1.Impact of compliance with enhanced recovery after surgery program on the prognosis among patients undergoing total knee arthroplasty

Objective:To study the effect of compliance with enhanced recovery after surgery (ERAS) program on the prognosis of patients undergoing total knee arthroplasty.Methods:A total of 229 patients who underwent unilateral total knee arthroplasty in Beijing Friendship Hospital, Capital Medical University from March 2022 to March 2024, the clinical data of patients were retrospectively analysed. The cohort included 57 males and 172 females, aged 61-79 years, the average age was (68.6±5.2) years. All patients received perioperative ERAS interventions, and compliance with each ERAS item was recorded. Based on overall ERAS compliance, patients were divided into high-compliance group ( n=140, compliance >89.9%) and low-compliance group ( n=89, compliance ≤89.9%). The characteristics of the two groups were collected, including gender, age, body mass index, smoking, comorbidities, Hospital for Special Surgery (HSS) score, preoperative visual analog scale (VAS) score, surgical time and estimated blood loss. Clinical outcomes indicators including HSS scores and knee range of motion (ROM) at 1 and 3 months postoperatively, length of hospital stay, complications within 3 months, and readmission at 3 months. Measurement data were expressed as mean±standard deviation ( ± s) and compared using t-test. Count data were expressed as cases and percentages and analyzed using Chi-square or Fisher exact probability method. Results:There was no significant statistical difference in age( P=0.167), gender( P=0.500), body mass index ( P=0.322), smoking( P=0.185), hypertension( P=0.118), diabetes( P=0.550), coronary heart disease( P=0.633), arrhythmias( P=0.564), chronic lung disease( P=0.460), depression( P=0.295), preoperative HSS( P=0.492), preoperative VAS( P=0.644), surgical time ( P=0.459) and estimated blood loss( P=0.171) between the high-compliance group and the low-compliance group. The postoperative knee ROM (115.2±8.5)° of patients with high-compliance was significantly higher than that of patients with low-compliance group (101.8±10.1)°, the difference was statistically significant ( P<0.001). The length of hospital stay was (13.7±3.7) d in the low-compliance group, which was more than that in the high-compliance group [(9.8±2.5) d], and the difference was statistically significant ( P=0.028). Conclusions:Patients undergoing total knee arthroplasty had a high overall compliance with the ERAS program. The prognosis of patients with higher compliance of ERAS is better, and ERAS compliance should be improved as much as possible in clinical work.
2.Analysis of monitoring results of iodine deficiency disorders in Zhongwei City of Ningxia Hui Autonomous Region after marketization reform of salt industry

Objective:To understand the surveillance of iodine deficiency disorders (IDD) in Zhongwei City of Ningxia Hui Autonomous Region after marketization reform of salt industry (2017).Methods:In 2018, Shapotou District, Zhongning County and Haiyuan County of Zhongwei City were divided into five sampling areas according to the location of east, west, south, north and middle. In each area, one township was selected, and three salt sales outlets were selected from each township; at the same time, three supermarkets were selected from the cities and towns of each county (district), and different kinds of salt samples in all sales outlets and supermarkets were tested semi-quantitatively. From 2016 to 2020, Shapotou District, Zhongning County and Haiyuan County of Zhongwei City were divided into five sampling areas according to the location of east, west, south, north and middle. One township was selected from each area, and one primary school was selected from each township, 40 non-boarding students aged 8 - 10 years old (age balanced, half males and half females) were selected from each primary school; at the same time, 20 pregnant women were selected from each township, and salt samples were collected from houses of children and pregnant women to test salt iodine content. From 2018 to 2020, according to the "Ningxia Iodine Deficiency Disorders Surveillance Program (2016 Edition)", random urine samples of children and pregnant women were collected to detect urinary iodine content. In 2018 and 2019, the thyroid volume of children was measured by B-mode ultrasound, and the goiter rate was calculated.Results:In 2018, 13 kinds of salt were supplied in the market of Zhongwei City, a total of 130 salt samples were tested, and 11 samples of non-iodized salt were detected, with a rate of 8.46%. From 2016 to 2020, the consumption rates of qualified iodized salt in Zhongwei City were 90.33% (813/900), 67.89% (611/900), 78.67% (708/900), 91.56% (825/901) and 94.44% (850/900), respectively. From 2018 to 2020, the medians urinary iodine of children aged 8 - 10 years old were 189.33, 195.64, and 222.10 μg/L, and the medians urinary iodine of pregnant women were 158.21, 158.01, and 171.84 μg/L, respectively. In 2018 and 2019, the goiter rates of children aged 8 - 10 years old were 1.75% (7/400) and 0.67% (4/600), respectively.Conclusion:After marketization reform of salt industry, the consumption rates of qualified iodized salt in Zhongwei City decrease in 2017 and 2018, however, children and pregnant women have sufficient iodine nutrition, and the goiter rate of children is controlled at a low level.
3.Clinical rescue for basicranial fracture complicated with massive hemorrhage

Objective To study the clinical rescue for basicranial fracture complicated with massive hemorrhage.Methods The therapies for 20 patients of basicranial fracture complicated with massive hemorrhage treated in our hospital within 3 years were analyzed retrospectively.Results Among 20 patients,16 cases were male,4 cases were female.7 cases were simple anterior basicranial fracture with massive hemorrhage,1 case was the delayed massive hemorrhage in cavernous fistula caused by anterior basicranial fracture,12 cases were anterior and mid basicranial fracture complicated with massive hemorrhage.In consequence,6 cases were secondary cerebrospinal leak,8 cases recovered,8 cases died.Conclusion The basicranial fracture complicated with massive hemorrhage is a very dangerous symptom with high death rate and high disability rate.The patients should be rescued actively.
4.Treatment of multiple system organs failure after severe craniocerebral injury

ObjectiveTo analyze the development mechanism and investigate the more effective therapeutic method of the multiple system organs failure (MSOF) after severe craniocerebral injury.MethodsThe clinical data of 21 MSOF cases after severe craniocerebral injury was analyzed retrospectively.ResultsOf all 21 cases, 2 cured, 7 mended and 12 died with death rate 57% and deformity rate 33.3%.ConclusionTo severe craniocerebral injury, comprehensive, timely and effective therapeutic method is the key to reduce the occurrence of MSOF and the rate of death and deformity.
