Chinese Journal of Clinical Laboratory Science 2025;43(3):167-173
Objective To identify serum metabolic markers served in the clinical diagnosis of ulcerative colitis(UC).Methods Ser-um samples from 29 UC patients,31 Crohn's disease(CD)patients, and 21 matched healthy controls(HC)admitted to Department of Gastroenterology,Nanjing Drum Tower Hospital during September 2022 and March 2024 were collected. The ultra-high-performance liquid chromatography coupled with quadrupole-orbitrap mass spectrometry(UHPLC-Q Exactive HF-X)technology was used to detect and analyze serum metabolites.A partial least squares discrimination analysis(PLS-DA)model was constructed, and the metabolites significantly up-regulated in UC were screened based on the variable importance in projection(VIP)score>1,P value<0.05,and fold change(FC)>1.2.The pathway enrichment analysis was performed using the Kyoto Encyclopedia of Genes and Genomes(KEGG)database to reveal the biological pathways involved in the metabolites. The area under the receiver operating characteristic curve(AUCROC)was calculated to evaluate the diagnostic potential of the differential metabolites. Results A total of 1 522 metabo-lites were identified from the three sample groups.Among them,4 metabolites, namely leucodopachrome(VIP=1.964,P<0.05,FC=1.916),tetrahydrodipicolinate(VIP=1.74,P<0.05,FC=2.65),N-ethylmaleimide(VIP=1.519,P<0.05,FC=1.597),and 5,6-dihydroxyindole(VIP=3.018,P<0.05,FC=1.575),were significantly up-regulated in UC.Their AUCROC values for distinguishing UC from CD were 0.788(95%CI:0.655-0.921),0.773(95%CI:0.639-0.907),0.834(95%CI:0.720-0.949),and 0.899(95%CI:0.821-0.977),respectively,while those for distinguishing UC from HC were 0.966(95%CI:0.924-1.000),0.926(95%CI:0.857-0.995),0.969(95%CI:0.928-1.000),and 0.910(95%CI:0.830-0.990),respectively.KEGG pathway analysis showed that the up-regulated metabolites in UC were primarily enriched in biological pathways such as tyrosine metabolism, glycerophospholipid me-tabolism,and arachidonic acid metabolism. Conclusion The serum metabolic profile of UC patients is significantly changed, and the four differential metabolites mentioned above may serve as effective biomarkers for the differential diagnosis of UC,CD,and HC.

Objective:To analyze the effect of joint management of type 2 diabetes mellitus (T2DM) between specialty and community under the model of National Diabetes Prevention and Control Center (DPCC).Methods:A total of 2 527 T2DM patients managed by DPCC Pingshan Center of Shenzhen from January 1, 2022 to December 31, 2024 were retrospectively included. After management, the rate of downturn, reexamination rate, blood pressure compliance rate, metabolic indicators (waist circumference, body mass index, fasting blood glucose, glycosylated hemoglobin, blood lipids) and screening rate of chronic complications of diabetes (atherosclerotic cardiovascular disease, microvascular disease, diabetic peripheral neuropathy) were analyzed. Those included 2022 ( n=564), 2023 ( n=1 477), and 2024 ( n=2 527). Results:The downturn rate in 2022, 2023 and 2024 increased year by year (22.8% vs 67.2% vs 89.9%, P<0.01), and the review rate (41.1% vs 62.2% vs 52.7%, P<0.01), complication screening rate (51.6% vs 85.3% vs 62.2%, P<0.01), blood pressure compliance rate (53.1% vs 78.0% vs 67.2%, P<0.01), body mass index compliance rate (13.2% vs 17.3% vs 28.6%, P<0.01), fasting blood glucose meeting rate (46.4% vs 60.2% vs 68.5%, P<0.01), glycated hemoglobin meeting rate (58.4% vs 63.2% vs 45.6%, P<0.01) were relatively improved. Waist circumference compliance rate (30.6% vs 27.7% vs 21.6%) and blood lipid compliance rate (33.6% vs 35.5% vs 31.9%) were not significantly improved, and the review rate, blood pressure compliance rate and complication screening rate in 2024 were lower than those in 2023 and higher than those in 2022. Conclusions:The combined management of type 2 diabetes under the DPCC model has significant effects on improving the down-conversion rate, rediagnosis rate, blood pressure compliance rate, metabolic index compliance rate and the screening rate of diabetes-related chronic complications in patients with diabetes. At the same time, it was also found that with the progress of hierarchical diagnosis and treatment, the review rate, complication screening rate, blood pressure, waist circumference, blood lipid and glycosylated hemoglobin reached the standard of patients decreased compared with the previous situation, which needs to be further analyzed and improved.

