1.Advances in posterior scleral reinforcement for pathological myopia

Pathologic myopia(PM)is characterized by progressive axial elongation, posterior staphyloma, and fundus degenerative changes, representing a leading cause of blindness in Asian countries.Posterior scleral reinforcement(PSR)is currently the only surgical intervention to slow axial elongation. Its mechanisms involve mechanical reinforcement, improved blood circulation, and biological stimulation of scleral remodeling. This article reviews recent advances in PSR: surgical techniques have evolved from complex, classic procedures to standardized single-strip methods, incorporating 3D MRI to quantify posterior staphyloma morphology for personalized graft design and facilitating minimally invasive, precision surgery. Among implant materials, biomaterials such as allogeneic dura mater are being continuously optimized, while biomimetic piezoelectric smart materials actively promote scleral collagen synthesis through electrical signals, achieving a paradigm shift from “passive support”to “active reinforcement”. Clinical outcomes indicate advantages in regulating axial length, enhancing visual function and blood circulation, and facilitating early intervention in children. Combined procedures with ICL implantation, cataract surgery or vitrectomy provide comprehensive treatment regimens for individual patients with varied conditions. This article also analyzes long-term issues and core controversies including ectopic posterior staphyloma and reoperation, and prospects future research directions such as artificial intelligence assistance, translation of smart materials and full-cycle prevention and treatment systems, so as to provide references for clinical practice and scientific research.

3.Clinicopathological features and prognostic analysis of 67 cases of marginal zone lymphoma derived from T-bet positive memory B cell

Purpose To explore the clinicopathological features and potential clinical value of marginal zone lym-phoma(MZL)derived from T-bet positive memory B cell.Methods Clinical data of 67 cases of MZL were collected.Hematoxylin-eosin,immunohistochemistry,and multiple immunofluorescence stains,B cell receptor high throughput sequencing technology were used to study the histology,immunophenotype,and immunoglobulin heavy chain variable gene(IGHV)repertoire.Results T-bet was expressed in some MZL patients(34/67,50.75%),which was correla-ted with clinicopathological characteristics such as gender,clinical stage,and Ki67 proliferation index(P<0.05),and also the progression-free survival was poor(P=0.012 2).T-bet positivity was a risk factor affecting the progres-sion of MZL.Microscopically,T-bet positive MZL frequently presented T-bet positive tumor B cells surrounded follicu-lar germinal center,"MALT ball"-type lymphoepithelial lesions,and IgG positive neoplastic plasma cells(P<0.05).T-bet had no biased influence on the VH gene usage(P>0.05).The common VH families were IGHV4 and IGHV3,and the segments were IGHV4-34 and IGHV3-30.The positivity of T-bet was associated with somatic hypermutation(SHM)state(P=0.014 9).The SHM was mainly in the range of 2%-4.9%in T-bet positive MZL,while in T-bet negative MZL the SHM was mostly greater than 5%.The VH gene usage was not correlated with the clinicopathological features of patients(P>0.05).IGHV4 was correlated with progression-free survival in T-bet positive MZL(P=0.038 2).Conclusion The expression of T-bet in MZL is closely related to the clinicopathological features such as histology,plasma cell immunophenotype and IGHV gene repertoire,and the prognosis of patients is poor,which may be a potential molecular marker affecting the progression of MZL.
4.Presence and clinical significance of tumor cells with macrophage phenotype in dif-fuse large B-cell lymphoma

Purpose To investigate the presence,proportion,clinical significance and origin of tumor cells with a macrophage phenotype in tumor tissues of patients with diffuse large B-cell lymphoma(DLBCL),and to explore wheth-er CD68 positive tumor cells can be induced in DLBCL cell lines in vitro.Methods The presence of CD68+CD163+CD20+PAX5-cells in the samples of DLBCL patients was first qualitatively detected by multiplex immunofluorescence staining,and then the proportion of CD79a+B lymphocytes,CD68+macrophages,and CD68+CD79a+double-positive cells were quantified.Patients were grouped according to the proportion of double-positive cells,and the differences in prognosis and clinicopathological features of DLBCL patients between subgroups were investigated.For cases with posi-tive BCL6 gene locus breaks,co-localization of CD68 with BCL6 gene breakapart was performed using combined immu-nofluorescence and immunological in situ hybridization to ascertain the tumor nature of B cell with a macrophage pheno-type.DLBCL cell lines(OCI-LY3,SU-DHL2)were treated with phorbol myristate acetate(PMA),and changes in the proteins levels of CD68 and PAX5 proteins were detected by flow cytometry.Quantitative real-time PCR(qRT-PCR)was used to detect mRNA levels of PAX5,an important transcription factor for B cell differentiation and develop-ment,and macrophage-related genes(CD68,ARG1,CD163,CD206,Dectin-1,PU.1,C/EBPα,C/EBPβ).In ad-dition,PMA-treated DLBCL cell lines(OCI-LY3,SU-DHL2)were co-incubated with pH-sensitive fluorescent dye pHrodo to detect the phagocytosis ability of PMA-treated DLBCL cells.Results The percentage of CD68+B lympho-cytes in 50 patients with DLBCL varied from 0 to 9.3%,and the overall survival(OS)ranged from 0.008 2 to 4.2 years.Patients with the low CD68+B lymphocytes group exhibited a significantly lower OS compared to those in the high CD68+B lymphocytes group(P=0.039).There was a significant difference in the molecular typing of DLBCL patients(P=0.009 5)between different subgroups for the proportion of CD68+B lymphocytes.CD68+B lymphocytes were derived from tumor cells in DLBCL patients.The proportion of CD68+cells and CD68+PAX5-cells significantly increased in DLBCL after treatment with PMA(P<0.05).The other macrophage markers CD68,ARG1,CD 163,CD206,Dectin-1,PU.1,C/EBPα,C/EBPβ,and the important B-cell transcription factor PAX5 were significantly different from the control group in terms of relative mRNA expression(P<0.05).Cellular phagocytosis was enhanced after PMA treatment of DLBCL cells.Conclusion Diffuse large B-cell lymphoma tumor tissue contains a certain per-centage of CD68+neoplastic B lymphocytes.The proportion of CD68+B lymphocytes is correlated with patient progno-sis and molecular typing.DLBCL cell lines can be induced to differentiate into CD68+tumor cells in vitro.

