1.Capacity building status of technical support institutions for occupational disease prevention and treatment in Ningxia Hui Autonomous Region

Objective To understand the capacity of technical support institutions for occupational disease prevention and treatment in Ningxia Hui Autonomous Region (hereinafter referred as "Ningxia"). Methods The basic data of 26 technical support institutions for occupational disease prevention and treatment in Ningxia in 2023 were collected through the "National Management Platform for Technical Support Institutions for Occupational Disease Prevention and Treatment". The status of these institutions was investigated, and their qualifications, human resources, equipment allocation, and testing capacities were analyzed. Results Among the 26 technical support institutions for occupational disease prevention and treatment in Ningxia, all were qualified occupational disease surveillance institutes, eight were qualified occupational hazard factor monitoring institutes, two were qualified occupational medical examination institutes, one was a qualified occupational diagnosis institute, one was a qualified occupational treatment institute, four were qualified for radiological health technical services, and one was qualified for occupational health technical services. A total of 168 professionals engaged in occupational diseases prevention and treatment, with the majority holding a bachelor degree and junior professional titles, accounting for 66.1% and 40.5%, respectively. The technical support institutions for occupational disease prevention and treatment of the district level,city level and county level of Ningxia were provided with 121, 68 and 32 instruments and equipment respectively on average,and the number of testing items of occupational hazard factors which the institutions of aforementioned levels are 50.0,17.0 and 1.3 respectively. Conclusion The technical support institutions for occupational disease prevention and treatment in Ningxia show insufficient qualifications and capacities, inadequate professional staff, imbalanced talent structure, incomplete equipment allocation, weak testing capacity, and an underdeveloped support system.
2.Cost-utility analysis of sugemalimab combined with chemotherapy as first-line treatment for advanced esophageal squamous cell carcinoma with high PD-L1 expression

OBJECTIVE To evaluate the cost-effectiveness of the first-line treatment using the combination therapy of sugemalimab and chemotherapy (hereinafter referred to as the “combination therapy”) for advanced esophageal squamous cell carcinoma (ESCC) with high programmed death-ligand 1 (PD-L1) expression from the perspective of the Chinese healthcare system. METHODS A partitioned survival model was constructed based on data from the GEMSTONE-304 study. The model cycle was set at 3 weeks, with a study duration of 10 years and a discount rate of 5%. The primary output parameters of the model included total costs, quality-adjusted life year (QALY), incremental costs, and incremental cost-effectiveness ratio (ICER). Cost- utility analysis was employed to assess the economic feasibility of the combination therapy compared to chemotherapy alone. The robustness of the base case analysis results was evaluated through univariate sensitivity analysis, probabilistic sensitivity analysis, and scenario analysis. RESULTS The ICER of the combination therapy compared to chemotherapy alone was 288 430.35 yuan/QALY, significantly exceeding the willingness-to-pay (WTP) threshold of 173 354.52 yuan/QALY which was set at 1.94 times the per capita gross domestic product (GDP) in 2023. The price of sugemalimab was the primary factor influencing the ICER. When the WTP threshold was set at 1.94 times the per capita GDP (173 354.52 yuan/QALY), the probability of the combination therapy being cost-effective compared to chemotherapy alone was 0. The combination therapy only became cost-effective compared to chemotherapy alone when the price of the drug dropped to 6 107.41 yuan per box (600 mg). CONCLUSIONS From the perspective of the Chinese healthcare system, the combination therapy for first-line treatment of advanced ESCC with high PD-L1 expression is not cost-effective; the combination therapy is cost-effective when the price of sugemalimab decreas by 50.65%.
3.TIPS with bare stents and covered stents for the treatment of portal hypertension:analysis of its long-term efficacy

Objective To evaluate the long-term efficacy of transjugular intrahepatic portosystemic shunt(TIPS)with bare stents and Fluency covered stents in the treatment of portal hypertension,and to discuss its clinical value.Methods The clinical data of 29 patients with intractable ascites or esophagogastric fundus varices rupture and hemorrhage caused by cirrhotic portal hypertension,who received TIPS with bare stents and covered stents at the First Affiliated Hospital of Xinjiang Medical University of China(25 patients)and the Lishui Municipal Central Hospital of China(4 patients)between August 2012 and December 2017,were retrospectively analyzed.The patients were regularly followed up to check the survival status.The postoperative cumulative shunt patency rate and cumulative survival rate of the patients were analyzed by Kaplan-Meier method.Results The technical success rate of TIPS was 100%.The mean portal vein pressure was decreased from preoperative(40.21±3.24)cmH2O to postoperative(24.55±3.55)cmH2O(P<0.05).The patients were followed up for 5.1-10.5 years.The postoperative 1-,3-,5-,7-year primary cumulative patency rates of the shunt were 89.7%,75.9%,75.9% and 52.5%,respectively.The postoperative 5-,7-,9-and 10-year cumulative survival rates were 100%,66.9%,66.9% and 33.4%,respectively.The incidence of hepatic encephalopathy was 13.8%(4/29).Conclusion Using bare stents combined with Fluency covered stents for TIPS is clinically safe and effective in the treatment of portal hypertension.This technique carries higher long-term shunt patency rate and low incidence of hepatic encephalopathy.Therefore,it can be used as a substitute for Viatorr stent when necessary.(J Intervent Radiol,2024,33:295-299)
4.Compound cottonrose hibiscus leaf gel plaster of optimal"Xiaozhongsan"formulation for knee synovitis

