1.Pathogenic bacteria distribution and serum indicator levels as well as their relationship with cardiac function in CHF patients complicated with PI

Objective To analyze the distribution of pathogenic bacteria,changes in serum levels of long non-coding ribonucleic acid(lncRNA)nuclear enriched abundant transcript 1(NEAT1),Ga-lectin-3,and monocyte chemotactic protein-1(MCP-1),and their relationship with cardiac func-tion in patients with chronic heart failure(CHF)complicated with pulmonary infection(PI).Methods A total of 278 CHF patients treated in the Second Cardiovascular Ward of the Third Affiliated Hospital of Henan University of Chinese Medicine from January 2020 to February 2022 were recruited,and according to the occurrence of PI or not,they were divided into a PI group(98 cases)and a non-PI group(180 cases).The distribution of pathogenic bacteria in the PI group was analyzed.ROC curves were plotted to evaluate the predictive value of serum indicators for PI in CHF patients.Results Among the 98 CHF patients with PI,161 strains of pathogenic bacteria were detected,with Gram-negative(G-)bacteria most prevalent,accounting for 114 strains(70.8%).Serum levels of lncRNA NEAT1,Galectin-3,and MCP-1 were significantly higher in the PI group than the non-PI group(P<0.01).Pearson correlation analysis showed that serum levels of lncRNA NEAT1,Galectin-3,and MCP-1 were positively correlated with cardiac function(r=0.794,r=0.523,r=0.709,P<0.01).ROC curve analysis revealed that the combined detec-tion of serum lncRNA NEAT1,Galectin-3,and MCP-1 had the highest predictive value for PI in CHF patients than each indicator alone,and the AUC value,sensitivity,and specificity of the com-bined detection was 0.926,84.54%,and 92.78%,respectively(P<0.01).Conclusion Gram-negative bacteria are predominant type of pathogenic bacteria in CHF patients with PI.Serum levels of lncRNA NEAT1,Galectin-3,and MCP-1 are increasing with the occurrence of infection and worsening of cardiac function.Combined detection of these indicators provides higher predic-tive value for PI in CHF patients.

3.The predictive value of 68Ga-PSMA PET/CT to distinguish between benign and malignant prostate disease before repeat prostate biopsy

Objective:To investigate the diagnostic performance of repeat biopsy 68Ga-PSMA PET/CT to distinguish between benign and malignant prostate disease. Methods:The clinical data and medical imaging of thirty-nine patients underwent repeat prostate biopsy were analyzed respectively in this study. The median age of patients was 65 years (range 46-81 years), the median PSA level was 11.0ng/ml (range 5.4-49.8 ng/ml), f/tPSA was 0.15(0.01-16.50)ng/ml, prostate volume was 43.80(7.79-108.63)ml, and PSA density was 0.24(0.09-2.31)ng/ml 2. All patients underwent pre-biopsy 68Ga-PSMA PET/CT and the standard transrectal ultrasound-guided systematic prostate biopsy. Based on the biopsy results, 68Ga-PSMA PET/CT images of all patients were visually and semi-quantitatively analyzed. By visual analysis, 68Ga-PSMA uptake in prostate was defined as focal, multimodal and inhomogeneous, and then the detection rate of prostate cancer in each subgroup was analyzed. The value of the ROC curve in the diagnosis of prostate cancer was analyzed based on the SUV max of prostate cancer(SUV max), tumor-to-normal-prostate background(SUV T/BGp)as semi-quantitative parameters of 68Ga-PSMA PET/CT. Results:Prostate cancer was detected in 18 patients (46.2%) and 12 patients (30.8%) had clinically significant disease. There were 11, 5 and 2 patients with prostate cancer respectively in men with a focal (12 patients), multifocal(7 patients) and inhomogeneous (20 patients) 68Ga-PSMA uptake. The ROC analysis revealed a SUV max 5.3 and SUV T/BGp1.8 as an optimal cut-off level to distinguish between non-prostate cancer and prostate cancer in 68Ga-PSMA PET/CT, the sensitivity and specificity were 100.0% and 85.7% for SUV max (AUC=0.979), 83.3% and 90.5% for SUV T/BGp (AUC=0.915). Conclusions:Pre-biopsy 68Ga-PSMA PET/CT could help to distinguish between benign and malignant prostate disease before repeat prostate biopsy and detect the foci of prostate cancer.
4.HPLC fingerprint of Zishen Yutai pills and simultaneous determination of 5 index components.

To establish the HPLC fingerprint and determine five index components (loganic acid, chlorogenic acid, loganin, sweroside and asperosaponin Ⅵ) of Zishen Yutai pills by high performance liquid chromatography, and provide a scientific basis for its quality control. The fingerprint chromatogram was analysed by the chromatographic fingerprint similarity evaluation system for tradition Chinese medicine (2012), fifteen common peaks were obtained at the wavelength of 254 nm. Different batches of Zishen Yutai pills showed a similarity of above 0.90 in HPLC fingerprint profiles. For the quantitive analysis method, The separation of five components showed good regression (>0.999 2) with linear ranges, and the mean recoveries were in the range of 97.62%-101.9%, with the RSD (=9) less than 3%. The established fingerprint and quantitative analysis methods are highly specific, simple and accurate, which can reflect the quality of Zishen Yutai pills more comprehensively, and can be used for its quality control.
5.Studies on chemical constituents from stem barks of Fraxinus paxiana.

OBJECTIVETo investigate the chemical constituents of Fraxinus paxiana.
METHODThe chemical constituents were isolated and purified by chromatographic techniques and the structures of the compounds were identified with or by spectroscopic methods.
RESULTFifteen compounds were obtained from the methanol extract of F. paxiana and their structures were elucidated as esculin (1), esculetin (2), fraxin (3), fraxetin (4), salidroside (5), osmanthuside H (6), liriodendrin (7), 3-(4-beta-D-glucopyranosyloxy-3-methoxy)-phenyl-2E-propenol (8), threo-syringylglycerol (9), euscaphic acid (10), 3-hydroxy-1-(4-hydroxy-3, 5-dimethoxyphenyl)-1-propanone (11), omega-hydroxypropioguaiacone (12), sinapyladehyde (13), betulinic acid (14) and mannitol (15).
CONCLUSIONAll compounds were obtained from this plant for the first time.
