1.Notoginsenoside R1 modulates mitophagy in human cardiomyocytes viathe Pink1/Parkin pathway after hypoxia/reoxygenation

ObjectiveTo investigate the mechanism by which Notoginsenoside R1 (NGR1) ameliorates hypoxia/reoxygenation (H/R)-induced injury in AC16 human cardiomyocyte cell lines through the regulation of mitophagy. MethodsCommon genes linked to hypoxia/reoxygenation injury and mitophagy were identified by intersecting data from GeneCards and MitoCarta databases. AC16 cell viability was assessed via CCK-8 assay under varying NGR1 concentrations (0, 6.25, 12.5, 25, 50, 100, 200, 300, 400, 500 μmol/L). AC16 cells were divided into the following groups: control group (Control), model group (H/R), and treatment groups (H/R + NGR1 at 100, 200 and 300 μmol/L). Mitochondrial membrane potential (ΔΨm) was measured using 5,5',6,6'-tetrachloro-1,1',3,3'-tetraethylbenzimidazolylcarbocyanine iodide (JC-1) staining. Transcriptional levels of mitophagy-related genes (Parkin, Pink1, P62) were quantified by reverse transcription-quantitative PCR (RT-qPCR). Protein expression of mitophagy-related markers (Parkin, Pink1, P62, and LC3BⅡ) was evaluated via Western blot analysis. Mitochondrial ultrastructure was visualized by transmission electron microscopy (TEM). ResultsCompared to the control group, cell viability in the H/R group significantly decreased (P<0.01). Treatment with NGR1 at concentrations above 100 μmol/L significantly enhanced the cell viability of AC16 cells compared to the H/R group (P<0.01). H/R induced a significant decrease in mitochondrial membrane potential (P<0.01), which was restored by NGR1 treatment (P<0.01). The mRNA levels of Parkin, Pink1, and P62 in the H/R group were upregulated compared to the control group (P<0.05), while NGR1 intervention downregulated their expression (P<0.05). Protein expression levels of Parkin, Pink1, and LC3BⅡ in the H/R group significantly increased, while P62 expression decreased compared to the control group (P<0.01). In contrast, different doses of NGR1 treatment significantly reduced the expression of Parkin, Pink1, and LC3BⅡ while increasing P62 expression (P<0.05). TEM revealed that the mitochondrial structure in the H/R group was severely disrupted, with fragmented and disorganized cristae, which was alleviated by NGR1. ConclusionNGR1 ameliorates H/R-induced AC16 cell injury, and its mechanism may be associated with modulating the Pink1/Parkin pathway to suppress excessive mitophagy.
2.Influence of Wenfei Guyuan Umbilical Moxibustion on Quality of Life and Immune Function in Patients with Chronic Obstructive Pulmonary Disease in Stable Phase