7.Obstacle analysis of evidence-based nursing application of standardized management of stress hyperglycemia during perioperative period in gastrointestinal tumor patients

Objective:To carry out evidence-based nursing for standardized management of stress hyperglycemia in perioperative period of gastrointestinal tumor patients, and to formulate indicators, analyze obstacles and promoting factors, formulate action strategies.Methods:Guided by the Johns Hopkins evidence-based nursing model, evidence were searched, evaluated and summarized. Clinical indicators and review methods were formulated to carry out quality review. From November 2021 to April 2022, the medical staff and patients in the gastrointestinal surgery department of the First Affiliated Hospital of Anhui Medical University who met the inclusion criteria were conducted, and the incidence of compliance rate was calculated. Based on the results of the baseline review, the obstacles and contributing factors were analyzed.Results:A total of 26 pieces of best evidence were included and 14 indicators were formulated for 48 medical staff and 45 patients to clinical review, among which the compliance rate of 7 indicators was less than 60%. The main obstacle factors were lack of procedures and instruments for management of perioperative stress hyperglycemia in gastrointestinal tumor patients, lack of knowledge of medical staff, etc. The main promoting factors were organizational support, good atmosphere of medical team cooperation, strong willingness to change, etc.Conclusions:There is a big gap between the clinical practice and the best evidence of perioperative stress hyperglycemia management in patients with gastrointestinal tumor. Action strategies should be put forward for obstacles and promoting factors to promote evidence transformation.

In January 2024,Cancer Statistics,2024 was published in CA:A Cancer Journal for Clinicians.This report estimated cancer cases and deaths in 2024 and described the secular trends of major cancers over recent decades.We summarized the report and integrated the latest data on the cancer burden in China to compare the prevalence cancer between two countries.We also comprehensively interpreted the underlying reasons for these trends to provide insights for cancer prevention and control strategies in China.
9.Effects of LINC02381 silencing on the miR-4500 and CCNE2 expression and biological behavior of lung adenocarcinoma cells

Objective:To investigate whether LINC02381 impacts the biological behavior of lung adenocarcinoma cells by regulating the miR-4500/CCNE2 axis. Methods:Cancer and paracancerous tissues were collected from 41 patients with lung adenocarcinoma treated at Huashan Hospital Affiliated to Fudan University,from June 2022 to May 2024. RT-PCR was used to detect the expression levels of LINC02381,miR-4500,and CCNE2 in lung adenocarcinoma tissues and cells. Calu-3 lung adenocarcinoma cells were divided into the control,si-NC,si-LINC02381,si-LINC02381+inhibitor NC,si-LINC02381+miR-4500 inhibitor,si-LINC02381+oe-NC,and si-LINC02381+oe-CCNE2 groups. qRT-PCR was used to determine the expression levels of LINC02381,miR-4500,and CCNE2 in each group of cells. The CCK-8 assay was used to detect cell proliferation. A monolayer scratch assay was performed to detect cell migration. The Transwell assay was used to detect cell invasion. Flow cytometry was performed to determine the apoptosis rate. Western blot was performed to detect E-cadherin,N-cadherin,vimentin,cleaved caspase-3,PCNA,MMP-2,and CCNE2 protein levels in the cells. The dual-luciferase reporter assay was used to verify the relation-ship among miR-4500,LINC02381,and CCNE2. Results:LINC02381 and CCNE2 expression was increased,whereas miR-4500 expression was decreased in lung adenocarcinoma tissues and cells. Compared with those in the control and si-NC groups,LINC02381 and CCNE2 expres-sion,the OD450 (24 and 48 h) values,scratch healing rate,number of invading cells,N-cadherin,vimentin,PCNA,MMP-2,and CCNE2 protein levels in Calu-3 cells in the si-LINC02381 group were reduced,whereas miR-4500 expression levels,apoptosis rate,E-cadherin,and cleaved caspase-3 protein levels were increased (P<0.05). Reducing miR-4500 expression or increasing CCNE2 expression weakened the inhibitory effects of LINC02381 silencing on the biological behavior of Calu-3 cells (P<0.05). Conclusions:LINC02381 silencing can result in increased miR-4500 expression,inhibition of CCNE2 expression,suppression of lung adenocarcinoma cell proliferation and migration,and promotion of apoptosis.
