1.Manganese porphyrin metal-organic framework nanoparticles loaded with DMXAA combined with sonodynamic therapy for the treatment of triple-negative breast cancer mouse xenografts

[摘 要] 目的:构建负载STING激动剂DMXAA的锰卟啉金属有机框架纳米颗粒(DPM),探讨其对三阴性乳腺癌(TNBC)细胞4T1及其小鼠移植瘤的治疗效果。方法:通过物理吸附法制备 DPM 纳米颗粒,利用透射电镜、扫描电镜及纳米粒度电位仪表征其形貌与理化性质。常规培养4T1细胞,细胞实验分为对照组、超声辐照组(US组)、DPM治疗组(DPM组)和DPM治疗联合超声辐照组(DPM + US组),用CCK-8法检测细胞活性,免疫荧光法检测高迁移率族蛋白B1(HMGB1)和钙网蛋白(CRT)的表达,WB法检测STING通路相关蛋白的表达。构建4T1细胞移植瘤小鼠模型,分为四组,处理同细胞实验,测量肿瘤体积,免疫荧光法检测移植瘤组织中Ki-67、HMGB1、CRT和缺氧诱导因子-1ɑ(HIF-1ɑ)蛋白的表达,TUNEL法检测细胞凋亡,流式细胞术检测免疫细胞活化情况,对主要器官进行H-E染色,以评估纳米材料的体内安全性。结果:DPM呈梭形,平均粒径(268 ± 3.302)nm,电位(33.1 ± 0.87)mV。细胞实验中,DPM联合超声辐照可明显抑制4T1细胞的增殖(P < 0.001),提高4T1细胞中ROS水平(P < 0.001),诱导4T1细胞CRT表达上调(P < 0.001),HMGB1从细胞核中移至细胞质,激活STING信号通路[p-STING、p-TBK1、p-IRF3蛋白表达均显著增加(均P < 0.001)]。体内实验中,DPM联合超声辐照可显著抑制4T1细胞移植瘤生长(P < 0.001)并促进免疫细胞表型转化(P < 0.001),抑制移植瘤组织中Ki-67、HIF-1α蛋白表达(均P < 0.01),谷胱甘肽(GSH)产生(P < 0.01),促进CRT、HMGB1蛋白表达、ROS产生(P < 0.001),对主要器官结构无明显影响。结论: DPM联合超声辐照可通过激活STING通路显著抑制4T1细胞及其移植瘤的生长,诱导抗肿瘤免疫应答,且对主要器官无明显毒性。
2.Construction and Evaluation of Mouse Model of Qi Deficiency and Phlegm Dampness Syndrome

ObjectiveQi deficiency and phlegm dampness syndrome is a common type of clinical traditional Chinese medicine(TCM) syndrome. However, there is no standard, scientific, and accurate report on the construction of animal models of Qi deficiency and phlegm dampness syndrome. This study aims to construct a mouse model of Qi deficiency and phlegm dampness syndrome by using a multi-factor composite modeling method and to evaluate the model. MethodsTwenty-one C57BL/6 mice were randomly divided into three groups with seven mice in each group, which were the normal group, model group, and Shenling Baizhusan (SLBZ) group. The control group was fed with ordinary diet and kept in a normal environment. The model group and SLBZ group were fed with a high-fat diet in a high-humidity environment. Swimming with heavy weights until exhaustion and gavage with cold water or lard were used to establish the mouse model of Qi deficiency and phlegm dampness syndrome. In order to test the syndrome by prescription, mice in the SLBZ group were treated with SLBZ for 14 days after model construction. The exhaustive swimming time, body weight, serum lipid levels, tongue changes, "Qi deficiency and phlegm dampness" assessment scale score, and cecal index of mice in each group were measured. The feces of each group of mice were sent for metagenomics and metabolome sequencing, and the changes in intestinal flora and metabolites were analyzed. ResultsAfter the modeling of Qi deficiency and phlegm dampness syndrome, the exhaustive swimming time of mice was obviously shortened (P<0.01). The serum total cholesterol, low density lipoprotein cholesterol, and non-high density lipoprotein cholesterol of mice were significantly increased (all P<0.01). The tongue of mice was significantly different from that of the normal group, and the score of the assessment scale was significantly higher than that of the control group (P<0.01). Cecal index decreased significantly (P<0.01). The serum lipid level, tongue image, assessment scale score, and cecal index were reversed in the SLBZ group. Metagenomic and metabolome sequencing results showed that intestinal flora and fecal metabolites were significantly changed in mice with Qi deficiency and phlegm dampness syndrome. Akkermansia_muciniphila, Faecalibaculum_rodentium, Eubacterium_plexicaudatum, Eubacterium sp 14_2, Candida glabrata, Romboutsia_ilealis, Turicibacter sp TS3, and other bacteria had significant changes, and the expressions of intestinal metabolites such as chenodeoxycholic acid, choline, L-phenylalanine betaine, and 2-phenylbutyric acid were significantly changed. Related metabolic pathways such as linoleic acid metabolism, primary bile acid biosynthesis, lysine degradation, arginine biosynthesis, and alpha-linolenic acid metabolism were affected. ConclusionThe Qi deficiency and phlegm dampness model of mice can be constructed by the multi-factor composite modeling method of high-fat diet feeding, high-humidity environment feeding, exhaustive swimming with heavy weight, and intragastric administration with cold water or lard. The blood lipid level, tongue change, score of "Qi deficiency and phlegm dampness assessment scale", cecal index, and changes in related intestinal flora and metabolites of mice can be used as key indicators for model evaluation.
3.Pregnancy probability prediction models based on 5 machine learning algorithms and comparison of their performance

