1.Comparison of differences in dosimetry and treatment efficiency of modified radiotherapy plans after left-sided breast-conserving surgery

Objective To compare the differences in dosimetry and treatment efficiency of three radiotherapy plans after left-sided breast-conserving surgery,including modified intensity-modulated radiation therapy(IMRT),cross-field volume-modulated arc therapy(VMAT)or improved VMAT,so as to provide references for clinical practice.Methods Three radiotherapy plans of modified IMRT,cross-field VMAT and improved VMAT were designed for 12 patients after left-sided-breast-conserving surgery.The modified IMRT with five irradiation fields and the improved VMAT with two arcs were modified by not setting cross-fields while determining the start and end angles with the rays passing through the least lung area.The cross-field VMAT had its start and end angles set based on the cross-fields.The doses to the target areas,peripheral organs at risk,heart and its substructures were evaluated,and dose verification was carried out.The three plans were compared in terms of treatment efficiency and gamma pass rate.SPSS 22.0 was used for statistical analysis.Results All the three plans behaved well in dose distribution.In terms of planning gross tumor volume dosimetry dosimetry,the improved VMAT and modified IMRT gained advantages than others in CI and D50,respectively,with the differences being significant(all P<0.05).In terms of planning target volume dosimetry,the modified IMRT had the V107 and D50 lower than those of the others,with the differences being significant(P<0.05).In terms of the protection of peripheral organs at risk,V5 of the left lung,Dmean of the right lung and Dmean of the healthy breast were lower in the modified IMRT plan than in the other 2 plans,with statistically significant differences(P<0.05);V20,V30,V35 and V40 of the left lung were lower in the modified VMAT plan than in the other 2 plans,with statistically significant differences(P<0.05).In terms of protection of heart and its substructures,the left ventricle V20,V30 and Dmean of the improved VMAT plan behaved better than those of other 2 plans,and the difference was statistically significant(all P<0.05).In terms of treatment efficiency,the cross-field VMAT plan had the lowest MU while highest treatment efficiency;the improved VMAT plan had the MU higher while the treatment efficiency lower than the cross-field VMAT plan;the modified IMRT plan had the highest MU while the lowest gamma pass rate,and the differences in the MUs and gamma pass rates among the three plans were statistically significant(P<0.05).Conclusion Under the same standard conditions,the cross-field VMAT and improved VMAT plans show technical advantages.Though the improved VMAT plan has the treatment efficiency lower than the cross-field VMAT,it decreases the possibility of radiocardiac injury in terms of dosimetry and thus can be used for radiotherapy after left-sided breast-conserving surgery.[Chinese Medical Equipment Journal,2025,46(4):45-51]
2.Effect of exercise during old age on cardiac function and APJ system in mice

Objective:We aimed to explore the effect of exercise on cardiac function and APJ system in old mice.Meth-ods:Male C57/BL6 mice were divided into young group(n=30),old control group(n=30),old continuous moderate-intensity training(CMIT)group(n=30),and old high-intensity interval training(HIIT)group(n=30).Mice in the CMIT group and HIIT group underwent 8-week exercise intervention,respectively.Mice in the young group and old con-trol group were placed on stationary treadmill simultaneously.The heart function parameters were measured,and the ex-pression of APJ/Apelin in myocardium and circulating Apelin/Elabela were compared among above-mentioned groups.Results:Compared yo mice in old control group,those in the CMIT group and HIIT group had significant higher left ven-tricular ejection fraction[(67.74±6.18)%,(70.69±7.43)%vs.(61.17±4.78)%]and left ventricular fractional short-ening[(33.07±4.29)%,(33.37±5.57)%vs.(28.05±3.61)%](P<0.05 or<0.01);and significant lower Tei index[(0.78±0.09),(0.78±0.12)vs.(0.88±0.08)]and left ventricular posterior wall thickness at diastole[(1.01±0.09)mm,(1.02±0.10)mm vs.(1.19±0.14)mm](P<0.05all).Compared to mice in old control group,those in the HIIT group had significant higher mitral valve E/A ratio[(1.63±0.11)vs.(1.42±0.18),P=0.043].Compared to mice in old control group,those in the HIIT group had significant higher expressions of APJ and Apelin in myocardium,as well as the levels of Apelin[(1186.23±393.74)pg/ml vs.(413.17±80.81)pg/ml]and Elabela[(5.38±2.76)ng/ml vs.(1.57±0.60)ng/ml]in the serum(P<0.05 or<0.01).Conclusion:Long-term exercise initiated late in life could im-prove heart function of mice.The APJ-Apelin/Elabela system may be involved in the exercise-mediated improvement of cardiac aging.

