1.Mechanism of Zuogui Pills in regulating bone metabolism through OXT/OXTR feed-forward loop based on theory of "all marrows dominated by brain".

Grounded in the theory of "all marrows dominated by brain", this study explored the therapeutic mechanism of Zuogui Pills in modulating the oxytocin(OXT)/oxytocin receptor(OXTR) feed-forward loop in the treatment of postmenopausal osteoporosis(PMOP). A PMOP rat model was established using ovariectomy, and 70 Sprague-Dawley female rats were randomly divided into the following groups: sham operation group, model group, estradiol group(17β-estradiol, 0.05 mg·kg~(-1)·d~(-1)), Zuogui Pills low, medium, and high dose groups(0.2, 0.4, 0.8 g·kg~(-1)·d~(-1), respectively), and an antagonist group(atosiban 0.9 mg·kg~(-1)·d~(-1) + 17β-estradiol 0.05 mg·kg~(-1)·d~(-1) + Zuogui Pills 0.4 g·kg~(-1)·d~(-1)). After 12 weeks of model establishment, treatment was administered by gavage once daily for another 12 weeks, followed by sample collection. Enzyme-linked immunosorbent assay(ELISA) was used to measure serum levels of estrogen(E_2), OXT, tartrate-resistant acid phosphatase(TRACP-5b), and bone alkaline phosphatase(BALP). Histopathological changes in the left distal femur were observed through hematoxylin and eosin(HE) staining. Micro-computed tomography(micro-CT) was used to analyze the microstructure of the right distal femur. Western blot was employed to detect the expression levels of OXTR, small GTP-binding protein Ras, Raf1 proto-oncogene(Raf1), mitogen-activated protein kinase kinase 1/2(MEK1/2), and extracellular signal-regulated kinase 1/2(ERK1/2), and their phosphorylated forms in tibial tissues. Compared with the model group, the Zuogui Pills medium and high dose groups showed significantly increased levels of E_2, OXT, and BALP, with a notable decrease in TRACP-5b levels. Morphologically, the trabeculae in the left distal femur were more tightly arranged. The fibrous structure in the right distal femur was significantly improved in the Zuogui Pills high dose group. Additionally, the expression of OXTR, Ras, p-Raf1, p-MEK1/2, and p-ERK1/2 proteins in tibial tissues was significantly increased. The therapeutic effect of the Zuogui Pills high dose group was partially inhibited when an OXTR antagonist was administered. These findings suggest that Zuogui Pills can regulate the OXT/OXTR feed-forward loop, activate the phosphorylation of the downstream Ras/Raf1/MEK/ERK signaling pathway, and ultimately improve bone mineral density, thereby exerting therapeutic effects in PMOP.

2.Therapeutic Strategy and Mechanism of Xijiao Dihuangtang in Recurrence of Psoriasis Vulgaris After Remission Based on Theory of "Latent Fire Causing Blood Stasis"

Psoriasis vulgaris is notoriously difficult to treat and prone to recurrence. Traditional Chinese medicine (TCM), however, has shown considerable efficacy in mitigating or suppressing such recurrence. The underlying reason lies in the TCM concept of "latent pathogens", which are prone to be reactivated by external pathogenic factors, thereby triggering relapse. At the early stage of recurrence, manifestations of "latent fire" often appear externally. If treatment is not thorough, the condition may shift into a state of "stalemate between healthy Qi and pathogenic factors", in which the disease appears on the skin but is rooted in deeper pathological layers, remaining unresolved and accumulating internally. Over time, blood stasis arises from fire, and the fire further congeals due to stasis, leading ultimately to recurrent flare-ups. This aligns with the modern immunological concept of "immunological memory" mediated by tissue-resident memory T cells (TRM) in the skin, which corroborates the TCM view of "latent fire inducing blood stasis". The interaction between TRM and keratinocytes (KC) parallels the entanglement of latent fire and latent stasis, both of which are deeply entrenched and difficult to resolve. The core pathogenesis of recurrent psoriasis vulgaris lies in "latent fire causing blood stasis". The hallmark is the deep concealment and persistence of latent fire and stasis, which linger and await an opportunity to reemerge. Based on this understanding, Xijiao Dihuangtang is employed to cool the blood, resolve stasis, and eliminate latent pathogens, and treatment is tailored according to the disease stage through three-phase syndrome differentiation. In the progressive stage, both exterior and interior are treated, with emphasis on clearing latent fire. In the stationary stage, the focus shifts to dispelling latent stasis and simultaneously regulating the Zang-fu organs. In the regressive stage, efforts are made to prevent the retention of latent pathogens and to strengthen healthy Qi. Accordingly, drugs effective in dispersing wind and clearing heat, pungent-moistening and dredging the collaterals, and tonifying deficiency and moistening dryness are often employed to achieve optimal outcomes. The precise mechanisms by which Xijiao Dihuangtang treats recurrent psoriasis vulgaris remain to be fully elucidated. Current research suggests it may intervene in the recurrence process through inhibiting KC proliferation via the PI3K/Akt/mTOR signaling pathway and glycolysis, regulating the Th1/Th2 and Th17/Treg cell balances to restore immune homeostasis, suppressing inflammatory cytokine production to alleviate the inflammatory response, modulating angiogenesis-related factors, such as vascular endothelial growth factor A (VEGF-A) and matrix metalloproteinase-9 (MMP-9), to control disease progression, and restructuring the gut microbiota to modulate systemic immunity and thereby influence the course of disease recurrence.
3.Preliminary application of human-computer interaction CT imaging AI recognition and positioning technology in the treatment of type C1 distal radius fractures.