Objective:To evaluate the role of hippocampal activating transcription factor 5 (ATF5) in cognitive impairment induced by neuropathic pain and the relationship with mitochondrial unfolded protein response(mtUPR) in mice. Methods:This study was conducted in 2 parts. Experiment Ⅰ Twenty-four SPF healthy male C57BL/6 mice, aged 6-8 weeks, weighing 20-25 g, were divided into 2 groups ( n=12 each) using a random number table method: sham operation group (S1 group) and neuropathic pain group (NP group). Neuropathic pain was induced by chronic constriction injury to the sciatic nerve. The mechanical paw withdrawal threshold (MWT) and thermal paw withdrawal latency (TWL) were measured before developing the model and at 7, 14, 21 and 28 days after developing the model. Mouse cognitive function was assessed using the novel object recognition test from 30-31 days after developing the model. After the end of the novel object recognition test, mice were sacrificed and the hippocampal CA1 region was harvested for determination of the expression of ATF5 (by Western blot) and the expression of ATF5 in neurons, microglia and astrocytes (by immunofluorescence double staining). Experiment Ⅱ Thirty-six SPF healthy male C57BL/6 mice, aged 6-8 weeks, weighing 20-25 g, were divided into 3 groups ( n=12 each) using a random number table method: sham operation group (S2 group), neuropathic pain + ATF5 up-regulation group (NA group), and neuropathic pain + empty virus group (NE group). On day 14 after developing the model, a virus that specifically up-regulated ATF5 expression in neurons and empty virus were injected into the hippocampal CA1 region. The MWT and TWL were measured at days 28 and 35 after developing the model. The novel object recognition test was performed on day 36 after developing the model to evaluate the cognitive function. After the end of the behavioral test, mice were sacrificed and the hippocampal CA1 region was harvested for detection of the expression of ATF5 and mtUPR marker proteins (Lon protease [LONP1] and heat shock protein 60 [HSP60]) by Western blot. Results:Experiment Ⅰ Compared with S1 group, no statistically significant change was found in the MWT and TWL before developing the model ( P>0.05), the MWT and TWL were significantly decreased on days 7, 14, 21 and 28 after developing the model, the discrimination index (DI) was decreased at day 31 after developing the model, the expression of ATF5 was down-regulated, the expression of ATF5 in neurons was down-regulated ( P<0.05), and no statistically significant change was found in the expression of ATF5 in mircrolia and astrocytes in NP group ( P>0.05). Experiment Ⅱ Compared with S2 group, the MWT and TWL were significantly decreased on days 28 and 35 after developing the model in NE group and NA group, DI was decreased, and the expression of ATF5, LONP1 and HSP60 was down-regulated in NE group ( P<0.05), and no significant change was found in NA group ( P>0.05). Compared with NE group, no significant change was found in the MWT and TWL in NA group ( P>0.05), DI was significantly increased, and the expression of ATF5, LONP1 and HSP60 was up-regulated in NA group ( P<0.05). Conclusions:Down-regulated ATF5 in the hippocampus is involved in the process of cognitive impairment caused by neuropathic pain, and the mechanism may be related to the inhibition of mtUPR.

