Geminiviruses cause substantial crop yield losses worldwide. The replication initiator protein (Rep) encoded by geminiviruses is indispensable for geminiviral replication. The Rep protein encoded by beet severe curly top virus (BSCTV, genus Curtovirus, family Geminiviridae) induces VARIANT IN METHYLATION 5 (VIM5) expression in Arabidopsis leaves upon BSCTV infection. VIM5 functions as a ubiquitination-related E3 ligase to promote the proteasomal degradation of methyltransferases, resulting in reduction of methylation levels in the BSCTV C2-3 promoter. However, the specific domains of Rep responsible for VIM5 induction remain poorly characterized. Although Rep proteins from several geminiviruses act as viral suppressors of RNA silencing (VSRs), whether BSCTV Rep also possesses VSR activity remains to be illustrated. In this study, we employed a transient expression system in the 16c-GFP transgenic and the wild-type Nicotiana benthamiana plants to analyze the VSR and the VIM5-inducing activities of different truncated Rep proteins haboring distinct domains. We found that the N-terminal domain (amino acids 1-180) of Rep suppressed GFP silencing in 16c-GFP transgenic N. benthamiana leaves. The minimal N-terminal fragment (amino acids 1-104) induced VIM5 expression upon co-infiltration, while C-terminal truncations lacked VIM5-inducing activity. Our results indicate that the N-terminal domain of Rep encoded by BSCTV mediates the suppression of RNA silencing and induces VIM5 expression. Thus, our findings contribute to a better understanding of interactions between geminiviral Rep and plant hosts.

Objective To assess the willingness of students with latent tuberculosis infection (LTBI) in Jiangsu Province to undergo preventive treatment and identify factors influencing their decision, aiming to provide insights for tuberculosis prevention and control strategies in school. Methods The physical examination information of tuberculosis latent infection cases was collected from screenings of new school enrollment and contacts of tuberculosis patients in 6 cities of Jiangsu Province from December 2022 to December 2023. Data on past medical history and understanding of preventive treatment were gathered through an online questionnaire survey on the website of Juanxing, and the influencing factors related to the willingness to take preventive medication were analyzed by logistic regression analysis model. Results In December 2022 to December 2023, a total of 13 school tuberculosis outbreaks occurred in 6 cities, and 1 661 contacts were screened, among which 162 cases met the criteria for prophylactic medication, 96 cases were included in the study by filling in the questionnaire. A total of 22 600 new students from 56 schools participated in the TB screening upon enrollment, of which 358 tested positive for the tuberculin skin test alone, meeting the criteria for preventive medication, and 251 of them completed the willingness survey. Finally, 347 students who met the criteria for preventive treatment were included in the study, with 164 expressing to accept preventive treatment representing a treatment acceptance rate of 47.3%. The results of multivariate analysis showed that university (OR=17.950, 95%CI: 3.078-104.686, P=0.001) and contact with the source of school tuberculosis epidemic (OR=19.542, 95%CI: 6.289-60.726, P<0.001) were associated with increased willingness to receive preventive treatment, while unclear whether to pay for the drugs themselves (OR=0.349, 95%CI:0.133-0.916, P=0.032) was associated with decreased willingness to receive preventive treatment. Compared with Huai'an City, the willingness to receive preventive treatment was significantly lower among students from Nantong City (OR=0.005, 95%CI:0.000-0.063, P<0.001), Nanjing City (OR=0.022, 95%CI: 0.003-0.703, P<0.001) and Lianyungang City (OR=0.074, 95%CI:0.008-0.703, P=0.023). Conclusions The acceptance rate of preventive treatment among LTBI students in Jiangsu Province is not high and is affected by multiple factors. Health education and medication mobilization for preventive medication are essential.

Objective To explore the association between outdoor air pollutants sulfur dioxide (SO2) and nitrogen dioxide (NO2) exposure and latent tuberculosis infection (LTBI) risk among middle school students. Methods Basic information and LTBI screening results were collected from students undergoing entrance physical examinations in 68 middle schools across six prefecture-level cities in eastern China from 2018 to 2021. The LTBI test was conducted using the purified protein derivative method. Additionally, data on outdoor air pollutant concentrations and meteorological factors in these six cities from 2015 to 2021 were extracted. A generalized additive model was employed to assess the impact of SO2 and NO2 exposure on the risk of LTBI among middle school students, and sensitivity and subgroup analyses were further performed. Results A total of 198 275 students were included in the final analysis, of which 11 721 students were diagnosed with LTBI. Compared with the non-LTBI group, the LTBI group had a higher proportion of males and Han ethnicity, as well as older age and higher body mass index (all P<0.001). With every 10 units increase in SO2 concentration, LTBI risk increased by 12.22% (95%CI: 4.47-20.54), 23.25% (95%CI:16.13-30.81), and 22.77% (95%CI:16.78-29.07) at lag times of one year, two years, and three years, respectively. Similarly, for every 10 units increase in NO2 concentration, LTBI risk increased by 22.20% (95%CI: 17.23-27.38), 27.22% (95%CI: 21.41-33.31), and 32.05% (95%CI: 25.76-38.65), respectively, at the corresponding lag times. Furthermore, the positive association between SO2 and NO2 concentrations and the risk of LTBI was robust in the sensitivity analyses and remained significant in both male and female students. Conclusions Outdoor air pollutants SO2 and NO2 exposure are positively associated with the risk of LTBI among middle school students. Since many developing countries face the dual challenges of high LTBI rates and severe outdoor air pollution, reducing outdoor air pollutant concentrations may help mitigate the tuberculosis burden in these countries.

