Objective To discuss the application of 3D printing technology in the adjuvant treatment of complex Stanford type B aortic dissection(SBAD)and abdominal aortic aneurysm.Methods The clinical data of 64 patients with complex SBAD and 64 patients with abdominal aortic aneurysm,who were admitted to the Mianyang No.404 Hospital of China from January 2022 to January 2024,were retrospectively analyzed.Of the 64 patients with complex SBAD,33 received preoperative 3D printing adjuvant treatment(observation group Ⅰ)and 31 received preoperative routine examination(control group Ⅰ).Of the 64 patients with abdominal aortic aneurysm,32 received preoperative 3D printing adjuvant treatment(observation group Ⅱ)and 32 received preoperative routine examination(control group Ⅱ).The changes in left-right diameter(LR)and anterior-posterior diameter(AP)of anatomical structure in observation group Ⅰ and observation group Ⅱ were analyzed.The perioperative situations were compared between observation group Ⅰ and control group Ⅰ,as well as between observation groupⅡ and control group Ⅱ.Results In patients with complex SBAD,LR of descending aorta diaphragm in S2(STL model)was significantly higher than that in S1(CTA image)and S3(plastic model,P＜0.05),and AP of descending aorta diaphragm in S2 was higher than that in S3(P＜0.05).LR of brachiocephalic trunk in S3 was significantly lower than that in S1 and S2(P＜0.05),AP of brachiocephalic trunk in S3 stage was significantly higher than that in S1 and S2(P＜0.05),and AP of brachiocephalic trunk in S2 was higher than that in S1(P＜0.05).LR of left common carotid artery in S3 was significantly higher than that in S1 and S2(P＜0.05),LR of left common carotid artery in S2 was higher than that in S1(P＜0.05),and AP of left common carotid artery in S3 was lower than that in S1(P＜0.05).LR and AP of left subclavian artery in S3 were significantly higher than those in S1 and S2(P＜0.05).In patients with abdominal aortic aneurysm,LR and AP of tumor neck in S3 were significantly higher than those in S1(P＜0.05),and AP of aneurysm neck in S3 was significantly higher than that in S2(P＜0.05).LR and AP of aneurysm in S3 and S2 were significantly higher than those in S1(P＜0.05),and LR and AP of aneurysm in S3 were significantly higher than those in S2(P＜0.05).LR of abdominal aortic bifurcation in S3 and S2 was significantly higher than that in S1(P＜0.05),LR of abdominal aortic bifurcation in S3 was significantly higher than that in S2(P＜0.05),and AP of abdominal aortic bifurcation in S3 was significantly lower than that in S1(P＜0.05).AP of left common iliac artery in S3 was significantly lower than that in S1(P＜0.05).In the observation group Ⅰ,the operation time,endovascular operation time and length of hospital stay were significantly shorter than those in the control group Ⅰ(P＜0.05),and the intraoperative blood loss and used dosage of contrast agent were lower than those in the control group Ⅰ(P＜0.05).In observation group Ⅱ,the operation time,endovascular operation time and length of hospital stay were significantly shorter than those in the control group Ⅱ(P＜0.05),and the intraoperative blood loss and used dosage of contrast agent were lower than those in the control group Ⅱ(P＜0.05).In patients with complex SBAD or abdominal aortic aneurysm,there was no internal leakage or stent displacement at 6 months after surgery.Conclusion Adjuvant treatment with 3D printing technology is helpful for improving anatomical structure measurement of lesion sites in patients with complex SBAD and abdominal aortic aneurysm.Preoperative 3D plastic model preview surgery is helpful for shortening the operation time and length of hospital stay and reducing the used dosage of contrast agent without affecting surgical treatment effect.

