolution images with a 5-pixel peritumoral extension. Representative clinical features were selected for subgroup analysis based on sample size and intergroup statistical differences between PNI-positive and PNI-negative patients. Forest plots were used to evaluate model applicability and robustness across subgroups. Results:The dSR5_ResNet18 model,built using super-resolution images of the tumor combined with a 5-pixel peritumoral region,achieved the best predictive performance,with an AUC of 0.867(95% CI=0.782 - 0.952)in the test set. Decision curve analysis demonstrated that the dSR5_ResNet18 model provided the greatest net clinical benefit. Forest plot analysis indicated strong generalizability of the models across subgroups such as pathological N stage,maximum lesion length,and lymph node enlargement,though relatively weaker performance was observed in the carcinoembryonic antigen(CEA)subgroup. Among all models,dSR5_ResNet18 exhibited the most consistent performance across subgroups,with the narrowest confidence intervals and highest robustness. Conclusions:The deep learning model incorporating ERUS-based super-resolution reconstruction demonstrated excellent performance in the preoperative prediction of PNI in rectal cancer. It offers significant advantages in image quality and generalizability,and may serve as a valuable tool to assist clinicians in formulating personalized treatment strategies.

Objective To investigate the distribution and antimicrobial resistance profiles of clinically isolated Haemophilus influenzae and Moraxella catarrhalis in hospitals across China from 2015 to 2021,and provide evidence for rational use of antimicrobial agents.Methods Data of H.influenzae and M.catarrhalis strains isolated from 2015 to 2021 in CHINET program were collected for analysis,and antimicrobial susceptibility testing was performed by disc diffusion method or automated systems according to the uniform protocol of CHINET.The results were interpreted according to the CLSI breakpoints in 2022.Beta-lactamases was detected by using nitrocefin disk.Results From 2015 to 2021,a total of 43 642 strains of Haemophilus species were isolated,accounting for 2.91%of the total clinical isolates and 4.07%of Gram-negative bacteria in CHINET program.Among the 40 437 strains of H.influenzae,66.89%were isolated from children and 33.11%were isolated from adults.More than 90%of the H.influenzae strains were isolated from respiratory tract specimens.The prevalence of β-lactamase was 53.79%in H.influenzae strains.The H.influenzae strains isolated from children showed higher resistance rate than the strains isolated from adults.Overall,779 strains of H.influenzae did not produce β-lactamase but were resistant to ampicillin(BLNAR).Beta-lactamase-producing strains showed significantly higher resistance rates to these antimicrobial agents than the β-lactamase-nonproducing strains.Of the 16 191 M.catarrhalis strains,80.06%were isolated from children and 19.94%isolated from adults.M.catarrhalis strains were mostly susceptible to both amoxicillin-clavulanic acid and cefuroxime,evidenced by resistance rate lower than 2.0%.Conclusions The emergence of antibiotic-resistant H.influenzae due to β-lactamase production poses a challenge for clinical anti-infective treatment.Therefore,it is very important to implement antibiotic resistance surveillance for H.influenzae and guide rational antibiotic use.All local clinical microbiology laboratories should actively improve antibiotic susceptibility testing and strengthen antibiotic resistance surveillance for H.influenzae.

Objective To understand the changing composition and antibiotic resistance of bacterial species in the clinical isolates from outpatient and emergency department(hereinafter referred to as outpatients)and inpatient children over time in various hospitals,and to provide laboratory evidence for rational antibiotic use.Methods The data on clinically isolated pathogenic bacteria and antimicrobial susceptibility of isolates from outpatients and inpatient children in the CHINET program from 2015 to 2021 were collected and analyzed.Results A total of 278 471 isolates were isolated from pediatric patients in the CHINET program from 2015 to 2021.About 17.1%of the strains were isolated from outpatients,primarily group A β-hemolytic Streptococcus,Escherichia coli,and Staphylococcus aureus.Most of the strains(82.9%)were isolated from inpatients,mainly SS.aureus,E.coli,and H.influenzae.The prevalence of methicillin-resistant S.aureus(MRSA)in outpatients(24.5%)was lower than that in inpatient children(31.5%).The MRSA isolates from outpatients showed lower resistance rates to the antibiotics tested than the strains isolated from inpatient children.The prevalence of vancomycin-resistant Enterococcus faecalis or E.faecium and penicillin-resistant S.pneumoniae was low in either outpatients or inpatient children.S.pneumoniae,β-hemolytic Streptococcus and S.viridans showed high resistance rates to erythromycin.The prevalence of erythromycin-resistant group A β-hemolytic Streptococcus was higher in outpatients than that in inpatient children.The prevalence of β-lactamase-producing H.influenzae showed an overall upward trend in children,but lower in outpatients(45.1%)than in inpatient children(59.4%).The prevalence of carbapenem-resistant Klebsiella pneumoniae(CRKpn),carbapenem-resistant Pseudomonas aeruginosa(CRPae)and carbapenem-resistant Acinetobacter baumannii(CRAba)was 14%,11.7%,47.8%in outpatients,but 24.2%,20.6%,and 52.8%in inpatient children,respectively.The prevalence of multidrug-resistant E.coli,K.pneumoniae,Proteus mirabilis,P.aeruginosa and A.baumannii strains was lower in outpatients than in inpatient children.The prevalence of fluoroquinolone-resistant E.coli,ESBLs-producing K.pneumoniae,ESBLs-producing P.mirabilis,carbapenem-resistant E.coli(CREco),CRKpn,and CRPae was lower in children in outpatients than in inpatient children,but the prevalence of CRAba in 2021 was higher than in inpatient children.Conclusions The distribution of clinical isolates from children is different between outpatients and inpatients.The prevalence of MRSA,ESBL,and CRO was higher in inpatient children than in outpatients.Antibiotics should be used rationally in clinical practice based on etiological diagnosis and antimicrobial susceptibility test results.Ongoing antimicrobial resistance surveillance and prevention and control of hospital infections are crucial to curbing bacterial resistance.