Objective:To evaluate the effect of age factor on the sedative potency of ciprofol. Methods:In this prospective study, patients of either sex, aged 40-80 yr, undergoing elective surgery under general anesthesia at the First Affiliated Hospital of Zhengzhou University, were selected and divided into middle-aged group (40-<65 yr) and elderly group (65-80 yr) based on their age. Effective sedation was defined as the Modified Observer′s Assessment of Alertness/Sedation scale score ≤1 and the bispectral index value ≤60 within 3 min following ciprofol administration. The initial dose of ciprofol was 0.4 mg/kg. The dose of ciprofol was determined by using the modified Dixon′s up-and-down method (increment or decrement of 0.04 mg/kg). The experiment was terminated after observing 8 transitions from effective sedation to ineffective sedation, starting from the initial transition event. The probit method was used to determine the median effective dose (ED 50) and 95% confidence interval of ciprofol for sedation. Results:A total of 61 patients was finally included in this study, with 35 in middle-aged group and 26 in elderly group. The ED 50 of ciprofol (95% confidence interval) was 0.296 (0.206-0.359) mg/kg in middle-aged group and 0.224 (0.185-0.257) mg/kg in elderly group. The ED 50 of ciprofol was significantly higher in middle-aged group than in elderly group ( u=181.00, P<0.001). Conclusions:Age can affect the sedative potency of propofol, with enhanced potency observed in elderly patients than in middle-aged patients.

Objective:To explore the therapeutic effect of micro-flaps carrying sensory nerve in treatment of high-pressure injection injuries of the digit (HPIID).Methods:From January 2022 to June 2024, retrospective analysis of 7 patients who had HPIID were admitted to the Department of Hand Surgery Division 1, Norinco General Hospital. The patients were 5 males and 2 females with ages from 25 to 59 years. The digital injuries were: 3 index fingers, 2 middle fingers and 2 thumbs. All patients received debridement under microscope in primary surgery, with the defects at 1.5 cm×2.4 cm - 2.5 cm×5.5 cm in size after debridement. In stage Ⅱ surgery, 5 patients received the treatment of free fibular great toe flap carrying peroneal nerve of the great toe, with the flap size at 1.8 cm× 2.0 cm - 2.7 cm×4.0 cm. Two patients received the treatment of transfer of free fibular medial plantar flap carrying medial plantar nerve, with flap size at 1.9 cm×2.6 cm - 4.5 cm×5.7 cm. Donor sites were directly sutured in 5 patients, and 2 patients received skin grafting. Five patients received postoperative follow-up at outpatient clinic, 1 patient via telephone interview and 1 via WeChat review. Results:All 7 flaps survived and all wounds had primary healing. All donor and recipient sites and skin grafting sites healed primarily. Postoperative follow-up lasted for 4 to 9 months, with an average of 6.4 months. All the affected digits had satisfactory appearance and function, except 1 which was slightly slimmer than the healthy side. Range of motion of the affected digits was evaluated according to total active movement (TAM): 5 were in excellent and 2 in good. Sensory recovery of the digits was evaluated according to the British Medical Research Council (BMRC): 1 digit was at S 2, 2 at S 3 and 4 at S 4. One patient had two-point discrimination (TPD) at 9.0-15.0 mm, 2 at 6.0-10.0 mm, and 4 at 3.0-6.0 mm. Conclusion:For HPIID with a defect, surgical treatment with transfer of micro-flap carrying sensory nerve should be a preferred treatment option.

The paper reports acupuncture treatment for one case of oculomotor nerve palsy after cranial-orbital communicating tumor surgery. The acupoint prescription was composed of the local acupoints of the eyes (Yansanzhen, Tijian, Cuanzhu [BL2], Yuyao [EX-HN4] and Sizhukong [TE23]), the acupoints on the head, face and neck (Yangbai [GB14], Sibai [ST2] and Fengchi [GB20]), Guanyuan (CV4) on the abdomen, and those on the four limbs (Hegu [LI4], Zusanli [ST36], Shenmai [BL62] and Zhaohai [KI6]). The point-to-point needling technique with the eyelid lifted was operated at Tijian, Cuanzhu (BL2), Yuyao (EX-HN4), and Sizhukong (TE23). Warm needling with moxa cone placed on the needle handle was operated at Guanyuan (CV4) and Zusanli (ST36), and the usual needling technique was delivered at the rest acupoints. The treatment was given once daily, discontinued for 1 day after every 6 treatments. One course of treatment was composed of 7 days, and 6 courses were required. After treatment completion, the upper eyelids were basically symmetrical and the bilateral eye cracks were equal, the double vision appeared occasionally. No recurrence and no aggravation were reported in 1 month of follow up visit.