Objective To construct 5 machine-learning models and compare their performance in predicting the associations between pre-pregnancy socio-psycho-behavioral exposures of both spouses and preconception outcomes.Methods Based on Chongqing Preconception Reproductive Health and Birth Outcome Cohort of volunteers recruited from Chongqing Health Center for Women and Children during January 2019 and March 2022,5 447 couples were recruited and surveyed through interviewer-interview for the demographic and social-psychological-behavioral data of both spouses(221 variables).According to the inclusion and exclusion criteria,4 097 couples were finally included,and randomly assigned into a training set(n=2 867 spouses)and a validation set(n=1 230 spouses)at a ratio of 7∶3.Feature analysis and collinear screening were applied to select the potential exposure factors.In consideration of difficulty to carry out semen parameters analysis in primary healthcare institutions,feature Set 1 including sperm parameters and feature Set 2 excluding semen parameters were constructed by including or excluding sperm quality simultaneously in the training set and the validation set.Five algorithms,that is,Logistic Regression,Naive Bayes,Random Forest,Gradient Boosting Machine,and Support Vector Machine,were used to construct preconception outcome prediction models,and the parameters of each model were optimized using random search combined with grid search.The predictive performance of each model was compared using precision,recall,F1 score,area under the receiver operating characteristic curve(AUC),and calibration curve.The optimal model was then selected by comparing the changes in the predictive ability of the questionnaire data for fertility outcomes with or without semen parameters.Results There were 24 variables screened out in feature Set 1,and 16 variables in feature Set 2.In feature Set 1,the gradient boosting machine performed better,with a relatively higher AUC value(0.651)and better F1 score(0.61).The logistic regression model performed stably(AUC value=0.647)and was suitable as the reference model.The random forest(AUC value=0.641),Naive Bayes(AUC value=0.641),and support vector machine(AUC value=0.634)performed second-best.By utilizing the gradient boosting machine,comparable results were found between the predictions from feature sets with or without semen parameters,as in feature Set 1,the AUC value of its validation set was 0.651(95%CI:0.629~0.681),the prediction accuracy was 0.63,the recall rate was 0.65,and the average precision value F1 was 0.61;and in feature Set 2,the AUC value of its validation set was 0.649(95%CI:0.624~0.663),and both the calibration curves were close to the ideal curve.The prediction results indicated that in feature Set 1,the features highly negatively correlated with preconception outcomes were female age,male age,and no pregnancy within 1 year without contraception,while the features highly positively correlated with preconception outcomes were female pregnancy history,total sperm vitality,and use of contraceptive measures before enrollment.Conclusion Among the 5 machine-learning algorithms performed in this cohort data,the gradient boosting machine shows slightly better performance.There are 24 factors being associated with preconception outcomes in both spouses,and the performance of the simplified model excluding semen parameters is not significantly declined.It is feasible to use machine-learning methods to predict human preconception outcomes through social-psychological-behavioral questionnaires.
4.The regression equation of cognitive impairment in patients with obstructive sleep apnea hypopnea syndrome was constructed based on Logistic regression analysis