Objective:To investigate the diagnosis and treatment of emergency inguinal hernia. Methods:The retrospective cross-sectional study was conducted. The clinical data of 236 patients with emergency inguinal hernia who were admitted to the First Affiliated Hospital of Soochow University from January 2015 to May 2020 were collected. There were 194 males and 42 females, aged (69±30)years. Hospitalized patients received routine blood biochemistry test and imaging examinations for evaluation of characteristics of hernia contents and intestinal obstruction. Manual reduction and surgical treatment were selected according to the conditions of patients. Observation indicators: (1) treatment; (2) follow-up. Follow-up using outpatient examination and telephone interview was performed to detect hernia recurrence and late-onset mesh infection up to August 2020. Measurement data were described as M (range) or M ( P25, P75), and comparison between groups was analyzed using the Wilcoxon rank sum test. Count data were represented as absolute numbers, and comparison between groups was done using the chi-square test. Results:(1) Treatment: of the 236 patients, 106 cases had successful manual reduction, 124 cases underwent emergency operation, 6 cases refused surgery. ① For 106 cases with successful manual reduction (including 4 cases guided by B-ultrasonography), the manual reduction time was 5 minutes (2 minutes,7 minutes). Ninety-three of 106 patients underwent selective operation after manual reduction, including 89 cases with indirect hernia, 2 cases with direct hernia and 2 cases with compound hernia. The time to selective operation was 3 days(2 days,5 days) after manual reduction. Patients underwent mesh repair, of which the operation time, volume of intraoperative blood loss, time to postoperative first flatus, duration of postoperative hospital stay were 44 minutes (29 minutes, 66 minutes),10 mL(5 mL,20 mL), 1 day(1 day,2 days), 1 day(1 day,2 days), respectively. Eleven patients didn't undergo selective operation. Two patients with abdominal pain and fever after manual reduction were diagnosed with perforation of intestine by emergency surgical exploration, and then underwent partial intestinal resection combined with high ligation of hernial sac. ② There were 93 of 124 patients undergoing emergency operation with indirect hernia, 18 cases with femoral hernia, 6 cases with obturator hernia, 6 cases with compound hernia and 1 case with direct hernia. There were 54 of 124 patients undergoing open operation, including 21 cases with Bassini surgery, 18 cases with Lichtenstein surgery, 9 cases with Mc Vay surgery, 6 cases with high ligation of hernia sac. There were 70 patients undergoing laparoscopic operation, including 57 cases with laparoscopic transperitoneal preperitoneal hernia repair (TAPP), 10 cases with laparoscopic explora-tion + tissue repair and 3 cases with laparoscopic exploration + closure of inner inguinal ring. The operation time, volume of intraoperative blood loss, time to postoperative first flatus, cases with short-term postoperative complications were 60 minutes (50 minutes,76 minutes), 20 mL(14 mL,30 mL), 2 days(1 day,2 days), 15 cases for patients undergoing open surgery, respectively. The above indicators were 56 minutes (47 minutes,77 minutes), 20 mL(10 mL,25 mL), 2 days(1 day,2 days), 21 cases for patients under-going laparoscopic surgery. There was no significant difference in the above indicators between the two groups ( Z=?0.88, ?1.37, ?1.56, χ2=0.07, P>0.05). Cases with intraoperative placement of mesh and duration of hospital stay were 18 cases and 5 days(3 days,8 days) for patients undergoing open surgery, versus 57 cases and 3 days(2 days,5 days) for patients undergoing laparoscopic surgery, showing significant differences between the two groups ( χ2=29.50, Z=?4.32, P<0.05). (2) Follow-up: of 236 patients, 192 were followed up for 2?60 months, with a median follow-up time of 19 months. Seven patients had recurrence of hernia after emergency operation, including 3 with high ligation of the hernia sac, 2 with Bassini surgery, 1 with Lichtenstein surgery, and 1 with laparoscopic exploration + closure of inner inguinal ring. One patient with late-onset mesh infection after Lichtenstein surgery was improved after mesh removal. No long-term complications such as hernia recurrence or late-onset mesh infection occurred to the 184 patients. Conclusions:Emergency inguinal hernia had different state of illness, manual reduction is suitable for partial patients with incarceration. Surgery is the first choice, and the surgical procedure needs to be individually selected.

Objective To study the impact of brief intervention on HPV cognition of medical students in Xinjiang, and to provide a reference for the development of HPV related education in medical students. Methods Sudents from 20 classes of a medical university in Xinjiang were randomly selected. The first 10 classes were enrolled into group 1, which was a self-controlled study group evaluating the effect before and after intervention. The other 10 classes were included in group 2, in which students in each class were randomly divided into control group (filling in questionnaires before the intervention) and intervention group (filling in questionnaires after the intervention). Medical students were given a 10 minutes presentation on the intervention of HPV by trained personnel. Results (1) For HPV related knowledge, the difference in the awareness rates between the two groups before and after intervention was statistically significant (P < 0.001), and the awareness rate after intervention was higher than that before intervention. Before the intervention, the awareness rates of medical students in group 1 and group 2 were 18.67% and 17.71%, respectively, which increased to 66.75% and 52.85% after the intervention (P<0.01). (2) Regarding HPV-related attitudes and behavioral content, the differences in the response results of the two groups of students before and after the intervention were statistically significant (P<0.05), showing a more positive state after the intervention. Before the intervention, there were 86.19% and 88.02% of the students in group 1 and group 2 willing to publicize HPV-related knowledge to their relatives and friends, and the numbers increased to 94.12% and 94.82%, respectively, after the intervention (P < 0.05). Conclusion Both self-controlled and parallel-controlled studies demonstrated that brief interventions improved medical students' awareness of HPV.
