1.Clinical characteristics and related factors of cognition disorders in elderly patients with hypertension

Objective:To analyze the clinical characteristics and related factors of cognition disorders in elderly hypertensive patients.Methods:It was a cross-sectional study. A total of 612 hypertensive patients with the age of (69.06±6.58) years (median 68.00 years) admitted in the Department of Cardiology, General Hospital of Chinese People′s Liberation Army from October 2022 to April 2024 were enrolled. The demographic and clinical data were collected, the cognition status was assessed with Mini-Mental State Examination (MMSE) at admission. The related factors of cognition disorders were analyzed with univariate and multivariate logistic regression.Results:The results showed that female hypertensive patients and those with older age, lower education, higher fasting blood glucose (FBG) and diabetes mellitus, higher low-density lipoprotein cholesterol (LDL-C) level, higher systolic blood pressure (SBP) and more cardiovascular comorbidities were likely to have cognition disorders (all P<0.05). Multivariate logistic regression analysis showed that smoking history, elevated SBP, elevated heart rate, elevated FBG, and elevated LDL-C were independent risk factors for cognition disorders in elderly hypertensive patients,while higher education level was an independent protective factor (all P<0.05). Conclusion:Smoking, increased SBP, increased heart rate, increased FBG, increased LDL-C and lower education level are independently associated with cognition disorders in elderly hypertensive patients.
2.Research on Data Mining Based on R Language:Tuina Techniques and Acupoint Selection Rules for Treating Adolescent Idio-pathic Scoliosis

[Objective]To analyze the manipulation and acupoint selection rules of Tuina treatment for adolescent idiopathic scoliosis(AIS).[Methods]Clinical research literature on Tuina treatment for AIS was searched in databases such as China National Knowledge Infrastructure(CNKI),VIP Database,WanFang Data,China Biology Medicine disc(CBMdisc)and PubMed from database establishment date to November 31,2024.Frequency statistics,association rules and cluster analysis were performed on the data by using R Studio 4.3.0 software,and the analysis results were visualized.[Results]A total of 122 clinical research articles on Tuina treatment for AIS were included,among which 116 were Chinese articles and 6 were English articles.These studies were extracted 122 tuina prescriptions,which involved 75 types of Tuina techniques,with a total application frequency of 376 times.The technique with the highest application frequency was the kneading method.A total of 19 acupoint prescriptions were extracted,involving 48 acupoints with a frequency of 119,among which Shenshu was the most frequently used acupoint.The acupoints mainly belonged to the Bladder Meridian of Foot-Taiyang,the Governing Vessel,the Kidney Meridian of Foot-Shaoyin,the Liver Meridian of Foot-Jueyin and some Extra-meridian Points.Among the combinations of manipulations and acupoints,kneading and Shenshu were used most frequently in combination with other manipulations and acupoints.Association rule network analysis showed that Dachangshu,Shenshu,Weizhong and Chengfu were strongly correlated among acupoints;kneading,pressing and Tuina were strongly correlated among manipulations.In the cluster analysis results,high-frequency acupoints formed 5 effective clusters and high-frequency manipulations formed 3 effective clusters.[Conclusion]Tuina treatment of AIS mainly uses loosening and pulling manipulations as a supplement,focusing on adjusting the balance of Yin and Yang of the spine;the selection of acupoints is mainly in local part combined with those along the meridians.
3.Application of artificial intelligence in the diagnosis and treatment of lumbar disc herniation:evolution towards standardization,efficiency,and precision of diagnosis and treatment methods

BACKGROUND:In recent years,artificial intelligence has been increasingly integrated into the diagnosis and treatment of lumbar disc herniation,enhancing the accuracy and efficiency of diagnostic procedures and diversifying therapeutic approaches.This integration has positioned artificial intelligence as a burgeoning focal point within the field.OBJECTIVE:To provide a comprehensive overview of the current applications of artificial intelligence in the diagnosis and treatment of lumbar disc herniation,to analyze the limitations of the relevant technologies.METHODS:A systematic computer-assisted literature search was performed in PubMed,CNKI,WanFang Database,and VIP Database for relevant publications regarding the application of artificial intelligence in the diagnosis and treatment of lumbar disc herniation from database inception up to August 2024.The search keywords included"lumbar disc herniation,artificial intelligence,machine learning,deep learning,big data,robot,neural network,model,algorithm."A total of 101 articles were selected based on predefined inclusion criteria and were reviewed.RESULTS AND CONCLUSION:Different artificial intelligence technologies have propelled the intelligent treatment of lumbar disc herniation,showing great potential for future development.Deep learning technology,based on advanced algorithms,constructs corresponding learning models to optimize the processing of X-ray,CT,and MRI images,achieving precise localization,identification,and analysis of degenerated intervertebral discs,and improving the accuracy of automated diagnosis.Big data technology utilizes data platforms to analyze vast medical data,simulate disease development trends,and provide a new perspective for disease assessment and prediction.Surgical robots,combined with robotic arms,3D high-definition vision systems,and 5G communication technology,support remote precise surgical operations,demonstrating significant technological advantages.In the future,the diagnosis and treatment of lumbar disc herniation by artificial intelligence will evolve towards standardization,efficiency,and precision through continuous optimization of algorithms and the professional development of data platforms.