OBJECTIVE The regression equation of cognitive impairment in patients with obstructive sleep apnea hypopnea syndrome(OSAHS)was constructed based on Logistic regression analysis,to lay a foundation for the prevention of cognitive impairment in OSAHS patients.METHODS A total of 302 OSAHS patients admitted to Huai'an First People's Hospital from October 2022 to October 2024 were selected for the study,and the cognitive impairment group(n=83)and the non-cognitive impairment group(n=219)were included according to the occurrence of cognitive impairment.The status quo of OSAHS patients complicated with cognitive impairment was analyzed,univariate and multivariate Logistic regression was used to analyze the early warning factors of OSAHS combined with cognitive impairment,regression equation was established,and receiver operating characteristic(ROC)curve was drawn to analyze the predictive value of regression equation on OSAHS combined with cognitive impairment.RESULTS Among 302 OSAHS patients,83 cases were complicated with cognitive impairment(27.48%).Multivariate Logistic analysis showed that age increase,physical exercise<3 times/week,apnea hypopnea index increase,longest apnea duration prolongation,serum levels of neuropeptide Y(NPY)increase were early warning factors of OSAHS combined with cognitive impairment.The increase of minimum blood oxygen saturation and serum gamma-aminobutyric acid(GABA)were protective factors of OSAHS combined with cognitive impairment(P<0.05).The above factors were included in the regression equation:logit(P)=-1.875+age×0.059+physical exercise<3 times/week×0.041+apnea hypopnea index×0.031+longest apnea duration×0.073-minimum blood oxygen saturation×0.081+NPY×0.043-GABA×0.049.The regression equation was evaluated,and the likelihood ratio of Chi-square=7.632,DF=7,P<0.001,the regression equation was statistically significant.Patients with OSAHS combined with cognitive impairment were included as positive,while those OSAHS without cognitive impairment were included as negative.The ROC curve for predicting OSAHS combined with cognitive impairment was drawn according to the regression equation.The results showed that when logit(P)>2.02,the area under the curve(AUC)was 0.896.The diagnostic sensitivity and specificity were 85.54%and 83.56%respectively.CONCLUSION OSAHS patients with cognitive impairment were not optimistic,early warning factors included increasing age,less than 3 times/week of physical exercise,increased apnea hypopnea index,prolonged longest apnea time,elevated serum NPY,protective factors included elevated minimum blood oxygen saturation and elevated serum GABA,the regression equation constructed on this basis had good predictive value.
5.AuNPs-FeCDs Dual Nanozyme Cascade System Integrated with A Smartphone Platform for Sensitive Detection of Glucose

A centrifugation-free,single-reaction colorimetric method for detection of glucose,utilizing a dual nanozyme cascade system based on gold nanoparticles(AuNPs)and iron-doped carbon dots(FeCDs),was developed in this work.The AuNPs exhibited glucose oxidase-like activity to catalyze glucose oxidation for generation of H2O2,while the FeCDs demonstrated peroxidase-like activity to subsequently catalyze the H2O2-mediated oxidation of 3,3',5,5'-tetramethylbenzidine(TMB).To prevent interference from the blue signal generated by self-aggregation of AuNPs in subsequent quantitative detection,the reaction system was terminated with HCl,converting oxTMB into a stable yellow product.Based on changes in the absorbance at 450 nm of this yellow solution,a quantitative relationship was established between glucose concentration and absorbance at 450 nm(A450).Experimental results demonstrated that this sensor achieved a linear detection range of 44 μmol/L to 11.11 mmol/L(R2=0.993)with a detection limit of 30.68 μmol/L and spiked recoveries of 97.9%-104.7%.By integrating smartphone-based color recognition capabilities,a rapid visual detection platform was established for quantification of glucose through RGB analysis.The validation experimental results using commercial glucose injection samples further confirmed the practical application potential of this methodology.