Objective To investigate the serum amino acid levels in patients with acute schizophrenia(SCZ)and their predictive effect on the improvement of cognitive function after treatment,so as to provide new insights into the clinical intervention of cognitive impairment in SCZ patients.Methods A total of 66 patients with acute SCZ were enrolled(case group-baseline period).Among them,36 cases completed the follow-up after 3 months of standardized treatment(case group-follow-up period);52 healthy controls(HCs)were included.The MATRICS Consensus Cognitive Battery(MCCB)was used to assess the cognitive function of all the participants.Liquid chromatography-tandem mass spectrometry(LC-MS)was employed to detect the concentrations of 18 amino acids in fasting serum of the case group-baseline period and the control group.Independent samples t-test was used to compare serum amino acid levels and cognitive function between the case group-baseline period and the control group.Paired t-test was used to compare the differences in cognitive function between the baseline period and the follow-up period of the case group.Spearman correlation analysis and multivariate linear regression model were used to investigate the correlation between serum amino acid levels at baseline in the case group and the improvement of cognitive function after 3 months of treatment.Results Compared with the control group,the cognitive function of SCZ patients in multiple dimensions at baseline was significantly reduced(P＜0.05).After treatment,the scores of Trail Making Test(TMT),Brief Assessment of Cognition in Schizophrenia:Symbol Coding(BACS),Wechsler Memory Scale-Ⅲ(WMS),and Brief Visuospatial Memory Test-Revised(BVMT)in patients were significantly improved(all P＜0.05).In addition,the levels of proline,methionine,histidine,phenylalanine,arginine,tyrosine,aspartic acid,tryptophan,lysine,and glutamic acid were significantly lower in the case group at baseline than in the control group(all P＜0.05).Among them,the baseline tyrosine level had a significant predictive value for the improvement of TMT(R2=0.136,P=0.029),Neuropsychological Assessment Battery(NAB)(R2=0.339,P＜0.001),and Mayer-Salovey-Caruso Emotional Intelligence Test(MSCEIT)test(R2=0.165,P=0.015).The baseline arginine level had a significant predictive value for the improvement rate of Fluency test(R2=0.113,P=0.048).Conclusion There is a decrease in various amino acid levels in patients with SCZ,and some amino acids can effectively predict the improvement of cognitive function after treatment.

Objective:To investigate the early clinical outcomes of anterior cervical discectomy and fusion (ACDF) in the treatment of two-segment cervical spondylotic myelopathy (CSM) using a cervical soft endoscopic minimally invasive system.Methods:A retrospective follow-up study was conducted on the medical records of 23 patients with two-segment cervical myelopathy who underwent ACDF using a soft endoscopic cervical spine minimally invasive system at the Third Affiliated Hospital of Southern Medical University between October 2022 and December 2023. The cohort included 15 males and 8 females, aged 37-79 years (58.52±11.77 years). The affected cervical segments included: C 3, 4 and C 4, 5 in 2 cases; C 3, 4 and C 5, 6 in 3 cases; C 4, 5 and C 5, 6 in 10 cases; C 5, 6 and C 6, 7 in 7 cases; and C 4, 5 and C 6, 7 in 1 case. Clinical outcomes were evaluated based on the Japanese Orthopaedic Association (JOA) scores and visual analog scale (VAS) for neck and shoulder pain, assessed preoperatively and at 1 week, 1 month, and 3 months postoperatively. Additional data recorded included incision length, intraoperative blood loss, operative time, postoperative complications, and the presence of prevertebral soft tissue edema. The improvement rate of JOA scores at the final follow-up was also calculated. Results:All patients successfully underwent surgery and completed follow-up, with follow-up durations ranging from 3 to 6 months (4.01±0.98 months). The mean operative time was 80.09±22.66 min (range: 53-127 min), and the mean incision length was 3.25±0.32 cm (range: 3-4 cm). Estimated blood loss ranged from 10 to 100 ml, with a mean of 34.78±24.1 ml. Postoperative drainage ranged from 0 to 80 ml (mean: 23.13±26.1 ml), and postoperative hospitalization durations ranged from 4 to 12 days (6.83±2.59 days). JOA scores improved significantly from a preoperative median of 9.00(8.00, 10.00) to 12.00(11.00, 14.00) at 1 week, 13.00(12.00, 14.00) at 1 month, and 15.00(15.00, 16.00) at 3 months postoperatively (χ 2=220.492, P<0.001). VAS scores for neck and shoulder pain also improved significantly from a preoperative median of 5.00(4.00, 6.00) to 3.00(2.00, 3.00) at 1 week, 2.00(2.00, 3.00) at 1 month, and 2.00(1.00, 2.00) at 3 months postoperatively (χ 2=170.869, P<0.001). No postoperative complications such as dysphagia, hoarseness, nerve injury, cerebrospinal fluid leakage, or intraspinal hematoma were observed. Imaging revealed no significant prevertebral soft tissue edema. At the final follow-up, the improvement rate of JOA scores resulted in 14 cases rated as excellent and 9 as good. Conclusions:ACDF using a cervical soft endoscopic minimally invasive system demonstrates satisfactory clinical outcomes for the treatment of two-segment CSM. This technique reduces the incidence of common complications associated with both open and traditional endoscopic surgeries.

