Central pontine myelinolysis (CPM) is a rare and fatal demyelinating disease of the central nervous system. Patients with CPM may have basic diseases, such as alcoholism and electrolyte disorders. There are various clinical symptoms of alcoholic central pontine myelinolysis, however, mental symptoms, such as hallucinations and delusions are rarely reported. This is a case report of alcoholic central pontine myelinolysis with mental symptoms, which suggests that mental symptoms may be one of the clinical manifestations of CPM and warrant further attention in clinical settings. Early MRI examination would be beneficial in reducing the missed diagnosis and misdiagnosis of the disease, and improving the prognosis of the patients.