Objective:To investigate the effect of dulagopeptide on physical examination indexes,plasma glucose metabolism and islet function in type 2 diabetes mellitus(T2DM)patients with poorly controlled plasma glucose.Methods:135 T2DM patients with poorly controlled plasma glucose who were admitted in our hospital from January 2023 to July 2024 were selected.A prospective randomized controlled design was adopted,they were divided into control group 1(received treatment with sitagliptin,n=45),control group 2(received treatment with insulin glargine,n=45),and observation group(received treatment with dulaglutide,n=45)according to the random number table method.Physical examination indexes,plasma glucose indicators,islet function,and incidence of adverse reactions were compared among the three groups.Results:12 weeks after treatment,body mass index(BMI),waist circumference,fasting plasma glucose(FPG),glycated hemoglobin(HbA1c),and postprandial 2-hour plasma glucose(2hPG)in the observation group were lower than those in control group 1 and control group 2(P<0.05).12 weeks after treatment,the observation group had the highest HbA1c compliance rate,reaching 71.1%(P<0.05).12 weeks after treatment,the fasting C-peptide(FC-P)and HOMA-islet(CP-DM)levels in the observation group were higher than those in control group 1 and control group 2(P<0.05).Conclusion:Dulagopeptide can effectively improve physical examination indexes,plasma glucose indicators,and islet function in T2DM patients with poorly controlled plasma glucose.
4.Efficacy and Mechanism of Action in Jishengshenqi Pill Combined with Dapagliflozin in the Treatment of Diabetes Nephropathy Based on NLRP3 Inflammatory Corpuscles

Objective:To explore the efficacy and mechanism of action in Jishengshenqi pill combined with dapagliflozin in the treatment of diabetes nephropathy(DN)based on NOD like receptor protein 3(NLRP3)inflamma tory corpuscles.Methods:140 DN patients were divided into control group(n=70,treated with dapagliflozin)and study group(n=70,treated with Jishengshenqi pill combine with dapagliflozin)used random number table.The TCM syndrome scores,blood glucose index,renal function index,peripheral blood NLRP3 levels,and related factor levels between two groups of patients were compared.Results:The scores of dysuria,fatigue and cold intolerance,edema,soreness and coldness in the waist and knees,abdominal distension,pale complexion and lower five shift diarrhea in the study group after treatment were lower than those in the control group(P<0.05).Fasting blood glucose(FBG),serum creatinine(SCr),2-hour postprandial blood glucose(2hPG),blood urea nitrogen(BUN),hemoglobin A1c(HbA1c),NLRP3 messenger RNA(mRNA),and cysteine protease 1(Caspase-1)levels in the study group after treatment were lower than those in the control group(P<0.05).Conclusion:After the treatment of Jishengshenqi pill combined with dapagliflozin in DN patients,the clinical symptoms and blood glucose can be effectively controlled,and renal function can also be improved,it is speculated that this is related to the inhibition of NLRP3 mRNA expression in peripheral blood mononuclear cells and the activation and amplification of downstream inflammatory pathways.

6.Working practices in eliminating the public health crisis caused by viral hepatitis in Hainan Province of China

