The overall health condition of patients significantly affects the diagnosis,treatment,and prognosis of endodontic diseases.A systemic consideration of the patient's overall health along with oral conditions holds the utmost importance in determining the necessity and feasibility of endodontic therapy,as well as selecting appropriate therapeutic approaches.This expert consensus is a collaborative effort by specialists from endodontics and clinical physicians across the nation based on the current clinical evidence,aiming to provide general guidance on clinical procedures,improve patient safety and enhance clinical outcomes of endodontic therapy in patients with compromised overall health.
2.Posterior capsule of prostatic apex sparing laparoscopic radical cystectomy and intracorporeal orthotopic neobladder (Studer technique)

Objective:To report the perioperative outcomes and short-term efficacy of posterior capsule of prostatic apex sparing laparo-scopic radical cystectomy and intracorporeal orthotopic neobladder (PCPAS-LRC-ICONB),also known as the Studer technique. Methods:A retrospective analysis was conducted on 34 male patients who underwent PCPAS-LRC-ICONB between July 2019 and July 2023 at Huai'an Hospital of Huai'an City and Tianjin Medical University Second Hospital. The perioperative and short-term outcomes of PCPAS-LRC-ICONB were summarized. Results:All 34 patients completed the PCPAS-LRC-ICONB procedure successfully. Neobladder-urethral anastomosis was performed without tension,and no cases required conversion to open surgery. The median operative time was 420 min (range 350-520 min),with a median estimated blood loss of 200 mL (range 150-600 mL). The median follow-up time was 22.5 months (range 4-60 months). No complications of Clavien grade three or higher occurred during the perioperative period. At 12 months postoperatively,the daytime and nighttime urinary continence rates were 88.9% and 66.7%,respectively. All the patients were satisfied with their urinary continence status. The median bladder capacity was 225 mL (range 100-400 mL) and the median residual urine volume was 20 mL (range 0-50 mL). Pulmon-ary metastases were detected in two patients at 6 and 15 months postoperatively,bone metastases in two patients at 18 and 25 months,liver metastasis in one patient,and local recurrence (one case of abdominal wall implantation) in two patients. By the end of the follow-up period,one patient died from cerebral infarction,one from myocardial infarction,one from other accidents,and six patients died from blad-der cancer recurrence and/or metastasis. Conclusions:PCPAS-LRC-ICONB reduced the difficulty of neobladder-urethral anastomosis,and the perioperative and short-term outcomes were satisfactory.

4.Serological characteristics and genotype analysis of D epitope blood group of RhD variant individuals in Guangzhou

【Objective】 To explore the characteristics of the D antigen epitope of individuals with RhD variants and the genetic molecular mechanism of gene mutations in Guangzhou. 【Methods】 A total of 59 samples of RhD variants were collected from blood donors and hospitals in Guangzhou from January to August 2019. Serological characteristics of D epitopes were further analyzed using two kinds of monoclonal anti-D reagents and D epitope detection kits, and RHCE phenotypic typing was performed. QuickGene DNA extraction kit was used to extract the genomic DNA of the samples, and PCR-RFLP method was used to analyze the RHD gene zygote type. The RHD gene sequence was detected by multiple ligation-dependent probe amplification(MLPA) genotyping, and the RHD exon(1~10) Sanger sequencing was performed on the samples still in doubt after the above detection. DNAStar/SeqMan analysis software was used for comprehensive analysis. 【Results】 In this group of individuals with RhD variants in Guangzhou, 27.12%(16/59) were detected from blood donors [accounting for 0.007%(16/232 793) of blood donors in Guangzhou during the same period], and difficult samples of patients sent by hospitals for determination accounted for 72.88%(43/59). RHD genotype detection: 40.68%(24/59) were RHD*weak partial 15, 25.42%(15/59) were RHD* DⅥ.3 and 33.90%(20/59) were rare RHD variants [76.92%(10/13) were RhD variants with 2 different alleles]. Serological D-screen revealed a relatively fixed pattern of RHD*DⅥ.3 in anti-D antibody(clone: P3*212 23B10), while the others was negative. The phenotypic distribution of RhD variant CE was Ccee 38.98%(23/59), ccEe 35.59%(21/59), CcEe 25.42%(15/59). 【Conclusion】 Weak partial D15 and DⅥ.3 were the most common RhD variants in Guangzhou Han population, and DⅥ can be preliminarily identified by serological methods such as D-Screen anti-D reagent, while the remaining RhD variants can only be identified by molecular biological methods, and >95% of the RhD variants were C+ or E+ phenotypes.
5.Surgical design of primary unilateral cleft lip repair accordingto distance measurement of the upper nasolabial region

