1.Effectiveness of rotator cuff repair surgery in treating proximal humeral fractures and its association with patient age and Neer classification

Objective:To investigate the effectiveness of rotator cuff repair surgery in treating proximal humeral fractures and its association with patient age and Neer classification.Methods:A retrospective analysis was conducted on the clinical data of 90 patients with proximal humeral fractures who underwent rotator cuff repair surgery at the Marine Police Corps Hospital of the Chinese People's Armed Police Force between January 2020 and January 2023. The clinical data of these patients were retrieved from the hospital's medical record database. Based on the age distribution of the patients, they were divided into four groups: Group 1 (> 18 years old and < 30 years old, n = 10), Group 2 (≥ 30 years old and < 45 years old, n = 15), Group 3 (≥ 45 years old and < 60 years old, n = 40), and Group 4 (> 60 years old, n = 25). There were no statistically significant differences in baseline data among the four groups (all P > 0.05), indicating comparability. Additionally, these patients were divided into three groups based on Neer classification: Group I (Neer type II, n = 28), Group II (Neer type III, n = 30), and Group III (Neer type IV, n = 32). Differences in various outcome measures related to treatment efficacy, including operation time, intraoperative blood loss, fracture healing time, Visual Analogue Scale (VAS) score, Constant-Murley score, Neer shoulder function score, and the overall incidence of postoperative complications, were recorded and compared across different age groups. Overall efficacy was assessed according to the Neer criteria: > 90 points as excellent, 80-89 as good, 70-79 as fair, and < 70 as poor. Results:After 6 months of surgery, the overall clinical treatment effectiveness rate was 95.56% (86/90). At 2 weeks postoperatively, the VAS score was (3.45 ± 1.35) points, which was significantly decreased compared with the level measured preoperatively ( P < 0.05). At 6 months postoperatively, the Constant-Murley score and Neer shoulder function score were (90.34 ± 6.34) points and (86.34 ± 5.46) points, respectively (both P < 0.05), which were significantly increased compared with the levels measured preoperatively. There were significant differences in fracture healing time ( F = 4.89, P < 0.05), intraoperative blood loss ( F = 5.28, P < 0.05), VAS score ( F = 5.02, P < 0.05), and Neer shoulder function score ( F = 5.32, P < 0.05) and Constant-Murley scores ( F = 3.85, P < 0.05) compared with before treatment. There was no statistically significant difference in the overall incidence of postoperative complications or surgical time among Groups 1, 2, 3,and 4 [0 vs. 5.00% (2/40) vs. 6.67% (1/15) vs. 12.00% (3/25), χ2 = 0.98, P > 0.05]. Group III had longer surgical time and fracture healing time compared with Groups I and II ( F = 4.55, 4.23, both P < 0.05), with greater intraoperative blood loss ( F = 5.24, both P < 0.05). Additionally, Group III had a higher VAS score compared with Groups I and II ( F = 5.06, P < 0.05), while the Neer shoulder joint function score and Constant-Murley score in Group III were lower ( F = 3.83, 4.56, both P < 0.05). There was no statistically significant difference in the overall incidence of postoperative complications among the Groups I, II, and III [9.38% (3/32) vs. 3.57% (1/28) vs. 6.67% (2/30), χ2 = 1.00, all P > 0.05]. Pearson correlation analysis revealed a positive correlation between patient age, Neer classification, intraoperative blood loss, fracture healing time, and VAS score ( r = 0.565, all P < 0.05), and a negative correlation with the Neer shoulder joint function score ( r = -0.620, P < 0.05). However, there was no significant correlation between patient age, Neer classification, and the Constant-Murley score ( r = -0.008, P > 0.05). Conclusion:The rotator cuff repair is effective in treating proximal humeral fractures, but the effectiveness varies depending on patient age and Neer classification. Patient age and Neer classification are positively correlated with intraoperative blood loss, fracture healing time, and VAS score, while they are negatively correlated with the Neer shoulder joint function score.
2.Application of the second generation snap shot freeze combined with artificial intelligence in free heart rate coronary CT angiography