BACKGROUND:Previous studies have confirmed that the new compound cottonrose hibiscus leaf gel plaster has a good effect in the treatment of acute soft tissue swelling. OBJECTIVE:To observe the clinical efficacy of compound cottonrose hibiscus leaf gel plaster in the treatment of synovitis of the knee joint. METHODS:Seventy-two patients with knee synovitis were selected from Ruijin Hospital Affiliated to Shanghai Jiao Tong University School of Medicine from December 2019 to May 2021.These patients were randomly divided into a trial group and a control group,with 36 cases in each group.The trial group was treated with compound cottonrose hibiscus leaf gel plaster,once a day,12 hours each time,while the control group was treated with Diclofenac Diethylamine Emulgel,twice a day.After 28 days of treatment,visual analog scale score,WOMAC Osteoarthritis Index score,quality of life score(SF-36),thickness of knee synovium and comprehensive curative effect were compared between the two groups. RESULTS AND CONCLUSION:(1)Visual analog scale scores after treatment were lower than those before treatment(P<0.05).Visual analog scale scores in the trial group after 7,14 and 28 days of treatment were lower than those in the control group(P<0.05).The WOMAC Osteoarthritis Index scores of the two groups after treatment were lower than those before treatment(P<0.05),and the WOMAC Osteoarthritis Index scores in the trial group after 7,14 and 28 days of treatment were lower than those in the control group(P<0.05).(3)The SF-36 quality of life score in the two groups after 28 days of treatment was higher than that before treatment(P<0.05).SF-36 quality of life score in the trial group after 28 days of treatment was higher than that in the control group(P<0.05).(4)After 28 days of treatment,the thickness of knee synovium in the trial group was less than that in the control group(P<0.05),and the effective rate in the trial group was higher than that in the control group(P<0.05).(5)These findings indicate that compared with Diclofenac Diethylamine Emulgel,the compound cottonrose hibiscus leaf gel plaster can better relieve knee pain,enhance knee joint function,reduce synovial hyperplasia,and elevate the overall quality of life of patients.
5.Measurement of three-dimensional parameters of lower cervical facet joints and design of transarticular facet screw guide

BACKGROUND:In recent years,cervical facet joints have been paid more and more attention to the pathogenesis and surgical treatment of cervical spondylosis,but there are few anatomical studies on adult lower cervical facet joints. OBJECTIVE:To measure three-dimensional parameters of the lower cervical facet to provide a basis for the design of the lower cervical transarticular facet screw guide. METHODS:From June 2021 to June 2022,100 cases receiving cervical spine CT examination in the Affiliated Hospital of Xuzhou Medical University were selected,with 50 males and 50 females,aged 20-50 years.After screening,each image showed no cervical spinal stenosis,cervical disc herniation,obvious bone hyperplasia,infection or tumor.The sagittal inclination angle of each segment of the cervical spine facet joint and the angle between the lower cervical spine facet joint surface and the transarticular facet joint screw at the C3-7 levels were measured after 3D reconstruction.According to the measurement results of statistical analysis,a lower cervical transarticular facet screw guide was designed using CAD software. RESULTS AND CONCLUSION:The inclination angle of the cervical facet joint surface on the sagittal plane was distributed in a U-shaped shape centered on C5,and the magnitude relationship was C7>C6>C3>C4>C5.The relationship between transarticular facet screw angles on the sagittal plane was:C6/7>C5/6>C4/5>C3/4,where the angle of C3/4,C4/5 and C5/6 was close to 90°,and the angle of C6/7 exceeded 100°.By measuring the sagittal inclination angle of the cervical facet joint and the angle of the transarticular facet screw,this study designed a guide that was perpendicular to the lower cervical facet joint surface in the sagittal plane.
6.Effect of knockdown IGSF10 on proliferation,migration and invasion capacity of lung adenocarcinoma cells

