1.Simulation Analysis on the Compensation Mechanism for High-Quality Development of Public Hospitals Based on System Dynamics Model

Objective:To propose policy recommendations for the compensation mechanism of high-quality development of municipal public hospitals.Methods:Taking the municipal public hospitals in Shanghai as the research object,the aspects of financial subsidies,the structure of medical service income,investment in scientific research and education,and other social compensations were examined to conduct a systematic simulation of the operation of the average municipal public hospital in Shanghai under different policy intervention scenarios.Results:If the current conditions remain unchanged,the average municipal public hospital in Shanghai will face a deficit starting in 2032,the proportion of income from examinations and laboratory tests will surpass that of medical service income in 2026 and 2031,respectively.If the hospitals follow a plan of Combination Plan 3,4,and 5,the average municipal public hospital in Shanghai will be able to break even from 2023 to 2033,and the structure of medical service income will be significantly optimized.Conclusion:To establish a new compensation mechanism for high-quality hospital development that enhances medical services,it is recommended to optimize the structure of government financial compensation,explore rational pricing strategies based on categories,increase investment in scientific research and education while paying attention to the transformation of scientific and technological achievements,expand compensation channels for public hospitals,and improve relevant laws and regulations.
2.Literature Analysis and Textual Research on the Classical Formula Nuangan Jian

Nuanjian Jian(Decoction for Warming Liver Meridian),originating from Jing Yue Quan Shu(Jingyue's Complete Works)written by Zhang Jingyue,is composed of Angelicae Sinensis Radix(Danggui),Lycii Fructus(Gouqizi),Poria(Fuling),Fructus Foeniculi(Xiaohuixiang),Cinnamomi Cortex(Rougui),Linderae Radix(Wuyao),and Aquilariae Lignum Resinatumd(Chenxiang)or Aucklandiae Radix(Muxiang).Nuanjian Jian is a widely-used classical formula for warming the liver and kidneys,promoting qi circulation and alleviating pain.It was included in the Ancient Classical Formulas Catalog(First Batch)published by the National Administration of Traditional Chinese Medicine in 2018,and was primarily used to treat lower abdominal pain,hernia,and other disorders differentiated as the syndrome of cold stagnation in the liver and kidneys.Based on the principles of key information verification for classical formulas,this article systematically reviewed the ancient and modern literature on Nuangan Jian,and conducted a comprehensive textual research and analysis of its origin,composition,dosage,method of making a decoction and taking medicines,processing,modifications,historical and modern applications,and experimental studies.The study concluded that the composition and dosage of this formula are relatively clear.The contemporary single-dose equivalents are calculated as Danggui 9.33 g,Gouqizi 11.19 g,Fuling 7.46 g,Xiaohuixiang 7.46 g,Rougui 5.60 g,Wuyao 7.46 g,and Chenxiang 3.73 g.The decoction is prepared by adding 300 mL of water and 4 g of ginger,boiling until reduced to 140 mL,and taken 2-3 times daily,2 hours after meals.Modern practitioners have expanded the indications of this formula,extending its use from hernia and lower abdominal pain to various disorders in andrology,gynecology,and gastroenterology.The indications have the core pathogenesis of cold stagnation in the liver meridian,and usually affect the liver while also involve the spleen,kidneys,and lower energizer.Diseases of the lower abdomen,genitals,and breasts belonging to the region of the liver-meridian route,are also commonly treated with Nuangan Jian.The findings of this study provide a reference for the further development of this classical formula.
3.Analysis of SUN Simiao's Treatment of Vomiting and Modern Clinical Application

[Objective]To summarize SUN Simiao's clinical experience in treating vomiting and explore the modern application of SUN's method for treating vomiting,taking diabetic gastroparesis vomiting(DGPV)as an example.[Methods]By excavating and organizing the special chapters or paragraphs related to vomiting in SUN Simiao's works Emergency Thousand Gold Prescriptions and Thousand Gold Wing Prescriptions,it comprehensively analyzes SUN's understanding of the causes and treatment of vomiting,and applies it to clinical practice based on the pathogenesis of DGPV.[Results]Vomiting is a common clinical symptom of digestive tract abnormalities.Although conventional western medicine treatment can temporarily alleviate vomiting,it is prone to recurrence.SUN Simiao had profound insights in the diagnosis and treatment of vomiting,believing that"abnormal lifting and descending of the middle-Jiao,upward invasion of stomach Qi""cold,heat,phlegm,water,food and various evils invading the middle-Jiao"and"long-term illness injuring the five Zang organs and insufficient Qi and blood"were the core pathogenesis of vomiting.Through inductive analysis of his works,six methods and corresponding prescriptions can be extracted:harmonizing the stomach and descending Qi to stop vomiting,warming Yang and dispelling cold to stop vomiting,resolving phlegm and promoting diuresis to stop vomiting,digesting food and resolving stagnation to stop vomiting,clearing heat and nourishing fluids to stop vomiting,and regulating organs and tonifying deficiency to stop vomiting.In the clinical treatment of DGPV,small prescriptions and corner herbs for treating vomiting should be used in a targeted manner based on the core pathogenesis of different stages.[Conclusion]SUN Simiao's thoughts on treating vomiting still have certain application value in modern medicine and provide guidance for clinical treatment of vomiting such as DGPV.