Objective To investigate the changing antibiotic resistance profiles of E.coli isolated from patients in the 52 hospitals participating in the CHINET program from 2015 to 2021.Methods Antimicrobial susceptibility was tested for clinical isolates of E.coli according to the unified protocol of CHINET program.WHONET 5.6 and SPSS 20.0 software were used for data analysis.Results Atotal of 289 760 nonduplicate clinical strains ofE.coli were isolated from 2015 to 2021,mainly from urine samples(44.7±3.2)%.The proportion of E.coli strains isolated from urine samples was higher in females than in males(59.0%vs 29.5%).The proportion of E.coli strains isolated from respiratory tract and cerebrospinal fluid samples was significantly higher in children than in adults(16.7%vs 7.8%,0.8%vs 0.1%,both P<0.05).The isolates from internal medicine department accounted for the largest proportion(28.9±2.8)%with an increasing trend over years.Overall,the prevalence of ESBLs-producing E.coli and carbapenem resistant E.coli(CREco)was 55.9%and 1.8%,respectively during the 7-year period.The prevalence of ESBLs-producing E.coli was the highest in tertiary hospitals each year from 2015 to 2021 compared to secondary hospitals.The prevalence of CREco was higher in children's hospitals compared to secondary and tertiary hospitals each year from 2015 to 2021.The prevalence of ESBLs-producing E.coli in tertiary hospitals and children's hospitals and the prevalence of CREco in children's hospitals showed a decreasing trend over the 7-year period.The prevalence of CREco in secondary and tertiary hospitals increased slowly.Antibiotic resistance rates changed slowly from 2015 to 2021.Carbapenem drugs(imipenem,meropenem)were the most active drugs amongβ-lactams against E.coli(resistance rate≤2.1%).The resistance rates of E.coli to β-lactam/β-lactam inhibitor combinations(piperacillin-tazobactam,cefoperazone-sulbactam),aminoglycosides(amikacin),nitrofurantoin and fosfomycin(for urinary isolates only)were all less than 10%.The resistance rate of E.coli strains to antibiotics varied with the level of hospitals and the departments where the strains were isolated,especially for cefazolin and ciprofloxacin,to which the resistance rate of E.coli strains from children in non-ICU departments was significantly lower than that of the strains isolated from other departments(P<0.05).The E.coli isolates from ICU showed higher resistance rate to most antimicrobial agents tested(excluding tigecycline)than the strains isolated from other departments.The E.coli strains isolated from tertiary hospitals showed higher resistance rates to the antimicrobial agents tested(excluding tigecycline,polymyxin B,cefepime and carbapenems)than the strains from secondary hospitals and children's hospitals.Conclusions E.coli is an important pathogen causing clinical infection.More than half of the clinical isolates produced ESBL.The prevalence of CREco is increasing in secondary and tertiary hospitals over the 7-year period even though the overall prevalence is still low.This is an issue of concern.
9.Effects of Scutellarin on Colonic Inflammatory Injury in Mice with Ulcerative Colitis by Regulating PI3K/Akt/NF-κB Signaling Pathway