Objective :
To investigate the effects of immunoglobulin gene superfamily 10 (IGSF10) on prolifera- tion,migration and invasion of lung adenocarcinoma cells.
Methods :
ioinformatics was applied to study the ex- pression levels of IGSF10 in tumor tissues and normal tissues. Western blot and quantitative real-time PCR ( qPCR) were used to detect the expression level of IGSF10 in lung adenocarcinoma cell lines and normal lung epi- thelial cells.Knockdown of IGSF10,the effect of knockdown of IGSF10 on proliferation,migration and invasion of lung adenocarcinoma A549 cells was examined using cell counting kit-8 ( CCK-8) ,Transwell migration and inva- sion assay,scratch assay and plate cloning assay.The effects of knockdown of IGSF10 on the expression of invasion and migration-related genes in A549 cells were examined by Western blot and qPCR assays.
Results :
IGSF10 ex- pression in lung adenocarcinoma tissues was lower than that in normal tissues (P <0. 05) .IGSF10 expression in lung adenocarcinoma cell lines was lower than that in lung epithelial cells (P<0. 05) .Knockdown of IGSF10 pro- moted the ability of lung adenocarcinoma A549 cells to proliferate ,proliferation ,migration and invasion ( P < 0. 05) .Knockdown of IGSF10 promoted the expression of regulatory epithelial-mesenchymal transition marker Neu- ral-cadherin (N-cadherin) and key transcription factors Snail family transcriptional repressor 1 (Snail) and Snail family transcriptional repressor 2 (Slug) (P<0. 05) and inhibited the expression of Epithelial-cadherin (E-cad- herin) (P<0. 05) .
Conclusion
Knockdown of IGSF10 may promote proliferation,migration and invasion of lung adenocarcinoma cells through activation of Snail,Slug / E-cadherin signaling axis,and this result may provide a po- tential new target for clinical diagnosis and treatment of lung adenocarcinoma.
7.Design of a movable monitoring device of preventing blood leakage from internal fistula

Aiming at the problem of blood leakage from internal fistula of patients who received dialysis during treatment,the existing Bluetooth technique was applied to design a circuit diagram.A movable monitoring device of blood leakage from internal fistula was designed,and an intelligent alarm mode for the blood leakage from internal fistula was established,which could real-timely remind the blood leakage from internal fistula.The device used the technique of moisture absorption cotton patch and real-time reminder device to supplement the design defects and related treatment schemes of the existing hemodialysis machine,which could realize the safe and effective management for blood leakage from internal fistula,and can avoid the occurrence of adverse events caused by delayed treatment.
8.Analysis of risk factors for sepsis in patients with severe trauma

Objective:To investigate the risk factors for sepsis in patients severe trauma.Methods:A retrospective case-control study was conducted to analyze the clinical data of 149 patients with severe trauma admitted to General Hospital of Ningxia Medical University from January 2021 to June 2022, including 112 males and 37 females, aged 18-93 years [(50.6±16.3)years]. According to whether the patients developed sepsis, they were divided into sepsis group ( n=66) and non-sepsis group ( n=83). A comparison was made between the two groups in gender, age, measurements of body temperature, heart rate, respiration, systolic blood pressure, diastolic blood pressure, mean artery pressure (MAP), oxygen saturation (SPO 2), white blood cell (WBC), absolute neutrophil count (ANC), percentage of neutrophils (NEUT%), red blood cell count (RBC), hemoglobin (HGB), platelet count (PLT), prothrombin time (PT), activated partial thromboplastin time (APTT), D-dimer level, level of lactic acid, level of blood glucose, quick sequential organ failure assessment (qSOFA), sequential organ failure assessment (SOFA), acute physiology and chronic health evaluation II (APACHE II) score, Glasgow coma scale (GCS), and injury severity score (ISS) within 24 hours of admission, causes of injury, injury sites, number of injury sites, hemorrhagic shock (HS), open injury, endotracheal intubation, length of ICU stay and total length of hospital stay. Univariate analysis and multivariate Logistic regression analysis were used to determine the independent risk factors for severe post-traumatic sepsis. Results:The results of univariate analysis showed that there were statistically significant differences in age, respiration, SPO 2, WBC and ANC, D-dimer level, blood glucose level, qSOFA, SOFA, APACHE II score, GCS, ISS, head and neck injury, open injury, tracheal intubation, length of ICU stay, and total length of hospital stay between the sepsis group and non-sepsis group ( P<0.05 or 0.01); whereas there were no significant differences in gender, underlying disease, body temperature, heart rate, systolic blood pressure, diastolic blood pressure, MAP, NEUT%, RBC, HGB, PLT, PT, APTT, lactic acid level, cause of injury, facial injury, chest injury, abdominal and pelvic injury, limb and pelvic injury, number of injury sites, and HS between the two groups ( P>0.05). Multivariate Logistic regression analysis showed that D-dimer level ( OR=0.97, 95% CI 0.96, 0.99, P<0.01) and tracheal intubation ( OR=15.80, 95% CI 2.14, 116.69, P<0.01) were significantly correlated with sepsis in patients with severe trauma. Conclusion:D-dimer level collected within 24 hours of admission and tracheal intubation are independent risk factors for sepsis in patients with severe trauma.
9.Tildrakizumab for moderate-to-severe plaque psoriasis in Chinese patients: A 12-week randomized placebo-controlled phase III trial with long-term extension