OBJECTIVE:
To explore the accuracy of human-computer interaction software in identifying and locating type C1 distal radius fractures.
METHODS:
Based on relevant inclusion and exclusion criteria, 14 cases of type C1 distal radius fractures between September 2023 and March 2024 were retrospectively analyzed, comprising 3 males and 11 females(aged from 27 to 82 years). The data were assigned randomized identifiers. A senior orthopedic physician reviewed the films and measured the ulnar deviation angle, radial height, palmar inclination angle, intra-articular step, and intra-articular gap for each case on the hospital's imaging system. Based on the reduction standard for distal radius fractures, cases were divided into reduction group and non-reduction group. Then, the data were sequentially imported into a human-computer interaction intelligent software, where a junior orthopedic physician analyzed the same radiological parameters, categorized cases, and measured fracture details. The categorization results from the software were consistent with manual classifications (6 reduction cases and 8 non-reduction cases). For non-reduction cases, the software performed further analyses, including bone segmentation and fracture recognition, generating 8 diagnostic reports containing fracture recognition information. For the 6 reduction cases, the senior and junior orthopedic physicians independently analyzed the data on the hospital's imaging system and the AI software, respectively. Bone segments requiring reduction were identified, verified by two senior physicians, and measured for displacement and rotation along the X (inward and outward), Z (front and back), and Y (up and down) axes. The AI software generated comprehensive diagnostic reports for these cases, which included all measurements and fracture recognition details.
RESULTS:
Both the manual and AI software methods consistently categorized the 14 cases into 6 reduction and 8 non-reduction groups, with identical data distributions. A paired sample t-test revealed no statistically significant differences (P>0.05) between the manual and software-based measurements for ulnar deviation angle, radial ulnar bone height, palmar inclination angle, intra-articular step, and joint space. In fracture recognition, the AI software correctly identified 10 C-type fractures and 4 B-type fractures. For the 6 reduction cases, a total of 24 bone fragments were analyzed across both methods. After verification, it was found that the bone fragments identified by the two methods were consistent. A paired sample t-tests revealed that the identified bone fragments and measured displacement and rotation angles along the X, Y, and Z axes were consistent between the two methods. No statistically significant differences(P>0.05) were found between manual and software measurements for these parameters.
CONCLUSION
Human-computer interaction software employing AI technology demonstrated comparable accuracy to manual measurement in identifying and locating type C1 distal radius fractures on CT imaging.

4.Effect of coping styles and psychological suzhi on relationship between depression and stressors in postgraduates

Objective:To explore the relationship between depressive symptoms and stressors among post-graduates and the role of coping styles and psychological suzhi between them.Methods:A total of 775 postgradu-ates were recruited.They were assessed with the Center for Epidemiological Studies Depression Scale(CES-D),Psychological Stressors Scale of Postgraduates(PSSP),Simplified Coping Style Questionnaire(SCSQ),and Col-lege Students Psychological Suzhi Brief Mental Health Version(CSPS-B-MH).Results:The detection rate of de-pression among postgraduates was 52.0%.Postgraduate stressors were positively associated with depression(β=0.47).Coping styles played a partial mediating role in the relationship between stressors and depression in postgrad-uates,with the mediation effect accounting for 24.76%of the total effect.Psychological suzhi was shown to moder-ate the effect of stressors on depression(β=-0.07)and positive coping on depression(β=-0.05).Conclusion:Postgraduate stressors could affect depression through the mediating role of coping styles and the moderating role of psychological suzhi.

6.Application of polymyxin B in treatment of patients with severe infections and distribution of plasma concentration

