1.Ultrasound discovery of focal nodular hyperplasia causing Budd Chiari syndrome:a case report
Chinese Journal of Ultrasonography 2025;34(3):256-259
Focal nodular hyperplasia(FNH),which usually occurs in the context of a normal liver,is the second most common benign hepatic lesion, with typical imaging manifestations of hypointensification in the portal and delayed phases of enhancement scans, and a stellate scar visible in the center. Budd-Chiari syndrome(BCS)is a rare hepatic vascular disease defined as obstruction of the hepatic venous outflow tract and is categorized as primary and secondary. The patient was a 20-year-old female, who came to the First Hospital of Jilin University for routine physical examination, without no obvious discomfort,no definite abnormality on physical examination, previous physical fitness,no abnormality on laboratory tests. Ultrasound(contrast-enhanced ultrasound)and enhanced CT suggested:FNH;BCS. In this case,BCS was due to compression of the right hepatic vein and inferior vena cava by a giant FNH,the patient refused to undergo invasive treatment due to the presence of an effective collateral circulation of the inferior vena cava with intrahepatic return and the absence of portal hypertension and other manifestations. Ultrasound and contrast-enhanced ultrasound can be used for real-time and dynamic imaging to observe intra- and extra-hepatic vascular obstruction and hemodynamic changes, with typical imaging manifestations of FNH and BCS,providing strong evidence for the diagnosis of this disease.

Objective:]To investigate the role of Arpin protein in bone repair by mediating migration of host bone marrow mesenchymal stem cells (BMSCs) to the bone defect area after transplantation of tissue engineering bone (TEB).Methods:Immunofluorescence was used to observe the expression and relative localization of Arpin and Arp2/3 proteins in BMSCs. Lentiviruses that ware designed to interfere with Arpin expression were constructed to transfect BMSCs for knockdown Arpin expression. Knockdown efficiency was verified by real-time quantitative reverse transcription PCR ( qRT-PCR) and Western blot. According to different levels of Arpin protein expression, experiments were divided into empty vector control group and an Arpin expression inhibition group in vitro and in vivo. In vitro experiments: the cell migration model was established with a migration chamber, then the cells from both groups were seeded on the up chamber, and the number of migrated cells were detected by fluorescence microscopy. Cells from both groups were seeded on six-well plates. Model of wound healing experiment was established and wound healing ratio was examined by microscopy. In vivo experiments: 8-week-old C57BL/6 mice were selected and assigned to empty vector control group and Arpin expression inhibition group according to the random number table, with 6 rats per group. Diaphysis of 2 mm and periosteum in the middle femur were excised to make a large segment of bone defects. Then, TEB was transplanted into the defect area and fixed.Green fluorescein-labeled BMSCs (1 million cells per mouse) from empty vector control group and Arpin expression inhibition group were injected through the tail vein. Number of BMSCs homing to the bone defect area was detected by immunofluorescence staining at day 2 and 7 after operation. At 4 weeks after operation, the femur was taken for a Micro-CT scan to analyze bone mass density(BMD), bone volume density (BV/TV), trabecular spacing (Tb.Sp) and trabecular thickness (Tb.Th). Then, the specimens were stained with pathological HE and MASSON staining to observe the quality of bone formation. Results:Mouse BMSCs expressed Arpin protein, which was located at the cell edge relative to Arp2/3. After transfection of lentivirus, BMSCs expressed green fluorescent protein, and the expression of Arpin gene and protein in Arpin expression inhibition group were decreased compared to empty vector control group ( P<0. 01). BMSCs migration was enhanced in Arpin expression inhibition group compared to empty vector control group [(76.6±6.6) vs. (105.7±6.5)] ( P<0. 01). Wound healing was accelerated in Arpin expression inhibition group compared to empty vector control group [(43.8±0.19)% vs. (62.6±3.2)%]( P<0.01). At day 2 after operation, immunofluorescence results showed no significant difference in cell migration between the two groups and almost no labeled cells migrated. At day 7 after operation, more cells migrated to the transplanted area in Arpin expression inhibition group compared to empty vector control group [(5.7±1.5) vs. (11.3±1.5)] ( P<0.01). At 4 weeks after operation, Micro-CT results showed that Arpin expression inhibition group had better bone formation quality than empty vector control group [BMD: (172.7±6.0)mg/cm 3vs. (140.0±6.0)mg/cm 3, BV/TV: (28.8±1.3)% vs. (23.4±0.9)%, Tb.Sp: (0.33±0.01)μm vs. (0.28±0.01)μm, Tb.Th: (0.11±0.01)μm vs.(0.15±0. 01)μm]( P<0.05). Pathological staining showed there were more new bone tissue in Arpin expression inhibition group ( P<0.01). Conclusion:Silencing Arpin protein expression promotes BMSCs to migrate to the bone defect area and improves bone repair effect.

Chinese Journal of Hospital Administration 2018;34(5):418-420
Objective To learn the command of basic theories, basic knowledge and basic skills, as well as strict requirement, strict attitude and strict behavior(three basics and three strictness)of clinicians at secondary and above public hospitals in Chongqing. Methods In this study, the examination scores of 1 650 clinicians from 55 such hospitals were assessed in terms of their three basics and three strictness performance during November 9-11 of 2016. The data were analyzed with t test and descriptive statistics. Results The theory scoring and skill scoring of all the clinicians averaged 61.25 ± 0.72 and 68.19 ± 21.12 respectively. Theory scoring and skill scoring of tertiary hospital clinicians were significantly higher than those at secondary(P<0.05). The skill scoring of teaching hospital clinicians averaged 74.87 ± 20.67, higher than those at non-teaching hospitals(67.38 ± 21.07, P=0.045). Conclusions The basic medical quality lacks due attention by medical institutions at all levels, and the training and assessment of clinicians on three basics and three strictness fall far behind. The clinicians rely heavily on large medical equipments for their diagnoses, neglecting most basic diagnostic skills. Basic medical quality of the city can only be enhanced by strengthening the training and assessment of three basics and three strictness for clinicians.

To understand the possibility of schistosomiasis transmission on the East Route Project of the South-to-North Water Diversion Project, a survey of endemic status of schistosomiasis in Weishan Lake area was conducted. A cluster sampling of 2086 boatmen were screened with DDIA Kit, and the positive ones were examined with Kato-Katz technique. Meanwhile, a questionnaire survey about schistosomiasis control was carried out to collect the data about the boatmen's general information and knowledge , attitude and practice (KAP). The results showed there were no schistosomiasis patients. However, there exist potential risk factors for schistosomiasis transmission because of the bad hygiene habits and the poor knowledge of schistosomiasis prevention in boatmen.

Objective To investigate the effects of VEGF-induced expression of Survivin by colon cancer cells (LOVO, HCT116) and human umbilical vein endothelial cells (HUVECs). Methods VEGF was used to stimu-late the LOVO, HCT116, HUVECs. We used RT-PCR and Western blot to estimate the expression of Survivin in different cells, and immunocytochemistry to investigate the expression level and site of Survivin protein. Results Survivin was expressed highly in the cytoplasm and was upregulated with the induction of VEGF. In HUVECs, Sur-vivin expression was not obvious but significantly upregulated with the inductioin of VEGF in the cytoplasm and nu-cleus. Conclusion The expression of Survivin mRNA and protein are upregulated by VEGF stimulation. The level and distribution of Survivin are different in tumor cell lines and non-tumorgenicity cells.
