The prescription of Qidong Yixin oral liquid is derived from the experience of national medical master Ren Jixue in treating viral myocarditis (VMC). It has the functions of tonifying Qi, nourishing the heart, calming the mind, and relieving palpitations. It is used to treat VMC and angina pectoris of coronary heart disease caused by deficiency of both Qi and Yin. However, the understanding of its efficacy evidence, advantageous aspects, dosage and administration, and medication safety remains insufficient in clinical practice. Therefore, the development of the Expert Consensus on the Clinical Application of Qidong Yixin Oral Liquid (hereinafter referred to as consensus) was initiated. Consensus strictly followed the process and methods of the expert consensus on the clinical application of Chinese patent medicines of the China Association of Chinese Medicine, successively completing multiple tasks such as the consensus project initiation, determination of clinical problems, evidence search and evaluation, formation of recommendation opinions and consensus suggestions, solicitation of opinions, peer review, submission for review and release, and so on. Consensus formed a total of 10 recommendation opinions and 12 consensus suggestions, clarifying the clinical positioning, efficacy advantages, syndrome differentiation, dosage and administration, combination therapy, timing of medication, adverse reactions, contraindications, and precautions of Qidong Yixin oral liquid, indicating that it has good clinical advantages and safety in the treatment of VMC and angina pectoris of coronary heart disease, providing norms and references for physicians to safely and rationally apply Qidong Yixin oral liquid. Consensus was reviewed and approved for release by the Standardization Office of the China Association of Chinese Medicine on December 23, 2024. Standard number:GSCACM-376-2024.

OBJECTIVE To investigate the mechanism by which Qiaobai cold compress solution (QBCS) improves acne vulgaris (AV) based on transcriptomics and animal experiments. METHODS Rats were randomly divided into a blank control group ( n =6) and a modeling group ( n =30). AV models were established in the modeling group by topical application of oleic acid to the inner surface of both ears, combined with subcutaneous injection of Cutibacterium acnes suspension into the auricle. Successfully modeled rats were further divided into the model group, positive control group (Tretinoin cream, 0.045 g/kg), and QBCS low-, medium-, high-dose groups [3.55, 7.11, 14.22 g/kg (calculated by the amount of crude drug) ] , with 6 rats in each group. Rats in each d rug group were treated with the corresponding drugs once daily for 14 consecutive days. After the final administration, changes in the appearance of the ears and histopathological changes in the ear tissues were observed, and serum levels of inflammatory factors, including tumor necrosis factor-α (TNF-α), interleukin-6 (IL-6) and IL-1β, were measured. Auricular tissues from the blank control group, model group and QBCS medium-dose group were collected for transcriptome sequencing. Differential expressed genes (DEGs) were screened and subjected to Kyoto Encyclopedia of Genes and Genomes pathway enrichment analysis, followed by validation using real-time quantitative polymerase chain reaction and Western blot assay. RESULTS Compared with the model group, rats in all QBCS groups showed alleviated auricular acne symptoms, with reduced epidermal thickening, sebaceous gland hyperplasia, and inflammatory cell infiltration. Serum levels of TNF-α (except for the QBCS low-dose group), IL-6 (except for the QBCS low-dose group) and IL-1β were significantly decreased ( P <0.05). A total of 590 DEGs were identified (blank control group vs. model group), and 596 DEGs were identified (model group vs. QBCS medium-dose group). Above DEGs (blank control group vs. model group) were mainly enriched in Toll-like receptor (TLR) and nuclear factor-kappa B (NF-κB) signaling pathways, etc. Validation experiments showed that, compared with model group, low-, medium- and high-dose of QBCS reduced, to varying degrees, the mRNA expression of TNF-α, TLR2, interferon-γ and CXC chemokine ligand 8 in the auricular tissues of AV rats, increased the mRNA expression of peroxisome-proliferator-activated receptor gamma and tumor protein 53, and inhibited the phosphorylation of NF-κB p65 protein as well as the expressions of TLR2 and myeloid differentiation primary response protein 88(MyD88) ( P <0.05). CONCLUSIONS QBCS can alleviate auricular inflammation and skin lesions in AV rats. This effect may be related to inhibition of the TLR/MyD88/NF-κB signaling pathway, thereby suppressing the expression of downstream inflammatory factors such as TNF-α.