In 2022, Hainan provincial government launched the project for the prevention and control of viral hepatitis with the goals of a hepatitis B screening rate of 90%, a diagnostic rate of 90%, and a treatment rate of 80% among people aged 18 years and above by the year 2025, and the main intervention measures include population-based prevention, case screening, antiviral therapy, and health management. As of December 31, 2024, a total of 6.875 million individuals in the general population had been screened for hepatitis B, with a screening rate of 95.6%. A total of 184 710 individuals with positive HBsAg were identified, among whom 156 772 were diagnosed through serological reexamination, resulting in a diagnostic rate of 84.9%. A total of 50 742 patients with chronic hepatitis B were identified, among whom 42 921 had hepatitis B-specific health records established for health management, with a file establishment rate of 84.6%. A total of 31 553 individuals received antiviral therapy, with a treatment rate of 62.2%. A total of 2.503 million individuals at a high risk of hepatitis C were screened, among whom 4 870 tested positive for HCV antibody and 3 858 underwent HCV RNA testing, resulting in a diagnostic rate of 79.2%, and 1 824 individuals with positive HCV RNA were identified, among whom 1 194 received antiviral therapy, with a treatment rate of 65.5%. In addition, 159 301 individuals with negative HBsAg and anti-HBs and an age of 20 — 40 years were inoculated with hepatitis B vaccine free of charge. Through the implementation of the project for the prevention and control of viral hepatitis, a large number of hepatitis patients have been identified, treated, and managed in the province within a short period of time, which significantly accelerates the efforts to eliminate the crisis of viral hepatitis.
7.Development and Initial Validation of the Multi-Dimensional Attention Rating Scale in Highly Educated Adults.

OBJECTIVES:
To report the development, validation, and findings of the Multi-dimensional Attention Rating Scale (MARS), a self-report tool crafted to evaluate six-dimension attention levels.
METHODS:
The MARS was developed based on Classical Test Theory (CTT). Totally 202 highly educated healthy adult participants were recruited for reliability and validity tests. Reliability was measured using Cronbach's alpha and test-retest reliability. Structural validity was explored using principal component analysis. Criterion validity was analyzed by correlating MARS scores with the Toronto Hospital Alertness Test (THAT), the Attentional Control Scale (ACS), and the Attention Network Test (ANT).
RESULTS:
The MARS comprises 12 items spanning six distinct dimensions of attention: focused attention, sustained attention, shifting attention, selective attention, divided attention, and response inhibition.As assessed by six experts, the content validation index (CVI) was 0.95, the Cronbach's alpha for the MARS was 0.78, and the test-retest reliability was 0.81. Four factors were identified (cumulative variance contribution rate 68.79%). The total score of MARS was correlated positively with THAT (r = 0.60, P < 0.01) and ACS (r = 0.78, P < 0.01) and negatively with ANT's reaction time for alerting (r = -0.31, P = 0.049).
CONCLUSIONS
The MARS can reliably and validly assess six-dimension attention levels in real-world settings and is expected to be a new tool for assessing multi-dimensional attention impairments in different mental disorders.

8.Effects of fangchinoline derivative LYY-32 on biological properties of BLM DNA helicase

Aim To investigate the effects of the fangchinoline derivative LYY-32 on the biological prop-erties of the BLM642-1290 DNA helicase,in order to lay a foundation for further research on its antitumor activity.Methods Fluorescence polarization assay,malachite green-phosphate and ammonium molybdate colorime-try,and fluorescein-labeled DNA gel electrophoresis experiments were conducted to study the effects of fangchinoline derivative LYY-32 on the DNA binding activity,ATPase activity,and DNA unwinding activity of BLM642-1290 DNA helicase.The effects of LYY-32 on the DNA unwinding activity of DNA helicase in cells were studied using fluorescent techniques and time-lapse microscopy.Ultraviolet spectral scanning was used to investigate the effects of LYY-32 on the confor-mation of the BLM642-1290 DNA helicase.Results At a concentration of 10 μmol·L-1,the inhibition rate of LYY-32 on BLM642-1290 DNA helicase binding to dsDNA was 53.17%.At a concentration of 5 μmol·L-1,the inhibition rate of LYY-32 on BLM642-1290 DNA helicase binding to ssDNA was 88.49%.The inhibition rate of LYY-32 on the ATPase activity of BLM642-1290 DNA he-licase was 89.3%at a concentration of 50 μmol·L-1.When the concentration of LYY-32 exceeded 5μmol·L-1,its inhibition rate on the DNA unwinding activity of BLM642-1290 DNA helicase was 100%.LYY-32 also significantly inhibited the DNA unwinding ac-tivity of DNA helicase in cells.However,LYY-32 had no effect on the conformation of BLM642-1290 DNA heli-case.Conclusion The DNA binding activity,AT-Pase activity,and DNA unwinding activity of BLM642-1290 DNA helicase could be significantly inhibi-ted by the fangchinoline derivative LYY-32.
9.Effects of deep hyperthermia on immune function during postoperative adjuvant chemotherapy in patients with colorectal cancer