The health of women and children serves as the cornerstone of comprehensive public health and represents a crucial barometer for measuring social progress and civilization. It directly impacts family well-being and social harmony. As a national-level guidance center for complex disease diagnosis and treatment, Beijing's critical maternal care center, and the designated consultation hospital for pregnancy complicated by rheumatic immune and endocrine disorders, Peking Union Medical College Hospital bears significant responsibilities in managing complex medical cases and implementing counterpart assistance programs. Confronted with diverse and complicated maternal sources, particularly emergency cases lacking standardized prenatal care, the hospital has established a multi-tiered, whole-process, and comprehensive maternal-infant safety management system. Through implementing high-risk maternal management protocols, optimizing critical care procedures, promoting informatization development, and enhancing multidisciplinary collaboration, Peking Union Medical College Hospital has substantially improved the efficiency of critical maternal care and maternal-infant safety assurance. This paper systematically summarizes the institutional development experience in maternal management and explores potential optimization approaches, aiming to provide valuable references for enhancing maternal-infant safety management systems in domestic healthcare institutions.
7.Fermentation improves the immune function of the Astragalus membranaceus (Fisch.) Bunge stem and leaf

ObjectiveTo improve the efficacy of Astragalus membranaceus (Fisch.) Bunge (A. membranaceus, Huang Qi), and to further develop and utilize it, fermentation technology was applied to the stem and leaf of A. membranaceus to enhance its immune function.MethodsIn this study, we fermented A. membranaceus stem and leaf (ASL) with probiotics and investigated its immune function. Firstly, we screened suitable strains for ASL fermentation and optimized the fermentation process. Secondly, we determined the antioxidant capacity of fermented ASL and its effect on inflammation in mouse monocyte-macrophage cell. Finally, the immunocompromised mice were treated with fermented ASL to investigate the changes in their immune ability.ResultsAmong the 10 selected probiotics, Lactobacillus plantarum was the most suitable strain for ASL fermentation. After optimization of the fermentation process, the content of saponins in fermented ASL was significantly increased. The fermented ASL exhibited strong anti-inflammatory and antioxidant activities in vitro. The in vivo immune efficacy improved by promoting the development of the spleen and thymus, as well as raising the immunoglobulin M, tumor necrosis factor-α, and interleukin-1β levels of in the serum.ConclusionThis study contributes to developing the non-medicinal parts of A. membranaceus, expands its medicinal resources, highlights the potential of fermentation technology to enhance these parts, and provides a reference for further development. Based on this approach, we can promote using non-medicinal parts of herbal medicines, minimize drug waste, and offer a reference for developing non-medicinal components in Chinese herbal medicines.
8.Castor stent implantation combined with subclavian artery bypass grafting for the treatment of aortic arch lesions

Objective To explore the short- and mid-term efficacy of Castor single branch aortic stent combined with subclavian artery bypass grafting for the aortic arch lesions. Methods A retrospective analysis of the clinical data of patients with proximal anchor zone insufficiency aortic arch lesions treated with Castor stent combined with subclavian artery bypass grafting at the Department of Cardiac Surgery, Nanjing Drum Tower Hospital, the Affiliated Hospital of Nanjing University Medical School from August 2020 to November 2023 was performed. Results A total of 22 patients were included, including 19 males and 3 females, with an average age of (56±16) years. There were 18 patients of aortic dissection and 4 patients of aortic arch tumors. The success rate of surgical technique was 100.0%, and the average postoperative hospital stay for patients was (10±4) days. The median follow-up time was 20 months. During the follow-up period, there were no major complications such as endoleak, paraplegia, cerebral infarction, and renal insufficiency, and all patients had no readmissions. Conclusion For proximal anchor zone insufficiency aortic arch lesions, the treatment method of using a Castor stent branch placed in the left common carotid artery can effectively extend the anchor zone, avoid the huge trauma of open chest surgery, and achieve a good short- and mid-term efficacy.
9.Assessing distribution characteristics and clinical significance of vertebral fractures in patients with osteoporosis based on whole spine MRI