Objective This study aimed to explore the feasibility of using a long short-term memory(LSTM)model based on empirical mode decomposition(EMD)and singular spectrum analysis(SSA)to predict the incidence of pulmonary tuberculosis in Shanxi Province.The goal was to provide a reliable prediction method to support the prevention and control of tuberculosis epidemics in the region.Methods Collecting and collating monthly data on the reported incidence of tuberculosis nationwide from January 2007 to December 2018.The LSTM、EMD-LSTM、SSA-LSTM models were established using the reported monthly incidence of tuberculosis reported in Shanxi Province from January 2007 to December 2017 as the training set and using the reported monthly incidence of tuberculosis from January to December 2018 as the test set.Mean squared error(MSE),mean absolute error(MAE),root mean squared error(RMSE),and mean absolute percentage error(MAPE)were used to evaluate the prediction effect of the models to determine the best model.Results The MSE,MAE,RMSE and MAPE of the EMD-LSTM model in predicting the incidence trend of pulmonary tuberculosis in the next year were 0.036,0.140,0.189 and 0.045,respectively.Compared with the LSTM model,the prediction performance increased by 66.36%,38.33%,42.38%and 41.56%,respectively.Compared with the SSA-LSTM model,it improved by 28.00%,9.68%,15.25%and 16.67%,respectively.Conclusion Compared with the single LSTM model,the fitting and prediction performance of EMD-LSTM and SSA-LSTM models are improved effectively.However,the prediction effect of EMD-LSTM model is better than that of SSA-LSTM model.Therefore,the EMD-LSTM model is more suitable for predicting the incidence trend of pulmonary tuberculosis in Shanxi Province,and can provide a theoretical basis for tuberculosis prevention and control policies.

Objective To explore the prediction performance of the autoregressive summation moving average(ARIMA)model based on empirical mode decomposition(EMD)for the prevalence trend of tuberculosis,to provide method support for the prediction of tuberculosis,and to provide ideas for the prediction of other infectious diseases.Methods The monthly data of pulmonary tuberculosis incidence in Shanxi Province from January 2008 to December 2018 were collected and sorted.The last three months,six months,nine months and one year of the data were used as the test set to evaluate the model prediction effect,and the training set was the remaining data of the corresponding sequence.The EMD-ARIMA model was constructed to predict and compared with the single ARIMA model.Results The predicted errors of EMD-ARIMA model for the next three months,six months,nine months and one year were all smaller than the errors of ARIMA model.Conclusion Compared with single ARIMA model,EMD-ARIMA model can improve the prediction accuracy and predict the incidence of pulmonary tuberculosis,and provide effective theoretical reference for disease control and prevention.

  Objective  To investigate the needs of eight-year program clinical medical students for the organization and contents of clinical oncology courses.  Methods  From September to November 2020, a questionnaire survey was conducted among eight-year program clinical medicine students in Peking Union Medical College to find out their knowledge base in oncology, teaching mode preference and course contents of interest.  Results  A total of 122 students participated in the survey, in which 89.3%(109/122) of the students showed interest in basic and clinical research projects related to oncology, 84.4%(103/122) thought it was better to use Simulation-based medical education (SBME), and 91.0%(111/122) hoped to learn throughoff-line discussion. In terms of course contents, eight-year program medical students were more interested in knowledge directly related to clinical context, such as diagnosis, treatment, multidisciplinary comprehensive treatment and evidence-based medicine. In terms of sub-analysis, traditional eight-year students (86%, 92/107) showed a higher acceptance of palliative care than students in the 4+4 reform program(60%, 9/15) and were more willing to act as scriptwriters in SBME(26% vs. 7%, P=0.013). The students in clinical phase gained a better understanding of oncology knowledge through research training and were more inclined to take on the role of scriptwriters in SBME than those in basic phase (27% vs. 11%, P=0.048).  Conclusions  The eight-year program clinical medical students are interested in the clinical oncology course and prefer study in the form of Simulation-based medical education (SBME).