Objective:To study the possible effect of soft tissue displacement in the surgical design of primary unilateral cleft lip repair.Methods:Charts of 52 unilateral cleft lip patients who got repaired from January 2014 to December 2015 in Peking Union Medical College Hospital were retrospectively reviewed. The nasolabial region measurements of these 52 cases during the lip surgery were especially collected. The distance between the key points in the upper nasolabial region were directly measured with a ruler and/or caliper. The cleft lips were repaired by rotation-advancement technique. The soft tissue along the cleft edge was sufficiently released, and the muscles were reconstructed.Results:Except the lip height and the distance between crista philtral point and subalar base on the noncleft side, most measurements between the key points on both sides got significant increase postoperatively ( P<0.05). The symmetrical ratio of the preoperative measurements on upper labial region were closing to 1 departing from the cleft region. The symmetrical ratio improved significantly postoperatively ( P<0.01). Conclusions:During the primary unilateral cleft lip surgery, the changes of the distances between the key points of upper nasaolabial region would not always follow the geometry. They are also affected by other factors, like the interaction between each layer and the elasticity of soft tissue, which should be considered and employed during the surgical design.
6.Dynamic contrast-enhanced MRI for the detection of testicular ischemia in experimental induced testicular torsion of rabbits

Objective To evaluate the dynamic contrast-enhanced MRI (DCE-MRI)in detecting experimentally induced testicular ischemia. Methods Thirty healthy male New Zealand rabbits were randomly assigned into 6 groups. There were 5 rabbits in each of the following experimental groups: ( 1 ) Normal control, (2) Sham-operated, (3) ischemia of 3 h group, (4) ischemia of 6 h group, (5) ischemia of 12 h group, (6) ischemia of 24 h group. In all experiment groups, the right testis served as the internal control while the left testis served as the experimental side. DCE-MRI for each animal lasts about 10 minutes. Signal enhanced ratios (SERs) of ROI for both sides of each group were calculated by a computer, and parameters of SERs of 30 s, 75 s, 120 s and maximal SER were used for statistical analysis.Time intensity curves (TICs) were made for two sides of each group via Excel 2003 software and classified into 4 types. Statistical analysis was performed to compare the differences of SERs between left and right testis by two independent Kolmogorov-Smirnov test. Results In group I and 2, significant enhancement was observed on both testes of 10 rabbits. The enhancement decreased gradually with the elongation of ischemia in torsion groups. Three cases of type Ⅰ and 2 cases of type Ⅱ were observed in group 1,5 cases of type Ⅰ in group two, 2 cases of type Ⅰ and 3 cases of type Ⅱ b in group three, 2 cases of type Ⅰ and 2 cases of type Ⅱ b in group four, 5 cases of type Ⅱ b in group five and 5 cases of type Ⅲ in group six were noticed in the left testes. And in TICs of right testes, all cases showed TICs of type Ⅰ except 2 cases of type Ⅱ a in group six. In four torsion groups, the values for SER75 of the left side were 0. 084%, 0. 076%, 0.164% and 0.065%, while the right side were 0.255%, 0.410%, 0.586% and 0.302% (P <0.05). The values for SER120 in group three, five and six were 0.221% , 0.158% and 0.059% for the left side, and 0.405%,0.522% and 0.207% for the right side(P <0.05). The values for MSER in group three, five and six were 0.217% ,0.164% and 0.072% for the left side, and 0.405%, 0.586% and 0.302% for the right side(P <0.05). Conclusion DCE-MRI technique may be useful in the diagnosis of testicular torsion, which shows potential in the clinical application.