OBJECTIVE To analyze the clinical characteristics of the patients with severe infections who were treated with polymyxin B and compare the plasma concentration of polymyxin B among the patients with severe infections with different creatinine clearance rates(Ccr).METHODS The clinical data were collected from 152 patients with severe infections who were treated with intravenous polymyxin B in intensive care unit(ICU)of Zhongda Hospital Affiliated to Southeast University from Jan.2021 to Mar.2023.The trough concentration(Cmin),median concen-tration(C1/2t)and peak concentration(Cmax)of polymyxin B were determined after 5 doses were completed.Based of the area under the curve(AUC)of drug concentration of polymyxin B(AUC0~24h)combined with the Ccr level[the ≤30 to<60 ml/min group(n=40),the 60 to<130ml/min group(n=79),and the ≥ 130ml/min group(n=33)],the AUC0~24h of polymyxin B were calculated,and the influence of Ccr on the plasma concentration was observed.RESULTS Among the 152 patients with severe infections,118 were male,and 34 were female,with the age ranging between 20 and 90 years old;the patients with high blood pressure accounted for 14.47%(22/152),the patients with diabetes mellitus 7.24%(11/152).Polymyxin B is primarily used in clinical settings for the treatment of pulmonary infections and bloodstream infections caused by carbapenem-resistant Acinetobacter baumannii and Klebsiella pneumoniae.The median initial dose and the median maintenance dose were 1.35(1.00,1.67)mg/kg q12 h and 1.07(0.83,1.25)mg/kg q12 h,respectively.The median AUC0~24h of polymyxin B was 76.57(54.65,106.47)μg·h/ml among the 152 patients,and the patients with AUC0~24 h ranging between 50 and 100 μg·h/ml accounted for 53.95%(82/152).Although the median AUC0~24h of polymyxin B of all the three groups ranged between 50 and 100)μg·h/ml,there were significant differences in Cmin,C1/2t,Cmax and AUC0~24h among the three groups(P<0.05).In addition,there were significant differences in the AUC0~24h of polymyxin B less than 50 μg·h/ml,ranging between 50 and 100 μg·h/ml and more than 100 μg·h/ml among the three groups of patients(x2=21.632,P<0.001).CONCLUSION Therapeutic drug monitoring(TDM)is nec-essary for the patients with severe infection who receive the polymyxin B for treatment,especially for the patients with Ccr ≤30 to<60 ml/min and Ccr ≥130 ml/min.

8.Study on the protective effect and mechanism of Zhilong Huoxue Tongyu Capsule on myocardial ischemia reperfusion injury mice based on serum metabolomics

Objective To observe the protective effect of Zhilong Huoxue Tongyu Capsule(Zhilong Capsule)on myocardial ischemia reperfusion injury(MIRI)in mice,and explore its regulatory mechanism using metabolomics.Methods Using a random number table method,30 C57BL/6J mice were randomly divided into the following three groups:sham operation group,model group,and Zhilong Capsule group(6.24 g/kg),with 10 mice in each group.In mice in the model group and the Zhilong Capsule group,a mouse MIRI model was established by ligating the left anterior descending branch,while mice in the sham operation group underwent threading without ligation.The Zhilong Capsule group began modeling one week after pre-administration and continued to receive intragastric administration for two weeks after modeling once daily.The cardiac function,including the left ventricular ejection fraction(LVEF)and left ventricular fraction shortening(LVFS),was assessed by color echocardiography.The myocardial fibrosis and apoptosis were observed by Masson staining and TUNEL staining,respectively.Enzyme-linked immunosorbent assay was used to measure the serum contents of lactate dehydrogenase(LDH)and brain natriuretic peptide(BNP).Liquid chromatography-mass spectrometry combined with multivariate statistical method was performed for serum metabolite detection and identification analysis.Results Compared with the model group,the mice in the Zhilong Capsule group exhibited an increase in LVEF and LVFS,a reduction in cardiac tissue structure disorder,a decrease in myocardial fibrosis,a decrease in cell apoptosis rate,and a decrease in serum LDH and BNP contents(P<0.05).Metabolomics result showed that intervention with Zhilong Capsule significantly regulated 30 differential metabolites related to MIRI.Important metabolic pathways involved 20 pathways related to tyrosine metabolism,arginine and proline metabolism,and vitamin digestion and absorption.Conclusion Zhilong Capsule has a protective effect on MIRI,and it may achieve this effect by regulating pathways related to tyrosine metabolism,arginine and proline metabolism,and vitamin digestion and absorption.
9.Safety Evaluation of Tianzhi Granules in Treating Mild-to-moderate Vascular Dementia

ObjectiveTo systematically evaluate the safety of Tianzhi granules used in the treatment of mild-to-moderate vascular dementia. MethodA randomized, double-blind, double-simulated, positive drug/placebo parallel controlled multi-center phase Ⅳ clinical trial and an open multi-center phase Ⅳ clinical trial of Tianzhi granules in the treatment of mild-to-moderate vascular dementia were conducted. Safety data of 1 492 patients were included and analyzed according to inclusion and exclusion criteria. The main evaluation measures were the incidence rate of adverse events/adverse reactions, laboratory indicators, vital signs, and electrocardiogram (ECG) results. ResultA total of six adverse events possibly related to the test drug occurred in 520 patients of the double-blind trial, and the symptoms were all mild and recovered. The incidence of adverse events was not statistically different among Tianzhi granules, donepezil, and placebo groups. Nine adverse events possibly related to the test drug were observed in 972 patients of the open trial, and the symptoms were mild and recovered. Laboratory tests (blood routine, urine routine, liver function, kidney function, and coagulation) and vital signs were compared before treatment (baseline) and after treatment of 12 and 24 weeks, respectively. There was no statistical significance in the main indicators before and after treatment. In the double-blinded trial, there was no significant difference in safety indicators between different groups before and after treatment. The most frequent adverse reaction was gastrointestinal discomfort, with an incidence rate of 6.64‰. ConclusionAdverse reactions occasionally occur in patients using Tianzhi granules, and it is safe to use Tianzhi granules to treat mild-to-moderate vascular dementia clinically.