Objective To evaluate the application value of the second generation snap shot freeze(SSF2)combined with artificial intelligence reconstruction in free heart rate coronary computed tomography angiography(CCTA).Methods The examination data of 37 patients undergoing CCTA were divided into two groups for reconstruction.Group A,reconstruction by artificial intelligence after SSF2 algorithm correction;group B,original images automatically split and multi-phase reconstruction by artificial intelligence.Image quality were compared on volume rendering(VR),curve planar reformation(CPR),maximum intensity projection(MIP)image,subjective evaluation,objective scoring,and signal-to-noise ratio(SNR).Independent samples t-test and Chi-square test were used,and P<0.05 was considered statistically significant.Results There were statistically significant differences in the image quality score and SNR of the two reconstruction methods(P=0.009).Group A scored better,with higher signal intensity,lower noise intensity,and better SNR.The difference in the number of right coronary artery(RCA)analyzable segments between the two groups was statistically significant(P<0.05).The excellent and good rate of subjective evaluation of coronary artery segments in group A RCA(98.6%)was higher than that in group B(69.6%).Conclusion Using SSF2 combined with artificial intelligence reconstruction technology can significantly improve the image quality of CCTA,improve the success rate of CCTA examination,and improve the overall work efficiency.
3.Establishment and application of a solid-phase blocking ELISA method for detection of antibodies against classical swine fever virus

Background:
Classical swine fever (CSF) is a severe infectious disease of pigs that causes significant economic losses to the swine industry.
Objectives:
This study developed a solid-phase blocking enzyme-linked immunosorbent assay (spbELISA) method for the specific detection of antibodies against the CSF virus (CSFV) in porcine serum samples.
Methods:
A spbELISA method was developed based on the recombinant E2 expressed in Escherichia coli. The specificity of this established spbELISA method was evaluated using reference serum samples positive for antibodies against other common infectious diseases.The stability and sensitivity were evaluated using an accelerated thermostability test.
Results:
The spbELISA successfully detected the antibody levels in swine vaccinated with the C-strain of CSFV. In addition, the detection ability of spbELISA for CSFV antibodies was compared with that of other commercial ELISA kits and validated using an indirect immunofluorescence assay. The results suggested that the spbELISA provides an alternative, stable, and rapid serological detection method suitable for the large-scale screening of CSFV serum antibodies.
Conclusions
The spbELISA has practical applications in assessing the vaccination status of large pig herds.
4.Association of regulator of G protein signaling 4 gene polymorphism and methylation with schizophrenia

Objective:To explore the differences of regulator of G protein signaling 4 (RGS4) gene polymorphisms and methylation between schizophrenia and healthy controls, as well as the association between gene polymorphisms and methylation.Methods:A total of 129 schizophrenia patients and 131 healthy controls from Southen Fujian were enrolled in this study. The peripheral blood DNA of all the subjects was extracted.The three polymorphic loci of RGS4 (rs10759, rs12753561 and rs951436) were amplified, sequenced, and then genotyped. In addition, 32 subjects were randomly selected from the two groups respectively and the gene methylation level of RGS4 was detected by sequencing after bisulfite treatment. SPSS 20.0 software was used for data analysis. The χ2 test and independent sample t-test were used to analyze the difference of gene methylation of RGS4.Two-way ANOVA was used to analyze the association between RGS4 gene polymorphism and methylation. Results:There were three genotypes of AA, AC and CC for rs10759 locus in the subjects of patient group and control group. And the distribution difference of genotypes between the two groups was statistically significant ( χ2=6.431, P=0.040), but there was no significant difference in allele frequency( χ2=1.270, P=0.260). For rs12753561, there were three genotypes of GG, GT and TT, and their distribution of genotypes was significantly different ( χ2=6.217, P=0.045). There was no significant difference for the allele frequency for rs12753561( χ2=0.021, P=0.885). For rs951436, there were three genotypes of AA, AC and CC, and there were no significant difference in genotype distribution and allele frequency distribution between the two groups( χ2=0.008, 0.007, both P>0.05). Methylated CpG sites were found in 26 patients and 27 healthy controls, and these were no significant difference between the two groups( χ2=0.110, P=0.740). There was no significant difference ( t=-0.318, P=0.752) of individual methylation rate (number of methylation sites/10) between schizophrenia patient group (0.24±0.11) and healthy control group (0.26±0.18). There was also no significant difference of methylation rate between male and female in both groups(both P>0.05). Finally, there was no significant difference of individual methylation rate among rs10759, rs12753561 and rs951436 genotypes (all P>0.05). Conclusion:RGS4 rs10759 and rs12753561 genotypes may be involved in schizophrenia, while RGS4 gene methylation has no association with schizophrenia. In addition, RGS4 gene polymorphism has no association with the methylation in the current experimental setting.
5.Study on Effects and Active Ingredients of Tibetan Medicine Lamiophlomis rotata against Rheumatoid Arthritis