ObjectiveThis paper aims to assess the effects of Wenfei Guyuan umbilical moxibustion on the quality of life and immune function in patients with chronic obstructive pulmonary disease (COPD) in stable phase. MethodsA multi-center randomized controlled trial design was employed,and the 220 cases of patients with COPD in stable phase from three grade A class-Ⅲ hospitals were included as research objects. The patients were randomly divided into the test group and control group,with each group consisting of 110 cases. Both groups received standardized treatment of western medicine,and the test group received Wenfei Guyuan umbilical moxibustion twice weekly for 13 weeks,followed by a 26-week follow-up period. Quality of life was evaluated by using the COPD assessment test (CAT),the modified COPD patient-reported outcomes (mCOPD-PRO) measure,and the modified effectiveness satisfaction questionnaire for COPD (mESQ-COPD) before treatment,four weeks, eight weeks, and 13 weeks of the treatment period,as well as 13 weeks and 26 weeks of the follow-up period. The number of acute exacerbation cases of patients in both groups was recorded during study period to evaluate the effect of Wenfei Guyuan umbilical moxibustion on acute exacerbations. 30 cases were randomly selected in both observation group and control group. Peripheral blood samples were collected before treatment and at 13 weeks of treatment. Enzyme-linked immunosorbent assay (ELISA) was used to detect the levels of immunoglobulin A (IgA),immunoglobulin G (IgG),immunoglobulin M (IgM),interleukin 10 (IL-10),interleukin 17A (IL-17A),transforming growth factor β1 (TGF-β1),and tumor necrosis factor α (TNF-α). Flow cytometry was used to detect cluster of differentiation 4 positive (CD4+),cluster of differentiation 8 positive (CD8+),T helper 17 (Th17),and Treg levels, thereby preliminarily exploring the effect of Wenfei Guyuan umbilical moxibustion on immune function. ResultsA total of 220 patients were included,with five cases dropping out. 215 cases were finally included in the per-protocol set,including 107 in the treatment group and 108 in the control group. Baseline characteristics of the first two groups before treatment were compared between the two groups. In terms of life quality evaluation, the main effect of group differences on the CAT scores was significant (F=15.108,P<0.01). The main effects of group differences on the physical domain (F=38.807,P<0.01),psychological domain (F=38.996,P<0.01),environmental domain (F=17.436,P<0.01),and total score of mCOPD-PRO (F=41.972,P<0.01) were significant. The main effects of group difference on clinical symptoms domain of mESQ-COPD (F=81.516,P<0.01),work-life ability domain (F=36.549,P<0.001),environmental adaptation ability domain (F=22.677,P<0.01),therapeutic effect domain (F=74.055,P<0.01),and total score of mESQ-COPD (F=73.251,P<0.01) were significant. Regarding acute exacerbations,during the entire study period,as well as the treatment period and follow-up period,the observation group showed fewer patients experiencing acute exacerbations compared to the control group,but the difference between the two groups was not statistically significant. In terms of immune indicators,after 13 weeks of treatment,the levels of IgA,IgG,and IgM in the observation group were significantly better than those in the control group (P<0.05). The level of IL-10 was significantly higher than that in the control group (P<0.05),and the levels of IL-17A,TGF-β1,and TNF-α were significantly lower than those in the control group (P<0.05). Compared with those in the control group,the level of CD4+/CD8+ in the observation group was significantly increased (P<0.05),while the levels of CD4+ and Treg were slightly increased,but the difference was not statistically significant. The levels of CD8+,Th17,and Th17/Treg were significantly decreased (P<0.05). ConclusionWenfei Guyuan umbilical moxibustion can improve the quality of life, and immune function in patients with COPD in stable phase. It is worth promoting in clinical practice.
3.Current quality status and management countermeasures of occupational health technical services in Zhejiang Province