Objective To observe the effect of scutellarin on colonic inflammatory injury in mice with ulcerative colitis by regulating phosphatidylinositol-3 kinase(PI3K)/protein kinase B(Akt)/nuclear factor kappa B(NF-κB)signaling pathway.Methods Ulcerative colitis(UC)mouse model was induced by dextran sulfate sodium(DSS).The successfully modeled mice were randomly divided into model group,scutellarin group,YS-49(PI3 K activator)group and scutellarin+YS-49 group,with 12 mice in each group.Another 12 BALB/c mice were set as normal group.After the corresponding intervention was given to each group,the disease activity index(DAI)of colitis symptoms in each group was scored.Hematoxylin-eosin(HE)staining was used to observe and score the symptoms of colonic inflammatory injury in each group.Evans blue(EB)assay was used to determine the intestinal permeability of mice in each group.Enzyme-linked immunosorbent assay(ELISA)was used to detect the levels of serum colonic injury markers intestinal fatty acid binding protein(I-FABP),diamine oxidase(DAO)and pro-inflammatory factors interleukin(IL)-1β,IL-17 and tumor necrosis factor α(TNF-α).Western Blot was used to detect the expression of inflammatory factors IL-1β,IL-17,TNF-α and PI3K/Akt/NF-κB pathway proteins in colon tissue.Results The DAI score,colon inflammation score,EB content of intestinal wall,serum I-FABP and DAO levels,serum IL-1β,IL-17 and TNF-α levels,protein expression levels of colon tissue IL-1β,IL-17 and TNF-α,colon tissue p-PI3K/PI3K,p-Akt/Akt,p-NF-κB p65/NF-κB p65 ratios in the model group were significantly higher than those in the normal group,and the differences were statistically significant(P<0.05).The DAI score,colon inflammation score,EB content in intestinal wall,serum I-FABP and DAO levels,serum IL-1β,IL-17 and TNF-α levels,protein expression levels of colon tissue IL-1β,IL-17 and TNF-α,colon tissue p-PI3 K/PI3 K,p-Akt/Akt,p-NF-κB p65/NF-κB p65 ratios in the scutellarin group were lower than those in the model group,and the differences were statistically significant(P<0.05).The DAI score,intestinal wall EB content,serum I-FABP and DAO levels,colon inflammation score,serum IL-1β,IL-17 and TNF-α levels,protein expression levels of colon tissue IL-1β,IL-17 and TNF-α,colon tissue p-PI3 K/PI3 K,p-Akt/Akt,p-NF-κB p65/NF-κB p65 in YS-49 group were higher than those in model group,and the differences were statistically significant(P<0.05).The DAI score,intestinal wall EB content,serum I-FABP and DAO levels,colonic inflammation score,serum IL-1β,IL-17 and TNF-α levels,protein expression levels of colon tissue IL-1β,IL-17 and TNF-α,colon tissue p-PI3K/PI3K,p-Akt/Akt,p-NF-κB p65/NF-κB p65 ratios in the scutellarin+YS-49 group were higher than those in the scutellarin group,and the differences were statistically significant(P<0.05).Conclusion Scutellarin can reduce inflammation in UC mice by inhibiting the activation of PI3K/Akt/NF-κB signaling pathway,alleviate colonic inflammatory injury and repair intestinal barrier function.
10.Palmitic acid increasing the entry of lipopolysaccharide into microglial cytosol and eliciting pyroptosis and apoptosis

Objective To investigate the types and mechanisms of microglial cell death induced by interaction between palmitic acid(PA)and lipopolysaccharide(LPS).Methods BV-2 microglial cells were divided into three groups for apoptosis research,BSA group,PA treatment group,and staurosporine(STA)group.They were further divided into four groups for necrosis research,BSA group,BSA+inhibitor group,PA group,and PA+inhibitor group.Western blotting was conducted to assess the expression levels of key proteins involved in apoptosis and necrosis pathways.The effect of PA on microglial cells was validated through feeding a high-fat diet to Institute of Cancer Research(ICR)mice.Results Apoptotic microglia were observed in both BSA group and PA group,PA significantly induced the activation of caspase-3,caspase-7,and poly ADP-ribose polymerase(PARP).However,compared to the BSA group,the level of activated Caspase-7 in the STA group did not change significantly.Inhibition of ferroptosis,necroptosis,or autophagy did not protect against PA-induced cell damage,while the Caspase-11 inhibitor,wedelolactone(WE),significantly improved PA induced cell damage.This study also found that PA could promote LPS entry into microglial cells and induce pyroptosis.This phenomenon and the protective effect of WE were further confirmed in a high-fat diet mouse model through immunofluorescent staining and Western blotting.Conclusion PA induces apoptosis and pyroptosis in microglial cells,while simultaneously promoting LPS entry into microglial cells and inducing pyroptosis.