OBJECTIVE:To study the effects and active ingr edients of Tibetan medicine Lamiophlomis rotata against rheumatoid arthritis (RA). METHODS :Fifty-six SD rats were randomly divided into normal control group (0.5% sodium carboxymethylcellulose),model group (0.5% sodium carboxymethylcellulose ),methotrexate group (positive control group ,3 mg/kg),L. rotata water extract low-dose ,medium-dose and high-dose groups (0.5,1,2 g/kg,by crude drug ),L. rotata total flavonoid group (200 mg/kg,by flavonoid extat ),with 8 rats in each group. Except for normal control group ,other groups were given Freund ’s complete adjuvant (FCA)on the rat ’s right hind footpad to induce adjuvant-induced arthritis model. The next day after injection of FCA ,rats in all groups were given relevant medicine intragastrically ,once a day (once every 3 days for methotrexate group )for 30 days. At 15th and 30th day of administration ,the degree of paw swelling of left hind foot was measured,and the arthritis index ,spleen index were calculated. At the end of 30th day of administration ,the levels of TNF-α,IL-1β, IL-6 and IL- 10 in rat serum were determined by ELISA assay ,the thymus index and spleen index were calculated ,the pathological changes of the ankle joints were observed by HE staining. RESULTS :Compared with normal control group ,degree of paw swelling,arthritis index at 15th and 30th day of administration as well as the spleen index and the levels of TNF-α,IL-1β, msxmX0146) IL-6 in serum at 30th day of administration were significantly com increased in model group (P<0.01);while the thymus indexyunbinji- and IL- 10 level at 30th day of administration were ang@swu.edu.cn significantly decreased(P<0.01);synovial cell proliferation and infiltration of articular cavity were observed in ankle joint. Compared with model group ,degree of paw swelling ,arthritis index at 15th and 30th day of administration as well as the spleen index and the levels of TNF-α,IL-1β,IL-6 in serum at 30th day of administration were significantly decreased in each medicine group (P<0.01);while the thymus index and IL- 10 level at 30th day of administration were significantly increased (P<0.01);the pathological changes of arthritis were significantly improved. Compared with L. rotata water extract high-dose group ,there were no significant differences in degree of paw swelling at 15th day of administration as well as the arthritis index ,spleen index ,levels of inflammatory cytokines and pathological changes of ankle joint in L. rotata total flavonoid group (P>0.05). CONCLUSIONS :Tibetan medicine L. rotata shows well anti-RA activity ,and total flavonoids may be the active ingredients of its efficacy.
6.Rethinking of neoadjuvant therapy for patients with initially resectable colorectal cancer liver metastases

Surgical resection is the best method for patients with colorectal cancer liver metastases. However, tumor recurrence rate is still high after surgery. Preoperative chemotherapy can help shrink the tumor, test biological behavior, and reduce recurrence rate; but it may also cause liver injury and delay surgery. There is still controversy whether neoadjuvant chemotherapy should be performed and how to select patients from chemotherapy before surgery. Thus, in this article, combined the research progress and the clinical experience of author's center, we discuss this issue in 4 aspects: the development of neoadjuvant chemotherapy; the indications and guideline recommendation for neoadjuvant chemotherapy; the selection of neoadjuvant chemotherapy regimens; common problems in neoadjuvant chemotherapy.
7.Interactions between belief index and apolipoprotein E gene polymorphism in relationship to the prevalence risk of mild cognitive impairment

Objective To explore the interactions between belief index and apolipoprotein E (ApoE) gene polymorphism in association of the risk of cognitive function. Methods Two thousand four hundred and ten community dwellings aged 55 years and over were enrolled in health check program in Ningxia Hui Autonomous Region, and fasting venous blood was collected to detect ApoE gene polymorphisms using high resolution melting curve method. The MMSE coupled with Duke University Religion Index was administered to assess the cognitive function and belief index. Crossover analysis and logistic regression model was employed detecting the interaction effect. Results Four hundred and sixteen (17.3%,416/2410) participants met the criteria of mild cognitive impairment (MCI), and 58.0%(1399/2410) participants with high belief index.The proportion of ?4 carrier, ?2 carrier were 14.9% (358/2410) and 14.5% (350/2410), respectively. The crossover analysis showed the synergy index was 0.40, indicated an antagonism between belief index and?4 allele. Participants with?4 allele carrier with low belief index had the highest risk of MCI (OR=1.95, 95%CI 1.24-3.07) compared with those without?4 allele carrier with low religiosity. Logistic regression model revealed a persist antagonism between belief index and ?4 allele (β=-0.76, OR=0.47, 95%CI 0.25-0.86,P=0.015) under controlled covariables age, gender, hypertension and blood glucose abnormalities. Conclusion The negative interaction of belief index with ApoE?4 allele may be significantly associated with the reduced risk of MCI.