Background The quality of occupational health technical services is directly linked to the protection of workers' health rights and the efficacy of occupational disease prevention and control. However, the industry still faces critical challenges: sporadic instances of institutional non-compliance and persistent irregularities in professional practice continue to undermine overall service performance. Objective To assess the current quality status of occupational health technical services in Zhejiang Province and propose countermeasures for quality improvement, providing a scientific basis for policy optimization and service delivery quality enhancement. Methods A total of 69 occupational health technical service institutions in Zhejiang Province that obtained formal accreditation as of April 30, 2024, were sampled, including 3 public institutions and 66 private institutions (comprising 3 formerly Class-A, 28 formerly Class-B, 11 formerly Class-C, and 24 newly certified institutions). Following the Technical Protocol for Quality Monitoring of Occupational Health Technical Service in Zhejiang Province and the Technical Protocol for Proficiency Testing of Occupational Health Detection in Zhejiang Province, a quality assessment task force comprising national and provincial experts was established. Evaluation was conducted across four dimensions: qualification maintenance and compliance, standardization of technical services, authenticity of technical services, and proficiency testing, utilizing a combination of document review, on-site inspections, and technical skill assessments. Results The occupational health technical service institutions in Zhejiang Province were predominantly private entities (82.5%), with significant disparities in overall service quality. The pass rates for qualification maintenance and compliance, technical service standardization, technical service authenticity, and the excellence rate for laboratory proficiency testing were 81.5%, 80.7%, 97.3%, and 90.4%, respectively. Regarding qualification maintenance, the pass rates for "environmental conditions" (49.8%, 56.7%) and "instrumentation and equipment" (58.2%、65.6%) were significantly lower for formerly Class-C and newly certified institutions compared to other categories. In terms of technical standardization, "standardized on-site inspections" recorded the lowest pass rate (67.4%), with newly certified institutions at only 48.0%. Regarding technical service authenticity, formerly Class-C institutions exhibited issues such as missing raw chromatograms for blank samples (85.7% pass rate). In laboratory proficiency testing, public and formerly Class-A institutions achieved 100% excellence rates, but the performance of formerly Class-C and newly certified institutions was comparatively weak; specifically, the failure rate for organic analysis in formerly Class-C institutions reached 20%; the failure rate for dust testing items in newly certified institutions was 10.3%. Conclusion The overall quality of occupational health technical services in Zhejiang Province still requires significant improvement, particularly in basic institutional conditions, the standardization of on-site inspections, and laboratory proficiency in organic and dust analysis. Formerly Class-C and newly certified institutions should be the primary focus of quality management efforts. Differentiated regulatory strategies are recommended, alongside strengthening interim and ex-post supervision to gradually enhance the quality of occupational health technical services across all institutions.

Background:
and Purpose Vertebrobasilar fusiform aneurysms (VBFAs) carry substantial morbidity and mortality, but optimal management for unruptured VBFAs remains unclear. We compared the safety and efficacy of conservative management (CM), stent-assisted coiling (SAC), and flow diverters (FDs) in patients with unruptured VBFAs, focusing on long-term prognosis.
Methods:
This study included data from a nationwide Chinese cohort of patients with vertebrobasilar dissecting aneurysms. Inverse probability of treatment weighting (IPTW) balanced confounders across groups. The primary outcome was poor prognosis (modified Rankin Scale score >2). Secondary outcomes included aneurysm rupture, ischemic stroke, compression symptoms, and VBFA-related deaths. Logistic regression estimated odds ratios (ORs) and 95% confidence intervals (CIs). Subgroup and sensitivity analyses were performed.
Results:
Among 1,115 patients with unruptured VBFAs, 838 (median age, 54 years; 655 men) were included. After IPTW, baseline characteristics were balanced. Median follow-up was 54 months. FD was associated with a lower risk of poor prognosis than CM (OR, 0.48 [95% CI, 0.30 to 0.77]; p=0.002), with no difference between CM and SAC. FD also reduced aneurysm rupture (OR, 0.20 [95% CI, 0.07 to 0.60]; p=0.004) and compression symptoms (OR, 0.30 [95% CI, 0.13 to 0.68]; p=0.004) versus CM. Time-to-event analyses further revealed significant differences in vertebral artery lesions and Type I–II VBFAs, whereas no significant differences were observed in basilar or vertebrobasilar junction lesions or in Type III–IV VBFAs.
Conclusions
Compared with CM, FD was associated with improved long-term outcomes in unruptured VBFAs, particularly in vertebral artery lesions and Type I–II VBFAs, although residual confounding cannot be excluded.