Objective:To explore the effects of deep hyperthermia on chemotherapy-related adverse effects and immune-inflammatory indicators in the patients undergoing postoperative adjuvant chemotherapy for colorectal cancer.Methods:This retrospective study included 52 patients who underwent surgery for colorectal cancer at the Affiliated Zhongshan Hospital of Dalian University from September 2021 to December 2023. The patients were divided into two groups based on treatment method: the combination group ( n=29) received postoperative adjuvant chemotherapy combined with deep hyperthermia, while the chemotherapy group ( n=23) received postoperative adjuvant chemotherapy alone. Both groups were treated with the XELOX regimen (oxaliplatin + capecitabine). The degree of bone marrow suppression during treatment was assessed by analyzing peripheral blood parameters, including hemoglobin, leukocyte count, neutrophil count, and platelet count. Immune-inflammatory indicators, including complement, procalcitonin (PCT), interleukin-6 (IL-6), systemic immune-inflammation index (SII), neutrophil-to-lymphocyte ratio (NLR), and platelet-to-lymphocyte ratio (PLR), were compared before and after treatment in both groups to evaluate the effects of deep hyperthermia on the immune-inflammatory response. Chi-square test or Fisher's exact test (two-tailed) was used to compare bone marrow suppression rates, and the immune-inflammatory indicators between the two groups were compared using t-tests or non-parametric tests, depending on whether the data conformed to a normal distribution. Results:In terms of myelosuppression, the incidence rates of moderate to severe decreases in leukocytes, neutrophils, platelets, and hemoglobin in the combination group were 31%, 31%, 21%, and 14%, respectively, compared to 52%, 61%, 48%, and 9% in the chemotherapy group. The change in PCT levels before and after treatment was significantly greater in the combination group than in the chemotherapy group ( P = 0.010). Both the combination group and the chemotherapy group showed significant reductions in SII, NLR and PLR after treatment, and the differences were statistically significant (all P < 0.05). The change in NLR before and after treatment was significantly greater in the combination group than in the chemotherapy group ( P = 0.031). Conclusions:Deep hyperthermia can alleviate chemotherapy-induced adverse effects such as thrombocytopenia and neutropenia in patients undergoing postoperative adjuvant chemotherapy for colorectal cancer. It also appears to improve the inflammatory response in these patients.
10.Optimization of simmering technology of Rheum palmatum from Menghe Medical School and the changes of chemical components after processing

OBJECTIVE To optimize the simmering technology of Rheum palmatum from Menghe Medical School and compare the difference of chemical components before and after processing.METHODS Using appearance score,the contents of gallic acid,5-hydroxymethylfurfural (5-HMF),sennoside A+sennoside B,combined anthraquinone and free anthraquinone as indexes,analytic hierarchy process (AHP)-entropy weight method was used to calculate the comprehensive score of evaluation indicators;the orthogonal experiment was designed to optimize the processing technology of simmering R.palmatum with fire temperature,simmering time,paper layer number and paper wrapping time as factors;validation test was conducted.The changes in the contents of five anthraquinones (aloe-emodin,rhein,emodin,chrysophanol,physcion),five anthraquinone glycosides (barbaloin,rheinoside,rhubarb glycoside,emodin glycoside,and emodin methyl ether glycoside),two sennosides (sennoside A,sennoside B),gallic acid and 5-HMF were compared between simmered R.palmatum prepared by optimized technology and R.palmatum.RESULTS The optimal processing conditions of R.palmatum was as follows:each 80 g R.palmatum was wrapped with a layer of wet paper for 0.5 h,simmered on high heat for 20 min and then simmered at 140 ℃,the total simmering time was 2.5 h.The average comprehensive score of 3 validation tests was 94.10 (RSD<1.0%).After simmering,the contents of five anthraquinones and two sennosides were decreased significantly,while those of 5 free anthraquinones and gallic acid were increased to different extents;a new component 5-HMF was formed.CONCLUSIONS This study successfully optimizes the simmering technology of R.palmatum.There is a significant difference in the chemical components before and after processing,which can explain that simmering technology slows down the relase of R.palmatum and beneficiate it.