6.Role and mechanisms of G-protein-coupled receptor 39 in brain injury after intracerebral hemorrhage in mice

Objective To investigate the effect and underlying mechanisms of G-protein-coupled receptor 39(GPR39)activation on neuroinflammation and brain injury after experimental intracerebral hemorrhage in mice.Methods Mouse model of intracerebral hemorrhage(ICH)was established by intracerebral injection of autologous blood.A total of 176 male C57/BL6 mice were randomly divided into 8 groups:Sham group(n=42),ICH group(n=34),ICH+Vehicle group(n=32),ICH+TC-G 1008 group(n=44),ICH+GPR39 siRNA group(n=6),ICH+Scramble siRNA group(n=6),ICH+TC-G 1008+666-15 group(n=6),and ICH+TC-G 1008+Vehicle 2 group(n=6).GPR39-specific agonist TC-G 1008 was administered via oral gavage at 1 and 25 h post-ICH modeling.Additionally,GPR39 siRNA and cAMP response element binding protein(CREB)inhibitor 666-15 were intracerebroventricularly injected 24 h before induction of ICH to inhibit the expression levels of GPR39 and p-CREB.At 48 h after ICH,modified Garcia test,forelimb placement test and corner turn test were used to evaluate the short-term neurological deficits in mice.Brain water content was determined by wet/dry method.Immunofluorescence assay was performed to detect the co-localization of GPR39 in neurons and microglia in the brain tissue surrounding the hematoma,as well as the expression of NOD-like receptor thermal protein domain associated protein 3(NLRP3)in neurons.ELISA was employed to measure IL-1β,TNF-α and myeloperoxidase(MPO)levels in peri-hematoma tissue.TUNEL staining was performed to quantify apoptotic neurons around the hematoma.Nissl staining was utilized to evaluate neuronal damage.Western blotting was conducted to detect the expression of GPR39,p-CREB,CREB,NLRP3,Cleaved caspase-1(C-caspase-1),and gasdermin-D protein(GSDMD)in peri-hematoma brain tissue.Results GPR39 expression peaked at 48 h post-ICH in mice(P<0.05),and it was expressed in both neurons and microglia.Activation of GPR39 by TC-G 1008(24 mg/kg)significantly improved the modified Garcia score,and increased success rate of left forelimb placement and the number of left turns(P<0.05).Brain edema in the ipsilateral basal ganglia(BG)and cortex(CX)was significantly reduced(P<0.05).The numbers of apoptotic and damaged neurons around the hematoma were obviously decreased(P<0.05).The expression of pyroptosis-related molecules,including NLRP3,C-caspase-1 and GSDMD and the levels of inflammation-related factors,including IL-1β,TNF-α and MPO were notably decreased(P<0.05).However,knockdown of GPR39 and downregulation of p-CREB expression significantly increased the expression of pyroptosis related molecules and inflammatory-related factors in peri-hematoma brain tissue post-ICH in mice(P<0.05).Conclusion GPR39 activation may inhibit neuroinflammation and brain injury after ICH in mice partly through the CREB signaling pathway.Therefore,GPR39 may be a potential therapeutic target for mitigating neuroinflammation and brain damage after ICH.

The senescence accelerate mouse prone strain was identified by Professor T.Takeda's laboratory in Japan from the offspring produced by the brother-sister inbred line of AKR/J mice donated by Jackson Laboratory in the United States SAMP),and SAMP8 mice showed a variety of aging characteristics,such as:Pathological changes such as memory and learning disorders,behavioral abnormalities,skin roughness,aging,hair loss,eye lesions(visual impairment,cataract,periocular lesions),hearing impairment,weight loss,muscle strength loss,reproductive system aging,liver and fat metabolism disorders,etc.,are widely used in experimental studies of related diseases or pathology,and have made considerable contributions to the research of diseases.However,there are also many problems in its application,such as the selection of month age,the use of intervention methods,the suitability of pathological changes and research content,and the understanding of models under different theoretical backgrounds,which are of great research value.Therefore,this paper systematically analyzes the above problems based on existing literature,in order to provide relevant basis for the application of SAMP8 mice in different fields and different mechanisms.

Androgenetic alopecia (AGA) is currently the most common hair loss disease, which is the result of a combination of genetic and environmental factors, and manifests itself as miniaturization of hair follicles around the top of the head and frontotemporal region, which eventually leads to hair reduction or disappearance. The pathogenesis of AGA is closely related to a variety of signaling pathways, which include the Wnt/β-catenin signaling pathway, bone-forming protein (BMP) signaling pathway, insulin-like growth factor (IGF) signaling pathway, fibroblast growth factor (FGF) signaling pathway, hedgehog (SHH) signaling pathway, and so on. Several cytokines are involved in these signaling pathways, including β-catenin, BMP2, IGF-1, and FGF5. An in-depth study of AGA-related signaling pathways and related cytokines can help to provide a comprehensive understanding of the pathogenesis of AGA, provide a certain theoretical basis for the discovery of new signaling pathways, and provide a new research direction for the treatment of AGA.