Background/Aims:
Liver transplantation (LT) following total hepatectomy is a life-saving treatment for hepatocellular carcinoma (HCC). The HCC recurrence after LT hinders the effectiveness of the procedure. The objective of this study is to develop a pre-operative risk stratification model based on a liquid biopsy.
Methods:
We conducted a comprehensive multi-omics study of 260 HCC patients from three centers, including clinical data, low-coverage whole-genome sequencing of cell-free DNA (cfDNA) from plasma, as well as whole-exome, single-nucleus RNA, and spatial transcriptomics from matched tumor and non-tumor tissues.
Results:
We identified cfDNA-derived copy number alteration (CNA) signatures associated with post-transplant recurrence. By integrating cfDNA-derived CNA profiles with single-cell transcriptomic data, we traced recurrence-associated cfDNA to a distinct subpopulation of malignant cells within the primary tumor. These cells were embedded in a pro-metastatic microenvironment of specialized endothelial subtypes and cancer-associated fibroblasts. Notably, most recurrence-associated lesions were detectable in cfDNA prior to liver transplantation (LT). Building on these insights, we developed the ZJU Criteria based on CNA fragments and tumor markers, a pre-LT risk prediction tool that integrates conventional clinical factors with cfDNA-derived CNA signatures, and validated it using internal and independent external cohorts.
Conclusion
Our findings suggest that post-transplant recurrence commonly originates from advanced subclones that emerge late during tumor evolution. The ZJU Criteria provides an accurate, non-invasive strategy that significantly improves pre-LT risk stratification and clinical decision-making for patients with HCC.
7.Application of pediatric ureteroscope-nested silicone catheter direct-vision catheterization in difficult urethral catheterization

Objective To evaluate the safety and efficacy of direct-vision catheterization using pediatric ureteroscopenested silicone catheter in male patients with difficult urethral catheterization, so as to provide a novel minimally invasive solution for the clinical management of difficult catheterization. Methods A retrospective analysis was conducted on 33 adult male patients with failed conventional urethral catheterization admitted to our hospital during Jan.2019 and Dec.2024.Directvision catheterization technique using a pediatric ureteroscope (F4.5/6.5) nested within an F18 silicone catheter was employed. The ureteroscope was inserted into the catheter lumen reaching the tip, utilizing the “sheath effect” to provide rigid support and a direct pathway.Pressurized irrigation connected to the independent channel of the three-way catheter provided sufficient fluid flow to identify the urethral course and guide catheterization under direct vision.Catheterization success rate, operation time, complication rate, and postoperative recovery were observed. Results Among the 33 patients, there were 26 cases of iatrogenic/traumatic urethral injury and 7 cases of underlying pathological structural abnormalities.Successful catheter placement was achieved in 32 patients (97.0%), 3 (9.4%) of which required zebra guidewire assistance, and 4 (12.5%) of which were successfully catheterized after adult ureteroscope dilation.One case with long-segment urethral stricture (approximately 4 cm) underwent suprapubic cystostomy.The median operation time was 12 (8, 18) minutes, with intraoperative blood loss less than 10 mL.No serious complications occurred during the procedure.After 14 days of catheter indwelling, patients achieved smooth voiding, median maximum flow rate was 18.2 (16.5, 20.4) mL/s.During the median follow-up of 6 (4, 9) months, no complications such as urethral stricture or urinary incontinence occurred. Conclusion Direct-vision catheterization using pediatric ureteroscope-nested silicone catheter is a safe and effective method for managing difficult urethral catheterization in male patients.

Japanese spotted fever(JSF)is an infectious disease caused by Rickettsia japonica,with nonspecific clinical symptoms and a high risk of misdiagnosis.We reported a case of JSF,in which Rickettsia japonica was detected in blood cells by metagenomic next-generation sequencing.The patient recovered after treatment with doxycycline.This report provides a reference for the clinical diagnosis and treatment of JSF.