OBJECTIVE To investigate the causal relationships between gut microbiota, blood metabolites and antidepressant treatment response from a genetic perspective, and to assess the potential mediating role of blood metabolites. METHODS This study utilized a two-sample Mendelian randomization (MR) design. Exposure data were derived from four large-scale gut microbiome genome-wide association study (GWAS) datasets and two blood metabolite GWAS datasets. The inverse variance weighted method was used as the primary method to evaluate the causal relationships between gut microbiota, blood metabolites and antidepressant effects. The robustness, heterogeneity and horizontal pleiotropy of the results were evaluated through various sensitivity analyses. Additionally, the false discovery rate (FDR) was applied to correct type Ⅰ errors caused by multiple hypothesis testing. Finally, MR mediation analysis was conducted to test the potential mediating effect of blood metabolites. RESULTS The s_ Bilophila was negatively associated with the effectiveness of antidepressant treatment ( P =8.030×10 -5 , then P =0.033 after FDR correction), and the f_Bacteroidales was positively associated with the effectiveness of antidepressant treatment ( P =3.275×10 -4 , then P =0.034 after FDR correction). Over a hundred blood metabolites were also screened out as being associated with antidepressant response, but after FDR correction, no significant causal relationship was observed. The P value of the mediation effect proportion of blood metabolites in the “gut microbiota-blood metabolites-antidepressant efficacy” pathway was greater than 0.05. CONCLUSIONS The s_ Bilophila may represent a risk factor for antidepressant effects, whereas the f_Bacteroidales may serve as a protective factor for antidepressant effects. The correlation between blood metabolites and antidepressant efficacy is not strong, and no genetic evidence is found to support that the investigated blood metabolites play a key mediating role between the gut microbiota and antidepressant response.

Objective This study conducted molecular biological identification of the viruses carried by Culex pipiens pallens specimens collected in Shunyi District, Beijing in 2023, and observed the changes in the infection rate of the viruses carried by Cx. pipiens pallens at different collection times. Methods Cx. pipiens pallens were collected using carbon dioxide mosquito traps. The mosquito samples were ground in batches and analyzed by molecular biology technologies. The virus infection rate at different collection times was analyzed statistically. Results 17 strains of Culex pipiens pallens densovirus(CppDNV)were identified from Cx. pipiens pallens samples collected in Shunyi District, Beijing, in 2023. The nucleotide sequence analysis of the virus genome coding region showed that CppDNV was a single-stranded DNA virus with a total length of 3 335 nt, encoding 2 non-structural proteins(NS1, NS2)and 1 capsid protein(VP). The nucleotide(amino acid)sequence lengths of the three proteins were 2 376 nt(791 aa),1 092 nt(363 aa)and 1 071 nt(356 aa), respectively. Phylogenetic analysis showed that CppDNV was located in genus Brevihamaparvovirus. Statistical analysis showed no significant difference in infection rates across collection times(χ2=4.429, P=0.194). Conclusions CppDNV was identified in Cx. pipiens pallens in Beijing, and it was stably maintained in this natural population.

China Pharmacy 2025;36(20):2599-2603
Serotonin-selective reuptake inhibitors (SSRIs), as widely used antidepressants in clinical practice, exhibit significant individual differences in antidepressant efficacy. Gut microbiota plays an important role in the development and progression of depression, and the use of SSRIs exerts a significant impact on the gut microbiota of patients with depression. Based on this, this article reviews the research progress on the interactions between the antidepressant effects of SSRIs and gut microbiota. It has been found that SSRIs can influence the diversity, abundance, and function of the gut microbiota directly or indirectly. Conversely, the composition of the gut microbiota and differences in its functional metabolic pathways, and other factors, can in turn affect the antidepressant effects of SSRIs. Therefore, in clinical practice, gut microbiota diversity can be utilized as a predictive indicator for the antidepressant effects of SSRIs. Probiotics can be employed as an adjunct therapy alongside SSRIs, and dietary adjustments, as well as fecal microbiota transplantation, can be used to enhance the therapeutic efficacy of SSRIs. In the future, large-scale, multicenter clinical studies should be conducted, enrolling a broadly representative cohort of patients with depression, to uncover the true association between the antidepressant effects of SSRIs and gut microbiota, thereby opening up more effective avenues for the comprehensive treatment of depression.