7.Discussing the Diagnosis and Treatment Ideas of Lumbar Disc Herniation from the Theory of Kidney Governor Qi Pulse

[Objective]To discuss the diagnosis and treatment of lumbar disc herniation from the theory of kidney governor Qi pulse founded by Professor LYU Lijiang.[Methods]Firstly,analyze the relationship among the kidneys,the Governing Vessel and the spine,clarify the roles of the kidneys and the Governing Vessel on the spine,and understand the theoretical basis of the theory of kidney governor Qi pulse.Then the etiology and pathogenesis of lumbar disc herniation were analyzed based on the theory of kidney governor Qi pulse.Finally,the treatment principles of lumbar disc herniation were analyzed from different treatment methods.[Results]The three elements of kidney-Governing Vessel-spine are closely related to each other.The fundamental pathogenesis of lumbar disc herniation lies in the deficiency of the kidney and stagnation of Governing Vessel.Deficiency of the kidney results in emptiness of Governing Vessel,while Governing Vessel stagnation leads to a dysfunction in the ascending and descending of Yin and Yang in Governing Vessel.This disruption causes an inadequate circulation of Qi,blood,essence and body fluids,leading to a lack of nourishment for the tendons and bones and an obstruction in the flow of Qi and vessels.The treatment principle is to benefit the kidney and pass the Governing Vessel.By supplementing and tonifying the kidney Qi and replenishing the kidney essence,the Governing Vessel becomes filled,allowing the ascent of Governing Vessel Yang Qi and the descent of Governing Vessel Yin Qi.This facilitates the transformation of essence and marrow,promoting the smooth circulation of Qi,blood,essence and body fluids.Consequently,the lumbar vertebrae becomes stable,the lumbar muscles strengthen,the meridians and collaterals regulate properly,and the flow of Qi and vessels becomes unobstructed.[Conclusion]The theory of kidney governor Qi pulse can guide the clinical diagnosis and treatment of lumbar disc herniation,benefiting the kidney and passing the Governing Vessel is an important principle of treating lumbar disc herniation,and it can be carried through the whole process of treating lumbar disc herniation.
8.A Prospective Cohort Study of Lever Positioning Manipulation for L4/5 Different Types of Lumbar Disc Herniation Based on Propensity Scoring

[Objective]Based on the propensity score matching method to explore the therapeutic effect of lever positioning manipulation in the treatment of L4/5 different types of lumbar disc herniation.[Methods]A prospective cohort design was used to select 122 patients with L4/5 lumbar disc herniation and divide them into unilateral group,bilateral group and central group.Samples with balanced covariates were obtained after matching,including 36 patients in unilateral group,35 patients in bilateral group and 35 patients in central group.All patients in the three groups were treated with leverage positioning technique,and all patients in the three groups were treated 3 times a week,once every other day,for a total of 2 weeks.Pain Visual Analogue Scale(VAS)score and Japanese Orthopaedic Association(JOA)score were observed before and after treatment to evaluate the differences in the postoperative effects of the three groups.[Results]The covariates of age,sex,course of disease,VAS score and JOA score were all balanced after matching(P>0.05).The results of following-up after 2 weeks and 6 months of treatment showed that the VAS score and JOA score were statistically significant(P<0.05),the distant and near term curative effect of lever positioning manipulation in the treatment of unilateral lumbar disc herniation was better than bilateral and central disc herniation(P<0.05).[Conclusion]The clinical efficacy of unilateral lumbar disc herniation is superior to bilateral and central disc herniation in the treatment of the same segment of lumbar disc herniation with leverage positioning manipulation.
9.Alpelisib-induced diabetic ketoacidosis in a non-diabetic patient

A 58-year-old female patient without disease history of diabetes mellitus was enrolled in the phase Ⅲ clinical trial of alpelisib combined with fulvestrant due to ineffective treatment of stage Ⅳ breast cancer. The treatment regimen was alpelisib tablets 300 mg orally once daily and fulvestrant injection 500 mg once a month intramuscularly. One month later, the patient experienced symptoms such as poor appetite, nausea, vomiting, drowsiness, and blurred consciousness. Laboratory tests showed arterial blood pH 7.1, random blood glucose >34.7 mmol/L, serum creatinine 210 μmmol/L, blood potassium 3.00 mmol/L, blood sodium 129 mmol/L, and urine ketone body (++). Diabetic ketoacidosis was diagnosed, and treatments such as intravenous insulin supplementation, rehydration, and correction of acidosis and electrolyte disorders were given. After 5 days, the patient′s symptoms were improved, but the blood glucose was poor controlled. Diabetic ketoacidosis was considered to be related to alpelisib. The dose of alpelisib was reduced by half, oral hypoglycemic drugs were added, and the insulin regimen was adjusted. Later, the blood glucose was relatively well controlled, and urinary ketone bodies turned negative.