9.Effect of neutrophil elastase inhibitor on acute respiratory distress syndrome in children

Objective:To investigate the efficacy of neutrophil elastase inhibitor in children with acute respiratory distress syndrome(ARDS).Methods:A total of 168 children with moderate to severe ARDS who were hospitalized in the Department of Pediatric Intensive Care Unit,the Affiliated Children′s Hospital of Xi′an Jiaotong University and Children's Emergency Center of Gansu Provincial Central Hospital from January 2022 to December 2023 were selected. Eighty-seven children receiving neutrophil elastase inhibitors were treated as the treatment group and 81 children receiving conventional treatment as the control group. The dynamic changes of general data,clinical indicators and ventilator parameters at 24 h,48 h and 72 h were compared between the two groups. Mortality at 28 days was the primary endpoint. Kaplan-Meier curve and Log-Rank test were used to evaluate cumulative survival.Results:There was no significant difference in general information and clinical characteristics between the two groups.Compared with the control group,arterial partial pressure of oxygen(PaO 2)/fraction of inspired oxygen(FiO 2)in the treatment group increased significantly at 48 h[(160.28±5.90)mmHg(1 mmHg=0.133 kPa)vs(141.04±4.01)mmHg, P<0.05]and 72 h[(227.58±6.85)mmHg vs(180.86±4.08)mmHg, P<0.05],and the differences were statistically significant.The platform pressure in the treatment group were lower than that of the control group at 24 h[(28.18±3.95)cmH 2O(1cmH 2O=0.098 kPa)vs(30.15±7.75)cmH 2O, P<0.05],48 h[(25.56±4.06)cmH 2O vs(29.07±5.01)cmH 2O, P<0.05],72 h[(24.95±2.82)cmH 2O vs(27.12±6.51)cmH 2O, P<0.05],and the differences were statistically significant. IL-8 in the treatment group were lower than that of the control group at 48 h[(78.26±14.05)ng/L vs(86.02±15.01)ng/L, P<0.05]and 72 h[(58.38±15.56)ng/L vs(68.68±18.05)ng/L, P<0.05],and the differences were statistically significant.The survival curve showed that the cumulative survival rate of the treatment group was higher than that of the control group,the difference between the two groups was statistically significant( χ2=4.549, P=0.033). Conclusion:Neutrophil elastase inhibitors can reduce the lung injury of ARDS patients by inhibiting the inflammatory response induced by neutrophils,and ultimately improve the prognosis of the disease.
10.Study on behavioral characteristics and neural mechanisms of obsessive-compulsive disorder model mouse induced by quinpirole

Objective Exploring the behavioral changes induced by quinpirole in obsessive-compulsive disorder(OCD)mouse,investigating the activation of neurons in different brain regions,and identifying differentially expressed genes(DEGs)and enriched biological pathways through transcriptome sequencing technology to elucidate the pathogenesis of OCD.Methods Randomly assign 32 male C57BL/6J mice,aged two months,to an OCD group and a control group(n=16).Administering quinpirole(0.75 mg/kg)via subcutaneous injection to the OCD group mice every other day for a total of 19 injections,while the control group mice received an equivalent volume of saline solution.Following the completion of the model construction,open field testing,elevated plus maze testing,and marble burying tests were conducted.After the completion of behavioral studies,tissue samples were collected.Neuronal damage was assessed using Nissl staining,while the expression of c-Fos and Iba1 proteins was examined through immunofluorescence staining.Transcriptome sequencing technology was utilized to screen for differentially expressed genes and to enrich relevant signaling pathways.The expression of inflammatory cytokines,including TNF-α,NF-κB p65,phosphorylated NF-κB p65(p-NF-κB p65),and IL-6,was detected using Western Blot analysis.Results Mouse induced with OCD by quinpirole exhibit anxiety-like behaviors and compulsive-like behaviors.Neurons in the hippocampal and hypothalamic regions exhibit signs of damage.The expression of c-Fos and Iba1 proteins is increased in the cortex,striatum,hypothalamus,and other brain regions.Western Blot result indicate a significant increase in the expression of pro-inflammatory cytokines such as TNF-α,p-NF-κB p65,and IL-6.Conclusions In OCD mouse,neurons in multiple brain regions are abnormally activated,microglia exhibit dysfunction,and neuroinflammation induced by the activation of the NF-κB signaling pathway accompanies the development of OCD.