9.The clinical diagnostic value of SYT-SSX gene detection combined with immunohistochemistry in synovial sarcoma

Objective To analyse the clinicopathological characteristics of synovial sarcoma (SS) from histomorphology,immunohistochemical (IHC) and fluorescence in situ hybridization (FISH),and to investigate the related factors influencing the prognosis of SS patients.Methods 94 cases were collected,including 60 SS cases and 34 in dined to SS cases.The expressions of common antibodies related to the diagnosis of SS Vimentin,CD99,AE1/AE3,EMA,CD34,Calponin,Ki-67 were detected by IHC,and the SYT-SSX fusion gene was detected by FISH.The clinical pathologic factors that affecting the prognosis of patients were analyzed through the statistical method.Results The positive rates of Vimentin,CD99,AE1/AE3,EMA,CD34,Calponin,Ki-67 were 100.00 %,74.47 %,36.17 %,28.72 %,17.02 %,90.43 % and 60.64 % respectively.FISH results showed that 81 patients were tested with SYT gene translocation,including 54 cases who were considered as SS,and 27 cases who were suspected/inclined to SS,and the number of positive cells surpassed 80 %.The gene translocation was not related with the patient' s age,sex,tumor position,histological type,differentiation of the cancer (all P > 0.05).Single factor survival analysis showed that the degree of differentiation,tumor diameter,the recurrence and metastasis had certain influences on patients' survival time (all P < 0.05).Multiple factors regression analysis showed that the degree of differentiation and tumor diameter were the risk factors influencing the prognosis of SS (risk coefficients > 1,P < 0.05).Conclusions IHC combined with FISH SYT-SSX fusion gene detection have the vital significance in the diagnosis of SS.The degree of differentiation and tumor size are the risk factors influencing the prognosis of SS.
10.Clinical study of bevacizumab combined with preoperative chemotherapy for colorectal cancer patients with liver metastases

ObjectiveTo explore the efficacy and safety of bevacizumab combined with preoperative chemotherapy for colorectal cancer patients with liver metastases.MethodsClinical data of 89 colorectal cancer patients with liver metastases admitted and treated in Sun Yat-sen University Cancer Center between May 2009 and August 2013 were retrospectively analyzed. The informed consents of all patients were obtained and the local ethical committee approval was received. According to the first-line chemotherapy regimens, the patients were divided into the bevacizumab combined with preoperative chemotherapy group (bevacizumab group,n=32) and the simple preoperative chemotherapy group (the chemotherapy group,n=57). Among the patients in the bevacizumab group, 24 were males and 8 were females with the age ranging from 29 to 74 years old and the median of 59 years old, 22 were with colon cancer and 10 were with rectal cancer. Among the patients in the chemotherapy group, 42 were males and 15 were females with the age ranging from 28 to 74 years old and the median of 57 years old, 42 were with colon cancer and 15 were with rectal cancer. The progression-free survival, response rate, resection rate and conversion rate of liver metastases and adverse effect incidence of preoperative therapy in two groups were observed and compared. The rates were compared using Chi-square test, and the survival analysis was conducted using Kaplan-Meier method and Log-rank test.ResultsThe median progression-free survival was 16 months in the bevacizumab group and 13 months in the chemotherapy group, and no significant difference was observed in the progression-free survival rate between two groups (χ2=0.030,P>0.05). The response rate, resection rate and conversion rate of liver metastases were respectively 59%(19/32), 69%(22/32) and 53%(17/32) in the bevacizumab group and 39%(22/57), 54%(31/57) and 40%(23/57) in the chemotherapy group, and no signiifcant differences were observed (χ2=3.561, 1.755, 0.983;P>0.05). The overall incidence of adverse events was 12%(4/32) in the bevacizumab group with 2 cases of neutropenia, 1 case of hand-foot syndrome and 1 case of gradeⅢ gums bleeding, while the overall incidence of adverse events was 9%(5/57) in the chemotherapy group with 3 cases of thrombocytopenia, 1 case of neutropenia and 1 case of liver function impairment. And no signiifcant difference was observed between two groups (χ2=0.313, P>0.05).ConclusionsBevacizumab combined with preoperative chemotherapy is safe and has potential curative effect to prolong the disease-free survival for colorectal cancer patients with liver metastases.