Background::There is a need for effective and safe therapies for psoriasis that provide sustained benefits. The aim of this study was to assess the efficacy and safety of tildrakizumab, an anti-interleukin-23p19 monoclonal antibody, for treating moderate-to-severe plaque psoriasis in Chinese patients.Methods::In this multi-center, double-blind, phase III trial, patients with moderate-to-severe plaque psoriasis were enrolled and randomly assigned (1:1) to receive subcutaneous tildrakizumab 100 mg or placebo at weeks 0 and 4. Patients initially assigned to placebo were switched to receive tildrakizumab at weeks 12, 16, and every 12 weeks thereafter. Patients in the tildrakizumab group continued with tildrakizumab at week 16, and every 12 weeks until week 52. The primary endpoint was the Psoriasis Area and Severity Index (PASI 75) response rate at week 12.Results::At week 12, tildrakizumab demonstrated significantly higher PASI 75 response rates (66.4% [73/110] vs. 12.7% [14/110]; difference, 51.4% [95% confidence interval (CI), 40.72, 62.13]; P <0.001) and Physician’s Global Assessment (60.9% [67/110] vs. 10.0% [11/110]; difference, 49.1% [95% CI, 38.64, 59.62]; P <0.001) compared to placebo. PASI 75 response continued to improve over time in both tildrakizumab and placebo-switching to tildrakizumab groups, reaching maximal efficacy after 28 weeks (86.8% [92/106] vs. 82.4% [89/108]) and maintained up to 52 weeks (91.3% [95/104] vs. 87.4% [90/103]). Most treatment-emergent adverse events were mild and not related to tildrakizumab. Conclusion::Tildrakizumab demonstrated durable efficacy through week 52 and was well tolerated in Chinese patients with moderate-to-severe plaque psoriasis.Trial registration::ClinicalTrials.gov, NCT05108766.
10.Application value of cone beam CT in accurate diagnosis of submandibular gland calculi

Objective To compare the detection rates of X-ray,ultrasound,computed tomography(CT)and cone beam CT(CBCT)for submandibular gland calculi in different positions,and to explore the auxiliary value of 3-dimensional visualization technology in diagnosis and treatment of submandibular gland calculi.Methods The X-ray,ultrasound,CT and CBCT examination data of 113 patients with submandibular gland calculi at General Hospital of Ningxia Medical University from Nov.2014 to Mar.2021 were retrospectively analyzed.The detection rates and overall detection rates of the 4 imaging methods for calculi in different positions were compared.Mimics 19.0 software and CBCT data were used for 3D reconstruction imaging,visually displaying the location,size,morphology,and number of calculi from a visual perspective.Results There was no significant difference in the total detection rate of submandibular gland calculi between CBCT and CT(98.2%[111/113]vs 95.6%[108/113],P>0.05),but both were significantly higher than that of X-ray(65.5%,74/113)and ultrasound(73.5%,83/113).The detection rates of ultrasound for calculi in the anterior and middle ducts were the lowest(63.6%[21/33]and 72.2%[26/36],respectively),which were significantly different from those of CBCT(97.0%[32/33]and 100.0%[36/36],respectively)and CT(93.9%[31/33]and 97.2%[35/36],respectively)(all P<0.05).The detection rate of X-ray for calculi in the posterior duct was significantly lower than that of ultrasound,CBCT,and CT(40.9%[18/44]vs 81.8%[36/44],97.7%[43/44],and 95.5%[42/44],all P<0.01).After CBCT data were imported into Mimics 19.0 software,submandibular gland calculi with different sizes,shapes,or numbers were successfully visualized and reconstructed.The reconstructed image was clear,and anatomical position relationship between the calculi and the mandible was visualized.Conclusion CBCT has good application value in the diagnosis of submandibular gland calculi and can replace traditional CT for the diagnosis of sialolithiasis.Clinically,CBCT examination can be directly performed for accurate diagnosis of calculi in the posterior duct which are not detected by double coincidence diagnosis but are indicated by ultrasound.For calculi in the anterior and middle ducts,X-ray and CBCT are preferred.Visualization technology helps clinical patient education and the development of personalized preoperative treatment plans.