BACKGROUND:Osteoporotic vertebral fractures are the most common complication in patients with osteoporosis.As a new imaging technique,spine magnetic resonance imaging(MRI)is much more sensitive than X-ray film in the diagnosis of osteoporotic vertebral fractures.However,total spine MRI is costly and takes a long time to scan.Therefore,there is no consensus on whether all patients with osteoporotic vertebral fractures need to undergo total spine MRI scan and which patients need to undergo total spine MRI. OBJECTIVE:To analyze the distribution characteristics of vertebral fractures and explore their clinical significance by observing the whole spine MRI data of osteoporotic vertebral fractures patients. METHODS:Data of cases and MRI images of all patients diagnosed with fresh osteoporotic vertebral fractures who visited the Department of Orthopedics,Affiliated Hospital of Southwest Medical University from August 2018 to September 2022 were retrospectively analyzed.903 patients were included in the study based on inclusion and exclusion criteria.General information(age,gender,and body mass index),medical history characteristics(duration of illness,history of trauma surgery,percussion pain area,and pain score)were collected.The characteristics of vertebral fractures were analyzed through whole spine magnetic resonance imaging.Firstly,based on the number of vertebral fractures in patients,they were divided into the single vertebral fracture group(484 cases)and the multi-vertebral fracture group(419 cases),and the differences were analyzed between the two groups.Then,based on whether the farthest interval between the fractured vertebrae was greater than or equal to 5,the multi vertebral fracture group was further divided into two subgroups.Among them,Group A(the farthest interval between the fractured vertebrae was less than 5)contained 306 cases;Group B(with the farthest interval between fractured vertebral bodies greater than 5)included 113 cases.The differences were analyzed between two subgroups. RESULTS AND CONCLUSION:(1)Among 903 patients,419 patients(46.4%)had more than two fractured vertebrae.There were 654 patients(72.4%)with thoracolumbar fractures,and 54 patients(6%)with fractures in the thoracic plus lumbar region and the entire thoracic to lumbar region.In group B,96.5%of patients had multiregional percussion pain.(2)Compared with the patients in the single vertebral fracture group and the multi-vertebral fracture group,there were significant differences in bone mineral density,whether the medical history was greater than or equal to 1 month,the history of low energy injury,and the distribution and number of axial percussion pain areas in the spine during physical examination between the two groups(P<0.05).Age,gender,body mass index,whether there was underlying disease,pain visual analog scale score,whether there was a history of elderly thoracolumbar fracture,and whether there was a history of thoracolumbar surgery,and the number of fractured vertebrae had no statistical significance(P>0.05).(3)There were statistically significant differences between the Groups A and B in bone mineral density,the distribution and quantity of percussion pain area,and the history of low energy injury(P<0.05).There were no significant differences in age,gender,history of old fractures,visual analog scale score,body mass index,whether the medical history was longer than or equal to 1 month,history of underlying diseases,and history of thoracolumbar surgery between the two groups(P>0.05).(4)Patients with multiple low-energy trauma history,history of more than 1 month,multiple percussion pain,and the lower bone mineral density should be alert to the occurrence of multiple vertebral fracture and jump fracture.We recommend the whole spinal MRI for these patients.
10.A Single-Arm Phase II Clinical Trial of Fulvestrant Combined with Neoadjuvant Chemotherapy of ER+/HER2– Locally Advanced Breast Cancer: Integrated Analysis of 18F-FES PET-CT and Metabolites with Treatment Response

Purpose:
This Phase II trial was objected to evaluate the efficacy and safety of adding fulvestrant to neoadjuvant chemotherapy in patients with estrogen receptor (ER)+/human epidermal growth factor receptor 2 (HER2)– locally advanced breast cancer (LABC). Additionally, the study aimed to investigate the association of 16α-18F-fluoro-17β-fluoroestradiol (18F-FES) positron emission tomography (PET)–computed tomography (CT) and metabolites with efficacy.
Materials and Methods:
Fulvestrant and EC-T regimen were given to ER+/HER2– LABC patients before surgery. At baseline, patients received 18F-FES PET-CT scan, and plasma samples were taken for liquid chromatography–mass spectrometry analysis. The primary endpoint was objective response rate (ORR). Secondary endpoints included total pathologic complete response (tpCR) and safety.
Results:
Among the 36 patients enrolled, the ORR was 86.1%, the tpCR rate was 8.3%. The incidence of grade ≥ 3 treatment-emergent adverse events was 22%. The decrease in ER value in sensitive patients was larger than that in non-sensitive patients, as was Ki-67 (p < 0.05). The maximum standardized uptake value, mean standardized uptake values, total lesion ER expression of 18F-FES PET-CT in sensitive patients were significantly higher than those in non-sensitive patients (p < 0.05). Moreover, these parameters were significantly correlated with Miller and Payne grade and the change in ER expression before and after treatment (p < 0.05). Thirteen differential expressed metabolites were identified, which were markedly enriched in 19 metabolic pathways.
Conclusion
This regimen demonstrated acceptable toxicity and encouraging antitumor efficacy. 18F-FES PET-CT might serve as a tool to predict the effectiveness of this therapy. Altered metabolites or metabolic pathways might be associated with treatment response.
